        기능성 보행운동이 만성관절염 여성노인 환자의 균형능력 및 보행능력에 미치는 영향: 기능성 밸런스 보행 킷의 개발과 적용 연구

        PURPOSE: This study aimed to provide fundamental data contributing to balance ability and gait ability in elderly with chronic arthritis by functional gait exercise on balance ability and gait ability. METHODS: Twenty voluntary subjects (10 exercise, 10 non-exercise) with an understanding toward the aims of this study were selected and their balance ability and gait ability, before and after the functional gait exercise, were measured. RESULTS: As for the balance ability, there were significant differences between pre- and post-values in the exercise group after completing the functional gait exercise program. Meanwhile, in the aspect of comparisons between post-values in both groups, there were significant improvements in up (U), down (D), right (R), left (L) rate of the exercise group. As for the gait ability, there were significant differences between pre- and post-values in the exercise group after completing the functional gait exercise program. In the aspect of comparisons between post-values in both groups, there were significant improvements in only the exercise group. CONCLUSIONS: The study results indicate that the functional gait exercise program is effective in improving the balance ability and gait ability of elderly with chronic arthritis.

        불안전 지면을 이용한 기능성 보행운동이 초등학교 비만아동의 균형능력과 보행능력에 미치는 영향

        본 연구는 불안전 지면을 이용한 기능성보행운동이 초등학교 비만아동(EG그룹 /20명, NEG그룹/20명)의 균형능력과 보행능력에 미치는 영향을 알아보기 위하여 그룹별 자세안정성 및 보행능력을 알아보았다. 불안전 지면을 이용한 기능성보행운동 후 균형능력의 변화는 EG 그룹에서 Up, Down, Left, Right에서 통계적으로 유의하 게 증가하였으며, 그룹 간 차이에서는 사후검사에서 Up, Down, Left, Right의 모든 변인에서 통계적으로 유의하게 증가하였다. 불안전 지면을 이용한 기능성보행운동 후 보행능력에 관한 연구에서는 EG 그룹에서 보행속도, 분속수, 보장, 활보장 에서 통계적으로 유의한 차이가 나타났으며, 그룹 간 차이에서는 사후검사에서 보행능력의 보행속도, 분속수, 보장, 활보장의 모든 변인이 통계적으로 유의한 차이가 나타났다. 위의 결과 로 보아, 불안전 지면을 이용한 기능성보행운동이 초등학교 비만아동의 균형능력과 보행능력의 변화에 긍정적인 효과가 있는 것으로 나타났다. This study was to investigate the effects of functional gait exercise for avail oneself of unsafe ground on balance ability and gait ability in the obese elementary students. The participants were divided into two groups: functional gait exercise group (EG,n=20) and non exercise group (NEG, n=20). The EG took part in the functional gait exercise for 12 weeks. The results of this study were as follows: First, As for the balance ability, there were significantly increased in up(U), right(R), down(D), and left(L) in EG compared with NEG after 12 weeks in static and dynamic balance ability. Second, As for the gait ability, there were significantly increased in velocity, cadence, step length and stride length in EG compared with NEG after 12 weeks. As conclusions, this study confirmed that the functional gait exercise program could improve the balance ability and gait ability of obese elementary students.

        하지 수술환자에게 적용한 로봇보조 보행훈련의 단기간 임상적 효과: 예비 연구

        Purpose: This study aimed to investigate the effect of robot-assisted gait training on the active ranges of motion, gait abilities, and biomechanical characteristics of gait in patients who underwent lower extremity surgery, and to verify the effectiveness and clinical usefulness of robot-assisted gait training. Methods: This study was conducted on 14 subjects who underwent lower extremity surgery. The subjects participated in robot-assisted gait training for 2 weeks. The active ranges of motion of the lower extremities were evaluated, and gait abilities were assessed using 10-m and 2-min walk tests. An STT Systems Inertial Measurement Unit was used to collect data on biomechanical characteristics during gait. Spatiotemporal parameters were used to measure cadence, step length, and velocity, and kinematic parameters were used to measure hip and knee joint movement during gait. Results: Significant improvements in the active ranges of motion of the hip and knee joints (flexion, extension, abduction, and adduction) and in the 10-m and 2-min walk test results were observed after robot-assisted gait training (p < 0.05). In addition, biomechanical characteristics of gait, spatiotemporal factors (cadence, step length, and velocity), and kinematic factors (gait hip flexion–extension, internal rotation–external rotation angle, and knee joint flexion–extension) were also significantly improved (p < 0.05). Conclusion: The results of this study are of clinical importance as they demonstrate that robot-assisted gait training can be used as an effective intervention method for patients who have undergone lower extremity surgery. Furthermore,the findings of this study are clinically meaningful as they expand the scope of robot-assisted gait training, which is currently mainly applied to patients with central nervous system conditions.

        10주간의 저항성 운동이 고령여성의 등속성 근력 및 보행능력, 균형능력에 미치는 영향

        The study was designed to examine effect of 10 weeks of resistance exercise on the isokinetic muscle function, gait ability and balance ability in 20 subjects(exercise group 10, control group 10) over 65years of age. The analysis of data revealed the following findings. 1. After 10 weeks of resistance training, the exercise group had significantly higher result than the control group on flexor/extensor peak torque of knee(p<.05). 2. Control group showed superior balance ability over exercise group. 3. In regards of gait ability, Control group's gait index was meaningfully lower than exercise group(p<.05). No significant difference was shown in gait velocity, way of walking and error rate. In conclusion, old women who had no exercise habit in their life were positively improved on isokinetic muscle function, gait ability and balance ability. It was shown that resistance training program was effective in decreasing improvement on weakness of muscle endurance strengh, decrease in walking ability.

        뇌졸중 환자의 보행훈련 효과 검증 - 근육활성화와 보행능력을 중심으로

        The purpose of this study was to investigate the effects of treadmill gait training with weight loading on the muscle activity and gait ability of the stroke patients. The design of this is a randomized controlled trial (RCT). Twenty-two stroke patients participated in this study. Participants were randomly assigned to the experimental group I (n=11) and experimental group II (n=11). Experimental group I performed general treadmill gait training, and experimental group II performed treadmill gait training with weight loading applied. The muscle activity and gait ability were compared to evaluate the effect on intervention. Significant differences in internal oblique muscle activity, gluteus medius muscle activity, stride length, single support were observed between the experimental group I and experimental group II (p<.05). A treadmill gait training with weight loading resulted in improved muscle activity of the oblique muscle activity, gluteus medius muscle activity and gait ablilty in stroke patients. Therefore, treadmill gait training with weight loading for stroke patients are effective method with clinical significance in improving the muscle activity and gait ability.

        동적 모의실험 및 분석을 통한 보행훈련장치 연구

        A gait training system for patients with gait disorder was developed. These systems leads gait joint movements given the patient who cannot carry out an independent gait. It provided a normal stance-swing ratio of 60 : 40 because of the eccentricity revolution of a gear. In this study, motion trajectory of a knee & ankle joint was evaluated and compared a measured trajectory with three-dimensional motion analyses system with the results of multi-body dynamics simulation during the gait training in a prototype of a developed gait training system. In general, predictions from the simulation study were in good agreement with experimental results from the motion analysis. The clinical application by a continuous dynamics simulation studies and gait analysis was expected to be developed by gait training system which would be helpful to recover gait ability of patients with gait disorder.

        Effects of therapeutic horse-riding program on the walking ability of students with intellectual disabilities

        The purpose of this study was to determine if an 8-week therapeutic riding (TR) program was effective in improving the walking ability of students with intellectual disabilities. Thirteen students diagnosed with intellectual disabilities participated in the TR program. TR sessions were conducted twice a week (30 min per session), with a total of 16 rides taking place over an 8-week period. A gait measurement analyzer was used to measure progress based on a turn test (6-m walking and turning test), walk test (10-m walking), and timed up and go (TUG) test. Measurements were made three times: before horse-riding (P0), after 4 weeks (8 rides) of horse-riding (P1), and after 8 weeks (16 rides) of horse-riding (P2). Data analysis was conducted using SPSS software (ver. 22.0). Descriptive statistics were generated on the general characteristics of the subjects, and the Kolmogorov-Smirnov test was used to verify the normality of the data. Because of the lack of normality, the data were analyzed using a nonparametric method and the significance level was set to 0.05. Measurements of the duration of the forward gait cycle (s) in the turn test and the forward gait speed (m/s) in the walk test indicated improved walking ability after the TR program (p < 0.001); the stride length (% height) also increased significantly (p < 0.05). The walk test revealed a significant effect of the program on the duration of the forward gait cycle (p < 0.05), while there were significant improvements on the left and right of the elaborated strides (p < 0.001). No significant improvement in TUG test performance was observed after the TR program. In this study, an 8-week TR program had positive results on gait. Therefore, further research is merited, where TR programs are likely to improve the walking ability of individuals with intellectual disabilities.

        다발성감각운동자극 치료가 뇌졸중 환자의 보행과 낙상위험도에 미치는 효과: 무작위배정예비임상시험

        Purpose : To determine whether an advanced rehabilitation therapy combined with conventional rehabilitation therapy consisting of sensorimotor exercises that would be superior to a usual treadmill training in gait ability and fall risk in subacute stroke patients. Methods : Thirty subjects randomly assigned to either multi-sensorimotor training group (n=19) or treadmill training group (n=18). Both groups first performed conventional physical therapy for 30 min, after which the multi-sensorimotor training group performed multi-sensorimotor training for 30 min, and the treadmill training group performed treadmill gait training for 30 min. Both groups performed the therapeutic interventions 5 days per week for 8 weeks. Gait ability was evaluated using the GAITRite system and Fall risk was measured using the Biodex Balance system before intervention and after 8 weeks. Results : There were no intergroup differences between demographic and clinical characteristics at baseline (p>.05). Both groups showed a significant improvement in gait ability (p<.05) and Fall risk (p<.05). In particular, the multi-sensorimotor training group showed more significant differences in gait velocity (p=.05), step length (p=.01) and stride length (p=.014) than the treadmill training group. Conclusion : The multi-sensorimotor training program performed on multiple types of sensory input had beneficial effect on gait ability. A large-scale randomized controlled study is needed to prove the effect of this training.

        편마비 환자의 트레드밀과 체중지지의 트레드밀 훈련이 균형능력 및 보행능력에 미치는 영향

        Purpose: The purpose of this study is to investigate and to verification of changes that the effect of treadmill and body weight support treadmill training on balance and gait ability for sub-acute stroke patients during 4 week. Method: 16 subjects who was diagnosed stroke were divided into 2 groups(8-treadmill training group, 8-body weight support treadmill group) by randomized control trial. Both training programs were consisted with 40 minuted, 5 times a week for 4 weeks and after training programs. Analysis: We analysed effects and changes on balance and gait ability. Analyses were performed using PASW ver. 18.0 and results were reported as mean ${\pm}$ standard deviation (S.D.). To investigate within group comparisons and to verification on effects of exercise, we did paired t test and repeated measured ANOVA test. Significance was set at p<.05. Result: Both training programs showed positive changes in Limit of Stability but significant results which is area of hemiside, area of intact side, area of posterior, total area were reported in body weight support treadmill training group. Changes of Berg Balance Scale was significantly increase and it had significant correlation between groups. Changes of 6 Minutes Walking Test was significant increase in both groups but there was no significant changes on Foot print and Romberg test. Conclusion: After considering all factors, both training programs showed effect on improvement of balance and gait ability in sub-acute stroke patients, but body weight support treadmill training group had better improvement in dynamic balance than treadmill training group. For the increase of balance and gait ability in sub-acute stroke patients, we need to continues study on difference of treadmill and body weight support treadmill and then we will give stroke patients a better satisfaction if we develop and provide a rehabilitation program for improvement of balance and gait ability.

        뇌졸중 환자의 탄력-비탄력 발목 테이핑 적용이 자세균형과 보행능력에 미치는 일시적 효과

        The purpose of this study was to investigate the immediate effects of ankle elastic and non-elastic taping on postural balance and gait ability in subject with stroke. Design: Cross-sectional study Methods: Twenty-seven subjects with stroke participated in this study. The subjects performed to stand quietly for 30s on the balance platform and walking test with three different ankle taping conditions. The sway length, sway area and sway velocity of center of gravity (COG) displacement was measured to assess the postural balance and the timed up and go test, 10m walking test, 6 minutes walking test was measured to assess the gait ability. Repeated meas- ured ANOVA was used to compare the postural balance parameters and gait ability according to three different ankle taping conditions. Results: Postural balance with non-elastic ankle taping was significantly improved compared to no ankle taping and elastic ankle taping condition(p<0.05). On the other hand, gait ability with elastic ankle taping was significantly increased compared to no ankle taping and non-elastic ankle taping condition(p<0.05). Conclusion: These findings suggest that an elastic ankle taping could effect to improve the gait ability, whereas a non-elastic ankle taping could effect to improve the postural balance in subject with stroke.
