Background: Breast cancer is the most common female cancer in Kerala, South India, with the incidence increasing in the past two decades, also in young women. However, there are limited data regarding the burden of disease, its epidemiology and histopathological characteristics in the state. Materials and Methods: This desciptive study covered 303 breast cancers evaluated during the period of December 2011 to August 2013 in the Department of Pathology, Government Medical College, Kottayam.The patients were also interviewed regarding selected risk factors. Results: The majority of the cases were 41-60 years of age with a mean at presentation of 53 years. Infiltrating ductal carcinoma was the most common subtype, followed by pure mucinous carcinoma and then lobular carcinoma. Of the cases, 6.6% were nullipara and 52.8% had fewer than or equal to 2 children. Median age at first child birth was 23 years (national value-19.8 years). A significant proportion (15%) had family history of breast cancer. Some 13.5%(41 cases) comprised the young breast cancer group (${\leq}40$years) with a mean age at first child birth in them was 27.4 years, 5 being nullipara and 6 having a positive family history. Conclusions: Breast cancer awareness, better availability of screening techniques and identification and targeting high risk groups all help to tackle the increasing load of breast carcinoma. A good proportion of cases comprised the young breast cancer group (under 40). Younger women should thus also be educated about breast carcinoma-risk factors, symptoms and diagnostic techniques to help in early detection and effective approach esto treatment.
Displacement-based seismic design of open ground storey buildings

Open ground storey (OGS) buildings are characterized by the sudden reduction of stiffness in the ground storey with respect to the upper infilled storeys. During earthquakes, this vertical irregularity may result in accumulated damage in the ground storey members of OGS buildings without much damage in the upper storeys. Hence, the structural design of OGS buildings needs special attention. The present study suggests a modification of existing displacement-based design (DBD) procedure by proposing a new lateral load distribution. The increased demands of ground storey members of OGS buildings are estimated based on non-linear time history analysis results of four sets of bare and OGS frames having four to ten storey heights. The relationship between the increased demand and the relative stiffness of ground storey (with respect to upper storeys) is taken as the criterion for developing the expression for the design lateral load. It is also observed that under far-field earthquakes, there is a decrease in the ground storey drift of OGS frames as the height of the frame increases, whereas there is no such reduction when these frames are subjected to near-field earthquakes.

The 20-substituted-40-selenoribofuranosyl pyrimidines3a–3j were synthesized from D-ribose and assayed foranticancer activity. The 20-azido and 20-fluoro groups with aribo configuration were introduced by the regioselectiveopening of the O2,20-anhydronucleosides with sodium azideand (HF)x-pyridine, respectively. Among the compoundstested, only 20-fluoro derivative 3j was found to exhibit significantanticancer activity, but was much less potent than thecorresponding 20-arabino analogue 2c. This study will providemedicinalchemistswith the insight into the identificationof structural requirements for the anticancer activity for thedevelopments of biologically active nucleosides.

Phytochemical investigation of the methanolic extract of Diospyros kaki leaves led to the isolation of osmanthuside H (1) and a new phenol glycoside, named gamnamoside [4-(3-hydroxypropyl)-2-methoxyphenol $\beta$-D-apiofuranosyl( 1 $\rightarrow$ 6)$\beta$-D-glucopyranoside] (2) along with (-) catechin (3) through a series of reversed phase column chromatography and preparative C18 HPLC. The structures of the isolates were determined by spectroscopic methods including IR, UV, HRTOFMS, and 2D NMR. Compounds 1, 2, and 3, showed good inhibitory activities ($IC_{50}$) of 175.4, 94.4, and 126.6 ${\mu}g/mL$ respectively, whereas a reversible ADH inhibitor, 4-methylpyrazole, showed the $IC_{50}$ of 326.6 ${\mu}g/mL$ against alcohol dehydrogenase (ADH).
Seismic behavior of concrete gravity dams

Dams play a vital role in the development and sustainment in a country. Failure of dams leads to the catastrophic event with sudden release of water and is of great concern. Hence earthquake-resistant design of dams is of prime importance. The present study involves static, modal and transient analyses of dam-reservoir-foundation system using finite element software ANSYS 15. The dam and the foundation are modeled with 2D plane strain element "PLANE 42" and the reservoir by fluid acoustic element "FLUID 29" with proper consideration of fluid-structure interaction. An expression for the fundamental period of concrete dams is developed based on modal analysis. Seismic response of gravity dams subjected to earthquake acceleration is evaluated in terms of peak displacement and stress.

Methemoglobinemia is a dyshemoglobinemia characterized by cyanosis and reduced oxygen saturation with increased methemoglobin values. The etiology may be congenital or acquired, with the latter being more common. We report a case of a full-term neonate who presented with transient cyanosis and methemoglobinemia caused by a mutation in the gamma chain of fetal hemoglobin.
