        복부둔상에 의한 간 손상의 임상적 고찰

        Because of the merit of nonoperative management of blunt hepatic trauma, which is the avoidance of operative morbidity, the use of nonoperative management has been extended. However the indications for such treatment have not been sufficiently determined. To suggest possible criteria for nonoperative management, we reviewed 95 cases with blunt hepatic injuries which were confirmed by abdominal exploration or abdominal CT in Gyeong Sang National University Hospital from January 1992 to October 2000. Of the 95 patients, 38 patients were treated with operation and 57 patients treated with nonoperative management. We analyzed the patients for age, sex, cause of injury, injured organ, injury grade, transfusion amount, and shock on admission, complications and mortality. The results are as follows; 1) The peak incidence of age was in the 3rd decade (20%), and the male-to-female ratio was 3:1. 2) The most frequent injury mechanism was traffic accidents (40, 42%). 3) The most frequently used diagnostic modality was abdominal CT (95 cases) in the nonoperative and the operative group. 4) On the liver injury scale of the American Association for the Surgery of Trauma (AAST), 13 were grade I, 18 were grade II, 10 were grade III, 13 were grade IV, and 3 were grade V in the nonoperative group ; 1 were grade I, 6 were grade II, 18 were grade III, 8 were grade IV, 4 were grade V, and 1 were grade VI in the operative group. 5) Between the nonoperative and the operative groups, there were significant differences in the systolic blood pressure in the first 8 and 24 hours, transfusion amount, and GOT/GPT level (p$lt;0.05). 6) In comparison of complication, there were 3 cases of heaptic abscess,1 case of intrahepatic hematoma, 1 case of bile leakage, 1 case of rebleeding in operative group (15.7%). There were 2 cases of intrahepatic abscess, 2 cases of intrahepatic hematoma, 4 cases of biloma, 1 case of subcapsular hematoma in nonoperative group (15.7%). In mortality cases, 6patients died in operative group. Among them, 5patiens died from DIC (disseminated intravascular coagulation) and multiorgan failure, 1patient died from ARDS (adult respiratory distress syndrome) in operative group and 2patients died from hypovolemic shock in nonoperative group. In conclusion, i) nonoperative management may be considered as a first choice in children and adults with blunt abdominal trauma and stable vital signs. ii) Patients with hemodynamically stable hepatic injury 8 hrs after resuscitation, even with an AAST OIS grade 4 injury, may be candidates for nonoperative management.

        NecroX-5 prevents breast cancer metastasis by AKT inhibition via reducing intracellular calcium levels

        <P>A major goal of breast cancer research is to prevent the molecular events that lead to tumour metastasis. It is well-established that both cytoplasmic and mitochondrial reactive oxygen species (ROS) play important roles in cell migration and metastasis. Accordingly, this study examined the molecular mechanisms of the anti-metastatic effects of NecroX-5, a mitochondrial ROS scavenger. NecroX-5 inhibited lung cancer metastasis by ameliorating migration in a mouse model. In human cancer cells, the inhibition of migration by NecroX-5 is cell type-dependent. We observed that the effect of NecroX-5 correlated with a reduction in mitochondrial ROS, but mitochondrial ROS reduction by MitoQ did not inhibit cell migration. NecroX-5 decreased intracellular calcium concentration by blocking Ca2+ influx, which mediated the inhibition of cell migration, AKT down regulation and the reduction of mitochondrial ROS levels. However, the reduction of mitochondrial ROS was not associated with supressed migration and AKT downregulation. Our study demonstrates the potential of NecroX-5 as an inhibitor of breast cancer metastasis.</P>

        미만성 및 분절성 호두까기 식도에서 임상양상 및 위식도역류

        목적: 호두까기 식도는 정상연동운동을 보이나 하부식도 수축파의 진폭이 현저히 증가된 경우로, 두 군데 하부 식도에서의 평균 수축파의 진폭에 근거하여 정의하고 있다. 그러나 고압의 연동성 수축이 하부 식도의 한 부위에서만 나타날수 있으며, 이러한 분절성 운동이상은 미만성 호두까기 식도로 변화할 수 있다. 본 연구에서는 호두까기 식도를 미만성과 분절성으로 확장하여 정의하고 이 두 군 사이에 임상 양상과 위식도역류에 차이가 있는지 알아보고자 하였다. 대상및 방법: 비심인성 흉통, 연하곤란 및 인후 이물감 등을 주소로 2002년 7월부터 2003년 10월까지 본원 소화기 내과를 방문하여 표준 식도내압검사에서 호두까기 식도로 진단된 50명(남자 28명, 여자 22명, 평균 51.4세)을 대상으로 하였다. 이들 중 35명에서 보행성 식도산도검사를, 42명에서 상부위장관 내시경검사를 시행하였으며, 호두까기 식도는 식도내압소견에 따라 미만성 및 분절성 두 군으로 구분하였다. 결과: 1)미만성으로 진단된 경우가 24명, 분절성으로 진단된 경우가 26명이었으며, 두 군 간에 고혈압, 허혈성 심질환, 흉통, 연하곤란, 흉부 작열감, 위산의 역류, 인후 이물감, 후두증상, 쉰목소리의 빈도에 있어 유의한 차이는 없었다. 2) 전체 검사시간 중 pH 4 미만인 전체, 기립시 및 앙와위시 측정시간 백분율, pH 4 미만의 역류가 일어난 횟수, pH 4 미만의 역류가 5분 이상 일어난 횟수, DeMeester 종합점수 또한 두 군 간에 차이가 없었다. 보행성 식도산도검사에서 위식도역류질환으로 진단된 경우는 미만성 호두까기 식도군에서는 28.6%, 분절성 호두까기 식도군에서는 38.1%로 유의한 차이는 없었다. 3) 상부위장관 내시경검사상 역류성 식도염으로 진단된 경우는 각각 23.8%, 14.3%로, 두 군 간에 유의한 차이는 없었다. 4) 24시간 보행성 식도산도검사와 상부 위장관 내시경 검사를 종합하여 분석하였을 때, 위식도역류질환으로 진단된 경우는 각각 33.3%, 38.5%로, 역시 두 군 간에 유의한 차이는 없었다. 결론: 미만성 호두까기 식도군과 분절성 호두까기식도군 사이에 임상 양상 및 위식도역류의 정도에 차이는 없었다. 그러므로 분절성 호두까기 식도의 진단에 대한 관심과 함께 더 많은 환자를 대상으로 한 추적 식도내압검사 및 치료반응에 대한 추가적인 연구가 필요하리라 생각된다. Background/Aims: The most common esophageal motility abnormality in patients with noncardiac chest pain is nutcracker esophagus. Most investigators regard nutcracker esophagus as a diffuse process involving the distal esophagus. Others consider it a segmental disturbance affecting any level of the esophagus. This study compared the clinical characteristics and gastroesophageal reflux between two groups of nutcracker esophagus patients: diffuse and segmental. Methods: From July 2002 to October 2003, 50 patients were diagnosed as nutcracker esophagus and were grouped into diffuse type (24 patients) and segmental type (26 patients). Of them, 35 patients underwent 24-hour ambulatory pH monitoring and 42 underwent gastroscopy. Results: The groups did not differ significantly with respect to the clinical characteristics such as hypertension, ischemic heart disease, chest pain, dysphagia, heartburn, acid regurgitation, laryngitis, globus or hoarseness. They did not show any significant difference in gastroesophagea reflux parameters by 24-hour ambulatory pH monitoring and gastroscopic findings. Gastroesophageal reflux disease was diagnosed in 33.3% of diffuse types and in 38.5% of segmental types, which was not significantly different ( p=0.706). Conclusions: There was no significant difference in clinical characteristics or gastroesophageal reflux between the diffuse nutcracker esophagus group and the segmental nutcracker esophagus group.

        전남지역「가고 싶은 섬」의 건강치유물질 특성

        본 연구는 전라남도가 2016년도에 선정된 전남지역「가고 싶은 섬」에 대한 섬 공기질의 건강치유물질인 테르펜 분포량과 음이온을 파악하기 위해서 실시하였다. 조사대상은 가고 싶은 섬의 침엽수림과 상록활엽수림에서 이루어졌으며 2016년 5월 ~ 9월 중 수행하였다. 테르펜 분포량 조사결과 침엽수림인 진도 관매도가 1,200 pptv로 가장 높게 나타났다. 수종별 테르펜류의 분포량은 침엽수림에서는 고흥 연홍도(1,181 pptv), 여수 낭도(1,150 pptv), 신안 반월도(1,029 pptv)순으로 타났으며, 상록활엽수림에서는 완도 소안도(851 pptv), 강진 가우도(748 pptv), 신안 박지도(627 pptv)순으로 나타났다. 테르펜류 성분별로는 침엽수림에서는 주로 α-pinene과 β-pinene이 검출되었으며, 상록활엽수림에서는 α-pinene과 δ-limonene, sabinene이 많이 검출되었다. 음 이온은 강진 가우도에서 최대값인 1,350 ea/cc으로 나타났으며, 전체 조사지점의 농도는 346 ea/cc∼ 828 ea/cc 범위로 측정되어 2015년에 조사된 도시공원(순천 장대공원)의 음이온 평균값 88 ea/cc에 비해 매우 높은 농도로 조사되었다. 건강에 유해한 물질인 총부유세균과 PM-10은 자연수준 상태로 조사되었다. 조사결과 전라남도「가고 싶은 섬」의 대기환경은 매우 안전하고 건강치유물질이 풍부한 환경으로 평가되었다. This study was conducted to provide basic data not only on the health healing material but also on the amount of terpene distribution to 「Islands that people hope to visit」 in Jeollanam-do. The investigations were conducted at coniferous forest and evergreen broadleaved forest which is stand of 「Islands that people hope to visit」 in Jeollanam-do visited frequently from May to September 2016. As a result, the amount of terpene distribution was 1,200 pptv(mean value) in the coniferous forests of Gwanmaedo, the forests of Yeonhongdo (1,181 pptv), Nangdo(1,150 pptv), Banwoldo(1,029 pptv), Soando(851 pptv), Gaudo(748 pptv), Bakjido(627 pptv). A large amount of α-pinene and β-pinene was mainly detected in the coniferous forests, while the amount of α-pinene and δ-limonene and sabinene took up a large share in the evergreen broad-leaved forest on 「Islands that people hope to visit」. The maximum value of anions was measured as 1,350 ea/cc in Gaudo. The mean value at all research sites was ranged from 346 ea/cc to 828 ea/cc. These were much higher than the average anion value of 88 ea/cc in the city park(Jang-Dae Park). Total airborne bacteria counts and PM-10 content being classed as a hamful influence material on the health were natural state level. As the result of investigation, the ambient air characteristics of「Islands that people hope to visit」in Jeollanam-do was estimated to very safe and pleasant environment.

        미산성 전해수로 처리한 생식원료의 저장 중 미생물 저감 효과 및 색도 변화

        본 연구에서는 생식 원료(현미, 보리, 당근, 감자, 신선초, 케일)를 미산성 전해수(SAEW)로 처리한 후 미생물 저감효과와 색도 변화를 측정하였다. 수돗물(TW)과 차아염소산나트륨수(NaOCl), SAEW 처리 간의 미생물 저감 효과와 색도 변화를 확인하기 위하여 각 저장온도(4, 10, 15, 20, 25, 30℃)와 저장기간(5, 10, 15, 20, 25, 30일) 동안 유통기한 연구로서 생식 원료를 저장한 후 변화를 확인하였다. 일반세균수는 생식원료 중 당근에서 가장 효과적인 저감효과를 나타냈으며 SAEW 처리가 NaOCl 처리보다 1.75 log CFU/g 더 감소하였다. 대장균군수는 보리에서 SAEW의 처리가 NaOCl 처리보다 1.54 log CFU/g 더 감소하여 생식 원료 중 가장 효과적인 저감효과를 나타내었다. 신선초와 케일에서 SAEW 처리는 lightness, redness, yellowness 값을 저하시켜 품질의 변화를 유지시킨 반면, 그 밖의 생식원료에서는 SAEW와 NaOCl 처리구간 효능의 차이는 없었다. 따라서 SAEW와 NaOCl 처리구간에 유의적인 차이(p<0.05)를 나타냈으며, 결과적으로 생식원료에 SAEW의 처리는 미생물학적으로 일반세균 및 대장균군수를 저감화시켜 저장기간 연장과 색도의 불변 효과를 나타냈으며 다른 품질의 변화 없이 생식의 원료를 효과적으로 위생처리가 가능함을 입증하였다. We investigated the changes in microbial and physicochemical characteristics of the raw materials of Saengsk (brown rice, barley, carrot, potato, Angelica utilis Makino and kale) during storage after treatment with slightly acidic electrolyzed water (SAEW). To confirm the antimicrobial effects of the treatment washing solutions as well as the physicochemical changes in chromaticity, we stored the raw materials of Saengsik for 5, 10, 15, 20, 25 and 30 days at storage temperatures of 4, 10, 15, 20, 25 and 30℃ in a shelf-life study. The effects of microbial inhibition were higher in carrot treated with SAEW than in that treated with NaOCl, as indicated by a 1.75 log CFU/g reduction. Moreover, a 1.54 log CFU/g reduction of total coliforms was achieved in brown rice in response to SAEW treatment, which was higher than NaOCl treatment at all storage temperatures and periods. The lightness, redness and yellowness values of the raw materials of Saengsik were not significantly affected by SAEW treatments, except for Angelica utilis Makino and kale, whereas SAEW treatment resulted in decreased chromaticity values when compared to the NaOCl treatment. Overall, there was a significant difference (p<0.05) between the efficacy of the SAEW and NaOCl treatments. These results indicate that washing with SAEW is an effective method to reduce the microorganisms and enhance the shelf-life of raw materials of Saengsik; therefore, it can be effectively used to sanitize ingredients of Saengik without affecting the other properties during storage.

        NASA LIS(Land Information System)을 이용한 한반도의 토양수분·증발산량 산출

        This study evaluated the accuracy of soil moisture and evapotranspiration by calculating the hydrological parameters in Korean peninsula using Land Information System(LIS) developed by US NASA. We used Noah-MP surface model to calculate hydrological parameters, and used MERRA2(Modern-Era Retrospective analysis for Research and Applications, Version 2) for hydrological forcing data. And, International Geosphere-Biosphere Program(IGBP) and University of Maryland(UMD) land cover maps were applied to compare the output accuracy, and Automated Synoptic Observing System(ASOS) of KMA was used as ground observation data. In order to evaluate the accuracy of the output data, the correlation coefficient(CC), BIAS, and efficiency factor (NSE, Nash-Sutcliffe Efficiency) were analyzed with soil moisture and evapotranspiration by ASOS ground observation data. As a result, the correlation coefficient of soil moisture using IGBP was 0.56 on average, and evapotranspiration was about 0.71. On the other hand, soil moisture using UMD was 0.68 on average and evapotranspiration was about 0.72, and the correlation coefficient by UMD was evaluated as high accuracy compared to the results by using IGBP. The correlation coefficient of soil moisture was an average of 0.68 and evapotranspiration was an average of 0.72 when MERRA2 was used as hydrological forcing data. On the other hand, the soil moisture applied with ASOS was an average of 0.66, and evapotranspiration was an average of 0.72. It is judged that the ASOS point data was reanalyzed as 0.65°× 0.5°grids, which is the same spatial resolution with MERRA2, resulting in differences in accuracy depending on the region.

        전구체 내의 셀레늄 함유량이 CuInSe2 박막의 물성에 미치는 영향

        We studied the effect on physical properties of CuInSe2 (CIS) thin films by change of the Se content in Cu-In-Se precursors. The three kinds of precursors with different Se content were deposited by a hybrid sputtering system which was equipped with the Selenium Knudsen cell (Se K-cell). The precursors were heated in an Se atmosphere for 15 minutes at 425 ℃, which produced crystalline CIS films. We characterized the crystalline CIS films using scanning electron microscopy (SEM), energy-dispersive x-ray spectroscopy (EDS), x-ray diffraction (XRD), and a Hall effect measurement system. The size of the island in the CIS films was reduced with an increasing Se content in the Cu-In-Se precursors. We found that the island structure in the CIS films disappeared with precursors of Se content over 19 at%. We were able to make a highly adhesive CIS film to Mo back contacts by using precursors with Se content of 19 at%. The solar cells fabricated with the CIS films using precursors with an Se content of 19 at% achieved 8.2% efficiency.
