<▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P>The most important differential diagnoses of acute monoarticular arthritis are septic arthritis and acute gout attack. Identifying infection is crucial in preventing the devastating outcome of septic arthritis. The delta neutrophil index (DNI) is a value that corresponds to the fraction of circulating immature granulocytes. As DNI reflects the burden of infection, we evaluated this index as a differentiating marker between septic arthritis and acute gout attack.</P><P>The medical records of 149 patients with septic arthritis and 194 patients with acute gout attack were reviewed. A specific cell analyzer, ADVIA 2120, was used to measure DNI. Clinical and laboratory markers associated with predicting septic arthritis were assessed by using logistic regression.</P><P>Patients with septic arthritis showed higher levels of DNI than those with acute gout attack (3.3 vs 0.6%, <I>P</I> < .001). Similar results were observed in patients without monosodium urate (MSU) crystal confirmation or those with normouricemia (3.3 vs 0.5 and 3.1 vs 0.7%, respectively; <I>P</I> < .001 for both). A DNI level of 1.9% was determined as the cutoff value for predicting septic arthritis. In the multivariate analysis, DNI was the most powerful independent value for predicting septic arthritis (odds ratio 14.003).</P><P>This study showed the possibility of using DNI as a differentiating marker between septic arthritis and acute gout attack at the crucial early phase. DNI showed its relevance regardless of confirmation of MSU crystal deposition or serum level of uric acid.</P></▼2>

Background: The pre-transplant model for end-stage liver disease (pre-MELD) score is controversial regarding its ability to predict patient mortality after liver transplantation (LT). Prominent changes in physical conditions through the surgery may require a post-transplant indicator for better mortality prediction. We aimed to investigate whether the post-transplant MELD (post-MELD) score can be a predictor of 1-year mortality. Methods: Perioperative variables of 269 patients with living donor LT were retrospectively investigated on their association with 1-year mortality. Post-MELD scores until the 30th day and their respective declines from the 1st day post-MELD score were included along with pre-MELD, acute physiology and chronic health evaluation (APACHE) II, and sequential organ failure assessment (SOFA) scores on the 1st post-transplant day. The predictive model of mortality was established by multivariate Cox`s proportional hazards regression. Results: The 1-year mortality rate was 17% (n=44), and the leading cause of death was graft failure. Among prognostic indicators, only post-MELD scores after the 5th day and declines in post-MELD scores until the 5th and 30th day were associated with mortality in univariate analyses (P<0.05). After multivariate analyses, declines in post-MELD scores until the 5th day of less than 5 points (hazard ratio 2.35, P=0.007) and prolonged mechanical ventilation ≥24 hours were the earliest independent predictors of 1-year mortality. Conclusions: A sluggish decline in post-MELD scores during the early post-transplant period may be a meaningful prognostic indicator of 1-year mortality after LT. A Reconfigurable Circularly Polarized Microstrip Antenna With a Slotted Ground Plane

<P>This letter describes a square patch antenna with a switchable circular polarization (CP) sense. The proposed antenna has four L-shaped slots on the ground plane, and the CP radiation can be generated by current perturbation due to the slotted ground plane. Because the CP sense of the proposed antenna is altered by the current path that is redirected with switched p-i-n diodes on the slots, the CP direction of the proposed antenna can be simply switched between the right-handed and left-handed CPs. As the slot and bias circuits are not placed on the patch side, the proposed antenna has a simple structure and can radiate a CP wave without altering the main beam direction. The experimental results show that antennas have excellent switchable radiation performance at 2.4 GHz.</P>
미생물에서 얻어지는 대두단백응고효소의 성질 및 대두 치즈화 식품(커드)에 대한 연구

Microorganisms, including the strain IJ-3 isolated from soil, were found to secrete an extracellular soybean protein coagulating enzyme and the strain, IJ-3 was identified as Genus Bacillus according to the Bergey's manual . The enzyme coagulated protein in soymilk , thus forming a curd at pHs 5.8-6.4 and at 55-75℃. The optimum temperature for soybean protein coagulating activity was 65-75℃ and the enzyme was stable at temperature below 50℃ and was found to be stable with about 60-100% of the original activity at a with pH ranges(pH6-7). The molecular weight of enzyme was estimated to be 28,000 by SDS-PAGE. The curd formed with the enzyme from Bacilus sp. IJ-3 has a smooth texture, and a mild taste without any bitterness or a beany flavor.

Trends of traction motor of railway vehicles have been shifted from induction motors to permanent magnet motors. Recently, studies have been conducted on dysprosium-free permanent magnets. Dysprosium is a heavy rare earth element that has a great impact on performance of motors. However, dysprosium aff ects the coercive force of a permanent magnet and can produce irreversible demagnetization. Conventional studies have focused on improving the performance of motors using permanent magnets without heavy rare earth elements. In this study, a robust rotor design process includes the way to restrain irreversible demagnetization using a dysprosium-free permanent magnet. First, the irreversible magnetization of bar-type and V-type magnets, which are basic models of interior permanent magnet synchronous motors, is analyzed. Next, expected demagnetized parts of the main magnets are used as a submagnets, which make magnetic fi eld bypass both sides of the submagnets, and magnets are added in submagnets regions to compensate gap of performance between target model and double type models. Finally, parameter analysis is performed on the design variables of the main magnet. The proposed the robust design process to restrict irreversible demagnetization proves its validity through fi nite element analysis.
광대역 응용을 위한 2.4 ㎓ 대역 전압 제어 주입 동기 발진기 설계

본 논문에서는 전압 제어 특성을 이용하여 광대역에서 사용 가능한 주입 동기 발진기를 제안하였다. 주입 동기 발진기의 공진 회로에 바렉터 다이오드를 포함시켜 다이오드의 바이어스 전압을 제어하여 자유 발진 주파수를 가변 가능하도록 하였으며, 이를 통해 주입 동기 신호의 입력 레벨이 낮더라도 광대역에서 동작이 가능하도록 하였다. 제안된 주입 동기 발진기는 0.8 ㎜ 두께를 갖는 FR-4 기판을 사용하여 제작되었으며, 0~5 V의 제어 전압을 주어졌을 때, 자유 발진 주파수는 2.39~2.52 ㎓로 가변이 가능하였다. 주파수 고정시 ?10 ㏈m의 입력 신호 레벨에서 주파수 동기 범위가 50 ㎒ 이상이며, 주파수 가변을 통해 ?30 ㏈m의 낮은 주입 신호레벨에서 90 ㎒의 주파수 동기 범위를 얻을 수 있었다. In this paper, a voltage controlled injection-locked oscillator(VC-ILO) is proposed for wideband applications. From the control of the free-running frequency by a varactor diode, the wide frequency locking range can be obtained for low-level injected signals. The proposed VC-ILO is implemented on an FR-4 substrate with a thickness of 0.8 ㎜. The free-running frequencies of the oscillator is 2.39~2.52 ㎓ at the control voltage of 0~5 V. While the frequency locking range of over 50 ㎒ is presented for ?10 ㏈m injected signal level at a fixed frequency, the locking range of over 90 ㎒ can be achieved for ?30 ㏈m by controlling the free-running frequency.
