      • 유방암 환자의 말초혈액에서 역전사효소연쇄중합반응을 이용한 Human Mammaglobin 측정의 임상적 유용성

        Background: The mammaglobin gene encodes a novel protein that is secreted from the mammalian epithelium of normal breast tissue as well as malignant breast cancer tissues. In order to ascertain the prognostic value of mammaglobin gene in breast cancer patients, we measured the expression of human mammaglobin (hMAM) by RT-PCR method in various stages of breast cancer patients. Methods: Peripheral blood samples from forty healthy volunteers and 114 breast cancer patients were obtained. Peripheral blood stem cells (PBSC) collected for the purpose of autologous stem cell transplantation in five patients with metastatic breast cancer and ten patient with high risk for relapse and no evidence of disease were used for hMAM assay. Results: All samples from peripheral blood of forty healthy individuals (twenty males and twenty females) were negative for hMAM, whereas 43 of 114 samples (38%) from breast cancer patients were positive for hMAM mRNA. All the normal breast tissues were positive for hMAM mRNA. hMAM mRNA expression was detected in 11 of 42 (26%) in breast cancer patients who underwent for curative resection and had no evidence of disease, in 8 of 25 (34%) with chemo-sensitive relapsed disease, and in 16 of 32 (53%) with chemo-refractory progressive disease. Eight (53%) samples from peripheral blood of 15 breast cancer patients with metastatic disease at diagnosis were positive for hMAM. Three (20%) samples from peripheral blood stem cells of 15 breast cancer patients for high dose chemotherapy were positive for hMAM. Conclusion : In contrast to healthy volunteers, hMAM transcripts were detected in the peripheral blood of breast cancer patients. The frequency of hMAM expression in peripheral blood was correlated with the clinical stages of disease, but, was not significant. The contamination of hMAM expressing cells in the stem cell pool warrants additional effective purging method before the transplantation. The clinical relevance of hMAM RT-PCR-based tumor cell detection in the peripheral blood of breast cancer patients should be further evaluated in prospective studies.

        포상기태와 융모상피암 환자의 갑상선기능

        It has been recognized that hyperthyroidism occur in patients with trophoblastic disease, either hydatidiform moles or choriocarcinomas. In the past decade, several lines of evidence have shown that human chorionic gonadotropin, secreted by the trophoblastic disease, is a thyroid stimulator and causes hyperthyroidism. In order to evaluate the relationship between level of thyroid hormone and human chorionic gonadotropin in patients with trophoblastic disease, level of thyroid hormone, serum -HCG and amount of urinary excretion of HCG were measured and analyzed in 24 patients with hydatidiform mole and 11 patients with choriocarcinoma who were admitted to Yonsei University Severance Hospital during the period from January 1981 to December 1986. The results are summarzed as follows: 1) Hyperthyroidism was observed in 33.3% of the patients with hydatidiform mole and in 18.2% of the patients with choriocarcinoma. 2) The amount of 24 hour urinary excretion of HCG in patients with hyperthyroidism was more than that in euthroid patients. 3) There was no difference in the level of thyroid hormone, serum -HCG, and amount of 24 hour urinary excretion of HCG in patients with hydatidifrom mole and choriocarcinoma. 4) The amount of 24 hour urinary excretion of HCG had significant correlation with FT4, whereas no similar correlation was observed between the levels of thyroid hormone and serum -HCG. In conclusion, the occurrence of hyperthyroidism is closely related with the amount of urinary excretion of HCG in patients with trophblastic disease.

        Bimodal SiC 입자강화 AZ91D 마그네슘 복합재료의 유동성 및 기계적 특성 평가

        The objective of this study was to investigate the influence of bimodal reinforcement on the fluidity and mechanical properties of SiC particle reinforced AZ91D Mg composites. The fraction of bimodal SiC particles was fixed at 20 wt% and the mixing ratios of 5 ㎛ to 50 ㎛ SiC particles were 0:1, 1:2, 1:1, 2:1 and 1:0. The results clearly showed that the fluidity and mechanical properties of the composites with bimodal reinforcement were dependent on mixing ratio. In terms of molten state fabrication and tailored mechanical properties, it was obtained that the proper mixing ratio seems to be 1:2.

        안면골 골절에 대한 임상적 고찰

        The facial bones are composed of several protuberance and arches(orbital rim, nasal pyramid, zygomatic arch and maxilla) and are apt to be damaged by the accidents. According to the intellectual development of the human, the social life is constantly complicated more than ever. The means of transportation has been increasing in scale and size, consequently the automobile accidents has also increasing the same as the transportation has been. Moreover the means of transportation has been expressed as the moving weapon which became very serious problem. Braunstein(1957), in a study of 1000 automobile accidents, reported that the head and face area were involved in 72.3% and that 7.2% of these also sustained fracture of one or more facial bones. The accurate diagnosis and treatment of the facial injurie swere always very important, but the interest of this field was very weakened. During recent years, numerous articles have documented the steady increase in the incidence of facial bone fracture which have been observed within the framework of our mobile and violent society. Authors reviewed and evaluated clinical patterns and proper management of facial bone fracture patients who were admitted and treated in Yonsei Medical Center from March, 1, 1973 to Setember 30, 1977. There were 245 cases of facial bone fractures in that period. The following results were obtained. 1. Monthly incidence of facial bone fracture showed 32 cases in May at the most and at the least in January 15 cases. 2. During this period, a total of 245 patients with fractures of the facial bones were admitted to the hospital. The sex incidence revealed 206 patients(84.1%) in male and 39 patients (15.8%) in female. The great incidence of the fracture occurred in the age of between 10 to 40 years. 3. Traffic accidents ranked the top as the cause of facial bone fracture. 4. Mandibular fractures were most common facial bone fracture(33.5%) during that period. 37 patients had multiple facial bone fractures. 5. Associated bracture is as follow in order of frequency. 1) Skull; 22 cases. 2) Upper extremities; 21 cases. 3) Lower extremities; 19 cases. 6. Soft tissue injury is as follow order of frequency. 1) Facial and peripheral laceration; 74 cases. 2) Severe skin abrasion and contusion, central; 5 cases. 3) Eye injury; 5 cases. 7. Life threatening associated injury as follow in order of frequency. 1) Cerebral contusion; 33 cases. 2) Airway obstruction; 8 cases. 3) Hemo-pneumothorax; 6 cases. 8. Almost 50% of the cases were treated within 48 hours after injury. 18(7.3%) patients were treated conservatively. 9. Closed reduction alone was done for 81(33.1%) cases of our patients, while 37(15.1%) done open reduction. 10. Complications are as follows. 1) Fever; 7 cases. 2) Local infection; 6 cases. 3) Pulmonary; 4 cases. 4) Death; 4 cases. Three out of four cases died from severe head injury and one case secondary to in traabdominal viscus rupture.

      • Influence of continental outflow events on the aerosol composition at Cheju Island,South Korea

        The chemical composition of aerosols measured at Cheju Island, Korea, over the 3-year period March 1992 to February 1995 are presented and discussed, with a particular emphasis on the Pacific Exploratory Mission in the Western Pacific (PEM-West B) time period. Cheju Island is under the influence of continental outflow conditions nearly 70% of the year, and as a result the aerosol loading of sea salt as well as continental aerosol components is high. The 3-year mean values derived from the daily tape filters are non-sea-salt (nss) SO₄²?=6.8㎍/㎥, NO₃?=1.2㎍/㎥, Cl??=1.9㎍/㎥, Na+=1.7㎍/㎥, Ca²?=0.5㎍/㎥, NH₄?=1.3㎍/㎥, Mg²?=0.3㎍/㎥, and K+=0.4㎍/㎥. Sea-salt components show peak values in winter, while calcium, nitrate, potassium, and to a lesser extent, sulfate and ammonium, show higher values in the spring, and all species exhibit a pronounced minimum in summer. Trajectory and principal component analysis show that elevated levels of primary aerosols (both sea salt and soil-derived) occur with strong wind conditions associated with winter and spring, and high concentrations of non-sea-salt components are most strongly associated with springtime continental outflow events. During the PEM-West B period, nitrate and calcium are found to be ∼60% higher than the annual mean, sea-salt components ∼30% higher, and sulfate ∼10% higher. The aerosol data are combined with gaseous SO₂ concentrations, precipitation chemistry data and companion aerosol measurements taken at the same site, to provide further insights into the aerosol composition at Cheju. At Cheju, sulfate and ammonium are found mostly in the fine fraction (∼80 to 90%), while calcium and nitrate reside in the coarse size fraction. sulfate and nitrate are not associated with the primary aerosols, but rather become associated with the aerosol during the long-range transport process.

        Pedicle Screw Placement in the Thoracolumbar Spine Using a Novel, Simple, Safe, and Effective Guide-Pin : A Computerized Tomography Analysis

        Objective : To improve pedicle screw placement accuracy with minimal radiation and low cost, we developed specially designed K-wire with a marker. To evaluate the accuracy of thoracolumbar pedicle screws placed using the novel guide-pin and portable X-rays. Methods : Observational cohort study with computerized tomography (CT) analysis of in vivo and in vitro pedicle screw placement. Postoperative CT scans of 183 titanium pedicle screws (85 lumbar and 98 thoracic from T1 to L5) placed into 2 cadavers and 18 patients were assessed. A specially designed guide-pin with a marker was inserted into the pedicle to identify the correct starting point (2 mm lateral to the center of the pedicle) and aiming point (center of the pedicle isthmus) in posteroanterior and lateral X-rays. After radiographically confirming the exact starting and aiming points desired, a gearshift was inserted into the pedicle from the starting point into the vertebral body through the center of pedicle isthmus. Results : Ninety-nine percent (181/183) of screws were contained within the pedicle (total 183 pedicle screws : 98 thoracic pedicle screws and 85 lumbar screws). Only two of 183 (1.0%) thoracic pedicle screws demonstrated breach (1 lateral in a patient and 1 medial in a cadaver specimen). None of the pedicle breaches were associated with neurologic or other clinical sequelae. Conclusion : A simple, specially designed guide-pin with portable X-rays can provide correct starting and aiming points and allows for accurate pedicle screw placement without preoperative CT scan and intraoperative fluoroscopic assistance.

        황산용액 중에서 Na2S2O8에 의한 코발트의 산화 침전

        Oxidation and precipitation of Co(II) in sulphate solution using Na2S2O8 as an oxidant was studied. The main factors affecting the oxidation of Co(II), the reaction time, temperature, pH of solution, agitation speed and amount of added Na2S2O8, were investigated. The results indicated that the oxidation rate of Co(II) was not affected by the agitation speed and the amount of the oxidant, but it increased obviously with an increasing pH of the solution and reaction temperature. The kinetics equation was established as .ln(1.X ) = k1.[H+].0.4755.exp ( 3800) .t with a first order reaction. The product precipitated by the oxidation T of Co(II) was CoOOH, and Co2O3 was obtained by calcination of CoOOH at 300 ℃ for 2 hours. The particle size of the precipitated product was influenced by the pH of the solution.

      • 한국 최초의 경주시 황남동 출토 유리 용융 도가니 및 유리 구슬에 대한 연구

        A piece of glass melting crucible whose inner face was lined with yellowish brown glass, a yellowish brown ellipsoidal bead and a greenish scrcew-shaped wound bead were unearthed in Kyongju city. These finds were dated to around the middle of the 6th century by pattern impressed earthenwares excavated in the same stratum. Chcmica analyses were carried out by means of energy diapersive spectrometry(EDS), atomic absorption analysis(AA), induced coupling plasma analysis(ICP) along with conventional wet analvsis. Also X-ray and elecrtron diffraction analvses were periomd in an attempt to identify any crystalline precipitates. Three glass items are classified iota lead-silica group showing the same compositions. This indicates that the two beads are from the same crucible. Ywllowish brown glass film contains 72.5% PbO and 25.2% SiO₂ and its softening point is 442℃ which is determined by differential scanning calorimeter(DSC). Melting point is found to be around 650℃ by practical measurement. Color differences in two beads are proved to be mainly due to copper contained in the glass, i.e., yellowish brown and greenish colors depend on reduction and oxidation atmosphere during melting, respectively. Chemical analysis shows that the glass melting crucible is a common earthenware.
