        Background: For effective education of social medicine, new and more efficient means of education are needed because social medicine consist of diverse background academics . Metohods: Workshop based lecture was carried out twice in social medicine class for medical students and was assessed with questionnaires survey. Results: Half of the students were satisfied with workshop-based learning and required more discussion time and pre-information. Conclusions: With some improvements, workshop-based adult learning method is expected to bring effectiveness in social medicine education.

        Background: Healthy and sustainable community is essential component of development. But there has been no systematic reporting about healthy municipality in specific city, although economic development results in considerable progress. Method: According to the Ontario progress monitoring tool partial monitoring has been done. Survey to the stakeholder including public officers were done and analyzed. Result: Economic area were most lowly reported area. By specific item, direction of improvement were induced in each area including social environment and human health area. Green space, waste treatment, traffic problem, small health service benefit were major agenda among the stockholders. Conclusion: According to the Ontario progress reporting, we got good picture about healthiness of one municipality and concrete direction of improvement.

        Many of the growth-promoting properties of growth hormone(GH) are mediated by insulin-like growth factor-I(IGF-I), a highly conserved circulating 70-amino acid peptide. Recent studies have shown that multiple mechanisms influence IGF-I gene expression, including transcription from two promoters, alternative RNA splicing, and variable polyadenylation. In thyroid tissue, thyroid stimulating hormone(TSH) and IGF-I are the most possible candidates for follicular cell proliferation and hypertrophy. Actually IGF-I had autocrine and paracrine effect for tissue growing. We prepared thyroid tumor tissue mRNAs using single step method for detecting IGF-I levels according to different tissues, i.e., thyroid adenoma or papillary thyroid carcinoma. We used Northern blot analysis for IGF-I mRNA and RNase protection assay (RPA) for IGF-I transcription start sites. For Northern blot, we used whole human IGF-I cDNA as a DNA probe and for RPA, we used IGF-I exon 1 containing noncoding promoter 1 as a riboprobe. We got good RNA bands from Northern blot analysis around 1 kb (IGF-IA) and 7.5 kb (IGF-IB) region. To clarify the amount of both IGF-IA and IB mRNAs, we measured autoradiographied signal of IGF-I mRNAs bands using densitometer. In IGF-IA signals, there's no change among liver and thyroid tissues, but in case of IGF-IB mRNA bands, the signal was markedly increased in thyroid carcinoma tissues than that of normal thyroid tissue (85% vs 14%). In the study of RPA, all thyroid tissues used the same transcription start sites as those of liver's. We concluded that that this different regulation of IGF-I mRNA was originated from tissue specificity. That meant some tissue specific transcription factor/s were related to tissue IGF-I expression.

      • Effects of Porous Carbon Cathode Additives on the Electrochemical Performance of Li—S Cells

        <P>In this paper, the effects of porous carbon additives, which included microporous, mesoporous and macroporous carbon, on the electrochemical properties of a sulfur cathode were examined. The type of carbon additive significantly affected the initial capacity reduction, though it did not affect the cycling stability of the sulfur cathodes. The cathodes that incorporated mesoporous and macroporous carbon additives delivered much higher initial capacities of similar to 970 and similar to 900 mAh/g, respectively, compared with those (similar to 715 mAh/g) with graphite and microporous carbon additives. The EDS results showed that these features are strongly related to the absorption characteristics of the carbon additives. Importantly, it was found that the macroporous carbons with a pore size of similar to 65 nm effectively confined dissolved polysulfides. Interestingly, the sulfur cathodes with microporous carbon additives exhibited extremely similar electrochemical behavior to those with nonporous graphite, which suggests that activated carbon particles do not serve as polysulfide reservoirs during discharge.</P>

        The Role of the Carbon Framework in Sulfur-Carbon Composite Cathodes in Li-S Batteries

        <P><B>Abstract</B></P> <P>Lithium-sulfur (Li-S) batteries are considered a promising energy storage solution for today’s society, which faces significant energy and environmental issues. Successful implementation of Li-S batteries requires the development of a robust and high-performance sulfur cathode. Unfortunately, the current approaches focus on the development of sulfur-carbon composites without a fundamental understanding of the reduction mechanism of sulfur and the role of carbon. To develop a clear understanding of the efficacy of carbon/sulfur composites, we investigated the electrochemical behavior of Li-S batteries employing different cathode structures. Our comparative study suggests that nanoscale electrical contact between sulfur and carbon is not essential for improving the electrochemical performance of Li-S batteries or for initiating electrochemical redox reactions of the sulfur cathode.</P>

      • Adding Radiotherapy to Immunotherapy Improves Survivals in Patients with Non-small Cell Lung Cancer

        Background Radiation therapy for lung cancer can be very effective for destroying cancerous cells locally. And systemic immune response from radiation therapy is emerging as an interesting issue in immune checkpoint inhibitors (ICIs) era. Here, we present the findings of a retrospective study investigating the impact of radiation therapy on outcomes with ICI treatment in patients with advanced NSCLC. Method Patients with NSCLC treated with radiotherapy for any reason and ICI between May 2016 and May 2020 were retrospectively analyzed. The optimal timing and site of radiotherapy in ICI treatment were analyzed via the difference in overall survival (OS) by sequence of radiation therapy. Results A total of 180 patients were diagnosed with NSCLC and received pembrolizumab, nivolumab and atezolizumab. The mean age was 67.0±9.4 years, and most patients were male. The major histological types were adenocarcinoma (47.2%) and squamous cell carcinoma (42.8%). Most patients had stage IV NSCLC (77.8%). The number of patients receiving radiation therapy was 41.7%, of which 52% received radiotherapy to the primary lesion, and 48% received radiotherapy to the distant metastatic lesion. 42 patients received radiation therapy before immunotherapy, 26 patients added concurrent radiation therapy to immunotherapy, and 16 patients received radiation therapy after immunotherapy. Whether radiotherapy was performed was not a statistically significant factor for OS (p=0.0831). when radiotherapy was introduced during ICI treatment, OS (p=0.016) and time to progression (TTP) of ICI treatment (p<0.001) were prolonged significantly. The median OS of patients who added radiotherapy during immunotherapy was significantly longer than receiving immunotherapy alone. (993 days vs 233 days, p=0.015) Conclusion Concurrently added radiotherapy augmented OS and TTP of ICI monotherapy in patients with advanced NSCLC. Radiation therapy Before and after ICI is not related with better survival outcome. We should consider radiation therapy adding on immunotherapy to gain more favorable patient outcome.

      • Acute Tubular Necrosis Associated with Pembrolizumab Treatment: A Case Report

        Introduction Immune checkpoint inhibitors have recently led to significant improvement of clinical outcomes in patients with metastatic non-small cell lung cancer (NSCLC). Renal toxicity associated with immune checkpoint blockade was uncommonly reported. We report the case of a 72-year-old male who had acute tubular necrosis after pembrolizumab treatment. Case A 72-year-old male was diagnosed with primary lung squamous cell carcinoma with multiple metastatic lymph nodes in July 2018. The patient has received the chemotherapy of gemcitabine/cisplatin for 4 cycles, and chest computed tomography (CT) followed up periodically, showed the progressive disease. Next, the patient treated with pembrolizumab, PD-1 inhibitor, for 4 cycles, the patient’s serum creatinine elevated from 0.91 to 2.0mg/dL. One week after receiving 5 cycle treatment, he was hospitalized with dyspnea and generalized edema. Emergency dialysis was performed under the diagnosis of acute kidney injury because renal function was worse and urine output was not maintained. A renal ultrasound showed normal kidney size and no hydronephrosis or other abnormalities. He was performed a renal biopsy to evaluate the cause of renal dysfunction. His renal biopsy findings showed acute tubular necrosis, thickening of capillary walls with mesangial expansion, with global sclerosis (20%). With suspicion for pembrolizumab as a potential cause for acute tubular necrosis (ATN), future infusions were held. Considering the possibility that ATN may have occurred as an immune-related adverse event, the patient was given a dose of IV methylprednisolone 500mg for 3 days. However, his renal function did not recover and he continued to require dialysis treatment. Unfortunately, he passed away by sepsis despite active treatment. Conclusion We report an interesting case of adverse events associated with pembrolizumab in lung cancer. This case suggests the importance of closely monitoring renal function in lung cancer patients treated with PD-1/PD-L1 inhibitors.

        Usefulness of presepsin in predicting the prognosis of patients with sepsis or septic shock: a retrospective cohort study

        Background: The diagnosis and prediction of prognosis are important in patients with sepsis, and presepsin is helpful. In this study, we aimed to examine the usefulness of presepsin in predicting the prognosis of sepsis in Korea. Methods: Patients diagnosed with sepsis according to the sepsis-3 criteria were recruited into the study and classified into surviving and non-surviving groups based on in-hospital mortality. A total of 153 patients (32 and 121 patients with sepsis and septic shock, respectively) were included from July 2019 to August 2020. Results: Among the 153 patients with sepsis, 91 and 62 were in the survivor and non-survivor groups, respectively. Presepsin (p=0.004) and lactate (p=0.003) levels and the sequential organ failure assessment (SOFA) score (p<0.001) were higher in the non-survivor group. Receiver operating characteristic curve analysis revealed poor performances of presepsin and lactate in predicting the prognosis of sepsis (presepsin: area under the curve [AUC] =0.656, p=0.001; lactate: AUC=0.646, p=0.003). The SOFA score showed the best performance, with the highest AUC value (AUC=0.751, p<0.001). The prognostic cutoff point for presepsin was 1,176 pg/mL. Presepsin levels higher than 1,176 pg/mL (odds ratio [OR], 3.352; p<0.001), higher lactate levels (OR, 1.203; p=0.003), and higher SOFA score (OR, 1.249; p<0.001) were risk factors for in-hospital mortality. Conclusion: Presepsin levels were higher in non-survivors than in survivors. Thus, presepsin may be a valuable biomarker in predicting the prognosis of sepsis.

        Cancer Selective Turn-On Fluorescence Imaging Using a Biopolymeric Nanocarrier

        <P>Most nanoparticle-based bioresearch for clinical applications is unable to overcome the clinical barriers of efficacy (e.g., sensitivity and selectivity), safety for human use, and scalability for mass-production processes. Here, we proposed a promising concept of using a biocompatible nanocarrier that delivers natural fluorescent precursors into cancerous cells. The nanocarrier is a biopolymeric nanoparticle that can be easily loaded with fluorescent precursors to form a fluorescent moiety via a biosynthesis pathway. Once delivered into cancerous cells, the nanocarriers are selectively turned on and distinctively fluoresce upon excitation. We, therefore, demonstrated the efficacy of the selective turn-on fluorescence of the nanocarriers in in vitro coculture models and in vivo tumor-bearing models.</P> [FIG OMISSION]</BR>

        High-Performance Soft Carbons Prepared by Treatment with Various Phosphorus Acids

        Phosphorus-doped soft carbons were successfully prepared by carbonizing petroleum cokes treated with phosphorus acids, such as $H_3PO_2$, $H_3PO_3$, and $H_3PO_4$. The effect of the type of phosphorus acids used on the composition, structure, and electrochemical performance of the soft carbons was extensively investigated. The soft carbons modified with $H_3PO_3$ and $H_3PO_4$ exhibited dramatically improved reversible capacities and outstanding rate capabilities.
