Background: Although international guidelines recommend palliative care approaches for many serious illnesses, the palliative needs of patients with serious illnesses other than cancer are often unmet, mainly due to insufficient prognosis-related discussion. We investigated physicians' and the general public's respective attitudes toward prognostic disclosure for several serious illnesses. Methods: We conducted a cross-sectional survey of 928 physicians, sourced from 12 hospitals and the Korean Medical Association, and 1,005 members of the general public, sourced from all 17 administrative divisions in Korea. Results: For most illnesses, most physicians (adjusted proportions – end-organ failure, 99.0%; incurable genetic or neurologic disease, 98.5%; acquired immune deficiency syndrome [AIDS], 98.4%; stroke or Parkinson's disease, 96.0%; and dementia, 89.6%) and members of the general public (end-organ failure, 92.0%; incurable genetic or neurologic disease, 92.5%; AIDS, 91.5%; stroke or Parkinson's disease, 92.1%; and dementia, 86.9%) wanted to be informed if they had a terminal prognosis. For physicians and the general public, the primary factor to consider when disclosing terminal status was “the patient's right to know his/her condition” (31.0%). Yet, the general public was less likely to prefer prognostic disclosure than physicians. Particularly, when their family members were patients, more than 10% of the general public did not want patients to be informed of their terminal prognosis. For the general public, the main reason for not disclosing prognosis was “psychological burden such as anxiety and depression” (35.8%), while for the physicians it was “disclosure would have no beneficial effect” (42.4%). Conclusion: Most Physicians and the general public agreed that disclosure of a terminal prognosis respects patient autonomy for several serious illnesses. The low response rate of physicians might limit the generalizability of the results.
온라인 커뮤니티 에브리타임을 통한 대학생의 일상정보 이용행태에 관한 연구

본 연구는 에브리타임이라는 온라인 커뮤니티를 통하여 대학생의 일상정보를 추구하고 공유하는 이용행태를 분석하기 위해 수행되었다. 일상정보 이용추구모델과 활동이론을 기반으로 연구를 설계하고, 질적연구 방법을 사용하여 다양한 대학 소속의 학생들을 면담하였다. 그 결과, 에브리타임 이용자들은 에브리타임을 일상정보의 추구 및 공유에 유용한 온라인 커뮤니티로 인식하고 있었다. 특히 학사일정, 수업정보, 졸업정보 등 학교 생활 정보 추구를 위해 가장 많이 이용하고 있었으며 그 외에는 건강정보, 맛집정보, 주거정보 등에도 활용하고 있었다. 코로나19 상황에 입학한 1, 2학년의 경우 에브리타임에 대한 의존도가 높았으며, 코로나19 이전의 대면 상황을 경험한 3, 4학년 또한 코로나19 이후에 에브리타임의 중요도가 높아졌다고 응답하였다. 에브리타임이 신속하고 유용한 정보를 제공하는 정보원임에도 불구하고 에브리타임의 적극적인 사용을 저해하는 주요 요인으로 일부 이용자의 도덕적 해이가 언급됐다. 본 연구 결과는 대학생 온라인 커뮤니티에 대한 정보학 연구와 대학생을 위한 온라인 커뮤니티의 개발과 운영의 기초자료로 활용될 수 있을 것으로 기대한다. This study aimed to analyze the usage behaviors of university students seeking and sharing everyday life information through an online community called Everytime. The study was designed based on everyday life information-seeking and activity theory models, and students from various universities were interviewed using a qualitative research method. Findings showed that Everytime users perceive Everytime as a valuable online community for pursuing and sharing everyday life information. It was primarily used to search for university life information, such as academic calendar, class, and graduation, and health, restaurants, and housing. In the case of the freshmen and sophomores who entered during the COVID-19 pandemic, their dependence on Everytime was high, and juniors and seniors who experienced university life before COVID-19 also responded that Everytime is one of the essential sources of information in university life. Although Everytime provides quick and valuable information, users mentioned the moral hazard as a major factor hindering the active use of Everytime. The results of this study are expected to be used as primary data for informatics research on the online community of college students and the development and operation of online communities for university students.

Purpose: Acetaminophen (APAP) is a widely available drug responsible for a large part of drug-induced hepatotoxicity in developed countries. Although acetaminophen overdose cases in Korea are being continuously reported, there are no reports related to the level of this drug in the patient’s blood or of laboratory analysis at emergency departments (ED). This study sought to analyze the acetaminophen overdose cases at a toxicological laboratory and to survey APAP analysis services offered at select EDs. Methods: We analyzed the demographic and analytic data at a toxicological laboratory run by the National Emergency Medical Center (NMC) in 2019-2020. We surveyed the APAP laboratory service in the 38 regional emergency medical centers (EMCs) and 68 local EMCs near the toxicological laboratory. Results: We studied 175 acute poisoning cases (112 women) with positive blood APAP results (mean age 47.0±24.1 years). Suicide attempts comprised 40.0% of the cases and 30.3% APAP overdose events. In the univariate analysis, we observed that patients were significantly younger, with fewer underlying medical diseases. There were a higher number of APAP overdose events, more favorable initial mental status, more toxic quantity intake in the above treatment line group (p<0.05), In multivariate analysis, the toxic amount intake was significantly more frequent in the above treatment line group (p<0.01). Hospital APAP analysis services were available in six EMCs (3/38 regional and 3/68 local). The hospital blood APAP level reporting intervals were shorter than outside-hospital laboratory services (p<0.01, regional 7.0±3.0 vs. 40.6±27.5, local 5.3±3.1 vs. 57.9±45.1 hours). The NMC toxicological laboratory reporting interval was shorter than the other outside-hospital laboratories (p<0.01, regional 5.7±0.6 vs. 50.2±22.7 local 7.5±3.0 vs. 70.5±41.5 hours). Conclusion: Over the treatment line group, toxic amount intake was significantly more frequent. Only six of 106 EMCs have their own APAP analysis service in their hospitals.

<P>The transmembrane domain (TMD) of the syndecans, a family of transmembrane heparin sulfate proteoglycans, is involved in forming homo-and heterodimers and oligomers that transmit signaling events. Recently, we reported that the unique phenylalanine in TMD positively regulates intramolecular interactions of syndecan-2. Besides the unique phenylalanine, syndecan-2 contains a conserved phenylalanine (SDC2-Phe-169) that is present in all syndecan TMDs, but its function has not been determined. We therefore investigated the structural role of SDC2-Phe-169 in syndecan TMDs. Replacement of SDC2-Phe-169 by tyrosine (S2F169Y) did not affect SDS-resistant homodimer formation but significantly reduced SDS-resistant heterodimer formation between syndecan-2 and -4, suggesting that SDC2-Phe-169 is involved in the heterodimerization/oligomerization of syndecans. Similarly, in an in vitro binding assay, a syndecan-2 mutant (S2(F169Y)) showed a significantly reduced interaction with syndecan-4. FRET assays showed that heteromolecular interactions between syndecan-2 and -4 were reduced in HEK293T cells transfected with S2(F169Y) compared with syndecan-2. Moreover, S2(F169Y) reduced downstream reactions mediated by the heterodimerization of syndecan-2 and -4, including Rac activity, cell migration, membrane localization of PKC alpha, and focal adhesion formation. The conserved phenylalanine in syndecan-1 and -3 also showed heterodimeric interaction with syndecan-2 and -4. Taken together, these findings suggest that the conserved phenylalanine in the TMD of syndecans is crucial in regulating heteromeric interactions of syndecans.</P>

Background: The gradually increasing demand for coffee worldwide has prompted increased interest in the relationshipbetween coffee and health issues as well as a need for research on metabolic syndrome in adults. Methods: Data from 3,321 subjects (1,268 men and 2,053 women) enrolled in the 2013–2014 Korean NationalHealth and Nutrition Examination Survey were analyzed. The subjects were divided into three groups according totheir daily coffee consumption. The odds ratios (ORs) and 95% confidence intervals (95% CIs) for metabolic syndromein the coffee-drinking groups were calculated using multiple logistic regression analysis by adjusting forconfounding variables. Results: The prevalence of metabolic syndrome was 15.5%, 10.7%, and 9.7% in men and 3.0%, 7.1%, and 6.5% inwomen according to their coffee consumption (less than one, one or two, or more than three cups of coffee perday), respectively. Compared with the non-coffee consumption group, the ORs (95% CIs) for metabolic syndromein the group that consumed more than three cups of coffee was 0.638 (0.328–1.244) for men and 1.344 (0.627–2.881)for women after adjusting for age, body mass index, household income, education, smoking, alcohol, regular exercise,and daily caloric intake. Conclusion: The OR of metabolic syndrome was not statistically significant in both men and women.
