        Decreased CRTH2 Expression and Response to Allergen Re-stimulation on Innate Lymphoid Cells in Patients With Allergen-Specific Immunotherapy

        Purpose: Group 2 innate lymphoid cells (ILC2s) have been implicated in the pathogenesis of allergic disease. However, the effect of allergen-specific immunotherapy (AIT) on ILCs remains to be clarified. The aim of this study was to evaluate the levels of ILC subsets in allergic rhinitis (AR) patients in response to house dust mite (HDM)-specific immunotherapy. Methods: We enrolled 37 AR patients undergoing AIT (16 responders and 11 non-responders) for 2 years, 35 HDM AR patients and 28 healthy subjects. Peripheral blood mononuclear cells (PBMCs) were analyzed by flow cytometry to identify ILC subsets. Stimulation of ILC2s with recombinant allergen-specific protein was used to determine ILC2's activation (CD69 expression). Results: Responder AIT patients and healthy subjects had a decreased frequency of circulating ILC2s compared to non-responder AIT and AR patients. Conversely, ILC1s from responder AIT patients and healthy subjects showed increased frequency compared to non-responder AIT and AR patients. The frequency of ILC3s natural cytotoxicity receptor (NCR)+ and NCR− in responder AIT patients was significantly lower compared to AR patients and healthy subjects. The ILC1: ILC2 proportion in responder AIT patients was similar to that of healthy subjects. PBMCs from patients who were responders to AIT had a significantly lower expression of the activation marker CD69 on ILC2s in response to allergen re-stimulation compared to AR patients, but no difference compared to non-responder AIT patients and healthy subjects. Conclusions: We propose that AIT might affect ILC responses. The activation of ILC2s was reduced in AR patients treated with AIT. Our results indicate that a relative ILC1/ILC2 skewed response is a possible key to successful AIT.

        Creating Innovative Packaging and Logo Design for the Golden Banana Product to Modernize and Enhance Distribution

        Purpose: This study focuses on the golden banana (Kluai Hom Thong) from the Ban Khok Banana Community Enterprise Group in Sang Khom District, Udon Thani Province, Thailand. Despite its potential as an export crop and minimal disease or insect management issues, the group's products struggled to reach international markets due to lack of marketing opportunities, distribution channels, and proper packaging. The research aims to analyze and develop packaging and logos to test market potential for the group's products. Research design, data and methodology: Data was collected through questionnaires developed in conjunction with three printing and packaging design experts. Market testing involved 100 randomly sampled consumers of the group's products. Results: The new packaging design was perceived as more international, modern, and suitable as a souvenir for adults. Conclusions: Integrating ethnic elements and local materials into packaging design can enhance product appeal. For example, creating cloth bags from fabric scraps for carrying local woven products can impress consumers. This approach could help overcome market barriers and improve prospects for golden banana products in international markets while maintaining the intact ecosystem and environment of the production area. The study highlights the importance of effective packaging and branding in addressing market access challenges for local agricultural enterprises, potentially opening new opportunities for community-based businesses.

        Comparative analysis of outcome between laparoscopic versus open surgical repair for vesico-vaginal fistula

        Objective Vesicovaginal fistula (VVF) causes detrimental psychosomatic effects on a woman. It is repaired using open abdominal as well as laparoscopic approach. Here we compare a series of open versus laparoscopic VVF repairs done at a single centre. Methods Retrospectively data of patients undergoing VVF repair in our department between January 2011 to December 2014 was analyzed. Patients who had a single, primary, simple VVF following a gynaecological surgery were included in the study. 26 patients met all the criteria. Out of these, thirteen patients had undergone a laparoscopic VVF repair (group 1) while thirteen had undergone an open transabdominal VVF repair (group 2). Results Mean fistula size was 2.14±0.23 cm in group 1 and 2.18±0.30 cm in group 2, which was comparable. Mean blood loss was 58.69±6.48 mL in group 1 and 147.30±19.24 mL in group 2, which is statistically significant (P<0.0001). Mean hospital stay was 4 days in group 1 and 13 days in group 2 which is statistically significant (P<0.0001). The analgesic requirement (diclofenac) was 261.53±29.95 mg in group 1 and 617.30±34.43 mg in group 2, which is statistically significant (P<0.0001). Fistula repair was successful in all the patients in both the groups. Conclusion The present study shows that laparoscopic VVF repair results in reduced patient morbidity and shorter hospital stay without compromising the results. So laparoscopic repair may be a more attractive treatment option for patients with post gynecology surgery VVF.

        Randomized, Double-Blind, Placebo-Controlled Trial of Vitamin D Supplementation in the Build-up Phase of House Dust Mite-Specific Immunotherapy

        Purpose: Vitamin D (VitD) is an immunomodulatory molecule capable of alleviating allergic symptoms. However, the effectiveness of allergen-specific immunotherapy (AIT) is not commonly evidenced in the early build-up phase. The aim of the study was to determine the potential of VitD supplementation in this treatment phase. Methods: Thirty-four house dust mite (HDM)-allergic adult patients treated with subcutaneous AIT were randomized to receive VitD2 60,000 IU/week or placebo for 10 weeks and followed up for 10 weeks. The primary endpoints were the symptom-medication score (SMS) and the treatment response rate. The secondary endpoints were eosinophil count and levels of plasma IL-10, Der p 2-specific IgG4, and dysfunctional regulatory T (CRTH2+ Treg) cells. Results: Of 34 patients, 15 in each group completed the study. Patients with VitD deficiency receiving a VitD supplement showed significantly lower mean change SMS than the placebo group in weeks 10 (mean difference −54.54%, P = 0.007) and 20 (mean difference −42.69%, P = 0.04). The percentage of treatment responders reached 78% and 50% in the VitD and placebo groups, respectively, and the effect remained in week 20 (89% and 60%). No significant difference was observed for the tested immunological read-outs, with the exception of the frequency of CRTH2+ Treg cells, which was remarkably reduced in the VitD-treated patients. Moreover, improvement in SMS was correlated to the number of CRTH2+ Treg cells. Our in vitro experiment indicated that VitD downregulated activation markers, whereas it improved the function of CRTH2+ Treg cells. Conclusions: VitD supplementation in the build-up phase of AIT could relieve symptoms and decrease Treg cell dysfunction, especially in patients with VitD deficiency.

        EMPIRICAL DETERMINATION OF CONVECTION PARAMETERS IN WHITE DWARFS. I. WHOLE EARTH TELESCOPE OBSERVATIONS OF EC14012-1446

        <P>We report on an analysis of 308.3 hr of high-speed photometry targeting the pulsating DA white dwarf EC14012-1446. The data were acquired with the Whole Earth Telescope during the 2008 international observing run XCOV26. The Fourier transform of the light curve contains 19 independent frequencies and numerous combination frequencies. The dominant peaks are 1633.907, 1887.404, and 2504.897 mu Hz. Our analysis of the combination amplitudes reveals that the parent frequencies are consistent with modes of spherical degree l = 1. The combination amplitudes also provide m identifications for the largest amplitude parent frequencies. Our seismology analysis, which includes 2004-2007 archival data, confirms these identifications, provides constraints on additional frequencies, and finds an average period spacing of 41 s. Building on this foundation, we present nonlinear fits to high signal-to-noise light curves from the SOAR 4.1 m, McDonald 2.1 m, and KPNO 2 m telescopes. The fits indicate a time-averaged convective response timescale of tau(0) = 99.4 +/- 17 s, a temperature exponent N = 85 +/- 6.2, and an inclination angle of theta(i) = 32 degrees.9 +/- 3 degrees.2. We present our current empirical map of the convective response timescale across the DA instability strip.</P>
