        口腔衛生士의 學校集團잇솔질意識에 關한 調査硏究

        Total 503 practicing oral hygienists were surveyed in this study. It was intended to confirm the gap between the dentistry's knowledge of collective tooth brushing at school and oral hygienists' knowledge on it. It also aimed to search for the ways to develop collective tooth brushing at school as an educational project to promote students' proper tooth brushing at school and also as a health project. The results were as follows; 1. The practicing oral hygienists didn't have a satisfactory level of knowledge on the period of use of permanent teeth, prevention of serious oral diseases, and time for tooth brushing. 2. Future oral hygienists should be fully taught about the fact that the first and second most common cause to make a Korean extract his permanent teeth are each dental caries and periodontal tissue illness and about the minimum number of time for tooth brushing from the dentistry curriculum to produce oral hygienists and from supplementary courses designed for oral hygienists. 3. All oral hygienists are supposed to help the public understand the fact fully that periodontal tissue illness and dental caries should be prevented in students to reduce the necessity of treatment, to cut down the national health insurance, and to prolong the life of teeth, At the same time, such efforts should also be taken as to let oral hygienists understand the effects of the prevention of periodontal tissue illness and dental caries in students on the needs of oral treatment, the national health insurance, and prolongation of tooth life. 4. All the oral hygienists in the country turned out to recognize the need to educate students about the proper tooth brushing time and methods and the need for schools to cultivate proper tooth brushing habits in students. 5. All the oral hygienists in the country agreed with the proposal that the tooth brushing at school project should be carried out as a health project as well as an educational project to promote the proper tooth brushing behaviors in students at school. 6. A great portion of the practicing oral hygienists, accounting for 3/4 of the whole, supported the idea that an orderly and systematic collective tooth brushing at school project should be carried out.

        칫솔질 방법 간 치면세균막 제거 효율 연구

        Objectives: This study aimed to measure the efficacy of different tooth-brushing methods for removing plaque in Korea. Methods: This study was conducted with the approval of the Institutional Review Board (IRB) of the Seoul National University School of Dentistry (S-D20180021). Thirty participants aged between 19 and 30 years, who did not have periodontal disease, were enrolled in this observational study. Participants were given the same type of toothbrush and toothpaste and asked to brush their teeth as they usually would. During brushing, participants were recorded with a camcorder that was attached to a mirror. Participants were aware they were being recorded. After they had finished brushing their teeth, a dental plaque staining and oral plaque index (PI) examination was performed. The PI score was measured using the Turesky modified Quigley Hein Index. Brushing methods were classified as rolling, horizontal, vertical, circling, and oblique. Skipped surfaces were recorded separately. Following this, statistical analysis was performed using SPSS software. Results: Most surfaces of the mouth were skipped. The most commonly used brushing method was the circling method, followed by the vertical, horizontal, rolling, and oblique methods. The most frequently used method on the vestibular surface was circling, with 52.92% of the oral surface skipped. The oblique brushing method had the lowest mean PI score with a mean±SD of 1.73±0.82. The mean PI score of the skipped surfaces was the highest with a mean±SD of 2.52±0.81. We also analyzed the linear mixed model considering the different lengths of time spent brushing. Both the brushing method used and the time spent brushing had a significant effect on the PI score, but no interactions between these were observed. In areas where a horizontal brushing method had been used, the PI score was significantly decreased. Conclusions: This study suggests that the horizontal brushing method is an efficient tooth-brushing method compared to the other methods. Additionally, tooth-brushing for more than 10 seconds on 3 to 4 teeth area was effective in removing dental biofilm.

        In Vitro Study on the Artificial Plaque Removal Effect by Use of 360 Degree Rotating Head with Sonic Tooth-Brush

        Objective: The authors have experimented for the artificial plaque removal effect of several kinds of 360 degree rotating head typed tooth-brushes with sonic vibratory actioned by using of automatic machine for horizontal scrub method in order to find the better toothbrush type for plaque removal. Methods: The experiment was conducted on three medium to 360 degree rotating head toothbrushes, a medium sized toothbrush and a medium sized toothbrush, and a flat toothbrush consisting of 30 ordinary toothbrushes. A brushing machine with horizontal scrubbing was manufactured and had variations of the end of the bristle attached to or near the surface of the teeth, a vibrating wave action force of 16,000 or 18,000 cycles per minute, and a working time of 2 or 3 minutes. The tooth removal effect was confirmed by scanning and analyzing images with a computer program after automatic brushing with the machine. The elimination rate results for each group were analyzed using the independent t-test and one-way ANOVA test. Results: It revealed the most in removal effect for the artificial plaque in such conditions as action at near the tooth surface with 18,000 cycle for 3 minutes in case of using A, B, and C tooth-brush. And it has more removal effect rate than for using the plane tooth-brush (p<0.05). Conclusion: It was recommended to develop the 360 degree rotating head and vibratory toothbrush focusing to use near the tooth surface with 18,000 cycles of vibration for 3 minutes at one site of the teeth area.

        Oral Health Care Program for the Disabled Persons Using the Toothpick Tooth-Brushing Method

        Objective: In order to promote the oral health level for severe disabled persons who were hospitalized at the disabled personal facility at Jeju community, tooth-pick method of the tooth-brushing have been provided individually and periodically to all members in the facility and check the oral health to compare the oral health level at before and the after. Methods: Forty of all members age 16 to 60 who have lived the Je-ju disabled nursing home disabled facility with severe disabled syndrome were participated through the written agreement by guardians or parents. Tooth-pick method of tooth-brushing was served individually for 10 to15 minutes service for individual, on every other week interval on Saturday or Sunday by visiting the facility with the dental examination instruments and tooth-pick toothbrush as 2 lane of the bristle tufts with zigzag line up, and individual dental care for 6 months. On performing the tooth-brushing, oral photo was taken in order to check such index as PHP (patient hygiene performance index), CPI (critical periodontal index) GI (gingival index) PMA (papillary, marginal, attached gingival index) and the oral malodor were checked. Results: Severe malodor and relatively high score of the periodontal index at first visit with the most of them with bleeding and sometimes complaint with the dull pain on the half of the subjects, otherwise it would be decreased the symptoms for 6 months and only 5 subjects got the slight bleeding when tooth pick tooth brushing. It had decreased the CPI, PMA, PHP, GI index and oral malodor level by time passing for 6 months later (p<0.05) and also decreased gingival pocket depth. Conclusion: Tooth-pick method tooth-brushing would be useful to decrease the periodontal index and recommended to apply in order to promote the oral health care for the disabled persons.

        강원특별자치도 일부 지역 성인의 가족 동거 상태에 따른 가족의 정서적 지지와 칫솔질 실천의 연관성

        Objectives: This study examines the association between emotional support and tooth brushing according to family cohabitation in the socio ecological model. Methods: The study used data from face-to-face interviews of 719 adults aged 30-90 years, living in Gangwon State. The dependent variables were tooth brushing after lunch and dinner (including before going to bed). The independent variables were living conditions and emotional support. Multiple logistic regression analysis was conducted to confirm the association between emotional support and tooth brushing. Results: The results showed that 81.4% of people lived with their family, and 85.9% had family (friends) with whom they could share oral problems (p< 0.01). Among those living with their family, 82.3% (p >0.05) brushed after lunch and 81.9% (p>0.05) brushed after dinner. Tooth brushing after lunch was 1.5 times more likely among those who shared oral problems with their family (friends); however, this was not statistically significant (95% CI: 0.92-2.29). Tooth brushing after dinner was 1.0 times more likely among those who shared oral problems with their family (friends); however, this was not statistically significant (95% CI: 0.43-2.35). Conclusions: Emotional support may help to promote tooth brushing practice among adults.

        A Clinical Study on the Plaque Removal, on the Teeth and Tongue and Oral Malodor Control Effect by Use of 360 Degree Round Head Tooth-Brush

        Objective: 360 degree round head tooth-brush was designed and produced in order to increase the plaque removal effect on the tooth surface and tongue surface, with the idea of the cleansing the proximal area and dorsum of the tongue effectively, and to reduce the oral malodor. Methods: A clinical test was performed with 60 adult volunteers by dividing with two groups as the experimental and the control. Tooth-brushing instruction as rolling method at buccal and lingual side as well as on the proximal site, and horizontal method on the occlusal surface, by use of 360 degree round head tooth-brush for experimental group, otherwise instructing with the rolling technique for the control group, both in teeth and the tongue. Patient hygiene performance index at the teeth and tongue, and oral malodor were checked by oral examination and B&B Checker, at before and 1, 2 and 4 weeks later, in order to compare the data with before and after as well as the experimental and the control group. Results: It revealed that the PHP index were more decreased in the experimental group than in the control, from the 2 weeks later, both at the teeth surfaces and tongue (p<0.05). Oral malodor was decreased in the experimental group than in the control, from a week later (p<0.05). Conclusion: 360 degree round head toothbrush was effective to control the dental plaque and tongue plaque, as well as for malodor control, under the well trained tooth-brushing technique.

        Proper Tooth-Brushing Technique According to Patient’s Age and Oral Status

        Tooth brushing is a basic technique to prevent major oral diseases (e.g., dental caries and periodontal disease). Several methods of tooth brushing technique devised by some dental scholars have been introduced. However, some guidelines to select the proper method are needed for an individual patient considering age and oral status. Methods such as rolling, Fones’, Bass, Stillman, Charters, Watanabe’s tooth-pick, and other modifications have been introduced. The rolling technique was recommended for general use, the Fones’ technique for preschool children, the Bass technique for the patient with gingivitis with gingival sulcus problem, the Stillman technique for widespread gingivitis, and the Charters technique for bridge wearer. Moreover, fixed-type orthodontic appliance wearer is recommended to use the mix-type methods as Bass, rolling, and Charters. Moreover, the Watanabe’s technique would be available for patients with a dental implant or periodontal problem. The denture wearer should clean the denture with a partial denture brush for a partial denture and a full denture brush for a full denture with the rolling method or its modification. The selection of the proper tooth brushing method suggested in this article would be one of the guidelines for clinical use of tooth brushing instruction for preventive measures.

        The Connection between Hand Washing and Brushing Teeth

        Background: The purpose of this study was to identify the connection between handwashing and toothbrushing, focusing on eating habits, and to verify whether eating habits can be used as an action cue for forming health habits.Methods: This was a cross-sectional study using secondary data from the 2019 community health survey. The participants included 229,099 adults aged 19 years or older, representative of the South Korean people. We employed two dependent variables: one was washing hands, and the other was brushing teeth. Eating habits was a major independent variable. Socioeconomic variables, such as age, gender, income, occupation, economic activity, education, and residence were adjusted as confounders. Multivariate logistic regression was performed to calculate adjusted odds ratio and 95% confidence intervals. Results: Most of the participants had good health behaviors: those who wash their hands and brush their teeth were each approximately 80%. Our finding indicated that brushing teeth and washing hands can be connected with eating habits. After adjusting for confounders, it was found that people who wash their hands before meals (compared to those who did not wash their hands before meals) had a higher toothbrushing rate after meals (i.e., socioeconomic status) (Adjusted Odds Ratio: 2.0, Confidence Intervals: 1.9 to 2.1). Conclusion: Those who practice either washing hands before meals or brushing teeth after meals were found to have a connection between washing hands and brushing teeth based on the results of practicing other health behaviors. This implies that eating habits can be connected as a behavior cue to promote health habits, such as washing hands before meals and brushing teeth after meals.

        Is Joint Tooth Brushing an Effective Program for Improving Dental Health among Elementary Students? : A Study from Jakarta, Indonesia

        Objective: To evaluate the effectiveness of a joint tooth brushing program at an elementary school in Jakarta. Methods: An intervention program to improve the oral health of elementary students was conducted over a 6-month period. The program consisted of dental health education and daily indoor two-minute sessions of joint tooth brushing. We sampled a total of 57 students from the first and second grades. Each one was interviewed and was subject to oral examinations before and after the program was implemented. Moreover, questionnaires were also administered to the teachers. Then they were trained as an instructor for the program. These programs were conducted daily before the beginning of class. Results: Significant changes in saliva pH and plaque pH scores was found for students. Moreover, teachers’ awareness of dental healthcare also increased. Finally, teachers were actively engaged when acting as tooth brushing program instructors. Conclusion: Teachers conducting daily, indoor, two-minute, joint tooth brushing programs could motivate students to maintain their oral health. This in turn leads to a decreased prevalence of dental caries among elementary school students.

        칫솔질 교육의 치면 세균막 억제 효과

        To maintain good oral hygiene, plaque control is essential and the most common and effective method is appropriate brushing. However accurate brushing is so difficult for children that the importance of brush is getting higher. We investigated the effect of tooth brushing instruction in plaque control for 40 children who came to Kyunghee university dental hospital, department of pediatric dentistry. We examined basic information by self-assessment, evaluated plaque index, tooth & tongue cleanness, malodor index for 4 times and compared values before and after tooth brushing instruction. Plaque index decreased(p<0.01) and tooth & tongue cleanness increased(p<0.01). There were no significant differences in mal-odor index. In conclusion, tooth brushing instruction is effective in removing dental plaque and its beneficial role is highly expected in establishing children’s good oral hygiene. 좋은 구강 위생 환경을 유지하기 위하여 치태 조절은 필수적이며, 가장 보편적이고 효과적인 방법은 올바른 칫솔질이다. 그러나 아동들은 올바른 칫솔질을 시행하기 어려워 칫솔질 교육의 중요성도 높아지고 있어, 경희대학교 치과대학 소아치과에 내원한 아동 40명을 대상으로 칫솔질 교육의 치태 조절의 효과를 조사하였다. 설문조사를 통해 기본 정보를 조사하고 치면 세균막 지수와 치아 및 혀 세정도, 구취 관리도를 측정하는 조사를 총 4회에 걸쳐 시행하여 칫솔질 교육 전과 후의 차이를 비교하였다. 치면 세균막 지수는 감소하였고(p<0.01), 치아 및 혀 세정도는 증가하였다(p<0.01). 구취관리도는 상관성을 보이지 않았 다(p>0.01). 이러한 결과 칫솔질 교육은 구강 내 치면 세균막 제거에 뛰어난 효과를 보여, 아동들의 좋은 구강 위생 환경의 확립에 도움 을 줄 것으로 기대된다.
