고등학생들의 음란물 접촉실태 및 관련요인

본 연구는 성에 민감한 청소년들의 음란물 접촉 실태를 알아보기 위하여 1998년 11월 30일부터 12월 30일까지 경상남도 J시 소재 고등학교 2학년 학생 732명을 대상으로 설문 조사 분석한 결과는 다음과 같다. 음란물 접촉실태는 대상자가 가장 많이 접촉하는 음란물은 성인용 비디오(76.9%)이었고 성인용 만화(70.1%), 성인용 주간지(55.9%), 음란 컴퓨터 프로그램(51.2%), 포로노 비디오(50.3%), 포로노 잡지(34.4%) 및 포로노 사진집(28.1%) 순이었다. 인문계 학생이 가장 많이 접촉하는 음란물은 성인용 만화이었고, 실업계 학생은 성인용 비디오였으며 남학생이 여학생보다, 실업계 학생이 인문계 학생보다 음란물 접촉 경험이 더 많았다.(p<0.01) 음란물 접촉 시 특성을 보면 대부분 학생이 "중학교"때 처음 접촉했으며 접촉경로응 "친구를 통해서"가 가장 많았고, 접촉이유는 "호기심 때문에"가 가장 많았다. 제반 변수들에 따른 음란물 접촉과의 연관성을 알아본 결과 자위행위, 성 경험, 이성교제, 학생성적, 흡연, 음주는 모든 음란물의 접촉과 상관성이 있는 것으로 나타났다.(p<0.01) 음란물 접촉 유무를 종속변수로 하여 다중 로지스틱 분석을 한 결과는 남자일수록, 자위행위를 할수록, 학교 성적이 낮을수록, 담배를 피울수록, 술을 마실수록, 외향성일수록 음란물을 보는 경험이 더 많았다.(p<0.01) 이상 결과들을 볼 때, 청소년들을 음란물로부터 보호하기 위해서는 성적충동을 올바르게 표출하고 승화할 수 있도록 개개인의 취향에 맞는 운동이나 취미활동을 권장하고 이를 개발할 수 있도록 도와주어야 할 것이며 유해환경에 대한 규제를 강화하여 학생들이 살고 잇는 사회의 성문화 정화에 모두 관심을 가져야겠다. This study was conducted to examine the sex-sensitive adolescents' actual condition of contact with lecherous things from November 11th to December 20th, 1998 with 732 high school sophomores in Chinju, Kyungsangnam-do. And the results are as follows. According to the actual condition of contact with lecherous things the most lecherous thing subject group came into contact was adult videos(76.9%) and adults cartoons(70.1%), adults magazines(55.9%), lecherous computer programs(51.2%), porn videos(50.3%), porn magazines(34.4%) and porn photo collections(28.1%) in highest order. The most lecherous thing academic high school students came into contact was adults cartoons and that of business high school students was adults videos and contact experiences to it of male students and business school students were higher than those of female students and academic school students respectively(p<0.01%). In view of students' characteristics of contact with lecherous things, most students made first contact with them in their middle school years and from their friends curiosity. In the result of study of relevance to the contact with lecherous things due to all sorts of variables, it shows that masturbations, sex experiences, intersexual acquaintances, school records, smoking and drinking are related to the all contacts with lecherous things(p<0.01). In the result of Multiple Logistic Analysis with the presence of contact with lecherous things as subordinate variable, the more they are men, doing masturbations, the person with bad school records, smoking, drinking and extroversive the more they are inclined to contact with lecherous things(p<0.01). To protect adolescents from the access to lecherous things, in view of above results, we have to encourage and help adolescents to enjoy and develop sports or hobbies of their own to express and purify their sexual impulses right. And we also have to concern the purification of sexual culture of the society in which students live by strengthening our regulatory systems for unsound circumstances.

DNA repair gene polymorphisms and benefit from gefitinib in never‐smokers with lung adenocarcinoma

<P><B>Abstract</B></P><P><B>BACKGROUND:</B></P><P>The objective of this study was to investigate whether polymorphisms in DNA repair genes affect clinical outcome of never‐smokers with lung adenocarcinoma (NSLA).</P><P><B>METHOD:</B></P><P>Common polymorphisms in the DNA repair genes ribonucleotide reductase M1 (<I>RRM1</I>), excision repair cross‐complementation group 1 (<I>ERCC1</I>), and x‐ray repair cross‐complementing group 1 (<I>XRCC1</I>) were genotyped in DNA samples from 158 patients among 313 NSLA who were randomized to receive either gefitinib or gemcitabine plus cisplatin (GP) as first‐line therapy. Immunohistochemistry for ERCC1 (n = 38) and direct sequencing of the epidermal growth factor gene (<I>EGFR</I>) (n = 42) were performed using tumor samples.</P><P><B>RESULTS:</B></P><P>Patients who had the <I>XRCC1</I> arginine (Arg)/Arg polymorphism at codon 399 (399Arg/Arg) had a higher response rate to gefitinib (71% vs 36%; <I>P</I> = .002) and had more <I>EGFR</I>‐mutant tumors (82% vs 29%; <I>P</I> = .001) than patients who had the glutamine (Gln) allele. Patients who had the <I>ERCC1</I> adenine‐adenine (AA) polymorphism at codon 8092 (8092AA) had a higher response to GP than patients who had the cytosine‐cytosine (CC) or the CA genotype (100% vs 44%; <I>P</I> = .043).When gefitinib was compared with GP, significantly longer progression‐free survival (PFS) was observed with gefitinib among patients who had the <I>XRCC1</I> 399Arg/Arg genotype (7.5 months vs 6.6 months; <I>P</I> = .013), the <I>RRM1</I> 2464 guanine‐guanine (GG) genotype (11.5 months vs 6.0 months; <I>P</I> = .004), and the <I>ERCC1</I> 8092CA genotype (7.5 months vs 6.4 months; <I>P</I> = .024). When the 3 genotypes were analyzed jointly, significantly longer PFS was observed with gefitinib among patients who had ≥2 genotypes (8.1 months vs 6.4 months; <I>P</I> = .009), whereas a trend for longer PFS was observed with GP among patients without the 3 genotypes (6.3 months vs 2.0 months; <I>P</I> = .06). In a multivariate Cox regression model, the greater number of specific genotypes independently predicted improved overall survival (hazard ratio, 0.5; 95% confidence interval, 0.3‐0.8; <I>P</I> = .006).</P><P><B>CONCLUSIONS:</B></P><P>Patients with the <I>XRCC1</I> 399Arg/Arg, <I>RRM1</I> 2464GG, and <I>ERCC1</I> 8092CA genotypes did benefit from gefitinib. Joint Replacement Surgery in Patients with Rheumatoid Arthritis in South Korea: Analysis of a Large National Database

Background: We aimed to investigate the current trend of joint replacement surgery incidence in patients with rheumatoid arthritis (RA) in South Korea and to compare the incidence of joint replacement surgery in each affected joint. Methods: We performed this big data analysis to investigate the current trend of joint replacement surgery incidence in patients with RA in South Korea and to compare the incidence of joint replacement surgery in each affected joint. This retrospective study was based on data from the Korea National Health Insurance claims database. Results: The prevalence of RA increased every year (0.13% in 2008, 0.25% in 2016). The number of newly diagnosed patients increased from 29,184 in 2010 to 38,347 in 2016. The incidence rate of joint replacement surgery in patients with RA increased from 0.72% in 2010 to 4.03% in 2016. The knee (68.3%) was the most commonly replaced joint. The relative risk (RR) of additional joint replacement surgery was highest for the shoulder joint (RR,1.454; 95% confidence interval, 0.763–2.771). The median time from diagnosis to surgery was the shortest in the elbow joint (379 days) and the longest in the shoulder joint (955 days). The median time for each joint was short in order of the elbows, ankles, hips, knees, and shoulders (p < 0.01). Conclusions: The most frequently and initially replaced joints were different, but the prevalence and incidence of RA, as well as those of joint replacement surgery, have recently increased in South Korea. Joint replacement surgery in RA was the highest for the knee joint. The median time from diagnosis to surgery was shortest for the elbow, followed by the ankle, hip, knee, and shoulder. Regardless of whether patients are symptomatic, evaluation of large joints such as the knee, elbow, ankle, and hip should be considered from an early stage.

Case Reports : Rupture of the Deep Femoral Artery during Proximal Femoral Nailing Following an Intertrochanteric Fracture: A Case Report

Recently, we experienced a case where the diagnosis and management of a deep femoral artery rupture was delayed. This vascular complication occurred during the insertion of a distal interlocking screw of a proximal femoral nail for the fixation of an intertrochanteric femur fracture. A 79-year-old male patient was diagnosed with a right intertrochanteric fracture after a fall. We fixed the fracture with a proximal femoral nail (Zimmer.. Natural NailTM System). One day after the procedure, the patient complained of pain and swelling on the anteromedial side of his middle thigh followed by hypotension, anemia and prolonged thigh swelling. Computed tomography angiography was performed 7 days after the procedure. We found a pseudoaneurysm of the perforating artery caused by injury to the deep femoral artery and an intramuscular hematoma in the anterior thigh muscle. We successfully treated the pseudoaneurysm using coil embolization. Throughout the management of intertrochanteric femoral fractures, it is important to be aware and monitor signs and symptoms related to the possibility of blood vessel damage. When a patient presents with swelling and pain on the middle thigh and/or unexplained anemia postoperatively, the possibility that these symptoms are caused by an injury to the femoral artery must be considered.

폐흡충증과 연관된 급성 호산구성 전격성 심근염 1예

전격성 심근염은 급성기에 높은 사망률을 보이나 약물치료나 기계적 순환 보조장치로 회복되기만 하면 급성 심근염 환자에 비해 장기 생존률은 좋은 것으로 알려져 있다. 현재까지 국내에서 기생충 질환에 의한 과민반응에 기인하여 양호한 경과를 보였던 호산구성 심질환은 보고 되었으나 폐 흡충증과 연관되어 호산구수가 증가하고 증상 시작 2일만에 혈압저하, 호흡곤란등 울혈성 심부전 경과를 보이는 급성 호산구성 전격성 심근염을 국내에서는 처음으로 경험하여 문헌고찰과 함께 보고하는 바이다. Fulminant myocarditis is characterized by critical illness at presentation. However, if affected patients recover with pharmacologic therapy and mechanical circulatory support, they may have a better long-term prognosis than patients with other forms of myocarditis. A 31-year-old man was admitted due to chest pain associated with dyspnea. Electrocardiogram showed ST-segment elevation in all leads except for aVR and aVL. Non-sustained ventricular tachycardia developed 12 hours after admission. The echocardiography showed diffuse hypokinesia, concentric edematous thickening of the left ventricular wall, and pericardial effusion. Serum cardiac enzymes and absolute eosinophil count were elevated. Since the ELISA (Enzyme-Linked Immunosorbent Assay) against Paragonimus westermani was positive, the patient was treated with praziquantel for 2 days. Eosinophil count normalized after 10 days, with conversion to negativity on ELISA after 4 weeks. We concluded that his myocarditis was probably caused by allergic reaction secondary to Paragonimus westermani infection.(Korean J Med 74:451-456, 2008)

슬관절에 발생한 결핵성 관절염으로 오인한 활막성 연골종증

활막성 연골종증은 관절 활액막으로부터 기원하는 증식성의 연골성 결절 양성 종양이다. 활막성 연골종증 환자의 진단에 있어서 영상의학적 소견이 중요한 부분을 차지하고 이는 다양한 모습으로 나타나는데 결핵성 관절염, 류마티스 관절염 등과 오인하게 되는 경우가 다수 있다. 자기공명영상 검사상 결핵성 관절염과 류마티스 관절염에서 흔히 보이게 되는 쌀소체(rice body)와 활액막성 연골종증의 소견이 유사하게 나타나는 경우가 많다. 영상소견상 결핵성 관절염 또는 류마티스 관절염으로 오인한 편측 무릎에 발생한 원발성 활막성 연골종증 1예에 대해서 보고한다. Synovial chondromatosis is a proliferative chondrogenic benign tumor originating from the synovial membrane of the joint. Radiological findings occupy an important part in the diagnosis of patients with synovial chondromatosis, which appear in various forms, and are often misunderstood such as tuberculous arthritis and rheumatoid arthritis. In many cases, the findings of rice body, which are common in tuberculous arthritis and rheumatoid arthritis, and synovial chondromatosis are similar on magnetic resonance imaging. A case of primary synovial chondromatosis that was mistaken for tuberculous arthritis or rheumatoid arthritis by imaging findings is reported.

The isothermal oxidation behavior of W-Mo-(29~48wt%)Cr alloys were investigated at 1000, 1100 and 1200℃ in air. The oxidized layers and their morphologies were analyzed by scanning electron microscopy, X-ray diffraction and electron probe microanalysis. The oxidation resistance was improved as the Cr content increased. The oxidation rates of W-Mo-Cr alloys containing 41~48wt % Cr followed a parabolic rate law and the measured rate constant(kp) of oxidation was approximately 1.92 × 10(-2)~9.55 × 10(-11)(g/cm2)2sec(-l) between 1000℃ and 1200℃. At the surface of this alloys, a Cr2O3 protective layer was continuously formed, and below the protective layer, CrZO3 internal oxide formed. The depth of internal oxidation zone was found to decrease with increasing Cr content. The oxidation resistance of W-Mo-(29~35wt%)Cr alloys was good at 1000℃, however, decreased rapidly as the temperature increased. The observed optimum Cr content for oxidation resistance was more than 35% at 1100℃ and 41% at 1200℃. The oxidation mechanism of W-Mo-Cr alloys was proposed.
