Food protein-induced enterocolitis syndrome (FPIES), though first reported in the 1970s, remains poorly understood and likely underdiagnosed. It is a non-immunoglobulin E (IgE)-mediated food allergy syndrome, most commonly identified in infancy and childhood. It can manifest as a constellation of symptoms following food ingestion, including repetitive and projectile emesis (1–4 hours), accompanied by pallor, lethargy, muscular hypotonia, and diarrhea (5–10 hours). In more severe reactions, significant leukocytosis with neutrophilia, thrombocytosis, metabolic derangements, methemoglobinemia, anemia, low albumin, and total protein may be present. Hypotension and ultimately hypovolemic distributive shock may occur in up to 15%–20% of cases. The diagnosis of FPIES is challenging and providers continue to face difficulties in management. This review article aims to highlight the most recent updates in epidemiology, natural history, pathophysiology, potential diagnostic markers, and guidelines for the management of FPIES.
Optimal designs for small Poisson regression experiments using second-order asymptotic

This paper considers the issue of obtaining the optimal design in Poisson regression model when the sample size is small. Poisson regression model is widely used for the analysis of count data. Asymptotic theory provides the basis for making inference on the parameters in this model. However, for small size experiments, asymptotic approximations, such as unbiasedness, may not be valid. Therefore, first, we employ the second order expansion of the bias of the maximum likelihood estimator (MLE) and derive the mean square error (MSE) of MLE to measure the quality of an estimator. We then define DM-optimality criterion, which is based on a function of the MSE. This criterion is applied to obtain locally optimal designs for small size experiments. The effect of sample size on the obtained designs are shown. We also obtain locally DM-optimal designs for some special cases of the model.
Securing Mobile Ad Hoc Networks Using Enhanced Identity-Based Cryptography

Recent developments in identity-based cryptography (IBC) have provided new solutions to problems related to the security of mobile ad hoc networks (MANETs). Although many proposals to solve problems related to the security of MANETs are suggested by the research community, there is no one solution that fits all. The interdependency cycle between secure routing and security services makes the use of IBC in MANETs very challenging. In this paper, two novel methods are proposed to eliminate the need for this cycle. One of these methods utilizes a key pool to secure routes for the distribution of cryptographic materials, while the other adopts a pairing-based key agreement method. Furthermore, our proposed methods utilize threshold cryptography for shared secret and private key generation to eliminate the “single point of failure” and distribute cryptographic services among network nodes. These characteristics guarantee high levels of availability and scalability for the proposed methods. To illustrate the effectiveness and capabilities of the proposed methods, they are simulated and compared against the performance of existing methods.

The effects of vertical component of earthquakes on torsional amplification due to mass eccentricity in seismic responses of base-isolated structures subjected to near-field ground motions are studied in this paper. 3-, 6- and 9-story superstructures and aspect ratios of 1, 2 and 3 have been modeled as steel special moment frames mounted on Triple Concave Friction Pendulum (TCFP) bearings considering different period and damping ratios. Three-dimensional linear superstructures resting on nonlinear isolators are subjected to both 2 and 3 component near-field ground motions. Effects of mass eccentricity and vertical component of 25 near-field earthquakes on the seismic responses including maximum isolator displacement and base shear as well as peak superstructure acceleration are studied. The results indicate that the effect of vertical component on the responses of asymmetric structures, especially on the base shear is significant. Therefore, it can be claimed that in the absence of the vertical component, mass eccentricity has a little effect on the base shear increase. Additionally, the impact of this component on acceleration is remarkable so the roof acceleration of a nine-story structure has been increased 1.67 times, compared to the case that the structure is subjected to only horizontal components of earthquakes.

5-Hydroxymethyl furfural (HMF) is known as a noteworthy platform in a biorefinery concept. HMF was prepared via fructose dehydration in aqueous and organic media, using three methods, i.e., conventional heating, ultrasonication and microwave irradiation. Water, methyl isobutyl ketone (MIBK), methyl ethyl ketone and ethyl acetate were used as media for HCl-catalyzed synthesis of HMF. FTIR and 1H-NMR spectroscopies were used for analysis. The synthesis yield and selectivity were investigated to optimize variables such as fructose concentration, catalyst dosage, temperature, irradiation power, solvent, and the reaction atmosphere. It was found that the yield in the organic media was superior to that of the aqueous ones. In addition, nitrogen atmosphere favored higher yield than air, due to lack of HMF oxidation. As conclusion, the highest yields of the conventional, ultrasonicated and microwave-assisted reactions were 87, 53, and 38%, respectively. In the reactions ultrasonically promoted, the reaction time scale was highly reduced from hours to minutes. The yield was varied with treatment times, so that ultrasonication was recognized to be the best approach in terms of yield, while the microwave method was the fastest one. Selectivity varied from 60 to 90% depending the reaction media and promotion method.

Purpose : Field of view and voxel resolution of cone beam computed tomography (CBCT) might affect the diagnostic capability. This study was performed to compare between the standard and HiRes zoom modes in the diagnosis of external root resorption (ERR) using CBCT. Materials and Methods : Sixty three small cavities (0.25 mm depth and 0.5 mm diameter) were simulated on the buccal, lingual, and proximal surfaces at three different levels of 16 roots of teeth. After covering the root with nail varnish, the roots were inserted in the sockets and the model was placed in a water-containing lacuna. CBCT scans were taken in both standard and HiRes zoom modes using NewTom VG (QR srl Company, Verona, Italy). Then, an observer assessed the images to determine the presence or absence of the cavities. This process was repeated by increasing the size and depth of cavities to 0.5 mm depth and 1 mm diameter. Data were analyzed by McNemar test. The sensitivity, specificity, positive predictive value, negative predictive value, and likelihood ratio in evaluation of the simulated cavities were calculated. Results : There was a significant difference between the two imaging modes in diagnosing the shallow cavities (p=0.02).The sensitivity of the standard zoom in detecting the shallow cavities was lower than that of the HiRes zoom. The likelihood ratio of the HiRes zoom was higher in the diagnosis of both cavity types. Conclusion : This study suggested that a smaller voxel size in the HiRes zoom mode of CBCT is preferred for diagnosis of ERR.
