        20대 여대생을 대상으로 한 치아우식활성검사 연구

        This study surveyed salivary flow rate, salivary viscosity, and salivary buffering capacity in order to intensively analyze salivary factors among factors of occurrence in dental caries for finding mutually different factors that function in occurrence of dental caries depending on each individual. Even the acid body within dental plaque has great influence upon dental caries. Thus, the comparative analysis was carried out by surveying the hydrogen ion concentration in dental plaque. The following results were obtained in this study. 1. The average decayed teeth in the survey subjects stood at 1.67 piece. The extracted teeth caused by dental caries stood at 0.47 piece. The filled teeth were indicated to be 6.31 pieces. Accordingly, the average permanent dental caries experience teeth were surveyed to be 8.44 pieces. 2. The results according to dental caries activity test method were indicated to be 12.56±4.15ml for the average stimulated salivary flow rate, 3.89±1.83ml for non-stimulated salivary flow rate, 1.49±0.69 for salivary viscosity, and 8.51±2.44 for salivary buffering capacity. The hydrogen ion concentration test in dental plaque was indicated to be 5.62±0.50 for before brushing teeth, 5.23±0.58 for 5 minutes after brushing teeth, 5.25±0.56 for 10 minutes after brushing teeth, 5.29±0.62 for 15 minutes after brushing teeth, 5.34±0.58 for 20 minutes after brushing teeth, 5.40±0.53 for 25 minutes after brushing teeth, and 5.61±0.59 for 30 minutes after brushing teeth. 3. Stimulated salivary and non-stimulated salivary flow rate,salivary viscosity, and salivary buffering capacity were indicated to be higher in group with non-caries than group with caries. However, it was statistically insignificant. The hydrogen ion concentration in dental plaque showed wholly statistical significant in the relationship with people with dental caries under progression. However, people without dental caries were indicated to be higher than people with dental caries. 4. As for correlation between caries activity test methods, the stimulated salivary flow rate had significantly positive correlation with non-stimulated salivary flow rate(p<0.001). Non-stimulated salivary flow rate showed negative correlation with salivary buffering capacity(p<0.01). The hydrogen ion concentration test in dental plaque showed positive correlation according to the passage of time after brushing teeth. However, there was no significant correlation with salivary viscosity and salivary buffering capacity(p>0.05).

        Salivary Parameters of Buffer Capacity, pH Saliva and pH Plaque Related to Dental Caries Activity in School Student

        Objective: Dental caries is a major worldwide oral disease problem in children. Although caries are known to be influenced by dietary factors, the disease results from a bacterial infection; thus, caries susceptibility may be affected by environment factors such as saliva. This study aimed to determine a possible correlation between caries prevalence and caries risk markers that important factors for caries development in school children age 7-8 year in 5 areas of DKI Jakarta. Methods: The study was conducted as a cross-sectional study which data were collected from 500 school children in DKI Jakarta age 7-8 years old who were received dental examination including dental caries was used score DMF-S, pH saliva, pH plaque and buffer capacity.The screening starts with simple clinical observations, expanding to a diversified pattern of tests to assess the un-stimulated buffer capacity, pH saliva and pH plaque. Results: The results show that the Mean±SD score DMF-Sof school children was 0.193±0.4015. While Mean±SD buffer capacity of un-stimulated saliva was 4.763±2.1628, the pH un-stimulated saliva 6.830±0.4991, the other result shows that pH plaque was 6.246±0.4094. There was significant correlation (R=0.229, p<0.0001) between score DMF-Sand buffer capacity, pH saliva and pH plaque. Conclusion: The findings suggest that the level of buffer capacity, pH saliva, and pH plaque are the importance of screening for caries risk markers.

        구강 프로바이오틱스와 에센셜오일 함유 양치액의 구강환경변화

        Purpose : The purpose of this study was to compare changes of salivary flow rate, salivary buffering capacity, and S. mutans after using a mouthwash containing essential oil or probiotics widely used as oral lactic acid bacteria in the oral environment. Methods : Fifty-three healthy adults aged 20 years or older voluntarily participated in this study after they were informed of the purpose of this study. Subjects were divided into a group treated with probiotics (L. reuteri) and a group treated with mouthwash containing essential oil. To evaluate changes in the oral environment, salivary flow rate, salivary buffering capacity, and S. mutans test were performed at baseline and 4 weeks later. Unstimulated and stimulated salivary flow samples were collected for 5 minutes and results were recorded in ml per minute. Salivary buffering capacity was divided into scores of 1 (low), 2 (moderate) and 3 (high) according to the color of the strip. The amount of S. mutans was classified as scores of 0, 1, 2, and 3 according to the decision table. Data collected in this study were analyzed at the 95 % significance level using the SPSS Version 23.0 program. Results : Both the unstimulated salivary flow rate and the stimulated salivary flow rate increased statistically after the intervention compared to those before intervention in the probiotics group. However, there was no statistically significant difference between the probiotics group and the mouthwash group. Salivary buffering capacity and S. mutans results showed no statistically significant differences between the probiotics group and the mouthwash group either. The salivary buffering capacity and the salivary flow rate showed a positive correlation. Conclusion : To improve the oral environment, such as salivary flow rate and salivary buffering capacity, an optimal product can be selected in consideration of individual preference and cost-effectiveness.

        다운증후군 환자의 치아우식증과 치아우식 원인균 및 타액내 면역항체의 연관성

        다운증후군 환자의 치아우식증과 구강내 여러 요인들과의 연관성을 평가하고자 9-18세의 다운증후군 환자 25명과 정상인 63명을 대상으로 치아우식경험지수, 치태지수를 조사하고 효소면역분석법을 이용하여 타액의 면역항체 IgA와 뮤탄스 연쇄상구균-특이 IgA 농도를 측정하고, Dentocult 키트를 이용하여 뮤탄스 연쇄상구균과 유산간균의 수, 타액의 완충능을 측정하여 두 군의 각 값들을 독립 t-test와 카이제곱 검정을 시행하였다. 다운증후군 환자에서 유치우식경험은 정상인에 비해 유의하게 낮았으나(p<0.001), 영구치우식경험은 유의한 차이를 보이지 않았다. 치태지수와 타액의 IgA의 농도는 두 군 간에 차이를 보이지 않았으며, 뮤탄스 연쇄상구균 특이-IgA 농도와 타액 완충능은 다운증후군환자에서 낮았다(p<0.05), 뮤탄스 연쇄상구균 수와 유산간균 수는 두 군간에 유의한 차이는 없었다. 9-11세 군에서 뮤탄스 연쇄상구균의 수는 다운증후군 환자에서 정상인보다 낮았으며(p<0.001), 뮤탄스 연쇄상구균 특이-IgA는 다운증후군 환자에서 유의하게 낮았다(p<0.05), 유치의 치아우식경험치면지수는 완충능, 뮤탄스 연쇄상구균의 수와 유의한 상관관계를 보였다. This study investigated the relationship between dental caries and various oral factors in subjects with Down' s syndrome. We compared 25 subjects with Down's syndrome with 63 healthy control. The dental caries index and plaque index were examined, and the total salivary immunoglobulin A and S. mutans specific salivary immunoglobulin A concentration were measured using ELISA. The S. mutans counts, Lactobacillus counts and buffer capacity were measured with Dentocult test medium. The decayed and filled surface index of deciduous teeth in subjects with Down' s syndrome was lower than in controls(p<0.001). The plaque index and total salivary immunoglobulin A concentration showed no difference, S. mutans specific salivary immunoglobulin A concentration and buffer capacity in subjects with Down' s syndrome were lower than in controls(p<0.001). There was no significant difference between two groups in the S. mutans counts and Lactobacillus counts. In 9-11 year age group, S. mutans counts in subjects with Down' s syndrome was lower than in controls(p(0.001) and S. mutans specific salivary immunoglobulin A concentration was lower(p(0.05). There was a high correlation among deciduous dental caries index and buffering capacity and S. mutans counts.

        생리적 구취환자의 타액요인에 영향을 미치는 융복합적 공통요인에 관한 통계적 분석 연구

        저자들은 2008년부터 2016년까지 고대의료원 구로병원 예방치과 구취클리닉에 내원한 구취 환자 중, 생리적 구취증 환자 171명에 대한 진료기록을 통하여 구취에 영향을 줄 수 있다고 인정되는 11개 독립변수들의 자료를 수집하였다. 이들 11개 독립변수들과 3개 종속변수들 중, 생리적 구취 발생에 융복합적 영향을 미치는 공통요인을 추출하여, 향후 구취 예방을 위한 구강보건교육자료 제작 등에 활용할 목적으로 본 연구를 시작하였다. 생리적 구취 발생과 밀접한 연관성이 있다고 알 려진 안정 시 타액량, 타액 완충능, 타액 침전율의 3개 종속변수에 11개 독립변수들이 미치는 영향력을 다중회귀분석과 경로분석을 이용하여 분석하였고, 각 변수들의 기술통계적인 결과 분석과 함께, 3개 종속변수에 통계적으로 유의하게 직접, 간접적으로 영향을 미치는 공통적인 융복합적 독립변수 요인들을 추출한 결과, 성별, 식음규칙성, 성격, 조식섭식습관의 네 가지 요인이 추출되었다. The data were collected from 171 physiologic malodor patients diagnosed in KUMC halitosis control clinic between 2008 and 2016. We selected 11 independent variables and 3 dependent variables, then planned to extract some convergent common factors affecting their physiologic malodor. We thought that those extracted convergent common factors could be utilized when preparing the contents of oral malodor preventive program. We used multiple regression analysis and path analysis method, for the analysis of influence of 11 independent factors to three salivary dependent factors(resting salivary flow rate, salivary buffering capacity, salivary precipitation rate). We have presented the physiologic malodor patients’ chracteristics by descriptive statistical analysis, and also statistically analysed convergent common factors influencing directly or indirectly to their three dependent factors. We could reason that the sex, the character, the intake habit of breakfast, and the regular food intake habit could affect resting salivary flow rate, salivary buffering capacity and salivary precipitation rate.

        Caries-related Microbiological Screening in Children under Three Years of Age

        3세 이하의 어린이에서 구강 내 미생물과 타액완충능이 치아우식증과의 관련성을 알아보았다. 87명의 어린이를 실험대상으로 하여 치태와 자극성타액을 각각 면봉과 면구를 이용하여 채취하였다. 0.94%의 lactic acid를 타액 표본에 첨가하기 전과 후의 pH를 각각 측정하였고 타액표본을 순차적으로 희석하여 선택배지와 비선택배지에 접종하였다. Mutans streptococci (MS), lactobacilli (LB), total viable count (TVC)의 집락수와 타액의 pH와 완충능을 어린이의 치아우식과 비교하여 다음과 같은 결과를 얻었다. 1. 자극성타액과 치태 모두에서 MS 와 LB 가 치아우식증과 높은 통계학적 유의성을 보였다. 2. 치아우식의 발생을 예측할 수 있는 미생물의 집락수는 다음과 같았다. 1) 자극성타액 1 ml 당 MS 집락수가 10^5 이상 2) 치태 1 ml 당 MS 집락수가 2×10^5 이상 3) 자극성타액 1 ml 당 LB 집락수가 10^3 이상 4) 치태 1 ml 당 LB 집락수가 10^3 이상 3. 타액의 pH와 완충능은 유아기우식증과 관련이 없었다. 4. MS 검사는 LB 검사 보다 높은 예측치 (predictive value)와 교차비 (odds ratio)를 보였다. 5. MS 집락수는 어린이에 있어서 치아우식 발생을 예측할 수 있는 가장 믿을만한 미생물학적 검사로 일반적인 세균배양법을 이용하거나 상품화된 검사기구를 이용하여 쉽게 임상에 적용할 수 있다. To evaluate microbial data and salivary measurements from clinically compatible, culture-based screening procedures employed with children younger than 36 months old. Plaque and stimulated saliva specimens were collected from 87 children. The pH of each saliva sample was measured before and after 0.94% lactic acid was added. Specimens were diluted and plated on selective media and non-selective media. Data collected were counts of mutans streptococci (MS) and lactobacilli (LB). In addition, total viable counts (TVC) of specimens, salivary pH and buffering capacity were also assessed. Each variable was compared to caries status of subjects. According to this study, the results were as followed: 1. Highly significant correlation with caries rates were found for counts of MS and LB. 2. The specific counts/ml saliva of plaque above which caries is predicted, or below which caries is not predicted were as follows: 1) Saliva MS; 10^5 2) Plaque MS; 2×10^5 3) Saliva LB; 10^3 4) Plaque LB; 10^3 3. Salivary pH and buffering capacity versus caries status were significant. 4. Microbial screening methods based on mutans streptococci had higher predictive values and odds ratios than methods for lactobacilli. 5. MS counts were clearly the best indicators of caries status in young children. This measurement can easily be obtained in a dental clinical setting both by conventional culture techniques, or commercial kits for MS recovery.

        신경모세포종의 화학 및 방사선요법이 치아우식활성도에 미치는 영향

        소아암 환자의 치료 방법으로 주로 사용되는 항암화학요법이나 방사선요법이 구강 건강에 미치는 영향은 매우 크다. 구강 점막염, 타액 분비율의 변화, 구강내 감염을 야기할 수 있고, 부분무치증, 왜소치, 치근 발육이상, 저석회화증이 나타날 수 있다. 본 연구는 신경모세포종으로 치료 받은 환아들에 있어 항암치료가 치아우식증 발생 위험도에 어떠한 영향을 미치는지 알아보기 위해 실시되었다. 삼성서울병원 소아청소년과에 신경모세포종으로 내원하여 화학 및 방사선치료요법을 받은 환아들을 대상으로 Dentocult.. SM, Dentobuff.. Strip을 이용하여 항암치료 후의 타액 분비율, 타액 완충능, 타액 내의 Streptococcus mutans 수를 알아보고 정상 어린이들의 결과와 비교하였으며, 신경모세포종 환아에서 처음 치료를 시작한 나이, 치료 종결 후 경과 시간에 따라 치아우식활성도가 어떻게 달라지는지에 대해서도 알아보았다. 그 결과 타액 분비율은 환아가 대조군에 비해 유의성 있게 낮은 결과를 보인 반면(p<0.05), 타액 완충능과 Streptococcus mutans 수는 환아와 대조군 간에 유의성 있는 차이를 보이지 않았다. 환아에서 치료를 시작한 나이나 치료 종결 후 경과 시간에 따른 치아우식활성도에서도 통계적 유의차는 없었다. Chemotherapy or radiotherapy used for the treatment of pediatric cancer may have many adverse effects on the oral cavity. Oral mucositis, reduced salivary flow, oral infection, hypodontia, microdontia, arrested root development, and enamel hypoplasia are common oral complications. The aim of this study is to evaluate the effects of cancer therapy on dental caries activities. The children who had been treated for neuroblastoma in the department of pediatrics, Samsung Medical Center, were included and healthy children served as controls. The salivary flow rate, salivary buffering capacity, and Streptococcus mutans counts of both groups were evaluated using Dentocult SM and Dentobuff Strip. The dental caries activity related to the age at the start of treatment and the time elapsed since treatment completion were also evaluated. As a result, neuroblastoma patients had significantly lower salivary flow rate than the controls, while there were no significant differences between two groups as for salivary buffering capacity and Streptococcus mutans counts. The dental caries activities related to the age at the start of treatment and the time elapsed since treatment completion were not significantly different.

        치위생과 학생의 치아우식경험도와 개량형 우식활성검사와의 관련성

        This study was implemented for 84 students of dental hygiene to show the correlation between dental caries experience and improved caries activity test. Dental caries experience for the sample groups was examined and stimulative saliva secreted for 5 minutes was collected into the tube to check saliva secretion rate. Dentocult LB test was executed to observe Lactobacilli colonies after 96 hour cultivation of culture slides moistened with stimulative saliva. Dentocult SM test(screening strip, site strip) was done to measure SM colonies distribution after 48 hour cultivation of culture strips applied with collected saliva and dental plaque respectively, and salivary buffering capacity was checked by means of Dentobuff strip kit. Following conclusions are obtained after examining the relation between Dentocult LB, Dentocult SM, Dentobuff strip test results and DMFT index, salivary secretion rate. 1. Showed no significant difference between Dentocult LB test results and DMFT index, salivary secretion rate. 2. Showed no significant difference between Dentocult SM(screening strip) test results and DMFT index, salivary secretion rate. 3. Showed significant difference between Dentocult SM (site strip) test results and DMFT index(pE0.05), but showed no significant difference between Dentocult SM(site strip) test results and salivary secretion rate. 4. Showed no significant difference between Dentobuff strip test results and DMFT index, but showed a very wide difference between Dentobuff strip test results and salivary secretion rate(pE0.01).

        녹차음용이 아동의 치아우식발생에 미치는 영향

        연구목적. 우리나라 국민의 우식발생을 억제시킬 목적으로 녹차폴리페놀의 우식예방효과를 구강내우식원세균수 변화와 구강내완충능 변화를 통해 간접적으로 관찰하여, 녹차의 우식예방효과를 측정하고자 한다. 연구방법. 우식발생가능성이 높은 연령대의 집단을 대상으로 녹차가 아동의 치아우식 발생에 미치는 영향을 조사하기 위하여서울시에 소재하는 초등학교 4학년 254명을 연구대상으로 하여 실험군(n=131)에게 아침 등교 후와 점심식사 후 매일 2회 신선한 녹차를 12개월 동안 마시도록 하였고 대조군(n=97)은 녹차를 마시지 않도록 하였다. 세계보건기구의 기준에 따라 연구시작시점과 6, 12개월 후의 우식경험영구치아를 조사 하였으며, 연구대상자의 자극성타액을 수집하여 구강 내 S. mutans수와 Lactobacillus 수 및 타액완충능의 변화를 비교 검토하였다. 연구결과. 우식경험영구치아수는 녹차비음용군보다 녹차음용군에서 둔화되었고(p=0.8065) S. mutans는 녹차음용군에서 녹차비음용군보다 감소하였다(p=0.0031). Lactobacillus는 녹차음용군과 녹차비음용군 간에 유의적으로 감소하였으며(p=0.0006), 타액완충능은 녹차음용군에서 녹차비음용군보다 증가하였다(p<0.0001). 결론. 녹차 음용에 의해 구강 내 S. mutans수의 증가가 억제되고 타액 완충능이 증가 되는 양상을 보이므로, 우식발생 가능성이 높은 집단에서의 녹차음용법은 구강건강을 증진시키는 모델로 활용될 수 있을 것으로 검토되었다.
