치조열 환자에 있어서 이차 골성형술 후의 예후에 관한 임상적 연구

Many kinds of anomalies have emerged in oral and maxillofacial area. Cleft lip and palate is one of the congenital anomalies of this area. Cleft alveolus appears in 75% of total cleft palate cases and various opinions are suggested for the correction of cleft alveolus. Treatment goal of the cleft alveolus is the restoration of normal function of the hard tissue, hence the bone graft is the essential procedure. But the operation timing is a matter of dispute. Primary bone graft is performed before the completion of the primary dentition, and the secondary bone graft is performed after the completion of the primary dentition. We have performed the secondary osteoplastic surgery for restoration of normal tooth-bearing function, for establishment of esthetic occlusion, and as a prerequisite to orthognatihic surgery in order to correct the undergrowth of the maxilla due to cleft palate and alveolus. The object of our study lies in the study of the alveolar bone change after secondary osteoplasty via the evaluation of bony bridge and the cleft side canine state after secondary bone graft into cleft alveolus. 29 patients between the age of 9 and 38 was the subject of this study and 21 cases were followed up. Follow up period is from immediate post-operative time to post-operative 20 months, of which the mean value is 7 months 10 days. The evaluation of bony bridge is based on the Abyholm criteria (1981), and the results were as follows : typeⅠ16, typeⅡ4, and typeⅢ1 cases, respectively. The canine state of follow up time were evaluated : Self erupting consists of 5 cases, and orthodontic guidance 4 cases of total 9 cases of unerupted canine at the time of osteoplasty. Erupting canine at the time of surgery were 5 cases.

The aim of this study was to investigate the prevalence of congenital missing, supernumerary teeth, and size, shape and rotation of the maxillary anterior teeth in patients with cleft lip and alveolus. Thirty- four individuals (male 13, female 21) with cleft lip and alveolus was selected. After look up the medical history, panorama X-ray and intraoral photos were investigated. The congenital missing teeth ratio was 26.5% (9/34) and all the missing tooth was maxillary lateral incisor. The ratio of supernumerary tooth was 17.6% (6/34) and 2 supernumerary tooth was mesial to the cleft and 4 was distal to the cleft. The ratio of small tooth was 67.6% (23/34) and 8 (34.8%) was located mesial to the cleft, 15 (65.2%) was distal to the cleft. Rotated tooth was 64.7% (22/34) and all the tooth was maxillary central incisor of cleft side. The result suggested that maxillary process influenced more than median nasal process in formation of lateral incisor. This result suggested that it is necessary to study the embryologic relationship between cleft lip and alveolus and dental abnormalities.

The cleft alveolus is one of three parts in cleft deformity. The purpose of cleft alveolus bone grafting is the recovery of normal esthetics, occlusion and speech. If a bony defect is extended to the nasal floor, especially wide bony defect at the ala base, it is difficult to condense the cancellous bone during bone transplantation and to reconstruct the normal anatomy at the alar base. We treated with above mentioned cleft alveolus patients using the autogenous cortical bone effectively. We report this technique with two cases and the literatures review.

The purpose of this study was to evaluate the stress distribution developed in the supporting structures by mandibular distal extension removable partial dentures with 4 different precision attachments. The examined precision attachments were Mini SG attachment, CM box attachment. Mini Dalbo attachment, Technoroach attachment. Concentrated force(vertical force 200N, horizontal force 50N) was applied on the artificial 2nd molar of the removable partial dentures and distributed force(vertical force 100N, horizontal force 25N) was applied on the each artificial 1st, 2nd molars of removable partial dentures. Total 4 cases were compared and analyzed with 3-dimensional finite element method. And then, stress distribution patterns were analyzed and compared. The results were as follows. 1. In case of concentrated force; Maximal stress distribution was showed on the alveolus under artificial 2nd molar and then, stress mainly distributed on the residual alveolus of the denture base. 2. In case of distributed force; Maximal stress distribution was showed on the distal alveolar crest of distal abutment, and then, stress mainly distributed on the alveolus of mesial and distal abutments. 3. The stress of residual alveolus of non rigid attachment was greater than that of rigid attachment. 4. The stress of abutments of rigid attachment was greater than that of non rigid attachment.

Simultaneous implant installation with bone graft was performed in 15 cases. Four cases were cleft alveolus patients. 56 implants were placed immediately with block bone grafts. 2 csses were cranial bone grafts and the others were iliac bone grafts. Three of 56 implants were lost(94.6% survival rate). One of three was cleft alveolus case. The cleft alveolus patients with simultaneous implants installation showed functional and esthetic results without infraocclusion and positional changes. Bergland index was considered to be type I after 12 months later. Immediate implant installation with bone graft is one of choice of treatment in closing cleft alveolus hoping sitnultaneous implant installation could be related with function which might result in less resoiption of graft. Functional and esthetic results are satisfactoiy ; there was no infraocclusion and positional changes.

This case report describes a successful orthodontic treatment with orthognathic surgery of a 17-year-old girl with skeletal Class III and unilateral cleft lip and alveolus on left side. The treatment included extraction of the maxillary premolars and asymmetric mandibular setback surgery. To induce regeneration of buccal alveolar bone on upper left lateral incisor, the palatal positioned left lateral incisor was aligned. After orthodontic treatment with orthognathic surgery, the patient had an improved facial profile, improved alveolar bone appearance on the cleft alveolus area, and favorable occlusion.

Background The outcome of alveolar grafting with synthetic bone substitute (Osteon III) in various bone defect volumes is highlighted.Methods A prospective study was accomplished on 55 patients (6–13 years of age) with unilateral alveolar bone cleft. Osteon III, consisting of hydroxyapatite and tricalcium phosphate, is used to reconstruct the defect. Alveolus defect diameter was calculated before surgery (V1), after 3 months (V2), and finally after 6 months (V3) postsurgery. In the t-test, a significant difference and correlation between V1, V2, and V3 are stated. A p-value of 0.01 is considered a significant difference between parameters.Results The degree of cleft is divided into three categories: small (9 cases), medium (20 patients), and large (26 cases).The bone volume of the clefted site is divided into three steps: volume 1: (mean 18.1091 mm3); step 2: after 3 months, volume 2 resembles the amount of unhealed defect (mean 0.5109 mm3); and the final bone volume assessment is made after 6 months (22.5455 mm3). Both show statistically significant differences in bone volume formation.Conclusion An alloplastic bone substitute can also be used as a graft material because of its unlimited bone retrieval. Osteon III can be used to reconstruct the alveolar cleft smoothly and effectively.

Two cases of alveolar bone graft with autogenous cancellous bone from proximal tibial metaphysis was done. Satisfactory amount and quality of autogenous cancellous bone was obtained. Considering its overall lack of morbidity, the proximal tibial metaphysis is a useful donor site for small to moderate bone defect in oral & maxillofacial region.
