Uterine sarcomas are relatively rare tumors of mesodermal origin that constitute 2-6% of uterine malignancies. Uterine sarcomas include endometrial stromal sarcoma (ESS), leiomyosarcoma (LMS), malignant mixed m llerian tumor (MMMT) of both homologous and heterologous type, pure heterologous sarcomas, blood vessel sarcomas, and lymphoma. Endometrial stromal tumors are divided into three types: 1) endometrial stromal nodule, 2) low-grade stromal sarcoma or endolymphatic stromal myosis, and 3) endometrial stromal sarcoma. The low grade endometrial stromal sarcoma is very rare. It shows occasional local recurrence, which might arise from endometrial stroma, from adenomyosis, and rarely from endometriosis. We have experienced a case of low grade endometrial stromal sarcoma of the uterus in a 54-year-old woman, which is presented with a review of literatures.

Endometrial stromal sarcomas (ESS) are rare neoplasms, comprising approximately 0.2% of all uterine malignancies, and about 10~15% of uterine sarcomas. These tumors are composed purely of cells resembling normal stroma. They can be divided into three types on the basis of mitotic activity; endometrial stromal nodule, low-grade endometrial stromal sarcoma, high-grade endometrial stromal sarcoma. Although the diagnosis may be determined by endometrial biopsy, transvaginal sonography, pelvic CT or MRI, the usual preoperative diagnosis is problematic. The confirmatory diagnosis is determined by pathologic finding after surgery. We experienced a case of endometrial stromal sarcoma which was diagnosed as endometrial carcinoma by imaging study and could not be diagnosed preoperatively by endometrial biopsy. Here we report this case with a brief review of literature.

Endometrial stromal sarcoma (ESS) is a relatively rare form of uterine sarcoma. Endometrial stromal sarcoma (ESS) of the myometrium compose just 0.2% of uterine malignant tumors. Low-grade stromal sarcoma has extended beyond the uterus in 40% of cases at the time of diagnosis, but the extra-uterine spread is confined to the pelvis in two thirds of the cases. A few cases of extra-uterine endometrial stromal sarcoma have been reported in literature concerning the ovary and pelvis. We have experienced a Post-total hysterectomy with bilateral salpingooophorectomy retroperitoneal endometrial stromal sarcoma, so we report this case with a brief review of literature.

Purpose: The goal of this study was to perform a retrospective analysis of the ultrasonographicfindings associated with low-grade endometrial stromal sarcoma. Methods: Ten pathologically confirmed cases of low-grade endometrial stromal sarcoma atour institution from January 2007 to April 2014 were retrospectively reviewed. All patientsunderwent a preoperative transvaginal ultrasound. Two radiologists came to a consensusregarding the location, size, margin, and echogenicity of the tumor, as well as the presence ofintratumoral cystic degeneration and its extent and configuration. Results: Low-grade endometrial stromal sarcoma manifested as an intramural mass protrudinginto the endometrial cavity (n=6) or as a purely intramural mass (n=4). The maximal diameterof the lesion ranged from 4 to 9.1 cm (mean, 6.2 cm). The imaging features of low-gradeendometrial stromal sarcoma were variable: six cases involved predominantly solid massescontaining cystic degeneration, one was a predominantly unilocular cystic mass, two were illdefinedinfiltrative solid masses, and one was a well-defined solid mass. Among the seven caseswith internal cystic degeneration, five patients showed a multiseptated cystic area or a cysticarea with multiple small clusters, while a unilocular cystic area within the tumor was found intwo patients. Conclusion: Low-grade endometrial stromal sarcoma is associated with variable ultrasonographicfindings with regard to the location, margin, and configuration of the lesion. Multiseptated cysticareas and multiple small areas of cystic degeneration are common.

Extrauterine endometrial stromal sarcoma arising from malignant transformation of the vagina is an extremely rare condition. The diagnosis is often difficult as the symptomatology and pathological features overlap with that of pelvic endometriosis. A 38 years old female presented with complaints of dyspareunia, dysmenorrhea, and painful defecation along with blood-stained vaginal discharge for a year. Examination revealed the presence of multiple brownish irregular nodules in posterior vaginal fornix and fixed tender nodules which on biopsy revealed florid vaginal endometriosis. She improved symptomatically on medical therapy. After 18 months of diagnosis, she presented again with a necrotic growth in posterior fornix, which on repeat biopsy revealed a low-grade endometrial stromal sarcoma. Laparotomy revealed a 7×5 cm mass in the pouch of Douglas, infiltrating the posterior vaginal wall and rectum. A complete cytoreductive surgery with retrograde hysterectomy, excision of posterior vaginal wall and rectosigmoid resection was done. The patient is disease-free at a follow-up of 65 months.

Low-grade endometrial stromal sarcoma is generally an indolent tumor rich in estrogen and progesterone receptors. Objective responses to hormonal therapy, most commonly with megestrol acetate, have been reported. The patient is a 50-year-old woman who presented with low-grade endometrial stromal sarcoma confined to the vagina in 1992 and was treated with local excision. Approximately 4 years later, the patient had recurrent right ovarian and ileal mass treated with right oophorectomy and segmental resection of ileum. In 2002, multiple lung metastatic lesions were discovered and treated with megestrol acetate. But megestrol acetate showed no response during the period. Letrozole was then given at a daily dose of 2.5 ㎎ with partial response for a duration of 10 months. Letrozole at a daily dose of 2.5 ㎎ may be effective in low-grade endometrial stromal sarcoma.

Endometrioid stromal sarcoma is a rare malignancy that originates from mesenchymal cells. It is classified into lowgrade endometrioid stromal sarcoma (LGESS) and high-grade endometrioid stromal sarcoma. Ultrasonographic findings of LGESS resemble those of submucosal myomas, leading to the possible preoperative misdiagnosis of LGESS as uterine leiomyoma. Electronic morcellation during laparoscopic surgery in women with LGESS can result in iatrogenic intraabdominal dissemination and a poorer prognosis. Here, we report a patient with LGESS who underwent a supracervical hysterectomy and electronic morcellation for a presumed myoma in another hospital. Disseminated metastatic lesions of LGESS in the posterior cul-de-sac and rectal serosal surface were absent on primary surgery, but found during reexploration. In conclusion, when LGESS is found incidentally following previous morcellation during laparoscopic surgery for presumed benign uterine disease, we highly recommend surgical reexploration, even when there is no evidence of a metastatic lesion in imaging studies.

Mixed endometrial stromal and smooth-muscle tumor is one of the uncommon forms of uterine sarcoma. Only a few cases of endometrial stromal tumors showing smooth muscle differentiation have been reported in the literature. We experienced a case of low grade endometrial stromal sarcoma with smooth muscle differentiation in a 44-year-old woman, so we report this case with brief review of literatures.

목적 : 이 연구의 목적은 저등급 자궁내막 간질성육종에 대한 치료와 예후를 평가하는데 있다. 연구 대상 및 방법 : 1993년부터 2002년까지 강남성모병원에서 치료받고 있는 세 환자를 대상으로 하여 저등급 자궁내막 간질성육종의 임상 병기, 치료 방법과 치료 결과를 분석하였다. 결과 : 세 명의 저등급 자궁내막 간질성육종 환자들은 초기 수술 후 호르몬 치료를 받으며 긴 생존률을 기록하고 있다. 두 명은 재발이 없는 상태로 생존해 있으며, 한 명은 호르몬 Objective : The aim of this study was to obtain information on the prognosis and treatment of the low grade endometrial stromal sarcoma. Sarcoma of the uterus is a malignant tumor arising from the muscle or connective tissue and very rare malignant tumor. There are only a few case reports in Korea. We have experienced fifteen cases of the uterine sarcoma between 1980 and 1989 for 10 year. The results were obtained as follows: 1. The distribution of uterine sarcoma by histologic-type was 5 cases(33.3%) for leiomyosarcoma, 6 cases(40.0%) fir malignant mixed mullerian tumor, 3 cases(20.0%) for endometrial stromal sarcoma. 2. The average age was 48.9 years and the most frequent occurrence was noted in the groups of fifties. 3. The clinical symptoms were abnormal uterine bleeding(60.0%), abnormal mass(20.0%), abdominal distension(13.3%), and abdominal pain(7.7%). 4. The primary sites were the cervix(7.7%), endometrium(46.7%), and myometrium(26.6%).
