        시뮬레이션 실습교육이 간호대학생의 임상판단력, 자신감, 임상수행능력에 미치는 효과

        본 연구의 목적은 시뮬레이션 실습교육이 간호대학생의 임상판단력, 자신감, 임상수행능력에 영향을 미치는 효과를 파악하고자 실시되었으며 단일군 사전-사후 실험연구이다. 연구 대상자는 S지역에 소재한 간호학과 2학년 시뮬레이션 실습교과목을 수강한 45명의 학생으로 자료는 설문지를 이용하여 시뮬레이션 실습교육 전 · 후에 수집되었다. 연구결과 임상판단력, 자신감, 임상수행능력 모두가 통계적으로 유의미하게 향상되었다. 그러므로 시뮬레이션 실습교육이 간호대학생의 임상판단력과 자신감, 임상수행능력에 미치는 긍정적인 효과를 확인하였다. 추후 임상판단력, 자신감, 임상수행능력을 향상시킬 수 있는 시뮬레이션 실습교육의 모듈 개발 및 효과를 검증할 수 있는 연구가 필요하다고 사료된다. The purpose of this study was to test the effects of simulation practice education on clinical judgment, self-confidence, and clinical performance ability of nursing student. This study used a one group, pre-post test design. The subject of this study were 45 second grade nursing students who participated in the simulation practice education. Data were collected before and after the simulation practice education using the questionnaire. As a result of this research, the improvements of the clinical judgment, self- confidence, and clinical performance ability were statistically significant. Therefore, the simulation practice education has a positive effect on nursing students in terms of the clinical judgment, self-confidence, and clinical performance ability.

        수혈간호 시뮬레이션 교육에서의 구조화된 디브리핑 방법에 따른 효과

        연구의 목적은 수혈간호 시뮬레이션 교육에서 구조화된 디브리핑 방법에 따라 간호대학생의 지식, 임상수행능력, 임상판단력에 미치는 효과를 규명하기 위함이다. 대상자는 간호학과 2학년생으로 실험군과 비교군은 각각 16명, 대조군은 15명으로총 47명이었다. 본 연구는 비동등성 대조군 사후 시차설계로 실험중재는 2일간의 수혈간호시뮬레이션 중, 대조군에게 구조화된 그룹토론 디브리핑, 비교군에게 구조 화된 구두 디브리핑, 실험군에게는 구조화된 비디오 디브리핑을 각각 실시하였다. 자료수집은 교육중재가 끝난 다음 날 임상수행평가와 자가보고식 설문을 통해 실시하였다. 수집된 자료는 기술통계, χ 2 test/Fisher’s exact test, one way ANOVA, Sheffé test를 이용하여 분석하였다. 연구결과, 실험군의 임상수행능력, 임상판단력이 비교군과 대조군보다 유의하게 향상되었으며, 지식은 대조군보다 유의하게 향상 되었다. 결론적으로 구조화된 비디오 디브리핑을 활용한 수혈간호 시뮬레이션 교육은 간호대학생의 지식, 임상수행능력, 임상판단력을 증진하였으므로 시뮬레이션 간호교육에서의 효과적인 디브리핑 방법으로 활용할 것을 권장하는 바이다. The purpose of this study was to investigate the effects of knowledge, clinical performance, and clinical judgment of nursing students according to structured debriefing methods in nursing simulation education for blood transfusion. The research is non-equivalent control group non-synchronized post test design. The subjects of the study were 47 nursing sophomore students. The experimental group was structured video debriefing, the comparative group was structured oral debriefing, and the control group was structured group discussion debriefing. The clinical performance skills and clinical judgement ability on blood transfusion care were evaluated by two trained experts through videotapes which recorded clinical performance examination. The data were analyzed by descriptive statistics, χ 2 test, Fisher’s exact test, ANOVA, Scheffé test, Intra-Class Correlation, and Pearson s Correlation Coefficient using SPSS WIN 20.0 program. The results were as follows: The mean of knowledge, clinical competence, and clinical judgement ability of experimental group were statistically significant the highest scores among three groups. Therefore, the use of structured video debriefing in simulation training is strongly recommended to improve knowledge, clinical competence, and clinical judgment. Further studies might focus on identifying effective strategies of debriefing which might include development of standardized guidelines in various clinical examples applied to simulations.

        간호대학생들을 위한 시뮬레이션 교육프로그램 개발 및 적용효과

        Purpose. This study was conducted to identify effects of the high fidelity simulation for nursing students. Methods. A non-equivalent quasi-experimental design was used to experimental group (N=43) and control group (N=39) for 8 weeks, 5 times from September 26 to November 9, 2013. Clinical performance was measured by using standardized patients. Critical thinking was measured through the questionnaire and clinical judgement was measured through observation by Lasater's rubric. Results. Clinical performance (F=211.15, p<.001), critical thinking (t=3.00, p<.004) and clinical judgement (t=7.60, p<.001) were significantly higher in experimental group with simulation education program than control group. Conclusion. Simulation education program were effective to improve clinical performance, critical thinking disposition and clinical judgement for nursing students. 본 연구는 간호대학생들을 대상으로 임상 시나리오를 적용한 고충실도 시뮬레이션을 진행하여 교육효과를 확인하고자 시도되었다. 비동등성 대조군 전후설계의 유사 실험 연구로 실험군 43명, 대조군 39명의 연구대상자에게 2013년 9월 26일부터 11월 9일까지 8주 동안 총 5회에 걸쳐 진행되었다. 실험군과 대조군에게 표준화 환자를 활용하여 임상수행능력을 측정하고, 임상판단력은 Lasater의 rubric으로 관찰을 통해 측정하였으며 설문지를 통해 비판적 사고성향을 측정하였다. 수집된 자료는 SPSS 20.0 프로그램을 이용하여 분석하였다. 분석결과는 시뮬레이션 교육프로그램을 적용한 실험군이 대조군보다 임상수행능력 점수(F=211.15, p<.001), 비판적 사고성향 점수(t=3.00, p<.004), 임상판단력 점수(t=7.61, p<.001) 모두 통계적으로 유의하게 높게 나타났다. 결론적으로 간호대학생들을 위한 시뮬레이션 교육프로그램 개발과 적용은 반복된 토의 및 술기 연습과 디브리핑 과정을 통해 임상수행능력, 비판적 사고성향과 임상판단력을 증진시키는데 유의미한 효과가 있었다.

        간호대학생의 임상의사결정능력에 대한 영향요인

        The aim of this study was to determine the influence on nursing student's clinical decision making. The participants were 123 nursing students in D city. Data were collected with a structured questionnaire. The data was analyzed with SPSS/WIN 19.0 using Analysis of variance (ANOVA), t-test, Pearson correlation, and stepwise multiple regressions. Nursing student's clinical decision making had significant correlations with a college-life satisfaction(r=-.205, p=.023), nursing professional values(r=.239, p=.008), critical thinking disposition(r=.224, p=.013). Factors that had an influence on clinical decision making included aㅡ college-life satisfaction(unsatisfied ß=-.248, p=.006), religion(christian ß=-.233, p<.010), age(21-22 year ß=-.216, p=.012) and nursing professional values(ß=.167, p=.046). These factors explained 20.9% of the variances in student's clinical decision making. The results of this study revealed that nursing professional values were important intrinsic factors in the clinical decision making of nursing student. Thus, it is necessary to help them to attain nursing professional values for increasing their clinical decision making. 본 연구는 간호대학생의 임상결정능력에 영향을 미치는 요인을 알아보기 위해 시도된 서술적 조사연구이다. 대상자는 D시에 소재한 3개 간호학과 대학생 123명으로 자가보고 설문지를 이용하여 조사하였다. 자료분석은 SPSS/Win 19.0을 이용하였고 기술통계, Analysis of variance (ANOVA), t-test, Pearson correlation, and stepwise multiple regression으로 분석하였다. 연구결과 간호대학생의 임상의사결정능력은 윤리적 가치관(r=-.205, p=.023), 간호전문직관(r=.239, p=.008), 비판적 사고성향(r=.224, p=.013)과 상관관계가 있었다. 간호대학생의 임상결정능력에 유의하게 영향을 미치는 요인은 대학생활만족도(ß=-.248, p=.006), 종교(ß=-.233, p<.010) 연령(ß=-.216, p=.012), 간호전문직관(ß=.167, p=.046)으로 나타났다. 이러한 요인들의 간호대학생의 임상결정능력에 대한 설명력은 20.9%로 나타났다. 따라서 본 연구결과를 바탕으로 간호대학생의 임상결정능력 향상을 위한 교육 시 대학생활만족도, 간호전문직관을 고려할 필요가 있고 간호대학생의 임상결정능력 향상을 위한 간호중재 프로그램개발이 요구된다.

        시뮬레이션 기반의 실습교육이 간호학생의 비판적 사고성향, 임상 판단력 및 학습 성취에 미치는 효과

        This The purpose of this study was to determine the effects of simulation practice education on critical thinking, clinical judgment, and learning achievement of nursing students. A quasi-experimental research design (one group pre-test and post-test design) and a questionnaire for measuring critical thinking, clinical judgment, and learning achievement were used in this study. The participants were 35 students in a college of nursing. There were statistically significant increases in differences of critical thinking and learning achievement between pre-simulation practice education and post. There was statistically significant increase in differences of clinical judgment between first case simulation practice and second case. According to the findings of this study, it is necessary that repeat this study to verify the effects of simulation practice education and approve the effects according to debriefing process or method. 본 연구는 간호학과 4학년 재학생 35명을 대상으로 시뮬레이션 실습 교육이 비판적 사고성향, 임상 판단력, 학습 성취에 미치는 효과를 분석하고자 시도되었다. 본 연구 결과, 시뮬레이션 실습교육에서 학습 전보다 학습 후에 비판적 사고성향, 학습 성취가 모두 유의하게 높아진 것으로 나타났다. 임상판단력은 1번째 환자 사례기반의 시뮬레이션 실습과 디브리핑을 통해 성찰학습을 마친 후 2번쨰 사례기반의 시뮬레이션 실습 후에 더 향상되어 유의한 차이를 보였다. 본 연구의 결과를 바탕으로 시뮬레이션 연계 실습교육을 적용하고 그 효과를 검증하는 반복 연구가 필요하며 디브리핑 과정이나 방법이 시뮬레이션 실습교육의 학습 효과에 미치는 영향에 대해 검증하는 연구가 필요하다.

        간호대학생의 SBAR인수인계교육이 의사소통능력, 셀프리더십과 임상판단력에 미치는 효과

        본 연구는 전문직간의 의사소통역량 개발을 위한 간호대학생을 대학생으로 SBAR인수인계 교육이 의사소통능력 및 셀프리더십, 임상판단력의 효과를 검증하기위한 비동등성 대조군 전후 유사 실험 연구이다. 연구기간은 2019년 3월 2일부터 8월2일까지 진행하였고, 연구대상자는 G시의 H대학교 간호학과 4학년을 학생으로 실험군 34명 대조군 35명이었다. 실험군에게 SBAR인수인계 교육을 진행하였고, 대조군에는 일반 인수인계 교육을 실시하였다. 연구결과, SBAR인수인계 교육을 진행한 실험군이 대조군에 비해 의사소통능력에 향상이 있었고(t=1.912, p<.1), 셀프리더십에 향상이 있었고(t=1.685, p<.1), 임상판단력이 향상되었다(t=7.127, p<.001). 본 연구를 통해 4학년 간호대학생을 대상으로 한 인수인계 교육은 의사소통능력, 셀프리더십, 임상판단력에 도움이되는 것을 확인하였고, 보다 구조화된 의사소통인 SBAR인수인계교육이 더 효과적임이 확인이 되었다. 이러한 결과는 SBAR인수인계교육이 예비간호사인 4학년을 대상으로 환자의 인수인계의 가이드라인을 제공함으로 효과적인 인수인계 교육방법이 될 것이다. This study is nonequivalent control group pretest-posttest quasi-experimental study before and after the non-identical control group to verify the effectiveness of the communication skills, self-leadership and clinical judgment skills of the nursing student for the development of communication skills among professionals as a college student. The research period was conducted from March 2nd to August 2nd, 2019, subjects of the study were 34 experimental groups and 35 control groups as students of the department of nursing at H University in G City. In addition, the acquisition patient handover training was conducted to the experimental group using SBAR handover education and in the control group, general handover training was conducted using. The study found that the experimental group that conducted patient handover education using SBAR handover education had better communication skills than the control group (t=1.912, p<.1), improved self-leadership (t=1.685, p<.1), and improved clinical judgment (t=7.127, p<.001). Through this study, patient handover education for fourth grade nursing students was confirmed to be helpful for communication skills, self-leadership and clinical judgment, and SBAR handover education, a more structured communication, was found to be more effective. These results will be an effective method of SBAR handover education for fourth graders nursing students, who are prospective caregivers, by providing guidelines for patient handover.

        간호대학생의 시뮬레이션실습 기반 임상판단력과 흡인간호수행능력

        Purpose: This study was conducted to explore the relationship between simulation-based clinical judgment and performance ability for tracheal suction in nursing students. Methods: With a convenience sampling, 207 nursing students participated in this descriptive study. Lasater clinical judgment rubric was used for self-reported clinical judgment in addition to observe the skill of tracheal suction using a checklist. Data were analyzed by descriptive statistics, t-test and Pearsons correlation coefficients using the SPSS/WIN 22. Results: A scenario with pneumonia patient was developed to observe the skill of tracheal suction during simulation-based practices. Then self-reported clinical judgment was scored. The mean score of total sum of clinical judgment, total mean of clinical judgment, and performance skill were 36.44±4.82, 13.44±1.71, and 42.32±5.05, respectively. Statistically, students having good skills in suction showed significant differences in clinical judgment of interpreting (p=.031) compared to students having fair skills. Conclusion: The results of this study show that a structured debriefing method utilizing Lasater clinical judgment rubric is helpful. Also, simulation-based practice related to adult nursing in the respiratory system was useful for increasing the core basic skills among nursing students.

        시뮬레이션 기반 인수인계 교육이 간호대학생의 지식, 비판적 사고 성향, 임상 판단력에 미치는 효과

        Purpose Clinical nurses work in three shifts due to the specificity of the ward's work, and there is a handoff of nursing work for each shift. The nurse's handoff maintains the continuity of nursing care, ensuring the safety of patients and the quality of nursing care and medical care. Handoff is an important nursing activity that exchanges patient information, and it is the responsibility and duty of medical staff to accurately deliver this information. In addition, simulation-based education is an educational method that provides opportunities for clinical practice without having harmful results in nursing and medicine. This study was conducted to evaluate handoff knowledge, critical thinking disposition, and clinical judgment by developing and applying a simulation-based handoff education program for nursing college students. Method This study is a one-group pretest-posttest design to develop and operate a simulation-based handoff training program to verify its effectiveness. The subjects were 4th year students enrolled in the Department of Nursing at the College of Nursing in Gyeonggi-do, The data were collected from February 19 to March 2, 2018. For data analysis, SPSS Version 22.0 was used, and general characteristics were analyzed by numbers, percentages, averages, and standard deviations. Before and after participating in the educational program, the subject's handoff knowledge, critical thinking disposition, and clinical judgment were analyzed using a paired t-test. Results Thirty people participated in this study as subjects. All participants were female and the average age was 22 years. The main results of the study are as follows. There was a significant difference in handoff knowledge from 4.63±1.61 before education to 5.83±0.95 after education (t=-3.71, p=0.001). There was a significant difference from 3.39±0.46 before education to 3.75±0.42 after education (t=-4.69, p<0.001). There was a significant difference in clinical judgment from 1.79±0.37 before education to 3.15±0.48 after education (t=-15.67 , p<0.001). Conclusion As a result of this study, it was confirmed that the simulation-based handoff education program was effective in improving the handoff knowledge, critical thinking disposition, and clinical judgment of nursing students. Therefore, based on the results of this study, it is expected that it will be used as basic data for establishing an education system to improve the performance of handoff in the future. In addition, it is considered that it can be used for education for new nurses and idle nurses, contributing to the improvement of the quality of handoff in the future.
