      • 선인장 양심실 보조장치의 설계 및 임상적용을 위한 평가

        현재 개발되고 있는 맥동형 이식형 인공심장은 완전이식형 인공심장과 좌심실보조장치 뿐이다. 좌심실보조장치를 장착한 환자의 10~15%가 우심실의 보조를 필요로하고 완전이식형 인공심장의 장착을 위해서는 자연심장을 제거해야하는 상황에서 이식형 양심실보조장치에 대한 필요성이 대두되고 있다. 본 연구진은 완전이식형 인공심장으로 개발된 한국형 인공심장을 개선하여 양심 실 보조장치를 개발하였다. 양심실보조장치는 이동형 작동기식 미케니즘을 이용하고 있으며 에너지 변환장치와 감속기로 구성된 작동기와 혈액주머니, 그리고 내장형 제어기로 구성되어 있다. 선인장 펌프로 명명된 KAH350은 캐뉼라의 연결을 위한 커넥터를 포함하여 길이 177mm, 폭 164mm, 높이 67 이고 무게 780g이며 최대 심박출량은 5L/min 이다. 양심실보조장치로 개발된 선인장펌프는 좌심실보조장치로도 응용이 가능하다. 이식적합성과 생체적합성을 평가하기 위하여 좌심실보조장치로 5회, 양싱실보조장치로 6회의 동물실험을 수행하였다. 각각 최장 28일간 생존하였으며 11회의 동물실험중 장치의 결함에 의한 것이 3회 있었는데 이들은 모두 전자장치의 결함과 방수처리문제였다. 현재 이식적합성과 일박출량을 향상시킨 KAH400모델이 개발중이다. The types of pulsatile implantable artificial hearts that are on the way of development and have been developed are totally implantable artificial heart and implantable left ventricular assist device (LVAD). Approximately 10% to 15% of all patients Implanted with wearable VADs have required right heart support with another device. And it is reluctant to patient who should remove his or her own heart to be implanted with total artificial heart. These situations drive the development of implantable bi-ventricular assist device (BVAD). The Korean BVAD was develophed by modifying the moving actuator type Korean artificial heart. This electro-mechanical BVAD comprises actuator including energy converter and reduction gear train, blood sacs, and internal motor and energy controllers. The KAH350 which is named as 'Cactus Pump' is 177mm in length, 164mm in width, and 67mm in thickness including connectors and nuts. The weight and maximum cardiac output of Cactus Pump is 780g and 5L/min, respectively. The Cactus Pump that was developed as BVAD could be used as LVAD by attaching compliance caps on the 2 ports of one ventricle. The animal experiments were undergone 5 times for LVAD, and 6 times for BVAD. The best records were 28 days survival in both applications. There were 3 times of device failure and they were all associated with electrical connection and hermetic sealing. The KAH400 that has improved anatomical fitting characterisitcs and stroke volume is on development.

        Anesthesia for ovarian teratoma resection using remimazolam and remifentanil in a patient with anti-N-methyl-D-aspartate receptor encephalitis -two case reports-

        Background: Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis is a type of autoimmune encephalitis that causes characteristic symptoms through the formation of antibodies against NMDA receptors. If ovarian teratomas are detected, surgical removal under general anesthesia is often considered. Many general anesthetic agents inhibit NMDA receptors. As such, anesthetic agents may have unexpected effects on disease progression. For anesthesiologists, providing general anesthesia for these patients is challenging and there are few studies on which anesthetic is most appropriate. Case: Two female patients were diagnosed with anti-NMDA receptor encephalitis and ovarian teratomas. Successful teratoma resection was performed under general anesthesia using remimazolam and remifentanil. After the surgery, one patient showed some improvement but died a month later. The other patient progressively improved over time. Conclusions: Remimazolam and remifentanil are useful general anesthetic agents for patients with anti-NMDA receptor encephalitis. Further studies are warranted.

        Background: Macrophage Migration Inhibitory Factor (MIF) is known as pro-in- fi ammatory and pro-oncogenic cytokine. Its role in gastric cancer has been revealed recently. We evaluated the relationship between MIF and H. pylori in the gastric carcinogenesis. Methods: Human gastric tissue samples from 522 patients were subdivided into 6 groups according to H. pylori infection status and pathologic diagnosis (cancer, dysplasia, or control). Tissue MIF mRNA level was measured by real-time PCR, shown as 2^delta-delta CT (2^ddCT). Results: Gastric cancer tissue expressed signifi cantly higher level of MIF mRNA (n=214, mean±SE; 9.16±1.5) than non-cancer counterpart (n=308, 4.22±0.9, p=0.005). There was no signifi cant difference in MIF mRNA level between H. pylori negative and positive tissues. In subgroup analysis, H. pylori-positive cancer tissue showed signifi cantly higher MIF expression (n=137, 10.27±2.1) than that of H. pylori-positive dysplasia (n=55, 1.91±0.5, p<0.05) or H. pylori-positive control (n=113, 3.29±0.6, p<0.05). However, no such fi nding was noted among the H. pylori-negative cancer/dysplasia/control group. (fi gure) Conclusions: Tissue MIF mRNA level showed signifi cant difference between gastric cancer and dysplasia only in H. pylori-positive group. From these results we can conclude that MIF could be an important factor in the H. pylori induced gastric carcinogenesis. Now the experiments regarding underlying mechanism are undergoing.

        Are pet foods safe? From perspective of mycotoxins, preservatives and antioxidants

        The aim of this study was to determine the contamination of mycotoxins and the concentration of preservatives and antioxidants in commercial pet food. 106 pet foods were purchased from online in Korea. Mycotoxin analysis were performed using liquid chromatography coupled to tandem mass spectrometry (LC-MS/MS) and preservatives and antioxidants were analyzed by high performance liquid chromatography (HPLC). The contamination of the mycotoxins in all samples was proved to comply with the Korea legislation with regards to animal feed, the analyses revealed presence of aflatoxins, ochratoxin A, zearalenone and fumonisins, with values below 1.6 ㎍/㎏, 12.1 ㎍/㎏, 478.5 ㎍/㎏ and 1873.5 ㎍/㎏, respectively. Furthermore, the simultaneous presences of different mycotoxins were observed in most of positive samples. The levels of dehydroacetic acid (DHA), sorbic acid (SA) and benzoic acid (BA) were, respectively, with values below 0.24 g/㎏, 4.82 g/㎏ and 6.35 g/㎏. The concentrations of butylated hydroxy anisole (BHA) and butylated hydroxy toluene (BHT) were demonstrated to be acceptable, with values below 0.04 g/㎏, and 0.02 g/㎏. This indicated the need for further investigation into the potential risk deriving from chronic exposure to low doses of mycotoxins. Additionally, a co-contamination of mycotoxins which interact in synergic manner are more concern. As DHA that not accepted was detected and very high levels of SA were identified in samples, an upper limit for preservatives in pet foods should be established in pet foods. BHA and BHT were detected within the recommended levels for antioxidant content, so these are considered as safe.

        <P>Neuronal SNARE proteins mediate neurotransmitter release at the synapse by facilitating the fusion of vesicles to the presynaptic plasma membrane. Cognate v-SNAREs and t-SNAREs from the vesicle and the plasma membrane, respectively, zip up and bring about the apposition of two membranes attached at the C-terminal ends. Here, we demonstrate that SNARE zippering can be modulated in the midways by wedging with small hydrophobic molecules. Myricetin, which intercalated into the hydrophobic inner core near the middle of the SNARE complex, stopped SNARE zippering in motion and accumulated the trans-complex, where the N-terminal region of v-SNARE VAMP2 is in the coiled coil with the frayed C-terminal region. Delphinidin and cyanidin inhibited N-terminal nucleation of SNARE zippering. Neuronal SNARE complex in PC12 cells showed the same pattern of vulnerability to small hydrophobic molecules. We propose that the half-zipped trans-SNARE complex is a crucial intermediate waiting for a calcium trigger that leads to fusion pore opening.</P>

      • Electrochemical Properties of Li Metal Batteries with P(PEGMA)-Coated Lithium Trivanadate Cathode and Li Powder Anode

        <P>Ionic conductive poly(poly(ethylene glycol) methyl ether methacrylate) (hereafter polymer) was coated onto a lithium trivanadate (LiV3O8) cathode by using an electrostatic spray deposition method in an effort to improve the ionic conductivity and prevent dissolution of vanadium into the electrolyte. The coating layer on the LiV3O8 electrode was identified by Fourier transform infrared spectroscopy, field emission scanning electron microscopy and transmission electron microscopy. Polymer was uniformly coated on the LiV3O8 electrode with a thickness of about 40 nm. Lithium powder, synthesized by the droplet emulsion technique, was used as the anode. Electrochemical charge-discharge tests showed that the cell based on the polymer-coated LiV3O8 cell exhibited an initial specific capacity of 186 mAh/g and the capacity retentions of the polymer-coated LiV3O8 after 100cycles was 87%, which is far improved that of uncoated cell (65%). Also, the coated cell showed 81% of the capacity retention after 300 cycles at 2 C-rate. These results demonstrate that the polymer coating on LiV3O8 is effective for suppressing capacity fading at high C-rate.</P>

        최근 6년 동안 경북대학병원에 입원하여 종양 절제술을 받은 남, 여 52명의 두개내에 발생된 수막종 환자에 대하여 임상적으로 분석검토하였다. 남, 여의 발생비는 1대 2.3이었으며, 40∼50세 연령층에서 가장 많았다. 천막 상부에서의 발생이 88.5%로서 대뇌 궁륭부, 접형골연부 및 시상동 인접부의 순으로 발생되었으며 천막 하부에서의 발생은 7.7%로서 주로 소뇌교각부이었다. 종양의 크기는 최장경이 3-5㎝가 가장 많았으며 조직 소견으로서는 Meningotheliomatous type이 가장 많았고 다음이 Fibroblastic type이었다. 방사선학적 소견으로서는 두개골에 변화를 일으킨 것이 48.1%, 뇌혈관 조영술상에서 종양상을 나타낸 것이 63.4%이었으며, 전산화 단층촬영상에서는 고밀도상이 82.2%, 저밀도상이 2.2% 그리고 석회침착이 15.4%였다. 또한 동반된 뇌부종은 대개 Grade Ⅰ과 Ⅱ로서 78.7%였다. 증상 출현시로부터 절제술시까지의 기간은 2년이내가 50.0%였다. 종양의 완전절제는 80.8%에서 가능하였으며 재발의 가능성이 없는 것은 75.0%였다. 사망율은 1.9%였으며 이환율은 19.2%였다. Clinical studies and analyses have been made on 52 male and female patients with intracranial meningioma, admitted to department of neurosurgery, Kyungpook National University Hospital during past 6 years. The ratio of male and female was 1 to 2.3, and 40-50 age bracket was most frequently affected. The meningioma occurred above tentorium was 88.5% of all, with cerebral convexity (32.7%) as the most common site followed by sphenoid ridge (15.4%), parasagittal (11.6), Parasellar region (7.7%) The one occurred below tentorium accounted for 7.7%, mainly at the cerebellopontine angle (5.8%). The most common size of the tumor was 3 to 5 ㎝ in diameter (30.8%). The most frequent type of tumor was meningotheliomatous, followed by fibroblastic type in the histological classification. Abnormal findings on the skull by the tumor covered 48.1% on plain films. The tumor blushing was 63.4% on cerebral angiograms. In the brain CT scanning without contrast enhancement, hyperdensity of the tumor covered 82.2%; hypodensity-2.2%; calcification-15.4%. The peritumoral edema was noted in 78.7%, mainly with grade Ⅰ and Ⅱ in the degree. The interval between the onset of symptoms and time of the operation was not more than 2 years in 50% of patients. Total removal of the tumor was possible in 80.8% of the whole cases, and the operative findings indicated a possible rate of recurrence as 25%. The rate of morbidity and mortality was 19.2% and 1.9% respectively.

      • 고립성 폐 결절로 나타난 경화성 혈관종의 폐엽 절제술에 의한 치험 1예 : A case report

        Patient was sixty-eight years old female. She was transferred to our department to resect the solitary pulmonary nodule on right lower lobe. Intraoperative frozen-section revealed bronchioloalveolar carcinoma. So, we performed right lower lobectomy and mediastinal lymph node dissection. Permanent pathologic diagnosis of this patient was sclerosing hemangioma. We report experience of treatment of sclerosing hemagioma presenting solitary pulmonary nodule by lobectomy with the review of literatures.
