        한방병원에 내원한 소아청소년기 말초성 안면마비 환자 284례를 통한 임상적 고찰

        Objectives The purpose of this study is to investigate the clinical characteristics in children and adolescents with peripheral facial palsy that was treated with Korean medicine. Methods The study was conducted based on 284 cases that consisted of 261 children (less than 19 years old) who visited the hospital of Korean medicine from August 2010 to August 2014. Then, this study was analyzed by reviewing the patients' charts. Results The incidence of facial palsy in pediatric patients was more common in boys than in girls and the mean age was 11.2 years. The prevalence of facial palsy was more common in winter and most were diagnosed with Bell's palsy. Recurrence was identified in 12.6% of the patients. The average age of the first attack was 9.5 years. It took about 3.2 years for recurrence to occur from the first onset. It took about 5 days to visit the hospital of Korean medicine from the onset. 81% of the patients came within 1 week from the onset. 50.7% of the cases did not receive any treatment before and 49.3% were previously treated at other hospitals. More than half of the patients (65.1%) received Korean medicine first. 54.6% of the patients were hospitalized and 45.4% received outpatient treatments in the hospital of Korean medicine. Outpatient treatments were administered about 11.5 times while hospitalized patients were treated for an average of 16.6 days. In general, hospitalized patients were treated more often than the outpatient group. An overall average duration of treatment was 70.6 days from the onset of facial palsy until the last day of treatment, and 53.0 days from the first day of treatment until the last day of treatment. 41.5% of the cases were treated only with Korean medicine and 49.0% were treated with a combination of Korean and Western medicine. The types of Korean medicine treatment used for facial palsy in descending order of frequency were acupuncture, Infra red, herbal medicine, SSP therapy, electro-acupuncture, carbone, hot pack, cupping, and moxibustion. The treatments that were most frequently used from Western medicines were steroid as monotherapy (61.7%) and a combination of steroids and antiviral therapy (37.6%). Conclusions This result showed that the risk of facial palsy recurrence among children is relatively high and that facial palsy requires a long-term treatment. It's helpful to explain about prognosis, treatment duration, and recurrence potential in children. Facial palsy in children is treated in various ways but more studies about the therapeutic effects of Korean medicine, Western medicine, and combination of Korean-Western medicine in children are needed.

        Objectives: The purpose of this study is to investigate the effectiveness of using Korean medical treatment for peripheral facial nerve palsy. Methods: We primarily treated the patient's peripheral facial nerve palsy during hospitalization (06/28/21~07/13/21) without any other steroids or antiviral drug treatments and additionally treated their spinal stenosis, using Korean medical treatments in both cases (herbal medicine, acupuncture, and herbal acupuncture therapy). The House-Brackmann grading system and Yanagihara's unweighted grading system were used to assess facial symptom changes, and the Numerical Rating Scale was used to evaluate the back pain caused by the spinal stenosis. Results: Following treatment, the patient's House-Brackmann and Numerical Rating Scale scores decreased, while their Yanagihara unweighted score improved. Conclusions: This study suggests that Korean medicine may be effective in the treatment of facial nerve palsy.

        ENoG를 이용한 末梢性 顔面神經麻痺의 臨床的 考察

        Objective : The purpose of this study is to report that ENoG may be used for concluding prognosis of peripheral facial palsy. Methods : The House-Brackmann facial grading scale of face was taken at onset and 3weeks after oriental medical treatment. The ENoG of face was taken at 1week after or before of onset. I compared the House-Brackmann facial grading scale with ENoG. Results and Conclusions : I analyzed a relation of House-Brackmann facial grading scale and ENoG. I guess that ENoG may be used for concluding prognosis of peripheral facial palsy. But further studies are requred for concluding prognosis of peripheral facial palsy by ENoG.

        전기와 자기장의 침 자극을 포함한 복합치료가 말초성 안면신경마비에 미치는 영향에 대한 관찰 연구

        Clinical Observations of Complex Therapy, including Electroacupuncture and Magnetic-acupuncture, for Treating Peripheral Facial Nerve Palsy Seo Young Oh, Hyun Lee and Jae Hui Kang* Department of Acupuncture & Moxibustion Medicine, College of Korean Medicine, Daejeon University Objectives : This study was performed to observe the effect of complex therapy, including electro- acupuncture and magnetic-acupuncture, on peripheral facial nerve palsy. Methods : Nine patients with peripheral facial nerve palsy were treated with acupuncture using electrical and magnetic stimulation. Acupoints in the face were stimulated with an electromagnetic field, as widely and as evenly as possible. To evaluate the effects before and after treatment we used Yanagihara’s unweighted grading system, House-Brackmann scale, and Sunnybrook facial grading system and image once a week. Results : After treatment, the scores of Yanagihara’s unweighted grading system, House-Brackmann scale and Sunnybrook facial grading system each improved (p-value < 0.05). Conclusion : Complex therapy using electro-acupuncture and magnetic-acupuncture might be an effective treatment to improve symptoms of peripheral facial nerve palsy. Further randomized- controlled trials are required to verify the efficacy and results of this study.

        Background : Sequelae symptoms of peripheral facial palsy include not only partial recovery of the paretic muscles but contracture, spasm, synkinesis and atrophy which cause significant functional, esthetic, and psychosocial disturbances to the lives of patients. Objective : This study is to investigate the prevalence, time of onset, patient's self assessment on the degree of understanding of the sequelae and the association of the degree of palsy with the appearance of sequelae. Methods : 106 patients with peripheral facial palsy were sequentially interviewed and examined. Results : 29 patients(27.4%) of 106 patients showed sequelae symptoms, of whom 19 patients(65.5%) showed sequelae symptoms during 4 to 6 months after the onset of palsy. The degree of understanding on the sequelae at the time of interview showed improvement compared to that at the time of first consultation to any physician but not to reach an sufficient understanding. No relation between the degree of palsy and the appearance of sequelae was observed.

        Treatment of Peripheral Facial Palsy with Skin Damage Caused by Folk Remedies Using Korean Medicine: A Case Report

        Peripheral facial palsy generally results from damage to the facial nerve. As facial asymmetry is observed, it accompanies not only functional problems but also psychological and social difficulties. Medical institutions treat most patients, however, there are still patients who rely on invasive methods by unqualified practitioners for fear of sequelae. This case describes a 61-year-old woman who experienced facial palsy twice. She visited our hospital with skin damage after folk patch therapy for her facial palsy. Combined Korean medicine treatment was administered during treatment period. After treatments, the symptoms of facial palsy and skin lesions improved. This case showed that Korean Medicine was efficacious in improving symptoms of facial palsy with damaged skin caused by folk patch therapy. Education and awareness of uncontrolled invasive treatments for facial palsy are needed.

        안면마비 후유증에 대한 매선치료의 효과

        Objectives : The aim of this study was to observe the effect of Needle-embedding therapy on sequelae of peripheral facial palsy. Methods : 27 patients with sequelae of peripheral facial palsy were treated with Needle-embedding therapy. Yanagihara score, number of symptoms, symptoms score were compared between before and after treatment to evaluate the effect of Needle-embedding therapy. Results : After treatment, Yanagihara score, number of symptoms, symptom score were significantly decreased(p<0.05). Conclusions : Needle-embedding therapy could be effective to improve symptoms of sequelae of peripheral facial palsy patients.

        봉약침 복합치료가 초기 회복지연에 따른 말초성안면마비 후유증에 미치는 영향

        Objectives : This study was to investigate the effectiveness of bee venom phamacopuncture complex therapy on the sequelae of peripheral facial palsy. Methods : We observed the effectiveness of bee venom pharmacopuncture complex therapy on three patients who have residual symptoms of Bell’s palsy although early stage(0 to 3months) elapsed. H-B grade and Yanagiha’s total score was used for evaluating the patient. Results : 1. Deviation of the bee venom therapy on the sequelae of peripheral facial palsy, One patient was improved from 19 to 39, another patient was improved from 25 to 40, the third patient was improved from 15 to 26 on Yanagiha’s total score. 2. In the bee venom therapy on the sequelae of peripheral facial palsy, compared with baseline, at final, H-B grade and Yanagiha’s total score was increased. Conclusions : Bee venom phamacopuncture can be available for relieving residual symptom of bell’s palsy after the early stage.

        정안침요법 복합치료를 이용한 말초성 안면마비 후유증 치료 5례

        Objectives : The purpose of this study is to report the effect of Jung-an acupuncture on the sequelae of peripheral facial palsy.Methods : Five patients suffering from sequelae of peripheral facial palsy were treated with Jung-an acupuncture. House-Brackmann grade system, the scale of Peitersen grades and picture, before and after the Jung-an acupuncture treatment were compared to evaluate its treatment efficacy.Results : House-Brackmann total grade of all subjects were improvement and Peitersen grades of all subjects were improvement. In two cases, the patients had improvement in synkinesisConclusions : This study showed that Jung-an Acupuncture treatment can be effective in improving functional and psychological outcome even for sequelae of peripheralfacial palsy. Randomized-controlled trial for further evaluation is necessary to verify the results and findings of this study.

        말초성 안면마비 후유증에 대한 정안침요법 증례 보고

        Objectives : This study was designed to evaluate the effect of Jung-an acupuncture on the sequelae of peripheral facial palsy. Methods : Four patients suffering from sequelae of peripheral facial palsy were treated with Jung-an acupuncture. Yanagihara’s total scores, the scale of Peitersen grades and picture, before and after the Jung-an acupuncture treatment were compared to evaluate its treatment efficacy. Results : Yanagihara’s total score of all subjects were increased. Peitersen grades of all subjects were decreased. Conclusions : As indicated by unequivocal increase in all subjects’ Yanagihara and Peitersen scores, Jung-an Acupuncture treatment is effective in alleviating the sequelae of peripheral facial palsy. Randomized-controlled trial for further evaluation is necessary to verify the results and findings of this study.
