        대전 지역에서 급성 A형 간염의 유전자형에 따른 임상 특성 고찰

        Background/Aims: Acute viral hepatitis A is a major health problem in Korea and the influx of genotype IIIA is thought to be one reason. We examined the differences in the clinical characteristics and laboratory findings of genotypes IA and IIIA in Daejeon. Methods: From November 2009 to June 2010, 81 patients positive for IgM anti-HAV were enrolled prospectively. The hepatitis A was genotyped using real-time polymerase chain reaction. The clinical characteristics and laboratory results were compared on the Results: The mean patient age was 32.6±7.4 years. The mean hospitalization was 7.7±2.4 days. The patient occupation varied. Clinically, vomiting and diarrhea were relatively more prevalent in genotype IIIA than in IA. Abdominal pain and skin spots were relatively more prevalent in genotype IA than in IIIA. The hemoglobin, peak aspartate aminotransferase (AST) level, and C-reactive protein were statistically higher in genotype IIIA than in IA. The distributions of the peak AST, alanine aminotransferase (ALT) and total bilirubin values tended to be perched in genotype IIIA than in IA. The international normalized ratio (INR) tended to be slightly prolonged in genotype IIIA than in IA. Conclusions: Recently, genotype IIIA of acute viral hepatitis A has become prevalent in Daejeon. Hepatitis A genotype IIIA probably causes worse laboratory abnormalities than genotype IA.

        수정-IPA를 통한 푸드카빙교육 효용성의 품질속성 분류에 관한 연구

        본 연구는 푸드카빙 교육을 받은 수강생들이 지각하는 푸드카빙 효용성은 어떠한지 살펴보고, 어떠한 효용성들이 있고, 효용성의 품질 속성은 어떠한지 살펴보고자 하였다. 이러한 연구목적을 규명하기 위하여 푸드카빙 교육을 받은 경험이 있는 수강생을 모집단으로 설정하였고, 설문조사는 2020년 7월 20일부터 8월 20일까지 한 달간 진행되었다. 설문지는 총 500부를 배부하였고, 435부가 회수되었고, 불성실한 설문지 38부를 제외하고 397부가 최종 분석에 사용되었다. 분석결과, 중요 실행 요소로는 푸드카빙 교육은 지루하지 않고, 남녀노소 할 수 있고, 잡생각이 사라지고, 자존감이 높아지고, 자신의 가치가 높아지고, 마음이 편해지고, 우울함이 사라지는 등의 수업 자체에서 오는 만족감의 속성들이 포함되었다. 매력적 요소에는 푸드카빙 기술이 수익으로 연결되고, 강좌를 개설하고, 적은 금액으로 공방을 오픈하는 등의 수익 측면과 관련된 항목들이 포함되었다. 아울러, 기본적 요소에는 집중할 수 있고, 기분이 좋아지고, 자격증을 취득하고, 전문가가 될 수 있다는 측면의 항목들이 포함되었다. 이는 대체로 푸드카빙을 처음 배우는 수강생들이 기본적으로 가지는 목적들이 포함되었다. This study aims to examine the food carving utility perceived by students who received food carving education, the types of the utility, and its quality attributes. To investigate this research purpose, students who had received food carving education were set as a population, and the survey was conducted for one month from July 20 through August 20, 2020. 500 copies of the questionnaire were distributed in total, and 435 copies were collected. 397 copies were used in the final analysis, excluding 38 unreliable copies. As a result of the analysis, important execution factors included the attributes of satisfaction with the class itself, e.g., Food carving education was not boring; it could be done by men and women of all ages; it made cockeyed ideas disappear; it promoted their self-esteem; it enhanced their value; it comforted their hearts; and it made their depression disappear. Attractive elements included revenue-related items, such as food carving skills connected to profits; they could open lectures; and they could open a workshop with little money. Along with this, the basic elements included the following: They could concentrate; they could feel better; they could get a certificate; and they could become an expert. They usually included the purposes the students who would learn food carving for the first time had basically.

      • Clinical Impact of Exosomal microRNA as a Novel Biomarker of Liver Fibrosis

        Aims: Many approaches have been suggested for the non-invasive diagnosis of liver fibrosis, including the use of serum biomarkers and ultrasound-based elastography, but none has yet replaced liver biopsy. MicroRNAs (miRNAs) have been suggested as potential diagnostic tools for liver diseases. We investigated alterations in the expression of serum exosomal miRNAs with the progression of liver fibrosis and evaluated their clinical applicability as biomarkers. Methods: This study prospectively enrolled 71 patients who underwent liver biopsy at a large-volume academic hospital in Korea. Exosomes were extracted from serum samples, and next-generation sequencing (NGS) of miRNAs was conducted in patients from different stages of liver fibrosis. Differential expression of miRNAs was quantified using targeted real-time quantitative polymerase chain reaction (RT-qPCR). A model was derived to discriminate advanced fibrosis based on miRNA levels using multivariate logistic regression. The performance of this model was evaluated and compared using area under the receiver operator characteristic (ROC) curve (AUC) and De- Long’s test. Results: NGS data revealed the relationship between exosomal miR-122 expression and liver fibrosis progression. The level of miR-122 decreased as the pathologic fibrosis grade progressed from stage 0 to 4. Patients with biopsy-proven advanced fibrosis had significantly lower levels of exosomal miR-122 (P<0.001) than those without advanced fibrosis. Exosomal miR-122 exhibited a fair performance in discriminating advanced fibrosis with an AUC of 0.77, which improved to 0.86 in combination with fibrosis-4 score (FIB-4) and transient elastography (TE). This value was higher than that reported for any other non-invasive modalities, including TE (AUC of 0.80) or FIB-4 (AUC of 0.57) alone. In a subgroup of patients with a non-viral etiology of liver disease, the performance of exosomal miR-122 as a biomarker improved, evident from the increase in the AUC value to 0.87. In this subpopulation, the combination model of miR- 122, FIB-4, and TE showed the best discrimination ability (AUC of 0.90), which was significantly higher than that of TE alone (AUC of 0.83; DeLong’s test P=0.046). Inhibition of miR-122 expression increased the proliferation of the human hepatic stellate cell line, LX-2, and upregulated the expression of collagen- 1A, a-smooth muscle actin, fibronectin, and transforming growth factor-ß. Conclusions: Exosomal miR-122 may serve as a novel biomarker for discriminating advanced liver fibrosis, and its accuracy may enhanced in combination with other non-invasive tests such as FIB-4 and TE.

      • TGF- <i>β</i> and Hypoxia/Reoxygenation Promote Radioresistance of A549 Lung Cancer Cells through Activation of Nrf2 and EGFR

        <P>Although many studies have examined the roles of hypoxia and transforming growth factor- (TGF-) <I>β</I> separately in the tumor microenvironment, the effects of simultaneous treatment with hypoxia/reoxygenation and TGF-<I>β</I> on tumor malignancy are unclear. Here, we investigated the effects of redox signaling and oncogenes on cell proliferation and radioresistance in A549 human lung cancer cells in the presence of TGF-<I>β</I> under hypoxia/reoxygenation conditions. Combined treatment with TGF-<I>β</I> and hypoxia activated epidermal growth factor receptor (EGFR) and nuclear factor (erythroid-derived 2)-like 2 (Nrf2), a redox-sensitive transcription factor. Interestingly, Nrf2 knockdown suppressed the effects of combined treatment on EGFR phosphorylation. In addition, blockade of EGFR signaling also suppressed induction of Nrf2 following combined treatment with hypoxia and TGF-<I>β</I>, indicating that the combined treatment induced positive crosstalk between Nrf2 and EGFR. TGF-<I>β</I> and hypoxia/reoxygenation increased the accumulation of reactive oxygen species (ROS), while treatment with N-acetyl-<SMALL>L</SMALL>-cysteine abolished the activation of Nrf2 and EGFR. Treatment with TGF-<I>β</I> under hypoxic conditions increased the proliferation of A549 cells compared with that after vehicle treatment. Moreover, cells treated with the combined treatment exhibited resistance to ionizing radiation (IR), and knockdown of Nrf2 increased IR-induced cell death under these conditions. Thus, taken together, our findings suggested that TGF-<I>β</I> and hypoxia/reoxygenation promoted tumor progression and radioresistance of A549 cells through ROS-mediated activation of Nrf2 and EGFR.</P>

        당뇨병이 없는 농촌지역의 건강한 성인 남녀에서 당화혈색소와 내장지방과의 관계

        Objective: A prediabetes hemoglobin A1c (HbA1c) level of 5.7%?6.4% is considered a risk factor for diabetes mellitus and cardiovascular disease (CVD) in the USA. In this study, we assessed the HbA1c and visceral fat levels as CVD risk factors in health check-up examinees who were not yet diagnosed with diabetes. Methods: Totally, 507 study subject were categorized as per criteria of the American Diabetes Association, depending on whether the HbA1c level was ≥5.7% or <5.7%. Lipid levels, blood pressure, BMI (kg/㎡), total abdominal, and visceral fat levels were measured by computed tomography. Results: The mean of HbA1c in the male group was larger than the mean in the female group and their values were, respectively, 6.03±0.82% and 5.88±0.72%(p<0.05). Only the mean values of age and visceral fat area were different between HbA1c≥5.7% and <5.7% in both male and female group(p<0.05). Visceral fat levels were significantly associated with HbA1c in the group of HbA1c ≥5.7% (odds ratio=1.005, 95% CI 1.002∼1.008). Conclusions: Visceral fat levels were significantly higher and correlated with the group which HbA1c level is ≥5.7%. This finding suggests that subjects who have high levels of HbA1c should be carefully monitored during prediabetes and should have chance to have health education programs.

        5-Lipoxygenase mediates RANKL-induced osteoclast formation via the cysteinyl leukotriene receptor 1.

        <P>5-Lipoxygenase (5-LO) catalyzes the formation of two major groups of leukotrienes, leukotriene B4 and cysteinyl leukotrienes (CysLTs), and it has been implicated as a promising drug target to treat various inflammatory diseases. However, its role in osteoclastogenesis has not been investigated. In this study, we used mouse bone marrow-derived macrophages (BMMs) to show that 5-LO inhibitor suppresses RANKL-induced osteoclast formation. Inhibition of 5-LO was associated with impaired activation of multiple signaling events downstream of RANK, including ERK and p38 phosphorylation, and IκB degradation, followed by a decrease in NFATc1 expression. Ectopic overexpression of a constitutively active form of NFATc1 partly rescued the antiosteoclastogenic effect of 5-LO inhibitor. The knockdown of 5-LO in BMMs also resulted in a significant reduction in RANKL-induced osteoclast formation, accompanied by decreased expression of NFATc1. Similar effects were shown with CysLT receptor (CysLTR)1/2 antagonist and small RNA for CysLTR1 in BMMs, indicating the involvement of CysLT and CysLTR1 in 5-LO-mediated osteoclastogenesis. Finally, 5-LO inhibitor suppressed LPS-induced osteoclast formation and bone loss in the in vivo mouse experiments, suggesting a potential therapeutic strategy for treating diseases involving bone destruction. Taken together, the results of this study demonstrate that 5-LO is a key mediator of RANKL-induced osteoclast formation and possibly a novel therapeutic target for bone-resorption diseases.</P>

        A FRET Assay for Celiac Disease

        To provide a basis for a homogeneous fluorescence resonance energy transfer (FRET) immunoassay for celiac disease, we carried out a FRET experiment using guinea pig tissue transglutaminase (tTG) and antibodies to tTG (anti-tTG) purified from rat serum. Fluorescein was utilized as the probe, and a nonfluorescent dye, QSY 7 served as the quencher. We labeled anti-tTG and tTG with fluorescein isothiocyanate and QSY 7 succinimidyl ester, respectively. Fluoresceinlabeled anti-tTG was the donor, and QSY 7-labeled tTG was the acceptor of the FRET experiment. When we titrated fluorescein-labeled anti-tTG with QSY 7-labeled tTG, we observed a large decrease in the steady-state fluorescence intensity, which was due to strong FRET from fluorescein-labeled anti-tTG to QSY 7-labeled tTG. Using time-resolved fluorescence spectroscopy, we could also observe a decrease in the fluorescence lifetime, which confirms the steady-state data. We expect that these results might be useful in the development of a novel fluorescence immunoassay for an easy screening and follow-up of celiac patients.

        자살 관련 행동의 과거력이 있는 한국군 장병에서의 스트레스, 우울 증상, 사회적 관계의 연관성

        Objectives This study investigates the relationships between soldiers’ social relations, stress, and depressive symptoms. We also investigated the effect of previous suicide-related behaviors on their correlations. Methods This study was conducted on a total of 1000 soldiers. Study subjects were tested using clinical scales including the Korean Mini-International Neuropsychiatric Interview, the Center for Epidemiological Studies for Depression Scale, the Worker’s Stress Response Inventory and the Lubbens Social Network Scale-6. Sociodemographic characteristics and history of suicide-related behaviors were also examined. In order to investigate the relationships between stress, depression, and social relations in groups with and without a history of suicide-related behavior, one-way correlation analysis was performed. Results Depressive symptoms and stress were positively correlated when sociodemographic factors are controlled, regardless of past history of suicide-related behaviors. Depressive symptoms and social relations showed a weak negative correlation. Furthermore, stress and social relations showed a weak negative correlation. When the stress response was additionally controlled, there was no significant correlation between depressive symptoms and social relations. There was a stronger correlation between the group with a history of suicide-related behavior and the group without suicide - related behavior. Conclusion There was a significant positive correlation between depressive symptoms and stress, and this correlation was greater in patients with a history of suicidal behavior than in those without prior suicidal behavior. In addition, social relations did not directly affect depressive symptoms but was found to mediate stress indirectly.

        Hepatocellular carcinoma (HCC) is the sixth most common tumor worldwide and the third leading cause of tumor mortality. The majority of tumors are diagnosed when advanced and then a 5-year survival rate of HCC is below 5%. However, recent progress in the diagnosis and treatment of HCC has made it possible for the patient to survive longer, and as a result, distant metastasis from HCC has increased and attracted more attention than before. HCC can be metastasized to all organs through blood and lymphatic channel. Of the various metastatic sites, the most common site is the lungs, followed by the lymph nodes, musculoskeletal, adrenal and omentum. Also, spleen, small bowel, large bowel and esophagus can be invloved. Brain metastasis can be occurred rarely, and then it is regarded to oncologic emergency. We report a patient who present with brain metastasis of progressing HCC despite of treatment.
