The purpose of this study is to propose a model for service programs and management strategies for the division of family education in Healthy Families Center. For the purpose, the researchers find out demands for the family education programs of people in Yong-San Gu, Seoul, exam the effect of each family education program of Yong-San Gu & Sookmyung Women's University Healthy Families Center from June to December, 2004. According to the results, people in Yong-San Gu need economic education service mostly, and they want to study at Saturday afternoon. The programs show positive effect on the improvement of family health.

Terlipressin is a vasopressin analogue that is widely used in the treatment of hepatorenal syndrome or variceal bleeding. Because it acts mainly on splanchnic vessels, terlipressin has a lower incidence of severe ischemic complications than does vasopressin. However, it can still lead to serious complications such as myocardial infarction, skin necrosis, or bowel ischemia. Herein we report a case of severe ischemic bowel necrosis in a 46-year-old cirrhotic patient treated with terlipressin. Although the patient received bowel resection, death occurred due to ongoing hypotension and metabolic acidosis. Attention should be paid to patients complaining of abdominal pain during treatment with terlipressin.

Background/Aims: Obesity is reported to be associated with erosive esophagitis (EE). However, the temporal association of obesity and abdominal obesity with EE is unclear. We conducted this study to investigate the temporal association of obesity, especially abdominal obesity with EE. Methods: Among 1,182 subjects who underwent health screening examinations including upper endoscopy in both 2003 and 2006, a total 1,029 subjects with a normal esophagogastric junction on upper endoscopy in 2003 were enrolled. All subjects completed questionnaires and anthropometric measurements were obtained twice by trained personnels. The patients with newly developed EE were compared to the subjects without newly developed EE. Results: Among 1,029 subjects, 42 (4.1%) were newly diagnosed with EE and 82 (8.0%) with hiatal hernia. The mean body mass index (BMI) in both examinations was significantly different between the two groups based on the development of erosive esophagitis (p<0.05 in both examinations). The mean waist circumference (WC) in both examinations was also significantly different between the two groups (p<0.01 in both examinations). The multivariate analysis demonstrated that EE was not associated with the BMI in 2003 and the increase of BMI; however, it was associated with the WC in 2003 (Odds ratio, 7.21; 95% CI, 1.78 to 29.19; >90 cm vs <80 cm). Conclusions: Our study showed that abdominal circumference is an independent risk factor for EE, demonstrating a temporal relationship between abdominal obesity and EE. Purpose: This study aimed to assess needs of training programs for APN practice courses and continuing education programs for APNs to provide baseline data to improve APN education and practice. Methods: A total of 98 questionnaires were distributed to professors in 38 nursing institutions and 64 (65.3%) were returned. Data collection was performed in November and December, 2007 through email. Results: Professors in charge of geriatric and home care nurse practitioner programs constituted 43.7% of the study participants. Forty-one percent answered that they were satisfied with the practice programs they currently have operated, while 36.5% responded to have difficulty in appointment of institutions for practice programs and 56.3% had problems in assignment of preceptorship. In case of developing training programs for APN practice courses, 58.7% answered that they are willing to participate, and 90.6% felt needs for development of continuing education programs for APNs. Regarding liability insurance for APN students, only 11.5% provided insurance policy. Conclusion: The study results indicated that standardized training programs are needed to assure quality of APN education, and continuing education programs are required to provide opportunities for APNs to improve competencies.
Effects of Silkworm Pupae on Bone Mineral Density in Ovariectomized Rat Model of Osteoporesis

Osteoporosis is characterized by the reduced density of mineralized bone matrix. The loss of bone has been attributed to an imbalance between bone formation and bone resorption. The silkmoth is one of the famous traditional tonic medicines. Silkworm pupa was evaluated for its antiosteoporotic activity in an ovariectomized rat model of osteoporosis. The rats were ovariectomized at 6weeks of age and divided into 7 groups. All the groups were fed without calcium diet. The OVX rats were treated with water and silkworm powder for 6 weeks. In OVX rats, the body weight, feeding and water intake quantity did not show a significant change, but the silkworm pupa powder immediately before the eclosion of Yeonrokjam was significantly increased the bone mineral density in the femoral bone. The silkworm pupa powder increased the bone with increasing calcium in serum. These results also showed its protective action through promotion of bone formation. The silkworm pupa powder has a definite antiosteoporotic effect; similar to estrogen, it is especially effective for the prevention of bone fracture induced by estrogen deficiency. The silkworm pupa powder is a promising anti-osteoporotic agent that can prevent bone fractures in osteoporotic patients. The silkworm pupa powder does not show serious side effects because natural herbal medicine origin.
Testosterone Secretion Effect according to the Growth Stage of Silkworm (Bombyx mori L.)

Until now, the main treatment for impotence has been the vasodilator injection, penile implants and etc. Among many methods, most effective way is known to be the medical therapy. The oral drug for an impotence remedy has to be above all, effective to the erection, and second, safety when taking in long-term. However, so far the drug in such condition and sufficiency has not been discovered. Consequently, it is crucial to develop the new medicine, made of natural materials only, without any uncertainty of side effect. The silkworm is very difference in physiological chemical change according to growth stage. Therefore the functional effect shows a very big difference according to preparation method with a growth stage. This research carry out the experiment with the pupa powder immediately before the eclosion, eclosion adult, the pupa extract immediately before the eclosion, and the silkworm powder of 5 instar 3rd day. The result showed not a big chance in weight, feed and water intake quantity. But testosterone hormone secretion effects of the freeze drying powder of the pupa immediately before the eclosion was very high 7.31 ng/ml, compared to 2.37 ng/ml of non treatment and 2.67 ng/ml of alcohol extraction method. As this result, the freeze drying pupa powder of the pupa immediately before the eclosion was confirmed to promote the secretion of the testosterone hormone more than alcohol extraction method of existing.

Silkworm powder, which contains 1-deoxynojirimycin (DNJ), is a promising complementary and alternative medicine (CAM) in Korea. Silkworm powder was produced from Yeonnokjam pupae at d 3 of the 5th instar at the National Academy of Agricultural Science. The powder was derivatized with 9-fluorenylmethyl chloroformate (FMOC-Cl), and the DNJ-FMOC content was measured by HPLC. We investigated the content of 1-DNJ in the silkworm powder and its glucose-lowering effect when it was treated at different temperatures. The content of 1-DNJ was the lowest at $150^{\circ}C$, while it was constant at other temperatures. The silkworm extract powder was orally administered to diabetic mice (20 mg/kg/d) for 4 wk. Water intake did not significantly change when compared with the control group (T0). The blood glucose levels significantly decreased when mice where administered silkworm powder treated at $60^{\circ}C$ (T60) compared to the control group, but no difference was observed between the groups T100 and T150. Moreover, the blood levels of TG significantly decreased compared with the control group. Based on these results, we surmise that the properties of the silkworm extract powder were stable upon heating at $100^{\circ}C$ but not at $150^{\circ}C$.
