        c-형강 및 ebb & flow 방식에 따른 경제성 분석

        관비재배와 저면관수인 ebb & flow, C-형강의 재배방식에 따른 고품질분화 생산을 위한 재배농가의 경제성을 분석하였다. 이를 위해 화훼농가의 비용구조를 분석하였고 자본 회수기간 및 내부수익률의 추정, 관수에 따른 인건비 절감 효과를 분석하여 재배방식에 따른 경제성을 비교하였다. 10a(991.7m²) 규모의 자동온실에 대해 관비재배방식을 선택한 시클라멘 재배농가의 총 비용은 9,993.8만원으로 추정되며, 이 중 시설비는 74%(7,410만원)를 점하고 연간 2,583.8만원의 경영비를 지출할 것으로 예상된다. 반면, C-형강방식을 선택한 재배농가는 초기 시설비용으로 약 8,520.0만원(총비용의 약 70%)을 지출하고, 매년 경영비로 3,597.0만원을 지출할 것으로 추정된다. 한편, 투자자금에 대한 자본회수 기간을 분석한 결과 10a 규모의 자동온실에서 시클라멘을 재배하는 농가에서 관비재배방식 혹은 C-형강방식을 선택할 경우에는 초기시설투자비 회수기간이 약 4 년 정도 소요될 것으로 예측되며, ebb & flow 방식을 선택할 경우에는 약 5년 정도의 시간이 소요될 것으로 예상된다. 또한, 내부 수익률을 추정한 결과 ebb & flow시설과 C-형강시설을 5년과 6년씩 사용한다고 가정할 경우, ebb & flow시설의 내부 수익률은 각각 9%와 14%로 나타났고, C-형강시설의 경우에는 각각 13%와 17%로 나타나 시중의 저축성예금 수신금리 4.82%와 비교하여 경제성이 높은 것으로 분석되었다. Economical efficiency for production of high quality-potted plants was analyzed by types of irrigation system such as conventional fertigation, ebb & flow, and C-channel as subirrigation systems. After cost structure was analyzed, payback period of capital, internal rate return (IRR), and effects of reduced labor cost were estimated. The gross cost of cyclamen-producing farmers using hydroponic systems in automated greenhouses (10a) was about 100 million won. Of the gross cost, installation and operation costs occupied 74% and 25%, respectively. In the C-channel subirrigation system, gross cost was 120 million won approximately. Installation and operation costs occupied 70% and 30%, respectively. Payback period of capital was 4 years for fertigated irrigation and C-channel subirrigation system, whereas it was 5 years for ebb & flow. IRR of ebb & flow and C-channel subirrigation systems was higher than the interest rates of 4.82%, indicating economic efficiency.

      • Impact of Interferon-Based Treatment on Quality of Life and Work Related Productivity from the Korean Cohort in the MOSAIC Study

        Aims: Chronic Hepatitis C Virus (HCV) infection increases the risk for progressive liver disease, hepatocellular carcinoma and negatively impacts the patient’s quality of life. HCV treatment is evolving with direct acting antivirals but IFN based therapy has been the standard of care for many years and remains available in some countries. The MOSAIC study aims to characterize patients with chronic HCV infection and assess the impact of IFN-containing treatment on health-related quality of life, work related productivity and health care utilization. Methods: MOSAIC is an international prospective multicenter observational study that has been conducted in 20 countries. Consecutive patients with chronic HCV infection were enrolled and those who initiated an IFN based regimen were prospectively followed for 48 weeks. We report results from the Korean cohort Results: 100 patients were enrolled: 86 were treatment naïve and 14 were treatment experienced. 33 patients initiated an IFN based regimen: 6 patients started IFN + RBV, 26 patients started Peg-IFN + RBV, none started Peg-IFN + RBV + DAA and 1 patient received other treatment. Among the treated cohort, demographic and disease characteristics were the following: the mean age was 54.5 years; 14 patients were male. 14 had minimal or no fibrosis, 2 portal fibrosis, 3 bridging fibrosis and 6 patients suffered from cirrhosis. HCV Genotype distribution was as follows: genotype 1: 11; genotype 2: 19 and genotype 3: 3. Table 1 describes the results at baseline and changes over 4, 12 and 48 weeks and end-of-treatment (EOT) for the quality of life and work productivity outcome measures (EQ-5D-5L, HCV-PRO and WPAI). Conclusions: Results from the Korean cohort of the MOSAIC study show a moderate trend for deterioration of health-related quality of life and work productivity associated with IFN based treatment for patients with chronic HCV infection during treatment period.         Changes in Physico-chemical Properties of Moss Peat Based Root Media and Growth of Potted Chrysanthemums as Influenced by Blending Ratios of Root Media in a C-channel Mat Irrigation System

        This experiment was conducted to investigate physical and chemical characteristics by volume fractions of root media using peatmoss, perlite, and vermiculite, along with effects on the growth of pot chrysanthemums (Dendranthema × grandiflorum ‘Vemini’) in a C-channel mat irrigation system. To evaluate the physico-chemical properties of 20 root media, the bulk density, particle density, total pore space, pore space, ash content, organic matter, pH, and electrical conductivity were measured and data were analyzed using principal component analysis (PCA). PCA scores revealed that physico-chemical properties changed by the blending of peatmoss, perlite, and vermiculite. The 20 root media were divided into three main groups by hierarchical cluster analysis. At the end of the experiment, the pH and EC of the root media were measured from media divided into four layers. The pH of root media without plants showed a strong linear relationship and the pH of root media with plants increased exponentially. The change of EC in the root medium was indicated as a hyperbolic curve. Plant growth characteristics according to growth in the 20 root media were analyzed by PCA. It was found that the mixing ratios of the root media affected plant growth characteristics. Therefore, mixing ratio is an important factor for pot-plant production in a subirrigation system.

        The nutritional risk is a independent factor for postoperative morbidity in surgery for colorectal cancer

        Purpose: The authors evaluate the prevalence of malnutrition and its effect on the postoperative morbidity of patients after surgery for colorectal cancer. Methods: Three hundred fifty-two patients were enrolled prospectively. Nutritional risk screening 2002 (NRS 2002) score was calculated through interview with patient on admission. Clinical characteristics, tumor status and surgical procedure were recorded. Results: The prevalence of patients at nutritional risk was 28.1 per cent according to the NRS 2002. The rate of postoperative complication was 27%. There was a significant difference in postoperative complication rates between patients at nutritional risk and those not at risk (37.4% vs. 22.9%, P = 0.006). Nutritional risk was identified as an independent predictor of postoperative complications (odds ratio, 3.05; P = 0.045). Nutritional risk increased the rate of anastomotic leakage (P = 0.027) and wound infection (P = 0.01). Conclusion: NRS may be a prognostic factor for postoperative complication after surgery for colorectal cancer. A large scaled prospective study is needed to confirm whether supplementing nutritional deficits reduces postoperative complication rates.

        Growth in Exclusively Breastfed and Non-exclusively Breastfed Children: Comparisons with WHO Child Growth Standards and Korean National Growth Charts

        Background: This study examined the relationship of infant feeding with anthropometric indices of children during their first six years of life relative to the Korean National Growth Charts (KNGC) and the World Health Organization Child Growth Standards (WHO-CGS). Methods: The study population consisted of 547,669 Korean infants and children who were 6 months-old to 6 years-old (born in 2008–2009) and participated in the National Health Screening Program for Infants and Children. Data on height, weight, and type of feeding during the first 6 months (exclusively breastfed [BF] vs. mixed- or formula-fed [FF]) were analyzed. Results: BF boys and girls were significantly shorter and lighter than FF counterparts from the age of 6 months to 4 years, but these differences were not significant after the age of 4 years. BF boys and girls only had significantly lower body mass index at the age of 2 years. Under the age of 2 years 6 months, and especially under the age of 1 year, BF boys and girls were significantly taller and heavier than the 50th percentile values of the 50th percentile value of the WHO-CGS. Conclusion: In this study using large-scaled national data, Korean breastfed children are shorter and lighter by 3 years 6 months–4 years 6 months, but afterward, there is no significant difference from those who had mixed- or formula-feeding. Substantial disparities in the anthropometric indices of Korean infants under the age of 1 compared to KNCG and WHO-CGS were found, regardless of their infantile feeding types. Our results emphasize the importance of constructing a nationwide reference chart based on actual measurements of BF Korean infants.

        Bioresorbable silicon electronic sensors for the brain

        <P>Many procedures in modern clinical medicine rely on the use of electronic implants in treating conditions that range from acute coronary events to traumatic injury(1,2). However, standard permanent electronic hardware acts as a nidus for infection: bacteria form biofilms along percutaneous wires, or seed haematogenously, with the potential to migrate within the body and to provoke immune-mediated pathological tissue reactions(3,4). The associated surgical retrieval procedures, meanwhile, subject patients to the distress associated with re-operation and expose them to additional complications(5-8). Here, we report materials, device architectures, integration strategies, and in vivo demonstrations in rats of implantable, multifunctional silicon sensors for the brain, for which all of the constituent materials naturally resorb via hydrolysis and/or metabolic action(9-12), eliminating the need for extraction. Continuous monitoring of intracranial pressure and temperature illustrates functionality essential to the treatment of traumatic brain injury(2,13); the measurement performance of our resorbable devices compares favourably with that of non-resorbable clinical standards. In our experiments, insulated percutaneous wires connect to an externally mounted, miniaturized wireless potentiostat for data transmission. In a separate set-up, we connect a sensor to an implanted (but only partially resorbable) data-communication system, proving the principle that there is no need for any percutaneous wiring. The devices can be adapted to sense fluid flow, motion, pH or thermal characteristics, in formats that are compatible with the body's abdomen and extremities, as well as the deep brain, suggesting that the sensors might meet many needs in clinical medicine.</P>

        Effects of estrogen inhibition formula herbal mixture for danazol-induced precocious puberty in female rats: an experimental study with network pharmacology

        Background: This study aimed at determining the effect of the herbal mixture estrogen inhibition formula (EIF) and its possible mechanisms by precocious puberty animal models and network pharmacology-based analysis. Methods: Precocious puberty animal models were established by a single injection of 300 μg danazol, then female rats were administered EIF, vaginal openings were monitored, uterus and pituitary indices were determined. The levels of ALP, E2, LH, and FSH were measured using ELISA kits. Real-time PCR was performed to evaluate the mRNA expression of GnRH, UNC5C, and netrin-1 in hypothalamic tissues. We applied network pharmacological analysis to predict potential targets and pathways of EIF. Results: EIF delayed danazol-induced early vaginal opening. In the onset model, EIF reduced the increased levels of serum ALP, E2, LH, and FSH; as well as mRNA expressions of GnRH, Netrin-1, and UNC5C. Moreover, long-term administration of EIF not only diminished all impaired factors but also had no effect on the normal development of the animals. The gene set enrichment analysis showed that the targets of EIF are mainly associated with the GnRH signaling and ovarian steroidogenesis pathways. Conclusion: EIF could be used in preclinical research for the treatment of precocious puberty by the inhibition of HPGA pre-maturation.

        Effects of combined mechanical stimulation on the proliferation and differentiation of pre-osteoblasts

        We observed how combined mechanical stimuli affect the proliferation and differentiation of pre-osteoblasts. For this research, a bioreactor system was developed that can simultaneously stimulate cells with cyclic strain and ultrasound, each of which is known to effectively stimulate bone tissue regeneration. MC3T3-E1 pre-osteoblasts were chosen for bone tissue engineering due to their osteoblast-like characteristics. 3-D scaffolds were fabricated with polycaprolactone and poly-L-lactic acid using the salt leaching method. The cells were stimulated by the bioreactor with cyclic strain and ultrasound. The bioreactor was set at a frequency of 1.0 Hz and 10 % strain for cyclic strain and 1.0 MHz and $30mW/cm^2$ for ultrasound. Three experimental groups (ultrasound, cyclic strain, and combined stimulation) and a control group were examined. Each group was stimulated for 20 min/day. Mechanical stimuli did not affect MC3T3-E1 cell proliferation significantly up to 10 days when measured with the cell counting kit-8. However, gene expression analysis of collagen type-I, osteocalcin, RUNX2, and osterix revealed that the combined mechanical stimulation accelerated the matrix maturation of MC3T3-E1 cells. These results indicate that the combined mechanical stimulation can enhance the differentiation of pre-osteoblasts more efficiently than simple stimuli, in spite of no effect on cell proliferation.

        Once-Daily Extended-Release Levetiracetam Improves Medication Compliance in Adolescent Epilepsy Patients

        Purpose: Since pharmacologic agents are the mainstay of epilepsy treatment, drug compliance is one of the most important factors in seizure control. Once-daily levetiracetam (LEV) has been proven to have the same efficacy as that of an immediate-release (IR) formulation. A reduced number of doses may improve drug compliance and patient satisfaction. The aim of this study was to assess drug compliance and patient satisfaction when changing from IR to an extended-release (ER) formulation. Methods: Adolescent patients diagnosed with epilepsy who were taking LEV from 2018 to 2020 were included in this study. Compliance charts were reviewed retrospectively. We compared the frequency of seizure occurrence with the frequency of skipping doses and adverse effects before and after changing formulations. Changes in subjective compliance and satisfaction were also investigated. Results: Among 585 patients taking LEV, 44 were included in this study. The average age of the included patients was 16.4±2.0 years. There was no significant change in the average seizure frequency (P=0.491) after switching formulations. Objective compliance based on chart records significantly improved after switching formulations (P=0.021). Additionally, 26 of 44 patients mentioned how they felt about switching formulations, of whom 25 (96.2%) were satisfied with the ER formulation. Thirteen of 24 patients (54.2%) reported better compliance. Conclusion: Our study shows that the efficacy of LEV ER was similar to that of the IR formulation. The reduced number of medication doses improved patient satisfaction and medication compliance. LEV ER may be preferable in adolescent epilepsy patients.
