Pycnodysostosis is an autosomal recessive disorder characterized by osteosclerosis, small stature, acro-osteolysis of the distal phalanges, loss of the mandibular angle, separated cranial sutures with open fontanels, and frequent fractures. One identified cause of the disease is reduced activity of the cysteine protease cathepsin K. A 48-year-old woman with a history of frequent fractures presented with a severe gait disturbance. Radiography, computed tomography, magnetic resonance imaging, and gene analysis were performed. Physical examination revealed open fontanels, and radiographs showed increased bone density. DNA sequence analysis revealed a deletion mutation of the cathepsin K gene. We diagnosed pycnodysostosis based on these findings. The magnetic resonance and computed tomography images demonstrated multilevel spinal canal stenosis due to ossification of the yellow ligament. We performed a laminectomy, and the patient’s neurological signs and symptoms improved. To our knowledge, this is the first case of pycnodysostosis with ossification of the yellow ligament.
Drop Metastasis of Adrenocorticotropic Hormone-Producing Pituitary Carcinoma to the Cauda Equina

The diagnosis of pituitary carcinoma cannot be made easily histologically, and most cases of pituitary carcinoma are diagnosed only after the clinical detection of metastasis. Distant metastasis of pituitary tumor occurs in 0.1% to 0.2% of cases and has been reported in the liver, bone and central nervous system, with only one case of metastasis to the cauda equine reported. This study describes a rare case of the drop metastasis of adrenocorticotropic hormone-producing pituitary adenocarcinoma to the cauda equina, causing cauda equina syndrome.

Backgrounds/Aims: The present study analyzed the postoperative outcomes in patients who underwent hepatectomy or pancreatectomy, with a history of intra-abdominal surgery involving other organs, to elucidate surgical efficacy. Methods: We examined the perioperative parameters in 28 patients who underwent hepatectomy (n=12) and pancreatectomy (n=16) after receiving prior abdominal organ resection (esophagectomy, n=2; gastrectomy, n=5; resection of small intestine, n=2; appendectomy, n=5; colorectal resection, n=9; hepatectomy, n=1; cholecystectomy, n=3; splenectomy, n=2, pancreatectomy ,right adrenectomy, nephrectomy and myoma uteri, n=1 each). Results: Age, gender, a history of comorbidities, and primary diseases were not significantly different between the groups. The present operation was predominantly indicated for liver metastases in all patients undergoing hepatectomy. Several diseases were detected in pancreaticoduodenectomy (PD) patients. Laboratory data were not significantly different between groups. Although operating time and blood loss during hepatectomy did not differ significantly between the groups, the operating time was significantly longer in patients undergoing PD compared with distal pancreatectomy (p<0.05). Red cell blood transfusion was most frequently used in patients who underwent major hepatectomy and PD (p<0.05). The prevalence of postoperative complications was not significantly different between groups. Hospital death was not observed and the period of hospital stay did not differ between groups. Conclusions: Carefully scheduled hepatectomy or pancreatectomy is safe even in cases with prior abdominal surgery under the present strategy.

Telmisartan is a biopharmaceutics drug classification system class II drug having a low solubility at neutral pH region. The purpose of this study is to clarify the process of dissolution and precipitation of telmisartan in the gastrointestinal tract from its oral formulation containing meglumine, a pH modifier (alkalizer). Granules of telmisartan containing different amount of meglumine were prepared, and its dissolution and precipitation profiles were investigated under various pH conditions in vitro. It was demonstrated that meglumine induced a supersaturation of telmisartan both at pH 5.0 and pH 6.8. Dissolved amount of telmisartan fluctuated over time and the pattern of dissolved concentration differed depending on the pH and the amount of meglumine. Powder X-ray diffraction analysis and differential scanning calorimetry analysis on precipitates from supersaturated solutions showed that pH of the dissolution medium profoundly affects the crystalline form of telmisartan. Furthermore, the effect of meglumine on oral absorption of telmisartan was investigated with dissolution/ permeation System in vitro. At pH 6.8, permeated amount was significantly higher with the formulation which contains larger amount of meglumine. Therefore, despite the very low solubility of telmisartan at neutral pH, meglumine significantly provided high solubility of telmisartan by induction and maintenance of its supersaturation.

Seismic isolation and vibration control techniques have been developed and put into practical use by challenging researchers and engineers worldwide since the latter half of the 20th century, and after more than 40 years, they are now used in thousands of buildings, private residences, highways in many seismic areas in the world. Seismic isolation and vibration control structures can keep the structures undamaged even in a major earthquake and realize continuous occupancy. This performance has come to be recognized not only by engineers but also by ordinary people, becoming indispensable for the formation of a resilient society. However, the dynamic characteristics of seismically isolated bearings, the key elements, are highly dependent on the size effect and rate-of-loading, especially under extreme loading conditions. Therefore, confirming the actual properties and performance of these bearings with full-scale specimens under prescribed dynamic loading protocols is essential. The number of testing facilities with such capacity is still limited and even though the existing labs in the US, China, Taiwan, Italy, etc. are conducting these tests, their dynamic loading test setups are subjected to friction generated by the large vertical loads and inertial force of the heavy table which affect the accuracy of measured forces. To solve this problem, the authors have proposed a direct reaction force measuring system that can eliminate the effects of friction and inertia forces, and a seismic isolation testing facility with the proposed system (E-isolation) will be completed on March 2023 in Japan. This test facility is designed to conduct not only dynamic loading tests of seismic isolation bearings and dampers but also to perform hybrid simulations of seismically isolated structures. In this paper, design details and the realization of this system into an actual dynamic testing facility are presented and the outcomes are discussed.
Experimental Flight of KIT Student’s Rocket in France

KIT Student's Rocket has been developed by a student group of Kyushu Institute of Technology lor rocket launch campaign held at La Courtine in France since 2006. This paper introduces the newest design of rocket. The rocket has the body length of 2120㎜, and weighs 14.6㎏ and can reach to an altitude of about 700㎜ by a solid rocket motor provided by CNES (the French Centre National D’Etudes Spatiales). The rocket is controlling rolling attitude during ascent phase and then deploying a parafoil at the apogee of the trajectory for recovery guidance to an aiming point.

To realize large-format compact array detectors covering a wide far-infrared wavelength range up to 200 $\mu$m, we have been developing Blocked-Impurity-Band (BIB) type Ge detectors with the room-temperature surface-activated wafer bonding technology provided by Mitsubishi Heavy Industries. We fabricated various types of $p^+$-$i$ junction devices which possessed a BIB-type structure, and evaluated their spectral response curves using a Fourier transform spectrometer. From the Hall effect measurement, we also obtained the physical characteristics of the $p^+$ layers which constituted the $p^+$-$i$ junction devices. The overall result of our measurement shows that the $p^+$-$i$ junction devices have a promising applicability as a new far-infrared detector to cover a wavelength range of 100--200 μm.
