        Analysis of Trend of Studies on Microneedle Treatment System (MTS)

        Objectives: The purpose of this study was to analyze the microneedle therapy system (MTS) and its research methods for the past 10 years in Korea. Methods: Data on microneedle therapy system were collected using NDSL, KISS, RISS, and OASIS electronic databases from January 2010 to August 2021. “microneedle,” “derma stamp,” “microneedle therapy system” were used as the keywords. The present study, however, excluded data that were 1) unrelated to the microneedle therapy system, 2) from review/meta/protocol studies, and 3) from overseas studies. Data selected through the primary screening process, animal studies, case reports, and clinical data were included in the analysis. However, information data not related to the microneedle therapy system were excluded from the study. Results: Among the MTS-related papers published from January 2010 to August 2021, 7 animal research, 2 clinical trials, and 10 case studies were published. Based on the research topics, there were 8 papers on skin improvement and skin diseases, 7 papers on hair growth and hair loss, 3 papers on stability, and 1 paper on peripheral facial paralysis. Conclusion: Most of the studies related to MTS focused on skin, hair, and stability. The effect of MTS on hair growth and skin improvement has been confirmed, and it has been proven to have significant effects on the treatment of acne, acne scars, and hair loss in clinical practice. No serious side effects were observed during the MTS treatment, and the safety assessment confirmed that it was safe for use.

        흉터복원술과 자동 미세침(Auto Microneedle Therapy System, AMTS)을 이용한 여드름 흉터의 치험례

        Objective : Acne is very common disease, it can leave permanent scars. However, there are few reports about it in oriental medicine academia. This study performed to evaluate the effect of reconstruction of skin scars and auto microneedle therapy system on acne scars. Methods : Six patients were treated by using reconstruction of skin scars and auto microneedle therapy system. We compared the degree of treatment of acne scars on pre and post case photos and Qualitative Global Acne Scarring Grading System(QGASC). Satisfaction degree were asked from the patients. Results & Conclusion : After comparing case photos and QGASC, observing patients satisfaction, it can be considered that reconstruction of skin scars and auto microneedle therapy system to be valid acne scars treatment.

        자동 미세침이 말초성 안면마비 환자의 증상 정도 및 안전성에 미치는 영향에 대한 임상 관찰

        Objectives : This study was to observe clinical application of auto-microneedle therapy system (AMTS) in patients with peripheral facial paralysis. Methods : 27 peripheral facial paralysis patients were observed after taking AMTS at Facial Palsy Center, Kyung Hee University Oriental Medicine Hospital from March 1, 2011 to January 9, 2012. We assessed the symptoms of facial paralysis with Yanagihara unweighted grading system, Sunnybrook facial grading system(SBGS) and facial disability index(FDI), and observed adverse events and total safety of the treatment. Results : The scores of facial palsy scales increased after AMTS in Yanagihara grading system and Sunnybrook facial grading system. AMTS-related adverse events were mild pain(5.9%) and fatigue(3.5%), which needed no extra treatment. The total safety evaluation was between ‘safe’ and ‘nearly safe’ level. There were no other serious adverse events. In addition, patients were satisfied with subjective improvement including facial tingling and numbness. Conclusions : AMTS can be applied as an adjunctive treatment for patients with peripheral facial paralysis due to its safety and clinical usefulness. It is easier to stimulate wide skin area in a short time. Further clinical research is required to investigate the effectiveness of ATMS in a more rigorous RCTs.

        약물 전달 시스템의 네트워크 연구: 마이크로니들과 피부

        Drug can be administered through oral. parenteral, transdermal, nasal, and pulmonary route, but each have its owndisadvantage. The drug delivery system(DDS) is an advanced technology that effectively delivers therapeutic drugs to desiredtargets and thereby reduces side effects and increase efficacy. Microneedle therapy system(MTS) is advancing in the field ofmedicine and skin care which can improve drug diffusion into skin with no pain. In this study, we introduce the basicconcepts of DDS and MTS including the structure and function of skin, so that researchers may benefit in their attentionon the production of microneedle and promoting research on the area.

        절개침과 미세피부침, 약초필링을 이용한 위축성 여드름 흉터 치료 7례 임상보고

        Objectives : The purpose of this study is to present the effect of subcision, microneedle therapy system and herbal peeling therapy which is treatment of Korean medicine to atrophic acne scar patients. Methods : We administered subcision, microneedle therapy system and herbal peeling therapy to atrophic acne scar patients and tested the results by Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Results : After the treatment, all of the patients showed improvements in Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Qualitative Global Acne Scarring Grading System of patients changed from grade 2.71±0.71 to grade 1.14±0.86 along with the change of Korean Acne Grading System from grade 1.57±0.57 to grade 1.00±0.00. Conclusions : Subcision, microneedle therapy system and herbal peeling therapy may be effective in the treatment of moderate to severe acne scars, and could be applied in clinical practice.

      • Skin Effect and Mechanism Using Microneedle Therapy System (MTS)

        This study is about microneedle therapy system (MTS)`s skin effect and mechanism. Microneedle makes micro-hole and the drug is penetrated easily and they induce natural skin rejuvenation by various cytokines and growth factors. As the mechanical and pharmaco-chemical factors stimulate the specific functional units in the skin, MTS takes effect in the tissue. There are 4 specific functional units: fundamental competence unit, immune competence unit, microcirculatory competence unit and neurovegetative competence unit. As the microneedle and drug`s reaction are combined, the drug is delivered to the skin tissues and these skin tissues are improved by activated receptor.

        Microneedle Therapy System (MTS) 자극에 의해 유발된 얼굴 홍조 와 부종에 대한 림프마사지의 완화효과

        In this study, the facial flushing and edema symptoms caused by Microneedle Therapy System (MTS) were observed, and the calming effect of Manual Lymph Drainage (MLD) on the flushing and the edema were quantitatively evaluated. The flushing and edema symptoms were remarkably increased just after the MTS stimulus, and reached its peak at 30 min. The facial swelling was occurred about 1.5 to 3.6% and the buccal region among the face was sensitive to the stimulus. In control group taken a rest only after MTS, the flushing symptoms were slowly become calm down, but there is little change in the edema. On the other hand, in the experimental group received the MLD treatment, the flushing and the edema symptoms were quickly reduced. The number and area of the hot flashes were reduced 63% and 79%, respectively, and the swelling face size was more reduced about 0.8~1.6% than the size before MTS stimulus. In conclusion, the MLD is one of the most promising methods to reduce the MTS- induced flushing and swelling symptoms.

        자동미세침치료기(Automicroneedle Therapy System, AMTS)의 진피증식 효과 및 피부 안전성 평가

        Background: Various topical cosmeceuticals and lasers have been used to treat photo-aged skin that has wrinkles, acne scars and dilated pores. The microneedle therapy system (MTS) that mechanically makes multiple holes on the skin has come into the limelight to treat these skin problems via stimulating collagen remodeling. The automicroneedle therapy system (AMTS) is a developed version of MTS and it has several advantages compared with conventional MTS. AMTS can achieve regular treatment results because of its automatically punching method. In addition, AMTS can treat smaller area and it has cost advantages due to the inexpensive disposable needle head. Objective: This study was designed to determine the dermal proliferative effects and safety of AMTS on the skin compared with that of the conventional MTS roller. Methods: Twelve hairless mice were divided into two groups; one group was treated with a 0.25 mm needle and the other group was treated with a 2 mm needle. The first group was subdivided into the AMTS-H and the MTS groups and the no treatment group as a control, while the second group was subdivided into the AMTS-H, AMTS, MTS and control groups. Each treated group underwent four procedures every other day. The dermal proliferative efficacies of the treatment were evaluated by the histology, including the dermal thickness and the densities of the collagen fibers. Western blot was also performed for the evaluation of the protein expression of procollagen type I and matrix metalloproteinase-13. For safety profiles, we performed gross observation, basal skin barrier function testing and histologic examination. Results: Treatment by AMTS significantly increased dermal collagen synthesis and the dermal thickness in the hairless mice. In addition, the expression of procollagen type I protein was increased, which accounted for the increased dermal collagen density. There was no specific safety problem related to the treatment. Conclusion: These results indicated that AMTS is an effective, safe modality for treating skin problems that require dermal proliferation. We anticipate that AMTS could be a new therapeutic option for inducing dermal proliferation or regeneration. (Korean J Dermatol 2010;48(11):955∼965)

        C57BL/6N 마우스 탈모모델에서 미세다륜침(MTS)과 황련해독탕 약침액 도포가 모발성장에 미치는 영향

        Objective : This study was carried out to investigate the effect of Hwangryeonhaedoktang pharmacopuncture solution (HRHDT) and microneedle therapy system (MTS) on hair growth in an alopecia model of C57BL/6N mice.Methods : Six-week old mice were depilated and separated in 4 groups ; CON (saline), MXD (3% Minoxidil), MTS and HRHDT+MTS. The treatments were applied twice a week for 17 days. The hair growth was determined photographically. The hair density, thickness and length were identified by Folliscope and the weights of body and organs were measured. In dorsal skin tissue, the expression of hair growth-related gene and protein was analyzed by RT-PCR. In addition, the hair follicles in the dermis were observed by H&E staining.Results : The promotion of hair growth was observed in HRHDT+MTS and MTS compared to CON. The hair density, thickness and length were also improved in HRHDT+MTS and MTS compared to CON. The mRNA expression of IGF-1, PRL and PL and the protein expression of VEGF and IGF-1 were increased in HRHDT+MTS and MTS compared to CON. The hair follicles and hair root growth were improved in HRHDT+MTS and MTS compared to CON. In the above results, HRHDT+MTS were more effective than MTS.Conclusions : These results suggest that HRHDT and MTS have a hair growth activity and can be useful for the treatment of alopecia.
