        신경계 재활의 전통적 중재와 대안적 중재 방법에 대한 문헌 고찰

        This paper is a review conducted to provide an overview of the history of the past and contemporary rehabilitation methods for patient with damaged central nerves. Neurorehabilitation is a complex medical process which an aim to facilitate a recovery from a nervous system injury, and to minimize and/or compensate for functional impairments arising from it. By focusing on all aspects of a person's wellbeing, neurorehabilitation offers a series of therapies from the psychological to occupational, teaching or re-training the patients on mobility skills, communication processes, and other aspects of that person's daily routine. Current viewpoint represents a paradigm shift in clinical intervention, and clinicians have differently considered the details of intervention for the nervous and musculoskeletal impairments. Such concepts require to see the overall history of physical therapy, we interpreted the hidden meaning in the history of physical therapy. The current and future physical therapists will be to look at the patients as a comprehensive perspective. Medical instruments have continuously been developed. However, it may not be competed by senior physical therapists working for a long time in rehabilitation field. It’s probably useless to find only a way to treat a variety of patients with neurological impairments, representing optimal effects of rehabilitation, because definite answer to the therapy does not exist and their symptoms should be handled case-by-case. Thus, we need to take the advantages of functional evaluation, and they will be integrated into intervention.

        코로나19 범유행 상황에서 물리치료사의 위험인식, 안전환경, 직무스트레스, 우울감에 대한 연구

        The purpose of the study is to investigate the relationship between Covid-19 pandemic and physical therapists' risk perception, safety environment job stress, and depression. The number of subjects has been calculated using the G*Power 3.1.9.2 program and the final 386 data were analyzed. The research tool used structured anonymously self-filling questionnaire, consisted of 12 questions of general characteristics, 11 questions of risk perception, 21 questions of safety environment, 14 questions of job stress, and 16 questions of depression. The data were collected from a one-month-long online survey that was conducted in March 2021. The collected data were analyzed using an independent t-test, one-way ANOVA, Pearson correlation analysis. The mean score of risk perception was 2.58 points, safety environment was 3.52 points, job stress was 2.82 points, and depression was 1.66 points. There was a positive correlation between the risk perception and job stress, the safety environment and depression, and the job stress and depression. And there was a negative correlation between risk perception and depression, the safety environment and job stress. Based on the results of this study, cautious observation on physical therapists' awareness of safe working environment and infection, and exposure to psychological risks such as stress and depression would be steadily given in acute respiratory infectious disease situations.

        트레드밀 훈련과 Lee Silverman Voice Treatment(LSVT)-BIG 프로그램이 파킨슨병 환자의 균형 및 보행능력에 미치는 효과 : 단일사례연구

        This study aimed to investigate the effect of treadmill training and Lee Silverman Voice Treatment(LSVT)-BIG program on the balance and gait functions of a patient with Parkinson's disease. A participant was a person who suffered from idiopathic Parkinson's disease. We used a single-subject experimental research design with reversal phases (A-B-A). Experimental session was eight sessions in three phases (a total of 24 sessions): (1) baseline phase (A), (2) intervention phase (B), and (3) withdrawal phase (A'). During all phases, a participant underwent daily routine rehabilitation therapy, including mat exercise, flexibility training, and therapist-guided functional activity. Furthermore, treadmill training and LSVT-BIG was additionally performed for 70 minutes, once per day in the intervention phase, which consisted of warm-up (5 minutes), LSVT-BIG (30 minutes), treadmill training (30 minutes), and cool-down (5 minutes). During the intervention phase, the TUG and 10MWT scores improved by 3.75 seconds (36.89 seconds to 33.14 seconds) and 2.28 seconds (26.07 seconds to 23.79 seconds) in average, respectively. And gains were maintained after the intervention. Furthermore, the BBS and DGI scores improved by 17 points (4 points to 21 points) and 11 points (6 points to 17 points) in average, respectively. These findings suggest that treadmill training and LSVT-BIG may be helpful to enhance balance and walking functions in a person with Parkinson’s disease. Further studies need to support these results.

        젊은 성인의 백팩과 숄더백 착용에 따른 위등세모근, 척추세움근의 근활성도 비교

        The purpose of this study was to find out how each bag affects the muscle activity of the upper trapezius and erector spinae muscles when walking with a backpack and a shoulder bag. The study subjects were 25, and 17 males and 8 females were recruited. After walking from the 1st to the 6th floor with a backpack and a shoulder bag weighing 10% of the person's weight, walking 10m marked on the laboratory floor was performed. Surface electromyography was used to determine the muscle activity of both dorsal and vertebral pectoral muscles, and a pain sensor was used to determine the degree of pain. With PASW 18, the paired sample t test and one-way analysis of variance were used, and the Bonferroni correction was used for the post-test. There was a significant difference in the activity of the left and right muscles of the upper trapezius when wearing a shoulder bag and the difference in activity of the left and right muscles of the upper trapezius without wearing a bag. When worn, there was also a significant difference in the activity of the left and right muscles of the upper trapezius. There was no significant difference in the activity of the left and right muscles of the erector spinae muscle when the bag was not worn, and when the backpack and shoulder bag were worn. In addition, the erector spinae muscle showed consistently high muscle activity, unlike the upper trapezius, which means continuous muscle contraction of the erector spinae, which is a factor that can increase muscle fatigue. Therefore, it should be strengthened with a focus on the erect vertebrae rather than the upper trapezius. The results of this study inform those who wear shoulder bags that shoulder bags cause an imbalance in muscle activity of the upper trapezius muscle and may cause musculoskeletal problems. It is recommended to wear a backpack rather than a shoulder bag, and it will help to prevent the imbalance of the body.

        노인의 기능적 운동성 임상 평가도구 : 문헌고찰

        Aging, in general, leads to changes in skeletal, collagenous, and cardiovascular tissues, body composition, nervous and immune systems, as well as the hormonal axis. As a result, most elderly people have remarkably impaired aerobic capacity/endurance, joint mobility, muscle performance, balance as well as abnormal position and gait patterns, cognitive disorders and depression. Based on age-related changes in the elderly, clinicians use measurements of the following: aerobic capacity/endurance (the 6-min walk test), position (range of motion and thoracic kyphosis index), strength (the grip strength test), gait (the 10-m walk test and dynamic gait index), balance (the Berg balance scale, timed ‘up and go’ test, and functional reach test), cognition and depression (the mini-mental state exam, activities-specific balance confidence scale, geriatric depression scale), and functional activity (the functional independence measure, Katz index of independence in activities of daily living, and modified Barthel index), as standard clinical assessment tools to establish appropriate therapeutic exercise programs for successful rehabilitation. This review describes several clinical assessment tools and elucidates implementation guidelines for the assessment of functional mobility among the elderly in clinical settings. Moreover, this review provides valuable information for clinicians and researchers for exploring the systematic evaluation strategies and optimal therapeutic programs to enhance functional mobility in the elderly.

        중등도 목기능장애지수(NDI)를 가진 환자의 압력통증역치, 관절위치감각, 정적 균형 및 보행에 대한 관계 분석

        The purpose of this study was to determine the correlation between pain pressure threshold, joint position sense, static balance and gait in patients with moderate neck dysfunction index (NDI). A total 27 subjects with chronic neck pain participated in this study. A NDI survey was conducted to select subjects with moderate ability (NDI score: 15~24). The pressure pain thresholds, joint position senses, static balance(sway length and foot pressure balance), and gait were measured, and the correlation between the NDI and each measuring element was analyzed. As a result, NDI had a positive correlations with joint position sense (r=0.708), sway length (r=0.506), and foot pressure on the front left foot (r=0.453). Among them, the correlation between NDI and sway length had a strong positive correlation. Whereas, NDI had not correlations with pressure pain thresholds, and gait factors. These results indicate that moderate neck dysfunction can decreased static balance ability and joint position sense of neck. Although there was no significant correlation between NDI and gait, further research is needed to apply task with gait or identify on the neck pain patients with severe ability.

        신경근육전기자극을 결합한 앉기 서기 동작이 만성 뇌졸중 환자의 하부 몸통과 하지 근력, 정적 및 동적 균형 능력에 미치는 영향 : 교차실험연구

        Various methods of sit-to-stand (STS) training for chronic stroke patients have been devised in the clinical rehabilitation field, aiming at reinforcing muscle strength and recovering balancing ability, including improving weight-bearing capacity. However, there is little research on STS training using neuromuscular electrical stimulation (NMES), which is known to be effective in enhancing muscle strength and balancing ability. Hence, this study investigated the impact of STS combined with NMES on the muscle strength of the lower trunk and extremities and balancing ability in chronic stroke patients. They were randomly divided into 12 experimental groups (STS with NMES) and 12 control groups (only STS). Measurements were used before and after the intervention. All measurements significantly increased in both the experimental group and control group after the intervention, respectively (p<0.05). However, after the intervention, between the groups, the experimental group was not more statistically significant than the control group in all measurements (p<0.05). This study illustrated that STS with NMES may not differ concerning the muscle strength of the lower trunk and extremities and balancing ability in chronic stroke patients compared to the application of only STS.

        20대 여성의 월경곤란증 정도와 자율신경계 기능, 근속성 및 균형의 연관성

        The purpose of the present study was to determine the association between autonomic nervous system (ANS) activity, muscle properties and the dysmenorrhea degree. The degree of dysmenorrhea was evaluated by self-reported Menstrual Distress Questionnaire (MDQ). Also, muscle properties (tone, stiffness, and elasticity) was measured using Myoton Pro on the muscles related to pain by dysmenorrhea: rectus abdominis (RA), erector spinae (ES), rectus femoris (RF) and biceps femoris (BF) muscles. In addition, the activity of ANS was estimated through the heart rate variability (HRV) which is considered an index of the functional status of the ANS. The average of MDQ score of the subjects was 97.95±30.73, and the mean of LF/HF ratio which means sympathetic activity was 2.99±2.41 (range from 0.82 to 9.47). Particularly, there was a significant positive correlation between MDQ total score and the LF/HF ratio (r=0.466, p<0.05). Whereas, MDQ total score and HF indicating parasympathetic activity showed a significant negative correlation (r=-0.463, p<0.05). There was no significant association between dysmenorrhea and muscle properties, moreover no association with balance was identified. However, there was a significant association between muscle properties and balance of the subjects who experienced dysmenorrhea (n=17, MDQ≥70). The elasticity of RA showed a significant positive correlation with OSI (r=0.517, p<0.05) and APSI (r=0.795, p<0.01). Muscle tone in ES showed a significant negative correlation with OSI (r=-0.546, p<0.05) and APSI (r=-0.699, p<0.01), and stiffness in ES showed a significant negative correlation with APSI (r=-0.607, p<0.01). These results suggest that dysmenorrhea symptom could induce the increase of sympathetic activity, while reduction of parasympathetic activity.

        슬링을 이용한 상하지 협응 및 기능훈련프로그램이 파킨슨병 치매 환자의 인지, 신체수행능력 및 우울증에 미치는 영향 : 단일사례연구

        This study aimed to examine the effects of sling exercise program involving coordination and functional training on cognitive function and physical performance in two patients with Parkinson’s disease dementia. The reversal design (A-B-A') of single-subject experimental design was used. In the intervention phase, sling exercise involving coordination and functional training were performed for 70 min/day daily for 24 days. Outcomes included the timed up and go test and sit-to-stand test that were measured immediately after the intervention, and the Mini-mental state examination, Berg balance scale, and 10-m walk test, 6-min walk test, modified Barthel index, and geriatric depression scale that were assessed before and after the intervention. The cognition, balance and walking abilities, activities of daily living and depression were remarkably increased after the intervention. These results demonstrate that the sling exercise program involving coordination and functional training may be helpful to improve cognition, physical performance, and depression of individuals with Parkinson’s disease dementia.
