Osteomyelitis of the pubic symphysis is not common and has often been reported to occur after urological or gynecological procedures, It can be spontaneous in origin but it also is associated with trauma, athletic exertion, pregnancy, and parturition. The early symptoms of osteomyelitis of the pubic symphysis mimic those of osteitis pubis and therefore, the differential diagnosis between these two entities is of clinical importance. A fifty nine-year-old man who had previously received debridement and wide excision of perianal necrotizing fasciitis visited our hospital with pain on both inguinal areas. The core biopsy of the pubic symphysis and aspiration culture were performed, from which Pseudomonas aeruginosa was recovered. He was treated with parenteral ceftazidime for 5 days followed by cefepime for 7 weeks and showed favorable clinical response. To our knowledge this is the first report on osteomyelitis of pubic symphysis resulting from debridement and wide excision of perianal necrotizing fasciitis in Korea.
세제세척 후 용기재질과 헹굼시간에 따른 계면활성제 잔류량 변화

일상생활과 실험실에서 널리 사용하는 대표적 용기를 선정하여 세제세척 및 헹굼 후 용기에 잔류하는 계면활성제의 양을 정량화하였다. 일반적 세제 사용량인 8 mL를 수세미에 묻혀 용기를 닦은 후 흐르는 물에 7초 및 15초간 헹궈주었다. 설거지가 끝난 용기에 증류수 100 mL를 붓고 흔들어 그릇에 남아 있는 계면활성제를 추출하여 분석하였다. 7초 헹굼과정을 거친 후 용기에 잔류하는 계면활성제 농도는 뚝배기 4.68 mg/L, 후라이팬 1.22 mg/L, 유리그릇 0.57 mg/L, 플라스틱 용기 0.25 mg/L 순으로 모든 용기에서 계면활성제가 검출되었다. 그러나 15초 헹굼과정을 거친 경우, 뚝배기에서만 계면활성제가 여전 히 검출되었고 다른 용기에서는 검출되지 않았다. 실험결과 다공성매체로 이루어진 재질일수록 계면활성제의 잔류가 많은 것으로 나타났다. 세제로 식기를 세척할 경우 충분한 헹굼과정이 있어야 세제 섭취를 줄일 수 있을 것으로 판단된다. 특히 많은 이공계실험실에서 실험실 용기가 세제세척 후 충분히 헹궈지지 않는다면 잔류 계면활성제가 이후 실험 결과에 지대한 영향을 미칠 수 있다. This study selected representative popular bowls used in daily life and laboratory and investigated the amount of residual surfactant resided in the bowls that were dish-washed. The bowls were washed by a scourer soaking 8 mL-detergent and then rinsed by flowing water for 7 or 15 seconds. One hundred milliliter of distilled water was placed in the washed-bowls to extract residual surfactant on the bowl surface and then analysed. All washed-bowls included residual surfactant after 7 second-rinsing. Residual surfactant concentrations in Ttukbaegi (earthen bowl), flying pan, glass bowl, and plastic bowl were 4.68, 1.22, 0.57, and 0.25 mg/L, respectively. However, the 15 second-rinsing showed that surfactant was detected only in Ttukbaegi and other bowls were clean. The results showed that surfactant would reside on the porous or rough surface of the bowl even after final rinsing. This study imply that all bowls and glasses should be throughly rinsed after detergent-washing to reduce continuos ingestion. Furthermore, residual surfactant in the glassware of laboratory may significantly affect experiment results.

<P>Loss of green color in leaves results from chlorophyll (Chl) degradation in chloroplasts, but little is known about how Chl catabolism is regulated throughout leaf development. Using the staygreen (sgr) mutant in rice (Oryza sativa), which maintains greenness during leaf senescence, we identified Sgr, a senescence-associated gene encoding a novel chloroplast protein. Transgenic rice overexpressing Sgr produces yellowish-brown leaves, and Arabidopsis thaliana pheophorbide a oxygenase-impaired mutants exhibiting a stay-green phenotype during dark-induced senescence have reduced expression of Sgr homologs, indicating that Sgr regulates Chl degradation at the transcriptional level. We show that the leaf stay-greenness of the sgr mutant is associated with a failure in the destabilization of the light-harvesting chlorophyll binding protein (LHCP) complexes of the thylakoid membranes, which is a prerequisite event for the degradation of Chls and LHCPs during senescence. Transient overexpression of Sgr in Nicotiana benthamiana and an in vivo pull-down assay show that Sgr interacts with LHCPII, indicating that the Sgr-LHCPII complexes are formed in the thylakoid membranes. Thus, we propose that in senescing leaves, Sgr regulates Chl degradation by inducing LHCPII disassembly through direct interaction, leading to the degradation of Chls and Chl-free LHCPII by catabolic enzymes and proteases, respectively.</P>

<P><B>ABSTRACT</B></P><P>Oxidative stress activates macroautophagy/autophagy and contributes to atherogenesis via lipophagic flux, a form of lipid removal by autophagy. However, it is not known exactly how endogenous antioxidant enzymes are involved in lipophagic flux. Here, we demonstrate that the antioxidant PRDX1 (peroxiredoxin 1) has a crucial role in the maintenance of lipophagic flux in macrophages. PRDX1 is more highly expressed than other antioxidant enzymes in monocytes and macrophages. We determined that <I>Prdx1</I> deficiency induced excessive oxidative stress and impaired maintenance of autophagic flux in macrophages. <I>Prdx1</I>-deficient macrophages had higher intracellular cholesterol mass and lower cholesterol efflux compared with wild type. This perturbation in cholesterol homeostasis was due to impaired lipophagic cholesterol hydrolysis caused by excessive oxidative stress, resulting in the inhibition of free cholesterol formation and the reduction of NR1H3 (nuclear receptor subfamily 1, group H, member 3) activity. Notably, impairment of both lipophagic flux and cholesterol efflux was restored by the 2-Cys PRDX-mimics ebselen and gliotoxin. Consistent with this observation, <I>apoe <SUP>−/−</SUP></I> mice transplanted with bone marrow from <I>prdx1<SUP>−/−</SUP>apoe<SUP>−/−</SUP></I> mice had increased plaque formation compared with <I>apoe<SUP>−/−</SUP></I> BM-transplanted recipients. This study reveals that PRDX1 is crucial to regulating lipophagic flux and maintaining macrophage cholesterol homeostasis against oxidative stress. We suggest that PRDX1-dependent control of oxidative stress may provide a strategy for treating atherosclerosis and autophagy-related human diseases.</P>

Bednar tumor was described by Bednar in 1957. The histologic pattern of this tumor shows similar to dermatofibrosarcoma protuberans (DFSP) but melanin pigments are scattered within the tumor, It s rare neoplasm accounting for approximately 1-5% of all case of DFSP. Clinically, this tumor is considered to be intermediated malignancy, because of slow growth and frequent local recurrence and lack of distant metastasis. The majority are located on the trunk and the upper and lower extremities, but extremely rate in the head and neck area. Microscopically, this tumor is characterized by tight storiform spindle cells and long slender cells that admixed with a small population of melanin containing dendritic cells. This dendritic cells are the primary features distinguising this lesion from conventional DFSP. Complete surgical excision and close follow-up case are necessany for this neoplasm because of probable intermediate malignancy. A patient was admitted to Our Department of Oral and Maxillofacial Surgery due to swelling on right parotid area and numbness of the right lower lip on September, 1994, By clinical examinations and C-T finding, we dignosed tentatively as myxoma or pleomorphic adenoma. Surgical excision of this tumor was performed with parotidectomy and mandibular osteotomy under the frozen biopsy. By final microscopic and electromicroscopic examination and immunohistochemical study, this tumor was diagnosed as Bednar tumor. So, we report a case of pigmented DFSP with review of literatures.
Recent trends of Multiple birth rates in Korea and fetal/neonatal/infant mortalities

Objective: This study was conducted to analyze recent trends of multiple birth rates (MBR) and fetal/neonatal/infant mortalities according to the number of gestations in Korea Methods: Data from 2009 to 2015 of live births, infant deaths and stillbirths were obtained from the Korean Vital Statistics. Neonatal mortality rate (NMR), infant mortality rate (IMR), and fetal mortality rate (FMR) in singleton, twin and triplet pregnancies were analyzed. Results: From 2009 to 2015, twin and triplet birth rates increased 34.5% and 154.3%, respectively. In twin births, NMR and FMR have been decreased significantly (from 10.92 to 8.62, p=0.034 and from 41.00 to 30.55, p< 0.001, respectively), but IMR did not show significant decrease. There was no significant change of NMR, IMR, and FMR, in triplet births. In singleton, twin, and triplet births, NMR was 1.26 ± 0.09, 10.6 ± 1.12, and 34.32 ± 11.72, respectively, and IMR was 2.38 ± 0.26, 14.52 ± 1.38, and 41.13 ± 12.2, respectively. FMRs were 12 ± 1.73, 35.99 ± 3.55, and 88.85 ± 16.55, respectively. The odds ratios of NMRs and IMRs in 2015 were approximately 9-fold and 6-fold higher, respectively, in twin births, and approximately 37-fold and 20-fold higher, respectively, in triplet births, than those in singleton births. There were no significant differences in odds ratios of NMRs and IMRs at GP 32-36 among single, twin, and triplet births, although the odds ratio of FMR at GP 32-36 in triplet gestation was significantly higher than those in singleton and twin gestation. Conclusion: Neonatal/infant mortality in multiple births is still significantly high, which is mainly related with preterm birth. Close fetal monitoring is needed to prevent fetal death in triplet pregnancies, after 32 gestational weeks.

Acute humoral rejection after renal transplantation is associated with a higher frequency of allograft dysfunction and graft loss. We report a case of acute humoral rejection which was treated successfully with plasmapheresis and intravenous immunoglobulin. A 31-year-old man developed azotemia after kidney transplantation. Kidney biopsy finding was compatible with antibody-mediated rejection, demonstrated by the infiltration of monocytes and neutrophils and the deposition of C4d on glomerulus and peritubular capillaries. We performed five plasmapheresis with concomitant treatment of intravenous immunoglobulin after each session. With aggressive treatment, there was improvement of oliguric acute renal failure, accompanied by decrease in the percentage of PRA and the titer of donor specific antibodies. Repeated kidney biopsy revealed persistent C4d staining on peritubular capillaries despite disappearance of donor specific antibodies. In conclusion, plasmapheresis and intravenous immunoglobulin are effective in treating acute humoral rejection. Korean J Nephrol 2006; 25(5):863-869)
