Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

선리(船里) 명문와 고찰

The National Museum of Korea keeps artefacts (many of them are merely fragments, although they are valuable as historical materials) collected during the colonial period, including 389 roof tiles that were collected by the National Museum of the Government General of Joseon in Seon-ri, Gwangju after the great flood of 1925, 337 of which contain inscriptions. However, their archaeological context has long been unclear due to a lack of detailed investigations or excavations. Some roof tiles are inscribed with the Chinese characters "(Place name)受國蟹口船家草 (Sugukhaeguseongacho)," which could be interpreted to mean the following: "(Place name) is requested to receive the roof tiles that we are dispatching. They were produced at a (Government-run) kiln near the ferry at the estuary of the river (whose name is associated with crabs)." Thus, it is thought that the place where the roof tiles were produced was Seonri, Gwangju and that they were supplied to the places whose names were stated. A total of twenty place names (for which the roof tiles were produced) inscribed on the roof tiles have been confirmed by the record in the section titled Jiriji (Geographical Book) of Samguksagi (History of the Three Kingdoms). They include areas around Bukhansan Mountain in Seoul, as well as in Yangpyeong, Anseong, Suwon, Gwacheon, Gimpo, Yangju, Goyang, Paju, Pocheon and Hanam in Gyeonggi-do and Incheon; Cheolwon; Gaeseong, Jincheon. Seon-ri roof tiles are thought to have been produced between the mid-8th century (place names were changed following the reorganization of administrative zones in 757) and the early Goryeo Period based on their patterns, changes in the inscriptions, and the place names inscribed upon them. The places to which the roof tiles were supplied generally correspond to the scope of Hanju, one of the nine provinces of Unified Silla. Thus, the roof-tile kiln in Seon-ri, Gwangju is thought to have been a state-run production center that supplied roof tiles from Hanju. The Chinese character 國('Guk') inscribed on the roof tiles corroborates this fact. This shows the characteristics of production and the distribution pattern of roof tiles during the Unified Silla Period. 국립중앙박물관은 일제강점기 당시 조선총독부박물관이 발굴하거나 지표 채집한 유물 중 대부분 편으로 잔존하고 있는 자료적 성격의 유물을 보관·관리해 왔다. 선리 기와 또한 일제강점기 당시 조선총독부박물관이 1925년 을축년대홍수 이후 발견·수습한 것으로 모두 389점에 이르며, 이 중 337점에서 명문이 확인된다. 하지만 수습에만 거치고 주변에 대한 정밀조사나 발굴조사 등을 실시하지 않아 유적의 성격이 밝혀지지 않았다. 기와에는 ‘(지명)受(國)蟹口船家草’라는 명문이 새겨져 있으며, 이는 ‘게내천 또는 게와 관련된 명칭의 하천 입구에 있는 나루터 부근 국영(가마)에서 제작한 기와를 보내니 (지명)에서 받아라.’는 의미로 해석된다. 이 중 ‘해구(蟹口)’는 게와 관련된 명칭의 하천 입구라는 의미로 기와 발견지 일대가 현재도 게가 살고 있는 지역이라는 점에서 기와 가마가 위치한 곳을 당시에는 ‘해구(蟹口)’라 불렀을 것으로 추정하였다. 따라서 생산지는 ‘해구(蟹口)’, 즉 선리가 되고 공급지는 명문 앞에 있는 각 지명들이 됨을 알 수 있다. 기와에 남아 있는 지명 즉 기와의 공급지는 『삼국사기』 지리지 비정을 통해 모두 19곳이 확인되는데, 이들의 현재지명을 비정한 결과 서울의 북한산 지역과 경기도의 양평·안성·수원·과천·김포·양주·고양·파주·포천·하남과 인천, 강원도 철원, 황해북도 개성지역, 충청북도 진천지역 등으로 확인되었다. 선리 기와는 문양과, 명문의 변화, 기와에 새겨진 지명이 통일신라지명과 고구려지명이 혼재되어 있는 점 등으로 보아 757년 행정구역 개편으로 지명 개명이 완료된 시점인 8세기 중후반부터 통일신라 말 또는 고려 초까지 제작된 것으로 파악된다. 또한, 서울·경기·인천·강원도 철원·황해도 개성 등 공급지역의 범위가 통일신라 9주 중 하나인 한주(漢州)의 범위와 대체로 일치하고 있어 선리의 기와가마는 한주를 공급지역으로 하는 관영공방으로 생각된다. 이는 기와의 명문에 새겨져 있는 ‘국(國)’을 통해서도 충분히 짐작할 수 있는 사실이다. 선리 기와가마가 한주의 관영공방이었을 가능성은 통일신라 당시의 기와 생산과 유통의 일면을 보여주는 것이라 할 수 있다.

모델 추종형 적응 퍼지 제어기의 설계

In this paper, reference-model following fuzzy controller (RMFFC) was proposed in order to overcome the difficulty of extracting rules and defects of the adaptation performance in the FLC. RMFFC comprised inner feedback loop consisting of the FLC and plant, and outer loop consisting of adaptation mechanism which of the FLC. A Referenced-model was used for design criteria of a fuzzy controller which characterizes and quantizes the control performance required in the overall control system. Tuning control rules of FLC is performed by the adaptation mechanism. For this, the fuzzy model for tuning the control rules is designed in accordance with the feature of error information. Proposed algorithm was verified through simulation for various plant models which have different characteristics. And the results are obtained as follows. 1) Control performance required by a skilled operator was reflected to the control. 2) Unnecessary rule modification did not occur in steady-state, after completion of learning, by using the following error information 3) Performance of RMFFC was improved in various disturbances, comparing with conventional FLC and MRAC.

렙토스피라증으로 오인된 전신홍반루푸스

Systemic Lupus Erythematosus (SLE) is autoimmune disorder involving systemic organs and charactehzed commonly by fatigue, fever, myalgia, and arthralgia. Leptospirosis is a kind of K-orean hemorrhagic fever disease and characterized commonly from fever, headache, myalgia severly to jaundice, renal failure, and hemorrhage. A 60-year-o1d woman was presented with febrile sense, myalgia, headache and general weakness, Initially, she was diagnosed as leptospirosis by serological test. Duhng treatment her clinical manifestation was not improved. On followed laboratory data, FANA positive with leukocytopenia and hemolytic anemia. Also anti-cardiolipin antibody, anti-dsDNA antibody and proteinuna was found. The serological titer of leptospirosis was not increased by four times. So, we coneluded that she had SLE and prescribed steroids. After that, her clinical manifestations were improved. Therefore, we report here on a case of SLE misdiagnosed as leptospirosis. 루푸스는 전신을 침범하는 질환으로 특히 임상 증상이 비특이적일 경우에 여러 다른 감염성 질환들로 오인될 수 있으며, 그에 따라 진다닝 늦어질수록 치료시작 시점도 늦어지게 되며, 치료 시기를 놓치게 되면 환자의 예후는 불량해진다. 본 증례는 렙토스피라증으로 입원한 환자에서 고식적인 항생제 치료를 하였으나 호전을 보이지 않았으며 임상 경과 과정중 시행한 자가면역 혈청 검사상 루푸스가 진단된 경우였다. 이에 렙토스피라증으로 오인된 루푸스 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

Fentanyl 마취하 개심술시 혈중 Catecholamine 농도의 변화

Serum epinephrine and norepinephrine concentrations were measured in 7 patients during fentanyl-oxygen anesthesia, who were undergoing elective open heart surgery(single valvular replacement operations) in Seoul National University Hospital. Sampling times and corresponding total fentanyl doses were as follows; 1) after arterial cannulation 2) 5 minutes after intubation(50 ㎍/kg of B. Wt) 3) 5 minutes after sternotomy(65 ㎍/ka) 4) after aortic dissection(75 ㎍/kg) 5) 15 minutes after initiating bypass 6) 60 minutes after initiating bypass [20 ㎍/kg of fentanyl was added to the priming solution before initiation of Cardio-pulmonary bypass(CP bypass)]. High Performance Liquid Chromatography(HPLC) was used to analyze hormones. Epinephrine concentrations(pg/ml) corresponding to the sampling times were as follows (mean±SE); 1) 213±59 2) 199±62 3) 246±83 4) 173±35 5) 270±70 6) 269±70 Norepinephrine concentrations(pg/ml) were as follows(mean±SE); 1) 609±107 2) 500±73 3) 645±152 4) 470±82 5) 494±65 6) 955±311 There no significant hormonal changes except the 6th norepinephrine value. There were no significant hemodynamic changes except blood pressure decrease at the beginning of CP bypass. It is concluded that high dose fentanyl-oxygen anesthesia was limitedly effective to block the stress of open heart surgery, general anesthesia and CP bypass.

Osteogenesis imperfecta is one of the groups of hereditary disorders of connective tissue which includes the Ehlers-Danlos syndrome, the Marfan syndrome, pseudoxanthoma elasticum, and Hurler syndrome. While cardiovascular involvement is associated with each of these disorders, it is least common in osteogenesis imperfecta and is overshadowed by the bony, ocular, otologic, cutaneous, and dental manifestations that are characteristic of the disorder. In evaluating patients with osteogenesis imperfecta, careful attention should be paid to cardiovascular findings and if valvular lesions are noted, patients should be instructed regarding the need for antibiotic prophylaxis for dental and surgical procedures. We report a case of osteogenesis imperfecta associated with aortic regurgitation.

Miniscalpel Needle Therapy with Integrative Korean Medical Treatment for Carpal Tunnel or Tarsal Tunnel Syndrome: Case Series of Three Patients

Objectives : This study reports the clinical effects of miniscalpel needle therapy in patients with carpal tunnel or tarsal tunnel syndrome. Methods : Three patients with carpal tunnel syndrome (CTS) or tarsal tunnel syndrome (TTS) (first case, patient with CTS and TTS; second case, patient with CTS; and third case, patient with TTS) were treated with miniscalpel needle (MSN) therapy and integrative Korean medical treatment. The Numeric Rating Scale (NRS), Neuropathic Pain Scale (NPS), Boston scale score, and AOFAS (American Orthopaedic Foot and Ankle Society) ankle-hindfoot score were measured. Results : In general, outcome measures after treatment showed improvement in all cases. In the first case (CTS and TTS), scores on the NRS, NPS, and Boston scale decreased, and AOFAS ankle-hind foot scores increased. In addition, Tinel's sign showed improvement. In the second case (CTS), scores on the NRS, NPS, and Boston scale, and Tinel's sign, were decreased. In the third case (TTS), scores on the NRS and NPS, and Tinel's sign, showed improvement, and AOFAS ankle-hind foot scores were increased. Conclusion : These results suggest that MSN therapy has a meaningful clinical effect in CTS and TTS.
