        Eccrine Spiradenoma Arising in the Breast Misdiagnosed as an Epidermal Inclusion Cyst

        Eccrine spiradenomas are rare, benign, cutaneous tumors that originate in the sweat glands. Eccrine spiradenomas in the breast are very rare and only a few cases have been reported. We report here on the case of a 47-year-old woman with superfi cial masses in the breast and these masses had gradually increased in size during follow-up. They were confi rmed to be an eccrine spiradenoma on pathologic examination. There have been a few reports about the radiologic fi ndings of eccrine spiradenomas of the breast. This is the fi rst case of an eccrine spiradenoma in the breast that was characterized by multiple imaging modalities, including mammography, ultrasonography and MRI. The lesion in our patient was fi rst diagnosed as an epidermal inclusion cyst based on the imaging fi ndings and the mass’s superfi cial location. Although the mammographic and ultrasonographic imaging fi ndings of eccrine spiradenomas and epidermal inclusion cysts are similar, the MRI fi ndings are different between epidermal inclusion cysts and eccrine spiradenomas. Eccrine spiradenomas should be considered in the differential diagnosis of cutaneous and subcutaneous lesions of the breast.

        왼쪽 다리에 발생한 낭종형 에크린 나선종

        Eccrine spiradenoma is a benign tumor that is differentiated into the direction of an eccrine gland. It is usually presents as a solitary, slowly-growing, intradermal or deeply subcutaneous nodule on the head and trunk. We report on a case of 32-year-old woman with a solitary, solid mass on the left leg that she`d had for 6 months. The mass was a 2.5×2×0.5 cm sized, round shaped, well demarcated solitary skin-colored nodule. The mass in our case was a relatively large and painful eccrine spiradenoma, and the mass showed cystic formation. The cyst was surrounded by cords of tumor cells. Hyaline materials were seen focally. There were two types of cells. Large pale nuclei existed adjacent to the cyst. Small dark nuclei existed at the periphery. To clarify the histogenesis of this tumor, immunohistochemical staining using various antibodies was also performed. We herein report on an unusual case of a large cystic eccrine spiradenoma.       • A case of eccrine spiradenoma on the proximal interphalangeal joint area of left thumb

        Eccrine spiradenoma (ES) is an uncommon benign adnexal tumor of the eccrine sweat glands. ES clinically presents as a painful, slowly growing, erythematous to brownish subcutaneous nodule. It typically occurs on the head and trunk, but can also appears on the upper and lower extremities. ES arises mostly in middle-aged adults without sexual predilection. Histopathologic finding of the ES reveals multinodular sharply-circumscribed tumor in the dermis. The tumor is composed of two cell types, peripheral small basaloid cells with hyperchromatic nuclei and central large cells with pale nuclei, which may form pseudorosettes. Treatment of choice is surgical excision and recurrence is uncommon. A 44-year old woman presented with 20-year history of a painful 1.5 Ⅹ 0.7 cm sized brownish smooth surfaced subcutaneous nodule on the proximal interphalangeal joint area of left thumb. Histopathologic findings from the excised specimen showed sharply-demarcated tumor arranged in sheets in the dermis, which was composed of central pale large cells and outer hyperchromatic small basaloid cells. The patient was diagnosed with eccrine spiradenoma. Herein we report a rare case of eccrine spiradenoma on the left proximal interphalangeal joint area of thumb.

        Eccrine spiradenoma is a benign uncommon neoplasm of skin adnexa which probably originates in the coil of the eccrine sweat gland. It presents usually as a solitary, slowly growing, sometimes painful, reddish-brown, and intradermal or deeply subcutaneous nodule. Lesions can arise on the head and trunk but also in a variety of locations. Histopathologically, eccrine spiradenoma may consist of one large, sharply demarcated lobule or several lobules that are often appear deeply basophilic because of the dense packing of nuclei on low magnification. The patient was 32-year-old woman with small subcutaneous soft nodule on the arm for 2 years. Due to its tenderness, the patient wanted it removed, and 4mm punch biopsy had been obtained. Histopathological examination had revealed the characteristic features of an eccrine spiradenoma.

        Eccrine spiradenoma is an uncommon benign adnexal tumor of the eccrine sweat glands. It presents as a painful and slow-growing, solitary nodule on the head and neck, sometimes on the trunk. Malignant transformation is rare, presenting as rapid increase in size of a long-standing lesion. Although it can occur at any age, it occurs most commonly in young adults without any sex predilection. Herein, we report a case of eccrine spiradenoma of the scalp in old age. A 77-year-old man presented with slightly tender, solitary, 3 cm-sized, skin-colored, mushroom-like nodule on scalp for several years. Dermatological examination with punch biopsy was undertaken, followed by surgical excision of the lesion. Histopathological examination revealed a sharply demarcated nodular tumor in the dermis composed of sweat ducts and circular rosettes of basaloid cells without malignant transformation. The case was diagnosed as eccrine spiradenoma.

      • A case report of rare malignant eccrine spiradenoma

        Malignant eccrine spiradenoma (MES) is a rare adnexal tumour derived from cutaneous sweat glands. It usually presents as a small, firm, reddish painful and small solitary nodule. Head and neck are rare locations. MES arises over a prior benign spiradenoma. Clinical behavior is aggressive with a high rate of recurrences and distant metastases. Diagnosis is based on histological findings and treatment must be aggressive from the beginning to achieve the best results. Wide local excision and close follow up are crucial in the management of MES. A 76-year-old female visited our center for mass on the left cheek, which developed about 3 years ago. We removed the mass by local excision. Histologically, the mass consisted of benign eccrine spiradenomas adjacent to carcinomatous area showing nuclear pleomorphism and marked mitotic activity. Here we report a case of MES that arises in a very unusual location.

        눈꺼풀에 발생한 에크린 땀샘종 1예

        목적: 눈꺼풀에 매우 드물게 발생하는 에크린땀샘종 1예를 경험하였기에 보고하고자 한다. 증례요약: 61세 여자가 발생일자 미상의 우안 윗눈꺼풀종괴를 주소로 내원하였다. 우안윗눈꺼풀종괴 외에는 다른 안과적 이상은 없었다. 우안 윗눈꺼풀 경계의 가운데 부분에 3 mm 크기의 옅은 분홍색을 띠는 결절성종괴가 관찰되었고 표면은 매끈하였다. 완전 절제 생검을 시행하였고 조직병리 검사에서 경계가 뚜렷한 결절들이 표피로 둘러싸여 있었으며 상피성 세포들이 코드 형태로 배열되었다. 특히 두 가지의 특징적인 세포들 중에 작고 진한 핵을 가진 세포들은 주변부에 위치하였고, 핵이 크고 연한 세포들은 중앙부에 위치하고 있었다. 조직병리 검사결과가 에크린땀생종에 합당하여 에크린땀샘종으로 진단하였다. 결론: 눈꺼풀에 발생하는 에크린땀샘종은 드물지만 단독으로 생긴 눈꺼풀의 낭종성종괴의 감별진단으로 고려하여야 한다. Purpose: To report a case of eccrine spiradenoma that presented as an eyelid nodule. Case summary: A 61-year-old female presented with a mass in the right upper lid margin; the onset time was unknown. An oph-thalmic examination revealed no abnormalities except the eyelid nodule. The nodule was 3 mm in size with a firm, smooth sur-face and light pinkish color, and was located near the lid margin. Excision and biopsy of the nodule was performed. The patho-logical findings of the specimen obtained showed islands of 2 types of lobular cells. Larger, paler cells were grouped around the central area, darker and smaller cells on the periphery. These findings were compatible with eccrine spiradenoma. Conclusions: Eccrine spiradenoma should be considered a differential diagnosis of a solitary eyelid tumor.         Lymphangiectatic Variant of Low Grade Malignant Eccrine Spiradenoma

        Low-grade malignant eccrine spiradenoma (spiradenocarcinoma) is a rare sweat glandtumor, which usually arises from a pre- existing benign eccrine spiradenoma. This paperpresents the case of a 55-year-old male who had a lesion in his right elbow for 10 years. Themicroscopic examination revealed a well-demarcated, multilobulated tumor in the dermisand subcutis, which presented with many blood-filled vessels and extensive hemorrhage. The tumor was composed of hyperchromatic, round to oval cells with nucleolar prominence,mild to moderate atypia, and increased mitotic index. Additionally, lymphangiectaticappearance was observed in areas with prominent stromal lymphedema. P53 and Ki-67 hadhigh positivity. Surgical excision of the lesion was performed with adequate surgical margins,and the dissected lymph nodes in the axilla were tumor-negative. After 15 months offollow-up, there was no recurrence or distant metastasis.
