        Changes in Structure and Connectivity of Auditory Cortex in Patients with Unilateral Hearing Loss

        The purpose of this study was to investigate cortical structural and connectivity changes in the auditory system due to the absence of unilateral input signals from the hearing-impaired ear in patients with unilateral hearing loss. Twenty-seven patients who have severe to profound unilateral hearing loss participated. T-1 images were acquired to calculate the normalized cortcial volume (nCV), and seed-based connectivity analysis was performed. It was shown that the nCV of Te 3.0 was decreased in the left unilateral hearing loss (LUHL). In LUHL group, a significant structural correlation was observed between region of interests (ROIs) within the auditory cortices of bilateral hemispheres, while right unilateral hearing loss (RUHL) group showed significant correlation between thalamus and auditory cortical regions. We proposed that cortical structural reorganization in unilateral hearing loss patients, that is different by deafness side. These results are expected to be related to individual binaural hearing performance in unilateral hearing loss.

        수술 후 발생한 일측성 난청에서 OsiaⓇ2 골전도 임플란트를 이용한 청각재활

        Various types of bone conduction hearing aids have been widely used for hearing rehabilita-tion for the last 30 years. Among them, the recently launched Osia®2 system is a new activetranscutaneous bone conduction implant system using piezoelectric effect. This can be ex-pected to deliver more efficient sound transmission, overcome sound attenuation, and improvehigh-frequency hearing than conventional passive transcutaneous hearing aids, and is consid-ered to be cosmetically superior to percutaneous hearing aids. Recently, we experienced twocases of Osia®2 implantation in patients with iatrogenic unilateral hearing loss. Both patientsshowed improved pure tone threshold and better Korean Hearing in Noise Test (K-HINT) scoreafter implantation. Furthermore, all of them had no complications after Osia®2 implantation.

        일측성 고심도 난청 영아의 조기청능재활 사례 보고

        Purpose: The purpose of this paper was to report a result of early intervention for an infant with a congenital profound unilateral hearing loss in the right ear. Methods: The infant was identified as his hearing problem through newborn hearing screening test and diagnosed with unilateral hearing loss at 5 months of age. The early intervention program was applied from 9 months to 12 months of age to prompt listening and spoken language acquisition. Korean aural rehabilitation for infants and toddlers, the program was applied to the infant, focused on parent education as a mean of facilitating the child’s language acquisition. The mother and grandmother participated in the intervention. The program was carried out once per two weeks for 3 months, and each session lasted 90 minutes. The listening, speech, and language evaluations were conducted before and after applying early intervention for 3 months to check the progress on each area. Results: The infant scored within normal range across evaluations applied on both, but largely improved especially on the vocabulary and general language development screening test at the second evaluation, taken at the 12 months of age. Conclusion: The review for what should be considered for children with the unilateral hearing loss was discussed.

        Hearing Rehabilitation Using Bone Conduction Implants in Patients With Single-Sided Deafness After Vestibular Schwannoma Removal

        Background and Objectives Vestibular schwannoma (VS) itself or treatment of VS cancause single-sided deafness (SSD), a unilateral sensorineural hearing loss. Patients of SSD ex-perience communication problems, which can be frustrating. For decades, many types of boneconduction implants (BCIs) have been developed and used to treat such hearing problems. Inthis study, we evaluated the effectiveness of BAHD surgery to treat SSD patients’ hearing im-pairment and analysed the complications patients suffer. Subjects and Method A retrospective chart review was conducted on 12 patients who hadundergone BCIs, such as Baha Attract, Bonebridge, and Sophono implantation, after removalof VS. From 2016 to 2021, one senior surgeon at a single tertiary hospital performed surgeryfor SSD rehabilitation. The authors analysed the clinical features, radiologic findings (CT andMRI), hearing tests (pure tone audiogram [PTA], speech audiogram [SA], and Korea hearingin noise test), subjective satisfaction scores (abbreviated profile of hearing aid benefit), andsurgical outcomes of the patients. Results All patients underwent temporal MRI, temporal bone CT scan, PTA, and SA atleast 3 years after VS removal and confirmed no residual tumour or serviceable contralateralhearing at the last follow-up. Among the patients (12), eight received Baha Attract implanta-tion, three received Bonebridge implantation, and one received Sophono implantation. On theaverage, patients’ ipsilateral hearing outcomes improved by 82.7 dB (from 115.8±9.8 dB to33.1±9.5 dB). Conclusion There are many methods of hearing rehabilitation in SSD that are used afterVS, but BCI seemed to be the most useful treatment.

        2차 대학병원에서 일측 소아 청소년 난청의 정기 검사 현황

        Backgrounds and Objectives : The purpose of this study was to describe the follow up status and rehabilitation of Unilateral hearing loss(UHL) in children and adolescents of Korean secondary referral hospital. Subjects and Method : This was a retrospective study based on clinical data collected from time of diagnosis of UHL. The study included data since November 2009. Serial audiograms were extracted from the clinical charts and follow up status and rehabilitation decisions were analyzed. Results : Of 101 children and adolescents, they were identified at a mean age of 8.9±4.9 years. Up to 45%(n=46) of subjects went through regular checking up. About a quarter of subjects(n=25) started one side hearing aids and 60%(n=15) used it successfully. A re-checked 85 of 11.8%(n=10) showed deterioration in hearing, including four subjects who developed bilateral loss. Conclusion : The regular follow up rate and rehabilitation ratio seems lower compared to advanced country. Regular monitoring of hearing and communication between professionals and guardians is crucial in order to minimize the potential negative effects of UHL.

        Increased Risk of Psychopathological Abnormalities in Subjects With Unilateral Hearing Loss: A Cross-Sectional Study

        Objectives. Although unilateral hearing loss (UHL) has been proven to be associated with educational and behavioral problems, few studies have investigated psychopathological abnormalities in this population. The aim of this study was to evaluate the psychopathological influence of UHL among Korean 19-year-old males. Methods. The authors retrospectively compared the objective personality test profiles of 602 subjects with UHL with those of 23,790 peers with normal hearing. All participants in the current study were 19-year-old males who underwent a physical examination and completed the Korean Military Multiphasic Personality Inventory for conscription at the Military Manpower Administration from February 2015 to December 2016. Results. Significantly higher scores were found on neurosis scales in the UHL group than in the normal-hearing group (50.9±10.8 vs. 44.9±6.0 for anxiety; 51.0±10.5 vs. 44.9±5.2 for depression; 51.1±10.4 vs. 45.1±6.81 for somatization, all P<0.001). The psychopathy scales were also significantly higher in the UHL group than in the normal-hearing group (49.3±9.4 vs. 46.3±5.7 for schizophrenia; 51.1±11.2 vs. 44.3±5.8 for personality disorders; 51.1±10.5 vs. 45.7±3.7 for paranoia, all P<0.001). Conclusion. Nineteen-year-old males with UHL tended to have more abnormal results on personality tests than controls with normal hearing, suggesting that UHL may be related with a higher risk of psychopathology.

        Selective Unilateral Inner Hair Cell Injury: A Case Report

        Sensorineural hearing loss (SNHL) is known to be associated with lesions at various sites, including outer hair cell (OHC) and inner hair cell (IHC) injury, as well as auditory dyssynchrony or vestibulocochlear nerve tumors. Therefore, it is important to establish the differential diagnosis of SNHL, which however is particularly challenging in patients with unilateral SNHL. The test battery approach is effective for accurate diagnosis in such cases. In this report, we discuss the usefulness of the test battery approach to accurately detect the site of the lesion in a patient with unilateral hearing loss. A 14-year-9-month-old adolescent who observed decreased hearing sensitivity in the left ear accompanied by difficulty with understanding speech at a distance was diagnosed with unilateral hearing loss in the left ear. In this report, we describe a rare and diagnostically challenging case of unilateral SNHL in a patient with normal OHC function and selective injury to IHCs, which was diagnosed using a test battery approach.

        Horizontal Localization in Simulated Unilateral Hearing Loss

        Background and Objectives: The ability to localize a sound source is one of the binauralhearing benefits in a horizontal plane based on interaural time difference and interaural intensitydifference. Unilateral or bilateral asymmetric hearing loss will affect binaural hearing and leadto sound locating errors. In this cross sectional analytical descriptive study, the localization errorwas investigated when participants turned their heads to the sound source with closedeyes and after simulating unilateral hearing loss by placing earplugs inside the right ear canal. Subjects and Methods: This cross sectional analytical descriptive study was carried out on30 right-handed adults, 22 female and 8 male (average: 25 years, standard deviation: 3.16). They were selected with the available random access method. Horizontal localization wasevaluated with five speakers located at 0, ±30, and ±60 degree azimuths at a 1-meter distancefrom the examinee. Narrow-band noise signals were delivered at 35 dB SL in two “withoutearplug” and “with earplug” situations and the results were compared. The study was performedbetween September and December 2016 in Tehran, Iran. Results: Significantdifferences were observed in localization errors between the “with earplug” and “without earplug”situations. The localization differences were greater for left-side speakers (-30 and -60degrees) compared with right-side speakers (+30 and +60 degrees). The differences weremore apparent at 4,000 and 6,000 Hz, which confirmed the effect of unilateral simulatedhearing loss on interaural latency differences. Conclusions: Simulating hearing loss by usingan earplug in one ear (right) increased localization errors at all frequencies. The errorsincreased at higher frequencies.

        Bone Anchored Hearing Aid(BAHA)를 이용한 청각 재활

        Background and Objectives Bone anchored hearing aid (BAHA) is an alternative method applicable to patients with chronic draining ear, congenital aural atresia and single sided deafness, who cannot benefit from conventional air conduction hearing aids. The objective of this study was to evaluate the experience of 14 patients who underwent BAHA surgery. Subjects and Method We retrospectively reviewed the medical records of 14 patients who underwent BAHA surgery. Preoperative pure tone air and bone conduction thresholds and air-bone gap, postoperative BAHA-aided thresholds were measured. Hearing improvement as a result of implantation and complications related to implant were evaluated. Results The most common indication for BAHA was congenital aural atresia (8 patients) and the rest consisted of chronic otitis media (3 patients) and unilateral sensorineural hearing loss (3 patients). The average threshold improvement with BAHA was 40 dB and 34 dB in patients with congenital aural atresia and chronic otitis media, respectively. Patients with unilateral hearing loss had a postoperative aided threshold of 25 dB. Complications were limited to the periabutment skin problem in two patients. One patient received revision surgery replacing the diseased skin with split-thickness skin graft from a thigh and the other patient received surgery to remove the abutment and the wound was closed with rotation flap, leaving the fixture underneath the skin. Conclusion BAHA could be one of the safe and reliable treatment options available for auditory rehabilitation. Systematic evaluation for candidate selection might be needed to increase hearing gain and decrease co-morbidity. Korean J Otorhinolaryngol-Head Neck Surg 2010;53:755-60
