Background and Objectives : Cigarette smoking is a major factor and the most modifiable risk factor for atherosclerosis. Endothelial dysfunction is an early event in atherosclerosis, before the structural change of the systemic arteries. We hypothesized that endothelial dysfunction might be present in the systemic arteries of young adult smokers and that this might be dose-dependent phenomenon. Therefore, we try to certify that smoking is associated with endothelial dysfunction in healthy young adults without other risk factor of atherosclerosis. Subjects and Methods : we studied noninvasively 29 subjects aged 3 to 4 decades without other risk factor of atherosclerosis, they were composed of the two groups, current smokers(n=15) and nonsmokers(n=14). Smoking history varied from 3 to 20 pack years. Using high-resolution ultrasound, vessel diameter and mean flow rate were measured at rest, during reactive hyperemia(with flow-mediated endothelium-dependent dilation), and after sublingual nitroglycerin(NG-induced endothelium-independent vasodilation). Result : Flow mediated endothelium dependent dilation was observed in the control subjects (9.4±3.44%) but was impaired in the smokers(5.8±2.51%)(p=0.004). NG-induced endothelium independent dilation is no difference in all subjects. Conclusion : Cigarette smoking is associated with endothelial dysfunction in healthy young adults.

본 연구는 만 0-5세 영유아를 양육하고 있는 조부모의 양육실태와 양육의 어려움, 사회적 지원에 대한 요구를 조사함으로써 조부모 양육참여를 도모할 수 있는 접근방법 및 조부모양육 지원의 방향을 모색하는 것을 목적으로 한다. 연구대상은 서울·경기 지역에 거주하고 있는 영유아기 손자녀를 양육하는 조부모 181명이다. 수집된 자료는 통계 프로그램(SPSS 23.0)을 사용하여 기술통계, t-test, ANOVA, Scheffe 검증을 실시하였다. 본 연구의 결과 첫째, 60대의 고졸 이상 조부모의 양육 참여비중이 높았으며, 성인자녀와 경제적으로 서로 독립되고, 손자녀와 따로 살면서 양육 돌봄을 하며, 성인자녀로부터 양육에 대한 보상을 받는 조부모의 형태가 많았으며, 대부분의 조부모가 양육만족감을 느끼고 있었다. 둘째, 조부모 양육참여에 있어서 생활지도참여가 높았고, 학습지도참여가 가장 낮게 나타났다. 셋째, 조부모의 양육 어려움 중 두드러진 요인은 체력적인 어려움이었으며 환경적, 교육적, 경제적 어려움 순으로 나타났다. 넷째, 조부모 양육지원요구 중 교육지원요구가 가장 높았으며, 환경지원요구, 경제지원요구, 체력지원요구 순으로 나타났다. 본 연구결과 조부모 양육참여의 제고를 위해서는 보다 체계적이며 정책적인 조부모 지원이 필요함을 시사한다. This study examined the current situation of the grandparents who are providing childcare to 0~5 year-old children, their childcare difficulties, and demand for social support. A survey was conducted on 200 grandparents who were providing childcare for young children of 0 to 5 years of age who were attending 12 kindergartens and day care centers located in Seoul and Gyeonggi Province area. As for survey method, visits to educational institutions were made to explain the purpose of research; then questionnaires were sent to the institutions and collected. 181 questionnaires were finally used and the data was analyzed by SPSS 23.0 program. The results are the followings. First, as for the current status of grandparents’ childcare, many grandparents in their 60s with high school or higher education were involved in grandparental childcare. Second, among the items of grandparents’ involvement in childcare, involvement in life guidance marked the highest score, followed by household guidance, leisure guidance, involvement in learning guidance. Third, the most outstanding difficulty in grandparental childcare was physical difficulty, followed in the order by environmental, educational, and economic difficulties. Fourth, among the items of demand for grandparental childcare support, demand for education support was the highest, followed by demands for environment support, economic support, and physical support. In order to boost grandparents’ involvement, more systematic and diverse forms of supports of grandparents are needed.

Purpose: The purpose of this study was to examine the effects of simulation-based learning on the knowledge about and performance confidence in the nursing care of children with asthma. Methods: A one-group pre- and post- study design was used. A total of 70 nursing students participated in the study. In groups of five, the participants experienced simulation-based learning during their child health nursing practicum. The given scenario was about nursing care for children with asthma. The simulation learning was provided for 3 hours, and included a group discussion before the simulation and a debriefing. Results: After the simulation, knowledge about and performance confidence in the nursing care of children with asthma significantly increased. The nursing students reported a high level of satisfaction with the simulation-based learning. There was a significant positive correlation between knowledge and performance confidence. Conclusion: Simulation-based learning was effective for nursing students in the nursing care of children with asthma. Our results suggest utilizing this new way of learning to strengthen the clinical experience of child health nursing in nursing students.

Kimchi is a widely consumed food in Korea, and the quality of the final fermented product is affected by properties of the cabbage used to produce it. The objective of this study was to determine optimal means of storing spring kimchi cabbage by analyzing kimchi produced from cabbage subjected to one of six combinations of pre-treatment and storage conditions. Kimchi quality evaluation was through sensory tests, measurement of antioxidant effects, and analysis of associated microorganisms. The kimchi produced from cabbage stored under T6 conditions (upside down orientation, pre-cooling, high density polyethylene (HDPE) film wrap, and photocatalytic storage) possessed a static pH, which implied a slow progression of ripening that was also verified in sensory tests by a low sour taste score of 3.60 and a high green taste score of 3.00. The kimchi produced from T3 cabbage (upright orientation and low-temperature storage) showed the highest consumer preference according to the sensory test results, with scores of 6.40 and 5.40 from the time of making kimchi through to the fourth week of ripening, respectively. Analysis of antioxidant effects showed that the highest phenol content of 243.75 GAE mg·g<SUP>-1</SUP> was in kimchi produced from T2 cabbage (upside down orientation, pre-cooling, HDPE film wrap, and plasma storage). Furthermore, both the highest flavonoid content of 70.54 QE mg·g<SUP>-1</SUP> and the highest DPPH radical-scavenging activity level of 70.54% was in kimchi produced from T4 cabbage (upside down orientation, pre-cooling, HDPE film wrap, and low-temperature storage). Over eight weeks of maturation, the volume of lactic acid bacteria constantly rose by 7.98 log CFU·mL<SUP>-1</SUP> in kimchi produced from T3 cabbage and by 7.81 log CFU·mL<SUP>-1</SUP> in kimchi produced from T1 cabbage (upright orientation and plasma storage). The results of this study indicate that the T3 cabbage storage conditions are optimal for producing high-quality kimchi.

<P><B>Introduction</B></P><P>Although it has been suggested that rare coding variants could explain the substantial missing heritability, very few sequencing studies have been performed in rheumatoid arthritis (RA). We aimed to identify novel functional variants with rare to low frequency using targeted exon sequencing of RA in Korea.</P><P><B>Methods</B></P><P>We analyzed targeted exon sequencing data of 398 genes selected from a multifaceted approach in Korean RA patients (<I>n</I> = 1,217) and controls (<I>n</I> = 717). We conducted a single-marker association test and a gene-based analysis of rare variants. For meta-analysis or enrichment tests, we also used ethnically matched independent samples of Korean genome-wide association studies (GWAS) (<I>n</I> = 4,799) or immunochip data (<I>n</I> = 4,722).</P><P><B>Results</B></P><P>After stringent quality control, we analyzed 10,588 variants of 398 genes from 1,934 Korean RA case controls. We identified 13 nonsynonymous variants with nominal association in single-variant association tests. In a meta-analysis, we did not find any novel variant with genome-wide significance for RA risk. Using a gene-based approach, we identified 17 genes with nominal burden signals. Among them, <I>VSTM1</I> showed the greatest association with RA (<I>P</I> = 7.80 × 10<SUP>−4</SUP>). In the enrichment test using Korean GWAS, although the significant signal appeared to be driven by total genic variants, we found no evidence for enriched association of coding variants only with RA.</P><P><B>Conclusions</B></P><P>We were unable to identify rare coding variants with large effect to explain the missing heritability for RA in the current targeted resequencing study. Backgroud: The stem bark of Kalopanax pictus (KP) has been used in traditional medicine to treat rheumatoidal arthritis, neurotic pain and diabetes mellitus in China and Korea. In this study, the mechanism responsible for anti-inflammatory effects of KP was investigated. Methods: We examined the effects of KP on NO production, nitric oxide synthase (iNOS) and HO-1 expression, NF-κB , Nrf2 and MAPK activation in mouse peritoneal macrophages. Results: The aqueous extract of KP inhibited LPS-induced NO secretion as well as inducible iNOS expression, without affecting cell viability. KP suppressed LPS-induced NF-κB activation, phosphorylation and degradation of IκB-α, phosphorylation of extracellular signal-regulated kinase 1/2 (ERK1/2) and c-Jun N-terminal kinase (JNK). Furthermore, KP induced HO-1 expression and Nrf2 nuclear translocation. Conclusion: These results suggest that KP has the inhibitory effects on LPS-induced NO production in macrophages through NF-κB suppression and HO-1 induction.
