        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

In this research, It is presented and implemented an FMS(Facility Management System) for efficient management of the ground and underground facilities on a campus. This studies is computerized the drawings, protocols. and ledgers. It is used a GIS(Geographic Information System) to get the geographical information of facilities The data more efficiently could be search and update on this system . The decision making process understanding the present facility condition, and analyzing the condition of location could be performed easily by the GUI(Graphic User Interface) of the system

      • 임팩트시 테니스 라켓 재질의 차이에 따른 근전도 분석

        The purpose of this paper is to select 7 male athletes and female athletes of high school, and to compare their active potential differences of upper arm muscles according to characteristic of a racket. the results obtained are as follows: 1. According to the accumulative distribution rate of racket material, graphite and wood material show better efficiency in case of brachio radialis muscle of male athletes, but the racket material shows no constant efficiency in brachii radialis muscle of female athletes. In case of flexor carpi radialis muscle, wood material provides better efficiency only for male athletes. In case of triceps brachii muscle, wood material gives better efficiency to males athletes and steel material gives better efficiency to females athletes. Especially in case of biceps brachii muscle, neither male nor female athletes are influenced by racket materials at impact. 2. In the muscle load rate according to racket material, wood has the highest muscle active potential and graphite has the lowest active potential. Therefore, graphite is thoght to be better in the efficiency of muscle load rate.

      • 위장관 분석의 임상적 고찰

        저자는 1979년 1월부터 1989년 12월까지 경북대학교 병원 외과에서 치험하였던 위장관분석 32예에 대하여 임상적 관찰을 하여 다음의 결과를 얻었다. 1) 성별분포는 남:여가 1.5:1로 남자에게 조금 빈발하였으며, 년령은 비교적 고른 분포를 나타내었다. 2) 과거수술력은 16예(50%)에서 있었으며 그 중 위수술이 11예로 가장 많았다. 3) 분석의 위치는 회장이 13예(40.6%)로 가장 많았으며 그 다음 위(8예), 공장(5예), 위 및 회장(3예), 십이지장(2예), 그리고 위 및 공장(1예)의 순이었다. 4) 수술 전 진단이 된 예는 전체 32예 중 13예(40.6%)로서 내시경이나 상부위장관 조영술로 진단된 예가 11예, 단순복부촬영으로 진단된 예가 2예 있었다. 5) 분석의 갯수는 단일분석이 24예(75.0%), 다발성인 경우가 8예였다. 6) 치료는 전체 32예 중 자연통과가 2예, 단순절개에 의한 제거가 27예, 미주신경절단술 및 유문성형술이 2예, 미주신경절단술 및 위전정부 절제술을 1예에서 실시했으며 사망예는 없었다. A review of 32 patients with gastrointestinal bezoars was conducted. Male to female ratio was 1.5:1. Sixteen patient had provious history of laparotomy of whom 11 cases underwent gastric surgery. The bezoars located most frequently in the ileum(13 cases). Thirteen patient(40.6%) were correctly diagnosed preoperatively. Twenty four patient (75.0%) had single bezoar and 8 cases had multiple in number. Enterolithotomy was performed in 27 cases, Vagotomy and pyloroplasty in 2 cases, and Vagotomy and antrectomy in 1 case for the treatment.

        발레 숙련도에 따른 센터에서 Grand Battement Jete´ a´ la seconde 동작의 운동학적 비교 분석

The purpose of this study was to investigate time of the phase, angle of the right ankle, knee, and hip joint, lateral angle of the trunk, mediolateral displacement of COM, and vertical displacement of COM between two groups while executing grand battement jets ?la seconde in a center exercise setting through 3D video analysis. The subjects participated in this study were skilled and unskilled 6 female ballet majors in Busan, respectively. The conclusions are as follows: 1. The time of the phase 2 was faster than P3. It shows a significant difference(p<.05) for P1 and P4 between skilled and unskilled groups. 2. The angle of the right ankle joint has a significant difference(p<.05) at E4 between skilled and unskilled groups. The angle of the right knee joint has no significant difference at all events between skilled and unskilled groups. The angle of the right hip joint has a significant difference(p<.001) at E3 between skilled and unskilled groups. 3. The lateral angle of the trunk has a significant difference(p<.05) at El and at E5 between skilled and unskilled groups. The skilled group of the lateral angle of the trunk was lower than the unskilled group. However the skilled group's lateral angle of the trunk was bigger than the unskilled group at E3. It has significant difference(p<.001) at E3 between skilled and unskilled groups. 4. The mediolateral displacement of COM has no significant difference at all events between skilled and unskilled groups. The vertical displacement of COM has a significant difference(p<.01) at E3 between skilled and unskilled groups.

      • 중량물 운반시 잡기유형에 따른 근전도 분석

        The purpose of this study was to examine EMG changes of the body according to method the holding during carrying object. The experiments were conducted with utilizing EMG analyses for five male and five female adults as subjects. The results were as followed. 1. The higher muscle action potential of vertical holding was shown than that of right holding when carrying objects in MMH work in proportion to the weight of the object. The came from the subjects using their brachial muscles much to maintain the angles of their wrist joints and elbow joints for keeping their stable postures while walking. 2. The higher muscle action potential of right holding was shown in right and left elector-muscles of their spines(L1-L2, L4-L5) while carrying in all stages of weights except 25kg objects than that of vertical holding. These results were caused from more use of brachial muscles than elector-muscles of spines in carrying weight objects. So vertical holding is more effective in carrying weight objects at a short distance than right holding.

        Relationship of the Lodging of Corn Hybrids and Wet Stress in Field Condition

        포장조건에서 22종의 옥수수교잡종을 과습조건에서 도복관련형질들의 상호관련성을 검정하였는데 좌절중 (1.0 m, 1.5 m)과 좌절부위등을 평가하였고 주요한 도복 저항성 형질인 좌절중, 간직경, 뿌리건물중, 중심고등의 상호 관련성을 함께 평가하였다. 본시험은 포트시험에서 일차 스크린 한 것을 논에 경사지를 만들어 경사지별로 수분함량과 도복저항성을 비교하기 위하여 관수시기(과습조건)를 파종후 20일, 5엽기, 출웅기, 출사기로 구분하였고 관수량은 100 mm/일 씩 5일간 처리하였으며 조사는 수확기에 수행 하였다. 본시험의 결과 출사기에 1.0 m, 1.5 m 높이에서 좌절중과 중심고는 정의 상관을 보였으며 파종후 20일차에 관수처리구에서는1.0m, 1.5m 모두 좌절강도와 간직경이 정의 상관을 나타냈다. 관수처리 5엽기에서는 높이 1.0 m에서 좌절강도와 간직경간의 강한 정의 상관을 나타냈으며 도복관련 형질인 간직경, 뿌리건물중,지상부 중심고, 좌절중, 좌절고등이지만 관수처리시 이들을 가장쉽게 판별 할 수 있는 방법은 5엽기에 관수처리구가 가장 효율적으로 사료된다. Under field conditions, 22 corn hybrids(CHs) were tested for several characteristics related to lodging resistance and excessive water stress, such as pushing resistance on 1.0 and 1.5 m hights of the corns and breaking point. The 22 corn CHs were selected because they showed strongest pushing resistance and breaking resistance of culm as well as the highest thickness of basal culm, leaf greenness, leaf area, and plant height based on two pre-pot experiments performed in phytotron. To make excess water condition, water irrigation was done at 1) 20-d after seeding, 2) 5th leaf stage, 3) tasseling stage, and 4) silking stage with 100 mm/day for 5 days. Lodging-related characteristics were measured: culm diameter, root dry weight, weight balance of above-ground plant, breaking resistance, and breaking height in field experiment. Among all the characteristics tested, we explained in the two critical points for lodging resistance: 1) breaking resistance and breaking point, 2) culm diameter with breaking resistance and breaking point, and they were further evaluated for the relationships with the 1.0 and 1.5m height of above-ground plant. In the corn hybrids, there was a positive correlation between breaking resistant and weight balance of corn at the silking stage water treatment at 1.0 and 1.5-m height. And breaking resistant and culm diameter had a positive relationship at the 20-d water treatment at both measuring height. In conclusion, selection for lodging resistant hybrids or germplasms to excess soilwater should be made at the 5th leaf stage water treatment, and basal culm thickness should be considered to evaluate the resistance of the corn hybrid plants to lodging.
