Neurolymphomatosis on F-18 FDG PET/CT and MRI Findings: A Case Report

Neurolymphomatosis is a rare manifestation of malignant lymphoma. A 74-year-old man, in complete remission from diffuse large B cell lymphoma, presented with a loss of pain and temperature sensation in the left hemiface and left upper extremity, and motor weakness in the left upper and both lower extremities. Cerebrospinal fluid analysis and brain magnetic resonance imaging (MRI) findings were negative. Combined fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) revealed multiple linear hypermetabolic lesions along the mandibular branch of the left trigeminal nerve, left brachial plexus, right axillary nerve, right suprarenal plexus, right adrenal gland, right femoral nerve, and both sciatic nerves, which corresponded to the patient's complex neurologic symptoms. C-spine and pelvic MRI revealed diffuse thickening with enhancement in the left brachial plexus and in the proximal portion of the left sciatic nerve, but negative findings for other sites identified by FDG-PET/CT. These findings suggest that FDG-PET/CTcan detect peripheral nerve infiltration by malignant lymphoma earlier than MRI. Thus, if a patient with a history of lymphoma presents with neurologic symptoms, FDG-PET/CT should be performed to evaluate neurolymphomatosis.

<P>Room temperature ionic liquids (RTILs), ether-functionalized imidazolium methanesulfonates, exhibit extremely high SO<SUB>2</SUB> solubility, at least 2 moles of SO<SUB>2</SUB> per mole of RTIL at 30 °C and at atmospheric pressure. The solubility of SO<SUB>2</SUB> in these RTILs increases with increasing number of tethered ether oxygen atoms and also with the pressure rise. FT-IR spectroscopic and quantum mechanical calculation results show that such high SO<SUB>2</SUB> solubility is originated from the combined interactions of SO<SUB>2</SUB> with methanesulfonate anion and ether oxygen atom or atoms on the imidazolium ring. The absorbed SO<SUB>2</SUB> gas can be readily and completely desorbed from the RTILs by heating at 100 °C in a N<SUB>2</SUB> flow, thereby allowing the RTILs to be reused up to 5 cycles without loss of their initial capacity.</P> <P>Graphic Abstract</P><P>Ether-functionalized imidazolium-based ionic liquids bearing a methanesulfonate anion are highly effective for the reversible absorption of SO<SUB>2</SUB>. <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c0ee00616e'> </P>

<P>This work shows the new engineering strategy of magnetic adsorbents by the combination of zeolitic imidazolate framework (ZIF) and microporous organic network (MON) chemistry. ZIF-67 nanoparticles containing Co<SUP>2+</SUP> ions were coated with MON. The thermolysis of ZIF-67@MON under argon resulted in hollow carbon materials bearing cobalt nanoparticles which showed promising performance as magnetic adsorbents for aromatic pollutants in water.</P> <P>Graphic Abstract</P><P>ZIF-67 nanoparticles containing Co<SUP>2+</SUP> ions were coated with a microporous organic network (MON). Dermatomyositis (DM) is an autoimmune idiopathic inflammatory myopathy with typical skin features, interstitial lung disease, myositis, mechanic’s hands. The patient is a 43-year-old Korean man. He visited local hospital and admitted. He developed right supraorbital edema and both leg cramping pain. Laboratory finding were D-dimer 395 ng/mL, AST 412 IU/L, ALT 217 IU/L, CPK 1,610 mg/dL and immunofluorescence for antinuclear antibodies, serum for anti-SM/RNP, anti-neutrophil cytoplasmic antibodies, anti-SSa,-SSb antibodies, anti-cardiolipin antibodies, anti ds DNA antibodies, anti-Jo-1 antibodies were all negative. Computed tomography showed that multifocal peribronchial consolidation and ground glass opacity in both lower lobe, especially in right subpleural area and effuision. A 1.5 cm-diameter filling defect in the distal right lower pulmonary vein. He refered to our hospital. He exhibitied DM-associated skin rashes; a heliotrope rash and edema on both eyelids, eruptions in both knuckles, prepatella, both shoulder and back. Neurologic examination showed no motor weakness. An electromyogram of muscles showed no definitive evidence of myopathy and polyneuropathy. Collectively, A diagnosis of amyopathic DM with suspected right pulmonary vein thromboembolism was made. Intravenous methylprednisolone (125 mg) and subcutaneous enoxaparin (5,000 IU) were given. Cutaneous symptoms and laboratory findings were improved gradually. After 10 days of treatment, the patient developed dyspnea, tachycardia, left thigh pain. His hemoglobin and blood pressure decreased 13.7 to 7.3 g/dL and 90/60 mmHg. Platelets, prothrombin time, activated partial prothromboplastin time, bleeding time were all normal. A CT scan showed hemorrhage of the left iliopsoas, iliacus and retroperitoneum. LMWH was stopped and 3 units of packed red cells were transfused. We applied a splint on his lower extremities with absolutely bed rest. Finally he recovered successfully and discharged.
Primary mediastinal abscess diagnosed by EBUS-TB NA: to resect or not to resect?

Here are two cases of amediastinal abscess, who were diagnosed by EBUS-TBNA. A 61-year-old asian female with a history of bronchiectasis for 20 years presented with one week of progressive dysphagia and throat pain. She complained anorexia, non-productive cough, and dyspnea. Oninitial presentation her blood pressure was 110/70, heart rate 96, respiratory rate 20 and pulse ox was 91%. For exclusion of esophageal cancer, EGD was perfomed, there are no specific lesion. A chest CT scan showed bronchiectasis with mucus plugging and patchy consolidations in the both hemithorax and a 6.5 cm sized multiloculated necrotic lesion in the mediastium. He was started on Piperacillin/Tazobactam. She underwent an EBUS-TBNA for rule out a malignancy. On EBUS the mass was consist of heterogenous hypoechoic mass, irregular margin. The histopathology of the mass showed only supprative inflammation exudates with small number or necrotic epithelial cell clusters. Sputum cultures grew Pseudomonas aeruginosa. We considered operation, but her pulmonary function is very poor (FEV1: 0.85L, FEV1/FVC 56%), and the patient refused. She was treated conservatively with antibiotics for 3 weeks, and then, discharged. The other case is a 50-year-old woman presented symptoms of cough and aggravating dyspnea. The patient subsequently had a chest CT scan that showed a 8.5 cm large mediastinal mass in middle and posterior mediastinum with adjacent upper thoracic vertebral bodies of destruction. EBUS showed heterogenous floating inner materials. A EBUS-TBNA was performed which showed only inflammatory exudates. She was operated through thoracic incision. The resected tissue was proven to be a chronic granulomatous inflammation with caseous necrosis. The immunohistochemical staining was positive for AFB, TB-PCR and negative for PAS and GMS. Sputum cultures grew Non T.B Mycobacterium. She had complication of esophageal leakage POD 42. So we treated her with esophageal stent. These cases are rare in that primary abscess in the mediastinum, are more uncommon in immunocomptent hosts. Patients presented with non-specific symptoms and a mass that was initially concerning for malignancy. Treatment of mediastinal abscess is controversial, yet.

<P><B>Abstract</B></P><P>The economic production of biofuels from renewable biomass using <I>Saccharomyces cerevisiae</I> requires tolerance to high concentrations of sugar and alcohol. Here we applied an inverse metabolic engineering approach to identify endogenous gene targets conferring improved alcohol tolerance in <I>S. cerevisiae</I>. After transformation with a <I>S. cerevisiae</I> genomic library, enrichment of the transformants exhibiting improved tolerance was performed by serial subculture in the presence of iso-butanol (1%). Through sequence analysis of the isolated plasmids from the selected transformants, four endogenous <I>S. cerevisiae</I> genes were identified as overexpression targets eliciting improved tolerance to both iso-butanol and ethanol. Overexpression of <I>INO1</I>, <I>DOG1</I>, <I>HAL1</I> or a truncated form of <I>MSN2</I> resulted in remarkably increased tolerance to high concentrations of iso-butanol and ethanol. Overexpression of <I>INO1</I> elicited the highest ethanol tolerance, resulting in higher titers and volumetric productivities in the fermentation experiments performed with high glucose concentrations. In addition, the <I>INO1</I>-overexpressing strain showed a threefold increase in the specific growth rate as compared to that of the control strain under conditions of high levels of glucose (10%) and ethanol (5%). Although alcohol tolerance in yeast is a complex trait affected by simultaneous interactions of many genes, our results using a genomic library reveal potential target genes for better understanding and possible engineering of metabolic pathways underlying alcohol tolerance phenotypes.</P>
