Background: Pulmonary paragonimiasis is caused by consumption of raw or improperlycooked crustacea infected with the laval stage (metacercaria) of Paragonimus westermani. The most characteristic symptoms were rust-colored sputum and cough. Paragonimiasis causes pleural thickening or effusion in 48% of the patients. Pleuro-pulmonary paragonimiasis can be easily overlooked by physicians who do not suspect this disease in the differential diagnosis. Method: We compared the outcomes of 11 patients with paragonimus empyema managed either through thoracotomy or pig-tail catheter drain. These patients were confirmed by food history, clinical and radiological findings, and laboratory data. Results: The male and female ratio was 1.75 : 1, and mean age was 40.0 ± 13.5 years. AII patients had pulmonary symptoms such as cough or chest pain. Serum-ELISA for paragonimiasis were all positive (mean titer was 0.57). AII patients had pleural effusion in radiological findings( 2 patients had bilateral pleural effusion). All patients received praziquantel (75 ㎎/㎏/day for 3days). Two patients were treated with thoracotomy and nine patients were treated with pig-tail drain. Hospital stay were 14.5 days in thoracotomy group and 5.6 days in pig-tail group respectively, Conclusion: Compared to the conventional thoracotomy grouP, the patients with paragonimus empyema who received pig-tail catheters had a significantly-decreased period of drain in situ, were clinically improved earlier, and were discharged earlier.

Scrub typhus is a potentially fatal infectious disease caused by the organism Orientia tsutsugamushi. Clinical manifestations are fever, skin rash, eschar and varying degree of respiratory distress. The pleural effusion in scrub typhus is rare and secondary to the destruction of vascular endothelium. Because the respiratory symptoms are generally mild and the pleural effusion in scmb typhus is rare than in interstitial pneumonia, there are few comments about the characteristics of scmb typhus with pleural effusion. So we made the comparative study of scmb typhus patients between with pleural effusion and without pleural effusion. 연구배경 치명적일 수 있는 감염질환 중 하나인 쯔쯔가무시중(Scrub typhus)에서 드문 흉막삼출을 동반한 환자에 대한 고찰은 다른 문헌에서도 자주 언급되지 않았다. 따라서 본 저자들은 흉막삼출이 있는 경우와 없는 경우에 따른 쯔쯔가무시중 환자들을 비교하고자 하였다. 방법 2003년에서 2006년까지 조선대학교 병원 내과에 입원중인 환자 중 쯔쯔가무시중 진단을 받은 총 109명의 환자를 대상으로 하여 흉부 X선 검사 및 혈청학적 검사(CPK, LDH, ESR, CRP, AST, ALT, ADA), 혈액학적 검사(WBC, PLT) 및 백분율 검사등을 실시하였다. 결과 흉막삼출이 있는 환자들의 경우 흉막 삼출이 없는 환자들에 비해 ESR 수치가 통계학적으로 의의있게 높았다(p-vaule < 0.05). 결론 흉막삼출 유무에 따른 쯔쯔가무시중 환자간의 비교에 있어 ESR 수치가 통계학적인 의의를 가졌다.
Epidemiology of Childhood Obesity in Korea

<P>Over the past several decades, the prevalence of obesity has increased dramatically worldwide and is increasing not only in developed countries, but also in developing countries. This increase may lead to an increase in the incidence of chronic diseases throughout the lifespan. In Korean children and adolescents, the prevalence of obesity increased from 6.8% in 1998 to 10.0% in 2013. Obesity is a state that more commonly influences children and adolescents of lower socioeconomic status (SES) than those with a higher SES. However, the prevalence of metabolic syndrome in a nationally representative sample of Korean adolescents decreased from 1998 to 2012. According to the Diabetes Fact Sheet of the Korean Diabetes Association, the prevalence of type 2 diabetes among children aged 18 years or younger was 153.5 per 100,000 in 2006 and 205.0 per 100,000 in 2013. Obesity is a complex disease influenced by many interacting factors, such as adipocytokines, lipopolysaccharide-binding protein, adenovirus 36 infection, birth weight, lifestyle, and endocrine-disrupting chemicals. Obesity in youth can adversely impact practically every organ system and lead to serious consequences, such as metabolic, gastrointestinal, pulmonary, cardiovascular, and psychosocial complications. Therefore, coordinated efforts by governments, organizations, communities, and individuals are needed to prevent and treat childhood obesity. In particular, a long-term policy to improve the social environment will also be necessary.</P>
Diabetes screening in South Korea: a new estimate of the number needed to screen to detect diabetes

Background/Aims: The Korean Diabetes Association (KDA) guidelines recommend adults aged ≥ 40 years and adults aged ≥ 30 years with diabetes risk factors for diabetes screening. This study aimed to determine the age threshold for diabetes screening in Korean adults. Methods: This study was based on the analyses of Korean adults aged ≥ 20 years using the Korea National Health and Nutrition Examination Survey (KNHANES) and the National Health Insurance Service-National Sample Cohort (NHIS-NSC). To evaluate screening effectiveness, we calculated the number needed to screen (NNS). Results: NNS to detect diabetes decreased from 63 to 34 in the KNHANES and from 71 to 42 in the NHIS-NSC between the ages of 30–34 and 35–39. When universal screening was applied to adults aged ≥ 35, the NNS was similar to that of adults aged ≥ 40. Compared to the KDA guidelines, the rate of missed screening positive in adults aged ≥ 20 decreased from 4.0% to 0.2% when the newly suggested screening criteria were applied. Conclusions: Universal screening for adults aged ≥ 35 and selective screening for adults aged 20 to 34, considering diabetes risk factors, may be appropriate for detecting prediabetes and diabetes in South Korea.
Development and Validation of the Korean Diabetes Risk Score: A 10-Year National Cohort Study

<P><B>Background</B></P><P>A diabetes risk score in Korean adults was developed and validated.</P><P><B>Methods</B></P><P>This study used the National Health Insurance Service-National Health Screening Cohort (NHIS-HEALS) of 359,349 people without diabetes at baseline to derive an equation for predicting the risk of developing diabetes, using Cox proportional hazards regression models. External validation was conducted using data from the Korean Genome and Epidemiology Study. Calibration and discrimination analyses were performed separately for men and women in the development and validation datasets.</P><P><B>Results</B></P><P>During a median follow-up of 10.8 years, 37,678 cases (event rate=10.4 per 1,000 person-years) of diabetes were identified in the development cohort. The risk score included age, family history of diabetes, alcohol intake (only in men), smoking status, physical activity, use of antihypertensive therapy, use of statin therapy, body mass index, systolic blood pressure, total cholesterol, fasting glucose, and γ glutamyl transferase (only in women). The C-statistics for the models for risk at 10 years were 0.71 (95% confidence interval [CI], 0.70 to 0.73) for the men and 0.76 (95% CI, 0.75 to 0.78) for the women in the development dataset. In the validation dataset, the C-statistics were 0.63 (95% CI, 0.53 to 0.73) for men and 0.66 (95% CI, 0.55 to 0.76) for women.</P><P><B>Conclusion</B></P><P>The Korean Diabetes Risk Score may identify people at high risk of developing diabetes and may be an effective tool for delaying or preventing the onset of condition as risk management strategies involving modifiable risk factors can be recommended to those identified as at high risk.</P>
Current status of managing diabetes mellitus in Korea

Diabetes mellitus is an increasing global health problem. Guidelines for diabetic care recommend management of lifestyle and risk factors (glucose, blood pressure, and cholesterol), as well as regular screening for complications associated with treatment of the conditions related to diabetes. The prevalence of diabetes increased from 8.6% to 11.0% from 2001 to 2013. According to the diabetes fact sheet 2015, the proportion of patients with diabetes treated with antihypertensive medications increased from 56.0% to 62.5% from 2006 to 2013, and 49.5% of those with diabetes were being treated with lipid-lowering medications in 2013, a 1.8-fold increase since 2006. According to the 2014 Korea National Health and Nutrition Examination Survey data, 45.6% of patients with diabetes achieved a hemoglobin A1c level of < 7.0%, 72.8% achieved a blood pressure of < 140/85 mmHg, and 58.0% achieved a low density lipoprotein cholesterol level of < 100 mg/dL. Only 19.7% of patients with diabetes had good control of all three of these parameters. Despite improvements in health promotion efforts, the rates of adherence to medication and risk-factor control are low. Therefore, a systematic approach to managing diabetes, including self-management education, is needed to prevent or delay complications. The government needs to establish a long-term policy to address the growing burden of diabetes.
