Examining Patterns of Polypharmacy in Bipolar Disorder: Findings from the REAP-BD, Korea

Based on Korean data from the Research on Asian Psychotropic Prescription Pattern for Bipolar Disorder, this study tried to present prescription patterns in biopolar disorder (BD) and its associated clinical features. Based on the information obtained from the study with structured questions, the tendency of prescription pattern was studied and analyzed. Polypharmacy was predominant, including simple polypharmacy in 51.1% and complex polypharmacy in 34.2% of patients. Subjects associated with simple or complex polypharmacy were significantly younger, had higher inpatient settings, a larger portion of onset with manic episode, a shorter duration of untreated illness, a shorter duration of current episode, were more overweight, used less antidepressants and used more anxiolytics. These findings can suggest higher polypharmacy rate in more severe BD and highlight the necessity of monitoring the weight of subjects with polypharmacy.

<P>Obesity is an important health problem caused by positive energy balance. Generally, low calorie dietary intake combined with regular exercise is the most common modality to lose bodily fat in obese people. Although this is the first modality of choice for obesity treatment, it needs to be applied to obese patients for at least 12 weeks or more and it does not provide consistent results because it is difficult to suppress increased appetite due to exercise. Recently, many researchers have been applying hypoxic conditions for the treatment of obesity, as many studies show that people residing in high altitudes have a lower percentage of body fat and fewer obesity-related illnesses than people living at sea level. Hypoxic therapy treatment, including hypoxic exposure or hypoxic exercise training, is recommended as a way to treat and prevent obesity by suppression of appetite, increasing basal metabolic rate and fat oxidation, and minimizing side effects. Hypoxic therapy inhibits energy intake and appetite-related hormones, and enhances various cardiovascular and metabolic function parameters. These observations indicate that hypoxic therapy is a new treatment modality for inducing fat reduction and promoting metabolic and cardiovascular health, which may be an important and necessary strategy for the treatment of obesity. As such, hypoxic therapy is now used as a general medical practice for obesity treatment in many developed countries. Therefore, hypoxic therapy could be a new, practical, and useful therapeutic modality for obesity and obesity-related comorbidities.</P>
Evaluation of Time-Based Arc Flash Detection with Non-contact UV Sensor

An arc fash is caused by electrical faults or mechanical non-contact such as insulation failure, short circuit, partial disconnection, and poor contact, emits high energy and strong light, so it can be detected by optical methods with relatively simple structures. The optical detection methods using the wavelength of light may malfunction depending on the surrounding environment, the sunlight and lighting. In addition, with such methods, it is difcult to measure the detection distance to the point where the arc fash occurs, measure the detection range of the detection sensor, and monitor how amounts of harmful from the arc fash. This paper aims to examine the techniques for quantitatively detecting arc fash caused by electrical faults. To this end, a system for detecting the time when an arc fash occurs and the techniques for calibrating this system were analyzed, respectively. A feld test was also conducted to detect the arc fash caused by mechanical non-contact between the pantograph of electric railway vehicles and the catenary. In the feld test, 9 number of arc fashes were detected using the percentage (%) technique for quantitative detection. The technique of detecting the time when an arc fash occurs is a new quantitative evaluation method, which is expected to be widely used to prevent negligent accidents in the electrical feld as well as being able to determine in real time whether there is a harmful arc.

<P>The objective of this study was to compare the virulence and pathogenicity of a combination of concurrent infections of two genotypes of porcine circovirus type 2 (PCV2) and two genotypes of porcine reproductive and respiratory syndrome virus (PRRSV) in terms of PCV2 viraemia, and PCV2-associated lesions and antigens in co-infected pigs. Pigs with PCV2a (or 2b)/type 1 (or type 2) PRRSV had significantly (<I>P</I><0.05) higher mean clinical respiratory scores and lower average daily weight gain compared with pigs with PCV2a (or 2b). Co-infection induced significantly lower levels of anti-PCV2 and anti-PRRSV IgG antibodies than infection with one genotype alone, regardless of the genotype of the two viruses. Pigs with PCV2a (or 2b)/type 2 PRRSV had significantly (<I>P</I><0.05) higher levels of PCV2 viraemia, more severe PCV2-associated lesions, and more PCV2 DNA within the lesions compared with pigs with PCV2a (or 2b)/type 1 PRRSV. However, there was no significant difference in these parameters in pigs with PCV2a/type 2 PRRSV or PCV2b/type 2 PRRSV. The results of this study demonstrate significant differences in the virulence and pathogenicity of type 1 and type 2 PRRSV but no significant differences in the virulence and pathogenicity of PCV2a and PCV2b with respect to the production of PCV2-associated lesions.</P>

Objective-We aimed to identify depressive symptom profiles that indicated the presence of depressive disorder and present optimal cut-off sub-scores for depressive symptom profiles for detecting depressive disorder in elderly subjects with chronic physical diseases including diabetes, chronic obstructive pulmonary disease/asthma, and coronary artery disease, using the Patient Health Questionnaire-9 (PHQ-9). Methods-Two hundred and thirty-one elderly patients with chronic physical diseases were recruited consecutively from a university-affiliated general hospital in South Korea. Results-Greater severities of all 9 depressive symptoms in the PHQ-9 were presented in those with depressive disorder rather than those without depressive disorder. A binary logistic regression modeling presented that little interest [adjusted odds ratio (aOR)=4.648, p<0.001], reduced/increased sleep (aOR=3.269, p<0.001), psychomotor retardation/agitation (aOR=2.243, p=0.004), and concentration problem (aOR=16.116, p<0.001) were independently associated with increased likelihood of having depressive disorder. Receiver operating characteristics (ROC) curve analysis presented that the optimal cut-off value of score on the items for little interest, reduced/increased sleep, psychomotor retardation/agitation and concentration problem (PHQ-9) for detecting depressive disorder was 4 with 61.9% of sensitivity and 91.5% of specificity [area under curve (AUC)=0.937, p<0.001]. Conclusion-Our findings suggested that the diagnostic weighting of little interest, reduced/increased sleep, psychomotor retardation/agitation, and concentration problem is needed to detect depressive disorder among the elderly patients with chronic physical diseases.
정부클러스터 정책이 기업 혁신활동에 미치는 영향 : 한국의료산업 클러스터를 중심으로

정부의 적극적인 첨단의료산업 육성정책에 따라 각 지역에 위치한 의료산업클러스터가 초기 조성단계에서 벗어나 본격적인 운영단계에 들어섬에 따라 효율적인 클러스터 활성화 정책에 대한 전략적인 접근을 필요로 하고 있다. 본 연구는 이 점에 주목하여 의료클러스터의 질적 성과에 대한 측정방법을 선행연구를 토대로 설정하고, 클러스터에 입주한 기업들의 성과와 형태의 다양성을 분석 하여 향후 정부의 클러스터 정책에 대한 개선방안을 제시한다. 클러스터 성과에 대한 측정항목과 기업의 활동 유형에 관한 명제를 도출 한다. 이와 같은 연구목표는 기존문헌에서 논의했던 클러스터 활동의 유형구분과 성과측정에 대한 논의를 종합하여 새로운 연구 프레임을 도출 한 후, 각 클러스터에서 제공된 현황자료와 특허자료를 이용해 발굴한 기업들의 분석을 통해 적용함으로써 설명 할 수 있었다. 여기서 도출된 세 가지 명제는 클러스터의 진화에 따른 성과 측정을 체계적으로 도출하여 정부 및 지방자치단체의 클러스터 육성정책 수립과 클러스터내 기업의 기술혁신과 비즈니스 모델 혁신 방법에 대한 방법적인 선택에 도움을 줄 것이다. According to Korean government"s aggressive policy to foster high-tech medical industry, medical industry clusters located in each region have entered a full-scale operation phase moving away from an initial phase of development. Taking this into account, this study sets up the measurement method of qualitative performance of the clusters on the basis of previous studies and analyzes the performance and type diversity of the corporations in the clusters in order to present improvement ways for future government support of the clusters. The proposition of the paper is about the measurement items of the cluster performances and the activity types of the enterprises. The research goal can be reached by examining the types of the cluster activities and their performance measurement discussed in the existing literature, designing a new research framework, and applying the corporate analysis that has been discovered using the status reports and patent data provided by each cluster. These findings, systematically derived from the evolution of the clusters, will help to establish policies to foster clusters of governments and local governments and select an effective method for technical innovation and business model innovation.
