        인산용액의 농도 및 적용시간 차이에 따른 상아질 표면의 형태적 변화

        The depth and patterns of demineralization according to the difference in concentration and application time of phosphoric acid were observed through the transmission electron microscope, and shear bond strengths to the acid-conditioned dentin were then measured and compared with the TEM results. To investigate the influence of polymer addition into the phosphoric acid and the effect of difference in concentration and application time of the acid, the specimens were randomly divided into 9 groups. Among the specimens, the exposed dentin surfaces were acid-conditioned with 10% polymer-thickened phosphoric acid(All Bond 2, Bisco, U.S.A.) and aqueous 10%, 20%, 30%, 40% phosphoric acid for 20 seconds, The rest of the specimens were acid-conditioned with 10% phosphoric acid for 15s, 30s, 60s, 120s respectively. The specimens were immersed in 4% glutaraldehyde in 0.1M sodium cacodylate buffer and postfixed with 1% osmium tetroxide without decalcification and then observed under a JEOL Transmission Electron Microscope(JEM 1200 EXⅡ,Japan). After the specimens were acid-conditioned as the above, primer and adhesive resin were applied to blot-dried dentin and shear bond strengths were then measured and analysed. The results were as follows : 1. The intertubular demineralization depth of 4.0-5.0㎛ in 10% polymer-thickened phosphoric acid gels was similar or slightly deeper than that of 4.0-4.5㎛ in aqueous 10% phosphoric acid solution. 2. The intertubular demineralization depth of aqueous 20%, 30% and 40% phosphoric acid solution was 6.5-7.0㎛, 6.5-7.5㎛ and 9.0-15.0㎛ respectively. It showed that the depth of dentin demineralization is partly related to the concentration of phosphoric acid solution. 3. The intertubular demineralization depth of aqueous 10% phosphoric acid solution in application time for 15s, 30s, 60s and 120s was 2.5-3.0㎛, 4.0-6.0㎛, 6.5-7.0㎛ and 8.5-14.0㎛ respectively. It showed that the depth of dentin demineralization is directly related tothe application time of phosphoric acid solution. 4. The partially demineralized dentin layer between demineralized collagen layer and unaffected dentin was showed to a width of 0.5-1.0㎛ in lower concentration groups treated with aqueous 10% phosphoric acid for 20s, 60s, 120s and 20% phosphoric acid for 20s. 5. The demineralization effect at the border of intertubular-peritubular junction was less evident than that in the peritubular and intertubular dentin. The collagen fibers in the intertubular dentin had a random orientation, whereas those that lined the tubules were circumferentially aligned. The cross-linkage of dentinal collagen in demineralized collagen layer was clearly seen. 6. A statistically significant difference of bond strengths according to the difference in phosphoric acid concentration did not exist among the groups treated with 10%, 20%, 30% and 40% acid solution (P>0.05). However, bond strengths to the treated dentin with 10% phosphoric acid solution for 30s were significantly higher than that for 120s (P<0.05).

        우측 수신증을 동반한 골반내 방선균증 1예

        Pelvic actinomycosis is a chronic granulomatous suppurative disease caused by an anaerobic grampositive organism Actinomyces israelii. It is com-monly associated with an intrauterine device(IUD) and can mimick pelvic or intra-abdominal malignant neoplasm. Ureteral obstruction leading to hydronephrosis is a rare complication of tubo-ovarian abscess. We experienced a case of hydronephrosis as a complication of pelvic actinomycotic abscess. The patient was a 46-year-old women presenting with fever and right flank pain. Leukocytosis and pyuria were present and a hydronephrosis was diagnosed by intravenous pyelography. Ultrasonography and a computerised tomography revealed a mass in right adnexum compressing the right ureter. Removal of retroperitoneal abscess and salphingo-oophorectomy were done and the diagnosis of actinomycosis was made by pathologic finding of resected mass. Postoperatively, the patient was treated with second-generation cephalosporin successfully. (Korean J Nephrol 2002;21(2):337-340)

        건강보험에서 5인 미만 사업장의 직장보험 편입 효과 평가와 정책 방향

        본 논문은 건강보험공단이 2001년 「월 이후 추진하고 있는 5인 미만 사려장의 직장보험 편입을 평가하고 향후 정책방향을 제시하는데 목적을 두고 있다. 5인 미만 사업장의 직장보험 편입은 보험 가입자의 보험료 부과의 형평성을 제고하고, 영세사업장 노동자들의 보험료 부담을 경감시키며 건강보험재정 안정 효과를 위한 것이다. 2001년 7월 이후 12월 31일까지 직장보험으로 신규편입한 대상자는 모두 약 64만명이었으나 편입이전지역보험 대상자는 약 53만명, 직장보험 대상자는 약 10만명이었다. 또한 동일 기간동안 직장보험으로 편입된 사업장은 모두 88,751개였다. 편입 전후 개인별 보험료 수준 변화를 살펴보면 보험료 부과대상 중 77.6%는 본인이 부담해야 할 보험료가 감소하였다. 대신 사업주의 보험료 부담은 상대적으로 늘어났다. 한편 2001년 7월 이후 12월까지 직장보험 편입으로 건강보험의 재정은 약 19,530백만원의 재정절감효과가 있었다. 직장보험은 7,331백만원의 보험료 수입 증가가 발생하였으며, 지역보험은 12,199백만원의 재정절감 효과가 있었다. 향후 5인 미만 사업장의 직장보험 편입이 원만히 추진되기 위해서는 피부양자 인정기준의 개선과 영세사업장 사업주의 보험료 부담을 경감시켜주기 위한 방안을 강구해야 하며, 사회적으로는 비정규직 노동자에 대한 사회보장 적용 확대를 위한 노력이 동시에 추구되어야 할 것이다. This study was conducted for the purpose of evaluating coverage of small companies with less than 5 persons by Corporate Insurance since July 2001 and of suggesting policy implication. This coverage expansion by Corporate Insurance was intended for improving equity level of contribution imposition, alleviating financial burden of employees and achieving financial stability of health insurance. While, from July 2001 to December, the number of newly covered beneficiaries was about 642 thousand persons, the number of Regional beneficiaries before the new coverage expansion by Corporate Insurance was 533 thousand persons. And the number of Corporate Insurance beneficiaries before the coverage expansion was 101 thousand persons. For the same period, the number of companies by the new coverage expansion of Corporate Insurance was 88,751. Before and after the expansion, taking into account changes of individual contribution levels, payment share levels of contribution were reduced to the 77.6% of the beneficiaries newly covered by Corporate Insurance. Instead, payment share level of contribution of employers was increased more than before. And 19,530 million won was increased in National Health Insurance Expenditure. 7,331million won was increased in Corporate Insurance revenue. Expenditure reduction effect of 12,199 million won was able to be obtained in Regional Insurance. In order to implement successfully the coverage expansion by Corporate Insurance in times ahead, employers' contribution share should be reduced, when considering low profitability levels of small companies.

        유산균 투여가 건강한 성인의 분변미생물 및 부패산물 생성에 미치는 영향

        유산균의 장내균총과 부패산물 생성에 미치는 영향을 알아보기 위하여, 건강한 성인 남녀 6명의 지원자에게 Lactobacillus acidophilus 와Bifidobacterium longum으로 제조한 유산균분말(각각 1.5×10 exp (9) cells)을 하루에 2회씩 2주동안 섭취하도록 하였다. 그리고 지원자 분변으로부터 균총수 변화와 부패산물인 암모니아, indole, skatole, p-cresole량을 측정하였다. 유산균의 투여 기간중에 분변내 bifidobacteria의 수가 8.78±0.39(log cfu/g feces±S.D)에서 9.27±0.29로 증가하였고(p<0.05) 투여 중지후에는 약간 감소하였으며, lactobacilli의 수는 6.15±0.80에서 6.76±0.48로 증가하였다(p<0.05). Enterococcus 균수는 투여 기간중에 6.66±0.80에서 7.72±0.40으로 증가하였다가 투여 중지후에는 감소하는 경향을 보였다. 그리고 혐기성균인 Bacteroides는 투여중에 8.45±0.34에서 9.15±0.21으로 증가하였으나(p<0.01), 유해균주로 알려진 Closridium(lecithinase negative)과 Staphylococcus 균수는 현저히 감소하였다. 그러나, 유산균 투여 실험기간인 6주 동안 분변내 암모니아, indole, skatole, p-cresole의 유의적인 농도변화는 없었다. To investigate the effects of lactic acid bacteria administration on fecal microflora and putrefactive metabolites in human being, Lactobacillus acidophilus and Bifidobacterium longum powder (1.5×10 exp (9) cells, respectively) was administrated to six healthy volunteers (average 28 years old) twice a day for 2 weeks. During the administration of lactic acid bacteria, the numbers of bifidobacteria, lactobacilli, and enterococci in feces were increased significantly, whereas those of Staphylococcus and lecithinase-negative Clostridium were decreased considerably. In addition, a number of anaerobic Bacteroides were increased. However, the contents of fecal ammonia and putrefactive metabolites (indole, skatole, p-cresole) were not changed during the administration.

      • 자기유변유체를 이용한 Fused silica glass의 미세연마공정특성에 관한 연구

        A new, commercially available polishing process called magnetorheological fluid polishing is used to fabrication optical glass and bio-chip. This polishing process guarantees high polishing quality by controlling the fluid density electrically. The applied material in experiments is fused silica glass. Fused silica glass is widely used in the optical and biochip field because of high degree of purity. For MR polishing experiments, MR fluid was composed with DI-water, carbonyl iron and nano slurry ceria. The wheel speed and electric current were chosen as the variables for analyzing the characteristics of MR polishing process. In results, the surface roughness according to the variation of wheel speed and electric current was analyzed.

      • Hypoxia-Responsive MicroRNA-101 Promotes Angiogenesis <i>via</i> Heme Oxygenase-1/Vascular Endothelial Growth Factor Axis by Targeting Cullin 3

        <P>Aims: Hypoxia induces expression of various genes and microRNAs (miRs) that regulate angiogenesis and vascular function. In this study, we investigated a new functional role of new hypoxia-responsive miR-101 in angiogenesis and its underlying mechanism for regulating heme oxygenase-1 (HO-1) and vascular endothelial growth factor (VEGF) expression. Results: We found that hypoxia induced miR-101, which binds to the 3 ' untranslated region of cullin 3 (Cul3) and stabilizes nuclear factor erythroid-derived 2-related factor 2 (Nrf2) via inhibition of the proteasomal degradation pathway. miR-101 overexpression promoted Nrf2 nuclear accumulation, which was accompanied with increases in HO-1 induction, VEGF expression, and endothelial nitric oxide synthase (eNOS)-derived nitric oxide (NO) production. The elevated NO-induced S-nitrosylation of Kelch-like ECH-associated protein 1 and subsequent induction of Nrf2-dependent HO-1 lead to further elevation of VEGF production via a positive feedback loop between the Nrf2/HO-1 and VEGF/eNOS axes. Moreover, miR-101 promoted angiogenic signals and angiogenesis both in vitro and in vivo, and these events were attenuated by inhibiting the biological activity of HO-1, VEGF, or eNOS. Moreover, these effects were also observed in aortic rings from HO-1(+/-) and eNOS(-/-) mice. Local overexpression of miR-101 improved therapeutic angiogenesis and perfusion recovery in the ischemic mouse hindlimb, whereas antagomiR-101 diminished regional blood flow. Innovation: Hypoxia-responsive miR-101 stimulates angiogenesis by activating the HO-1/VEGF/eNOS axis via Cul3 targeting. Thus, miR-101 is a novel angiomir. Conclusion: Our results provide new mechanistic insights into a functional role of miR-101 as a potential therapeutic target in angiogenesis and vascular remodeling. Antioxid.       • Central nervous system aquaporin-4 autoimmunity presenting with an isolated cerebral abnormality

        <P> Advances in the understanding of central nervous system aquarporin-4 autoimmunity have promoted the recognition of diverse clinical presentations beyond the traditional view of neuromyelitis optica. We describe a patient who developed hemiparesis caused by an extensive cerebral lesion as an initial manifestation of central nervous system aquarporin-4 autoimmunity. Although the patient had no history of optic neuritis or myelitis, not only was serum anti-aquarporin-4 antibody positive, but an imaging, treatment response and histopathological features also revealed characteristic findings suggestive of central nervous system aquarporin-4 autoimmunity. The present case highlights the importance of a comprehensive evaluation for anti-aquarporin-4 antibody even in patients presenting with isolated cerebral lesions. </P>
