Hepatopulmonary syndrome (HPS) is a serious complication of end-stage liver disease, which is characterized by hypoxia, intrapulmonary vascular dilatation, and liver cirrhosis. Liver transplantation (LT) is the only curative treatment modality for patients with HPS. However, morbidity and mortality after LT, especially in cases of severe HPS, remain high. This case report describes a patient with typical findings of an extracardiac pulmonary arteriovenous shunt on contrast-enhanced transesophageal echocardiography (TEE), and clubbing fingers, who had complete correction of HPS by deceased donor LT. The patient was a 16-year-old female who was born with biliary atresia and underwent porto-enterostomy on the 55th day after birth. She had been suffered from progressive liver failure with dyspnea, clubbing fingers, and cyanosis. Preoperative arterial blood gas analysis revealed severe hypoxia (arterial O2 tension of 54.5 mmHg and O2 saturation of 84.2%). Contrast-enhanced TEE revealed an extracardiac right-to-left shunt, which suggested an intrapulmonary arteriovenous shunt. The patient recovered successfully after LT, not only with respect to physical parameters but also for pychosocial activity, including school performance, during the 30-month follow-up period. Interleukin-21 is a common γ-chain cytokine that controls the immune responses of B cells, T cells, and natural killer cells. Targeting IL-21 to strengthen the immune system is promising for the development of vaccines as well as anti-infection and anti-tumor therapies. However, the practical application of IL-21 is limited by the high production cost. In this study, we improved IL-21 production by codon optimization and selection of appropriate signal peptide in CHO-K1 cells. Codon-optimized or non-optimized human IL-21 was stably transfected into CHO-K1 cells. IL-21 expression was 10-fold higher for codon-optimized than non-optimized IL-21. We fused five different signal peptides to codon-optimized mature IL-21 and evaluated their effect on IL-21 production. The best result (a 3-fold increase) was obtained using a signal peptide derived from human azurocidin. Furthermore, codon-optimized IL-21 containing the azurocidin signal peptide promoted IFN-γ secretion and STAT3 phosphorylation in NK-92 cells similar to codon-optimized IL-21 containing original signal peptide. Collectively, these results indicate that codon optimization and azurocidin signal peptides provide an efficient approach for the high-level production of IL-21 as a biopharmaceutical.
충주 온천지역의 관광환경자원의 과학적 평가 : 타 온천 지역과의 대기오염상태 비교 Comparison of Atmospheric Environmental State

Since the concept of clean air at hot spring resorts in Chungju city depends upon the scientific evidence of air pollution state, the atmospheric data at this city should be compared with other cities. The aim of this study was to investigate a degree of atmospheric cleanness at Chungju city comparing with other ones. The measurement results at this city showed the concentration of sulfur dioxide (S0_2; 0.0043 ppm/yr ~ 0.04 ppm/yr), total suspended particulate(TSP; 38.5 ㎍/m^3/hr ~ 90 ㎍/m^3/hr), ozone(0_3; 0.0133 ppm/hr ~ 0.0267 ppm/hr), nitrogen dioxide(N0_2; 0.0078 ppm/yr ~ 0.037 ppm/yr), and carbon monoxide(CO; 3.9 ppm/hr - 21.2 ppm/hr). The atmospheric cleanness at Chungju is involved in high group (p<0.05; high group vs low group) among the hot spring resort cities - Changwon, Seoul, Pusan, Soowon, Taejon, and Kangrung. As Chungju has a lake and less developed manufacturing area, the establishments of frontier industry, the leisure industry, the spa-medical industry and education facilities are strongly recommended for the municipal policy of the city.
좌새골하정맥을 통하여 중심정맥도관 삽입 3 일 후에 발생한 상대정맥천공과 우흉수 : 증례 보고 A case report

A 67-year-old male patient had coronary artery bypass graft. Before the induction of general anesthesia, 16 gauge central venous catheter was inserted via left subclavian vein. Initial chest anteroposterior (AP) radiograph showed adequate position, and the central venous pressure measurement through it was satisfactory. This patient's progress was satisfactory, and he was extubated on the second postoperative day. Daily chest AP radiograph had been unremarkable until the third postoperative day, when a right-sided pleural effusion was seen. A day later, the patient complained severe chest pain and dyspnea. We suspected perforation of superior vena cava wall by tip of central venous catheter on the chest computerized tomograph (CT). The injection of radiopaque dye through the central venous catheter confirmed spillage into the mediastinum on the chest CT. Right chest tube was inserted. The patient's condition dramatically improved after chest tube insertion. (Korean J Anesthesiol 2002; 43: 525~530)
Ca-alginate로 고정한 녹조류 Chlorella pyrenoidosa에 의한 구리의 생물 흡착시 경쟁 간섭의 영향

Batch experiments were carried out to asses the coexistence effect of lead and cadmium ions on the biosorption of copper ions by Chlorella pyrenoidosa immobilized with Ca-alginate. The uptake of copper ions was examined for single and binary solutions at different initial concentrations. The experimental results showed that the biosorption equilibrium was reached within 1 hour and the adsorption capacity of copper increased with increasing initial copper ions concentration in solution. The biosorption capacity of copper was found to be adversely affected by the presence of lead or cadmium ions in the binary solution, although decrease of the biosorption capacity in the presence of cadmium was less than in the presence of lead. The equilibrium data on the single and binary metal biosorption were fitted to two isotherm models: Langmuir and Freundlich; the Langmuir isotherm gave the better fit for the equilibrium data.

This study analyzes the foreign consumers' preferences and the willingness to pay for the Korean traditional dried persimmons by conducting surveys to Japanese, Americans, Canadians, Europeans, Australians, and New Zealanders. The results show that the potential export demand is largest to Japanese and those to U.S. and Europe are sufficient. The results also indicate that the price has to be reduced and the quality has to be improved in order to extend the export demand for the Korean traditional dried persimmons.
金剛山댐 建設에 따른 北漢江 上流 水域의 環境 모니터링에 관한 硏究

Aquatic ecosystem was studies in Lake Paro to monitor the effects of the Keumkangsan Dam construction in North Korea. Water quality, primary prodectivity, and microbes were serveyed bimonthly in 1987. The texture and composition of sediments, and meterological sapects were also investigated. The most remarkable change of water quality was the increase of turbidity. due to the dam construction. Overall chemical and biological aspects were greatly changed because of the high turbidity. The concentration of phosphate increased and subsequently phytoplankto standing corp increased. The effect of phosphate increase was compensated by the decrease of light penetration, and, therefore areal primary prodectivity was not so high as expected from the chlorophyll concentration. From the standing corp of microbes, the trophic state of lake Paro was evaluated as mesotrophy. As for the horizontal variation of microbial activity, the microbial heterotrophy was higher in the upstream regions than in the downstream regions. Sediment types are almost clayey silt and the mean sizes of sediments were homogeneous through the whole lake area. Organic carbon contents of the sediment seemed to be influenced by seasonal variation of primary productivity.
매복된 하악 제1대구치의 외과적 노출술을 이용한 치험례

제1대구치는 저작과 혼합치열기 이후의 수직적 교합관계형성에 핵심적인 역할을 하고 악골의 성장발육에 관여한다. 제1대구치의 매복은 하안면 고경의 감소, 낭의 형성, 치관 주위염, 인접치의 치근 흡수, 부정 교합 등의 문제점을 야기할 수 있다. 치료방법으로는 주기적 관찰, 외과적 노출술, 교정적 견인, 외과적 재위치술, 발치 등이 있으며 이중 외과적 노출술이 가장 기본이 되는 술식이다. 외과적 노출술시에는 맹출로의 개방성을 유지하는 것이 중요하며 개방성을 유지시키기 위한 방법으로는 레진관의 접착, 산화 아연 유지놀 시멘트, 치주포대 등을 노출된 부위에 충전시키는 방법 등이 있다. 본 증례에서는 매복된 하악 제1대구치에서 장애물의 제거와 외과적 노출술을 시행한 결과 자발적인 맹출을 관찰할 수 있었다. 또한 surgical pack과 투명상유지장치를 사용하여 간편하고 효과적으로 맹출로의 개방성을 유지할 수 있었다.
水稻種子의 Paclobutrazol浸漬處理가 苗素質에 미치는 影響

This experiment was carried out to evalute the effects of paclobutrazol (PP-333) on the characters of mechanical transplanting rice seedlings at the Chungbuk National University in 1986. The results obtained were summarized as follows. 1.Paclobutrazol appeared to be very effective to reduce plant height of seedlings. Highly significant difference in plant height of seedlings among paclobutraszol concentration was noted for all varieties tested. 2.Paclobutrazol at higher than 10 ppm increased the number of leaves of 40-day-seedlings for all varieties. It appeared that number of leaves was increased as paclobutrazol concentration was increased. 3.Although dry weight of seedlings was reduced, dry weight/plant height, which indicates the degree of seedlings substantiality, tended to be increased as paclobutrazol concentration was increased.
