        악성 흉수, 결핵성 흉수 및 부폐렴 삼출액과 농흉에서 Interferon-gamma, Interleukin-2 Receptor, Interleukin-6, Interleukin-10 농도의 비교

        연구배경 : Cytokine은 주요한 염증 매개 물질로서 질환에 따라 다른 양상을 보일 수 있어, 흉수를 일으키는 다양한 원인 질환의 감별에 이용되어 왔다. 이 중에서 악성 흉수, 결핵성 흉수, 부폐렴 삼출액 및 농흉에서 INF-${\gamma}$, IL-2R, IL-6, IL-1를 동사에 측정하여 비교한 연구는 많지 않아 본 연구를 시행하게 되었다. 방법 : 흉수를 주소로 내원한 환자 중에서 흉수 천자로 삼출액이 확인된 환자들을 대상으로 흉수를 채취하여, ELISA kit를 이용하여 INF-${\gamma}$, IL-2R, IL-6, IL-10 농도를 측정하였다. 결과 : 악성 흉수, 결핵성 흉수, 부폐렴 삼출액 및 농흉에서 흉수 내의 cytokine의 순서대로, INF-${\gamma}$는 $16.7{\pm}50$, $295.5{\pm}585.5$, $10.0{\pm}0$ pg/ml, IL-2R은 $3247.4{\pm}1713.3$, $7423.5{\pm}3752.8$, $3790.2{\pm}3201.1$ pg/ml, IL-6는 $600{\pm}12.8$, $556.4{\pm}161.7$, $514.4{\pm}224.8$ pg/ml, IL-10은 $28.2{\pm}55.5$, $11.3{\pm}11.7$, $98.4{\pm}141.7$ pg/ml로 각각 측정되어, 결핵성 흉수가 다른 원인의 흉수에 비해 IL-2R 농도가 의 있게 높았으며, IFN-${\gamma}$ 농도도 높은 경향을 나타내었으며, 부폐렴 삼출액에서는 IL-10농도가 다른 원인의 흉수에 비해 의미있게 높았다. 결론 : 결론적으로 흉수 내의 IL-2R농도 측정은 결핵성 흉수와 다른 원인의 흉수를 감별하는데 어느 정도 도움이 될 것으로 사료되며, IL-10 측정은 부폐렴 삼출액과 다른 원인의 흉수를 감별하는데 어느 정도 도움을 줄 것으로 생각된다. Background : Cytokines are chemical mediators that control and modulate many inflammatory processes. They work in different fashions in a variety of diseases. Discriminating between malignant effusion, tuberculous effusion, and parapneumonic effusion are crucial from the clinical view-point in Korea. In the current study, interferon-gamma (IFN-${\gamma}$), soluble interleukin-2 receptor (IL-2R), interleukin-6 (IL-6) and interleukin-10 (IL-10) were measured for this purpose. Methods : Pleural fluids from patients with malignant disease, tuberculosis, parapneumonic effusion and lung empysema were collected and gauged using commercial ELISA kits. Results : 34 patients were enrolled in this study. Among these 15 cases were malignant effusions, 12 were tuberculosis pleurisy and 7 were parapneumonic effusion and lung empyema. The levels of cytokines measured in this study were as follows, in order of frequency, malignant effusion, tuberculous effusion, parapneumonic effusion and lung empyema. The levels of INF-${\gamma}$ were higher in tuberculous effusion than in malignant or parapneumonic effusion ($295.5{\pm}585.5$ vs. $16.7{\pm}50$ vs. $10.0{\pm}0$ pg/ml, p>0.05). The levels of IL-2R were higher in tuberculous effusion than in malignant or parapneumoruc effusion ($7423.5{\pm}3752.8$ vs. $3247.4{\pm}1713.3$ vs. $3790.2{\pm}3201.1$ pg/ml, p<0.05). No significant differences were found in the levels of IL-6 between the groups ($600{\pm}12.8$ pg/ml in malignant effusion, $556.4{\pm}161.7$ pg/ml in tuberculous effusion, $514.4{\pm}224.8$ pg/ml in parapneumoruc effusion). IL-10 levels were higher in parapneumoruc effusion than in malignant or tuberculous effusions ($98.4{\pm}141.7$ vs. $28.2{\pm}55.5$ vs. $11.3{\pm}11.7$ pg/ml, p<0.05). Conclusion : These results suggest that the measurement of IL-2R levels in pleural fluids may be a useful means of differentiating between tuberculous effusion and pleural effusions of other origins, and that the measurement of IL-10 levels in pleural fluids may be useful to differentiate between parapneumonic effusion and pleural effusions of other origins.

        정상근과 근막 유발점이 활성화된 근육의 초음파 영상의 비교

        Purpose: The objective of this study was to offer primary clinical data examining whether change of imaging structure and quantitative evaluation of muscle activity on myofascial trigger points can lead to implementation of an analytical technique for evaluation of myofascial pain diagnoses. In addition, we examined the effect of a variety of mediation techniques, in order to examine neuromuscular physiological characteristics of myofascial trigger points muscle by comparing differences in pressure pain threshold and ultrasound imaging. Methods: Participants in the study included 30 adults in their twenties. The subjects were divided into the normal and myofascial trigger points groups. Clinical outcomes were evaluated by pressure pain threshold for pain and ultrasound imaging was performed for evaluation of the structural characteristics of muscle. Independent t-test was used for statistical analysis. Results: The two groups showed statistical significance in the change in pressure pain threshold (p<0.05). Findings of ultrasound imaging analysis showed no significant differences, increased muscle thickness was observed (p>0.05). Findings of ultrasound imaging analysis showed significant differences, increased muscle echodensity was observed (p<0.05). Findings on ultrasound imaging analysis showed significant differences, increased muscle white area index was observed (p<0.05). Conclusion: From these results, active myofascial trigger points muscle showed quality deterioration on ultrasound imaging. Thorough evaluation of imaging structure and physiological characteristics can be useful quantitative analytical techniques for diagnosis of myofascial pain syndrome and a primary factor reflected in physical therapy intervention.

        척수 손상 환자에 관한 실태 조사

        The incidence of spinal cord injury increase due to traffic accident, industrial accident and leisure sports. Spinal cord injury damages motor and sensory function below the injury level, also affects autonomic functions associated with voiding and defecation. Sexual dysfunction and psychosocial, vocational maladaptations are also some of the unwanted consequences of injury. The purpose of this study is look for means to prevent and to manage complications in spinal cord injury through investigation and analysis. The subjects of this study in spinal cord injured patients were admitted to the department of physical therapy, Kwangju christian Hospital, Nam Kwang Hospital, Chun Nam university Hospital and Cho Sun university Hospital, from April, 1, 1995 to March, 31, 1996. The results are as follows: 1. The subjects comprised 96 cases of spinal cord injury, ranging from 17 to 85(mean-40.8 yrs) and included 72 males and 24 females. Among these patients, 58 were cervical injury, 20 were thoracic injury and 18 were lumbar injury. 2. As for a major causative of spinal cord injury were traffic accident(59.4%), fall down (27.1 %), and motocycle(4.2%).. 3. The bladder control were taken by indwelling cathetar(41.7%), Crede maneuver(37.5%) and self voiding(16.7%). The bowel control were taken by all aid(61.5%), assitance(32.3%) and self defecation(6.2%). 4. Possible of sexual function were 35 cases (47.9%). 5. The device of transfer used wheel chair(69.8%) and bed(16.7%). 6. The patients with higher cord lesion got more serious pain than lower cord lesion. Also the patients with higher cord lesion got a serious spasticity. 7. The incidence of decubitus ulcer among 96 patients were in case 46(47.9%). The largest group of the pressure sore sites were sacral portion(82.0%), less than 1 month of onset occured a large numbers(50%). Incidence of pressure sore by spasticity occured many patients in case of mild or moderate. Incidence of pressure sore by pain occured many patients in case of severe pain.

        소아에서 발생한 삼차신경초종

        Trigeminal schwannoma is an uncommon intracranial tumor and constitutes less than 0.08-8% of all intracranial schwannoma. Trigeminal shchwannoma is even rarer in childhood and only 11 cases of trigeminal schwannoma under the age of 14 have been reported including our case. We experienced a case of trigeminal schwannoma in a 9 year-old girl without the stigmata of neurofibromatosis. She presented with a 2 week-history of right abducens nerve palsy. Magnetic resonance image demonstrated a dumbbell-shaped tumor extending into right middle and posterior fossas with a size of $60{\times}45{\times}35mm$. The tumor was successfully removed by right transzygomatic subtemporal extradural approach. Pathologic examination of the mass revealed typical features of schwannoma.

        재활원의 물리치료대상 뇌성마비아의 임상적 고찰

        The prognosis and result of treatment are influenced by associated cerebral handicaps. 38 cerebral palsied children in rehabilitation center were analized and neuromotor type severity associated defects, and their correlation were studied. The results were follows: 1. neuromotor type distribution was 65.78% of spasticity, 21.05% of athetoid, 2.63% of Ataxia, Tremor, and Rigidity, 5.26% of mixed type. 2. Mean values of I.Q, S.Q and M.Q were 84.3, 63.0 and 26.6 respectively, and percentages of strabismus, speech disturbance, shoulder abnormality, spine abnormality and hip abnormality were 26.3%, 81.5%, 5.2%, 52.6% and 15.9% respectively. 3. Significant differences were noted between each group of mild, moderate, and severe in S. Q, M. Q, and spine abnormality and between moderate and severe group in speech disturbance.

        항에스트로겐이 성체 수컷 마우스 수출소관의 형태적 변화에 미치는 효과

        본 연구는 수컷 생식기관내 에스트로겐 작용을 구명하기 위한 과정의 일환으로 항에스트로겐 물질인 ICI 182,780을 투여하고, 이로 인한 수출소관의 형태적 변화를 관찰하였다. 대조군과 처리군 간에 수출소관의 관강 직경은 처리후 8일째에 팽창하기 시작하여 59일째까지 지속되었고, 처리군 마우스의 수출소관내의 상피세포 높이는 대조군과 비교해 볼 때, 처리 후 경과기간 내내 감소되었다. 사이세포 높이가 최대로 감소된 곳은 처리된 마우스 수출소관의 근위부로 38% 감소하였으며, 수출소관 상피세포내의 핵상 세포질의 높이는 처리군 마우스의 근위부, 원추부, 원위부에서 각각 46%, 39%, 33% 감소된 것으로 나타났다. 항에스트로겐, ICI 182,780을 수컷 마우스에 투여한 본 실험에서, ${\alpha}ERKO$ 유전자 변형 동물에서 관찰되었던 것과 유사한 생식기관의 형태적 변화가 나타났다. 즉 항에스트로겐이 에스트로겐 수용체와 결합하여 에스트로겐의 기능을 차단하고 이로 인해 수출관의 관강 분비물 재흡수가 억제되어 분비물이 축적됨으로써 다양한 형태적 변환가 초래된 것으로 사료된다. 따라서 수컷 생식기관내의 중요한 에스트로겐 기능중의 하나는 수출소관 부위의 분비물 재흡수 작용이라고 할 수 있다. This study was designed to investigate the function of estrogen in related with the effects of the antiestrogen on morphologic changes of efferent ductules of the adult male mouse. Estrogen is synthesized in the male reproductive system and is found in high concentrations in rete and seminal fluids. Adult male mice, 30 days old, were injected subcutaneously with ICI(5mg/mouse) once per week for 8 weeks. Tissues were fixed by vascular perfusion on days 8, 10, 12, 14, 16, 25 and 59 post-treatment. The lumen of efferent ductules began to dilate from the day 8 and continued to the day 59 post-treatment. Epithelial cell height in the efferent ductules of the treated mice decreased all the time periods, compared to the control. Cell height in proximal region of the efferent ductules in the treated groups decreased most by 38%. Supranuclear cytoplasmic height in epithelial cell of the ducts also showed the decrease of 46%, 39% and 33% in proximal, conus and common regions in the treated groups, respectively. ICI 182,780 treatment in the male mouse reproduced a similar morphological abnormalities that were observed in the efferent ductules of the ${\alpha}ERKO$ mouse.

        전자기록 환경에서의 기능평가 프로세스 분석

        본고에서는 기록물의 내용이 아닌 기록이 생성된 기능에 기반을 두고 행해지는 전자기록 환경에서의 기능평가 프로세스를 분석하였다. 전자기록 환경에서 기능평가는 가장 보편화된 평가방식으로, 기록물의 생산 이전단계부터 수행되는 연속적인 업무 프로세스를 형성하게 된다. 종국적으로 기능평가를 기반으로 하는 전자기록 환경에서의 평가는 업무맥락 및 기록품질을 지님과 동시에, 조직의 영위 및 업무 수행에 필수적인 전자기록을 선별해 주게 된다. This article minutely examines functional appraisal process that focuses on not content but function in which the records are created. Taking everything into consideration, functional appraisal in electronic record environments constitutes consecutive process that is performed organically in pre-creation stage of records. Ultimately functional appraisal in electronic record environments is organic process selecting electronic records across phases of whole record management, which assure business context and recordness, and need to run organization and perform business.
