Proceedings of International Symposium on East-West Medicine, Seoul. 212-230, 1999. -Hepatitis B is caused by hepadnavirus. Hepatitis B virus replicates through 3.5kb pregenomic RNA intermediate which is regulated by core promoter. Pathogenesis of hepatitis B virus has been bilieved the result of host immune response. But recently many studies have reported that high level of viral replication caused by mutation in core promoter might result in severs hepatitis. But these studies were performed in vitro, not in vivo. So there is yet debate about which factor, viral of host factor, is more important in pathogenesis of hepatitis B virus. So we measured real viral replication level in 204 chronic hepatitis B patients by quantifying HBV DNA from sera by our novel PCR-based more sensitive method, and compared these results with ALT level measured from same sera, which indicates liver cell damage. Surprisingly there are no significant correlation between HBV DNA quantity and ALT level. Then we cloned core promoter region. In SSCP, we found that many viral mutants coexist in one patient. Base on SSCP result, we chose main viral core promoter type in each patients, which is thought to determine overall viral replication level in this patient. Main type of core promoter region of each 41 patients were directly sequenced. And with these we measured promoter activity by luciferase assay system and compared promoter activity with on another. We found tha there were some differences in promoter activity according to core promoter sequences. And we constructed replication-competent viral constructs with core promoter from 41 patients and Transfected these into HepG2 cell and measured HBV DNA by southern blot. There were also differences in HBV DNA quantity according to core promoter sequences. On these all results we investigated correlation between the effect of HBV core promoter on viral replication in vitro and HBN DNA quantity, ALT level from sera of each patients. We found there is no significant correlation among them. As a result, we concluded that in determining severity chronic hepatitis B patients, host factors of each patient is more important rather than replicative activity of virus itself.

The 5 harmonic detecting methods are simulated and their characteristics for the active filter application are compared using simulation results.

        Keumho river is flowing across Taeku city, which is α-mesosaprobic according to annual and situal average level. Sillyong stream inflowing to upper branch is β-mesosaprobic, and Omok stream is α-mesosaprobic, and Namchon, Pulno, and Palkye stream were polysaprobic, respectively. Then, these streams impact on main branch. But Tonghwa stream is a little clean and dilutes a very polluted middle main branch. Among them, Namchon, Pulno, Pakye, and lower main branch are polysaprobic zones with McNaughton`s dominance DI above 0.8, Kno¨pp`s B.E.O.L. above 90, Pantle-Buck`s S above 3.2, Beck-Tsuda`s BI below 10, Shannon`s diversity below 1.5, Yoon-Kong`s TBS below 30. Therefore these zones seem to be target area to be ameliorated in Keumho basin. In the fluctuation of biotic indices, B.E.O.L., Saprobic index S, SI, dominance DI in last season of 1990 were lower than those in same period of 1991, but diversity H`, BI, and TBS were higher.

        Evidence-based clinical recommendations for hypofractionated radiotherapy: exploring efficacy and safety - Part 1. Brain and head and neck

        Advances in radiotherapy (RT) techniques, including intensity-modulated RT and image-guided RT, have allowed hypofractionation, increasing the fraction size over the conventional dose of 1.8–2.0 Gy. Hypofractionation offers advantages such as shorter treatment times, improved compliance, and under specific conditions, particularly in tumors with a low α/β ratio, higher efficacy. It was initially explored for use in RT for prostate cancer and adjuvant RT for breast cancer, and its application has been extended to various other malignancies. Hypofractionated RT (HFRT) may also be effective in patients who are unable to undergo conventional treatment owing to poor performance status, comorbidities, or old age. The treatment of brain tumors with HFRT is relatively common because brain stereotactic radiosurgery has been performed for over two decades. However, re-irradiation of recurrent lesions and treatment of elderly or frail patients are areas under investigation. HFRT for head and neck cancer has not been widely used because of concerns regarding late toxicity. Thus, we aimed to provide a comprehensive summary of the current evidence for HFRT for brain tumors and head and neck cancer and to offer practical recommendations to clinicians faced with the challenge of choosing new treatment options.

        Anti-Proliferative and Pro-Apoptotic Activities of 4-Methyl-2,6-bis(1-phenylethyl)phenol in Cancer Cells

        It has been found that 4-isopropyl-2,6-bis(1-phenylethyl)phenol (KTH-13), a novel compound isolated from Cordyceps bassiana, is able to suppress tumor cell proliferation by inducing apoptosis. To mass-produce this compound, we established a total synthesis method. Using those conditions, we further synthesized various analogs with structural similarity to KTH-13. In this study, we aimed to test their anti-cancer activity by measuring anti-proliferative and pro-apoptotic activities. Of 8 compounds tested, 4-methyl-2,6-bis(1-phenylethyl)phenol (KTH-13-Me) exhibited the strongest anti-proliferative activity toward MDA-MB 231 cells. KTH-13-Me also similarly suppressed the survival of various cancer cell lines, including C6 glioma, HCT-15, and LoVo cells. Treatment of KTH-13-Me induced several apoptotic signs in C6 glioma cells, such as morphological changes, induction of apoptotic bodies, and nuclear fragmentation and chromatin condensation. Concordantly, early-apoptotic cells were also identified by staining with FITC-Annexin V/PI. Moreover, KTH-13-Me highly enhanced the activation of caspase-3 and caspase-9, and decreased the protein level of Bcl-2. In addition, the phosphorylation levels of Src and STAT3 were diminished in KTH-13-Me-treated C6 cells. Therefore, these results suggest that KTH-13-Me can be developed as a novel anti-cancer drug capable of blocking proliferation, inducing apoptosis, and blocking cell survival signaling in cancer cells.

        Cases of acute mercury poisoning by mercury vapor exposure during the demolition of a fluorescent lamp factory

        Background: In 2015, workers dismantling a fluorescent lamp factory in Korea were affected by mercury poisoning from exposure to mercury vapor. Case presentation: Eighteen out of the 21 workers who participated in the demolition project presented with symptoms of poisoning and, of these, 10 had persistent symptoms even at 18 months after the initial exposure to mercury vapor. Early symptoms of 18 workers included a general skin rash, pruritus, myalgia, sleep disturbance, and cough and sputum production. Following alleviation of these initial symptoms, late symptoms, such as easy fatigue, insomnia, bad dreams, and anxiety disorder, began to manifest in 10 out of 18 patients. Seven workers underwent psychiatric care owing to sleep disturbance, anxiety disorder, and depression, and three workers underwent dermatologic treatment for hyperpigmentation, erythematous skin eruption, and chloracne-like skin lesions. Furthermore, three workers developed a coarse jerky movement, two had swan neck deformity of the fingers, and two received care at an anesthesiology clinic for paresthesia, such as burning sensation, cold sensation, and pain. Two workers underwent urologic treatment for dysfunction of the urologic system and impotence. However, symptomatic treatment did not result in satisfactory relief of these symptoms. Conclusion: Awareness of the perils of mercury and prevention of mercury exposure are critical for preventing health hazards caused by mercury vapor. Chelation therapy should be performed promptly following mercury poisoning to minimize damage.

      • [P322] A case of intralymphatic histiocytosis associated with breast cancer

        Intralymphatic histiocytosis (IH) is a rare disease with variable and nonspecific clinical features, but with characteristic histopathological findings. Histopathologically, dilated lymphatic vessels containing aggregates of mononuclear cells are observed in the reticular dermis. IH is often associated with other diseases; such as rheumatoid arthritis, osteoarthritis, joint metal implantation, and breast cancer occurred over mastectomy scar. Although the pathogenesis of IH is not defined, some authors suggest that lymphatic vessel engorgement by histiocytes migrating from neighboring inflammatory reactions may play a key pathogenic role. A 50-year-old female presented with multiple tender erythematous plaques on both breast and right shoulder. On past history, she underwent both breast conservation operation 3 years ago. Laboratory findinds were WBC 12,340/mm3, neutrophil 88.1%, ESR 10mm/hr, CRP 9.73mg/dL and procalcitonin 0.089ng/mL. Histopathologic findings showed irregularly dilated vessels with intraluminal cells in dermis. Endothelial cells lining the vessels were positive for D2-40 staining, which is a marker for lymphatic endothelial cells, and intraluminal cells were positive for CD68 staining, which is a marker for histiocytes. She improved with conservative therapy. To our knowledge, this is the first case of IH in association with breast cancer in the Korean dermatologic literature.

        Hand, foot, and mouth disease (HFMD) is anenteroviral illness typically affecting children, which is characterized by fever, maculopapular, or vesicular rash of the extremities, mouth, and buttocks. Atypical HFMD with severe eczematoid symptoms is termed“Eczema coxsackium (EC)”. A two-year-old boy with a history of atopic dermatitis(AD) was hospitalized with 10-day history of fever and rashes starting on his both hands.Physical examination revealed numerous erythematous vesicles, eroded papule and bullae on his face, trunk, and extremities. Based on his atopic diathesis and clinical findings, eczema herpeticum or impetigo was suspected at first,so intravenous acyclovir and amoxicillin/clavulanatewereinitiated. Laboratory tests including bacterial culture were not remarkable. Histologic examination showed reticular degeneration with intraepidermalvesiculation with no multinucleate giant cells.Additionally viral antibody titer against CVA6 was1:2048. On the basis of clinical, histologic, and serologic findings, he was diagnosed as EC caused by CVA6 infection and discharged after 7-day conservative therapy.On follow-up for 2 months,he was healed completely without any complications.In our case,EC caused by CVA6 infectionshowed more severe symptoms than classic HFMD. We think that CVA6 infectionshould be considered when severe vesicular exanthems in an AD patientare observed.

        Dexmedetomidine attenuates H<sub>2</sub>O<sub>2</sub>-induced cell death in human osteoblasts

        Background: Reactive oxygen species play critical roles in homeostasis and cell signaling. Dexmedetomidine, a specific agonist of the ${\alpha}2$-adrenoceptor, has been commonly used for sedation, and it has been reported to have a protective effect against oxidative stress. In this study, we investigated whether dexmedetomidine has a protective effect against $H_2O_2$-induced oxidative stress and the mechanism of $H_2O_2$-induced cell death in normal human fetal osteoblast (hFOB) cells. Methods: Cells were divided into three groups: control group-cells were incubated in normoxia without dexmedetomidine, hydrogen peroxide ($H_2O_2$) group-cells were exposed to $H_2O_2$ ($200{\mu}M$) for 2 h, and Dex/$H_2O_2$ group-cells were pretreated with dexmedetomidine ($5{\mu}M$) for 2 h then exposed to $H_2O_2$ ($200{\mu}M$) for 2 h. Cell viability and apoptosis were evaluated. Osteoblast maturation was determined by assaying bone nodular mineralization. Expression levels of bone-related proteins were determined by western blot. Results: Cell viability was significantly decreased in the $H_2O_2$ group compared with the control group, and this effect was improved by dexmedetomidine. The Hoechst 33342 and Annexin-V FITC/PI staining revealed that dexmedetomidine effectively decreased $H_2O_2$-induced hFOB cell apoptosis. Dexmedetomidine enhanced the mineralization of hFOB cells when compared to the $H_2O_2$ group. In western blot analysis, bone-related protein was increased in the Dex/$H_2O_2$ group. Conclusions: We demonstrated the potential therapeutic value of dexmedetomidine in $H_2O_2$-induced oxidative stress by inhibiting apoptosis and enhancing osteoblast activity. Additionally, the current investigation could be evidence to support the antioxidant potential of dexmedetomidine in vitro.

        Effects of Bio-degradable Mulches on the Yield of Maize and the Density of Soil Microbe

        The use of polyethylene film has a problem such as increasing rural environmental contamination, collection costs and farmers’ workload. The objective of this study was to evaluate bio-degradable films in terms of yield of maize and soil environment. Treatments were bio-degradable film A (BDF A), bio-degradable film B (BDF B), high density polyethylene (HDPE), and non-mulched (NM) soil. Daily mean values of soil temperature (10 cm depth) under BDF A, BDF B, and HDPE were higher than in NM soil by 2.2, 2.8, 3.1℃ respectively. In the mulching cultivation of maize, bio-degradable film began to degrade from 50~60days after the planting. The degradation was much progressed in the harvest time and almost decomposed in the following spring. The weight of ear of maize was not shown significantly by mulching treatments. There were little changes of soil chemical properties for the bio-degradable film mulching. After using bio-degradable films, the contents of biomass-C and dehydrogenase activity increased from 92 to 137~147 mg kg<SUP>-1</SUP>, and from 87 to 123~168 mg kg<SUP>-1</SUP> respectively.
