Purpose: To compare the short-term effects of intravitreal versus posterior subtenon injection of triamcinolone acetonide for diabetic macular edema. Methods: This is a prospective and interventional study. Sixty eyes of 60 patients who had diffuse diabetic macular edema were assigned to receive a single intravitreal injection (4 mg) or a single posterior subtenon injection (40 mg) of triamcinolone acetonide. The central retinal thickness was measured using optical coherent tomography before injection and at 1 and 3 months after injection. Visual acuity and intraocular pressure (IOP) were also measured. Results: Both intravitreal and posterior subtenon injections of triamcinolone acetonide resulted in significant improvements in visual acuity at 1 month and 3 months after injection. Both groups resulted in a significant decrease in central macular thickness (CMT) at 1 month and 3 months post-injection. IOP in the intravitreal injection group was significantly higher than in the posterior subtenon injection group at 3 months after injection. Conclusions: The posterior subtenon injection of triamcinolone acetonide had a comparable effect to the intravitreal triamcinolone injection and showed a lower risk of elevated IOP. Posterior subtenon injection of triamcinolone acetonide may be a good alternative for the treatment of diffuse diabetic macular edema.

This survey was performed to estimate the levels of pathogenic microorganisms, antibiotic residues, and heavy metals in seven Korean freshwater aquaculture species including Anguilla japonica, Cyprinus carpio nudus, Oncorhynchus mykiss, Pseudobagrus fulvidraco, Semisulcospira coreana, Silurus asotus, and Trionyxs sinensis. None of the ten foodborne pathogens tested in this study were found in any of the species collected from any of the aquaculture farms. Furthermore, no banned chemicals or antibiotic residues were found in any of the species collected from any of the aquaculture farms, except enrofloxacin, which was below guideline limits (0.1 mg/kg). Finally, no species had lead, cadmium, total arsenic, or total mercury concentrations above the Ministry of Food and Drug Safety (MSDF) guidelines (0.5, 0.5, 0.1, and 0.5 mg/kg, respectively). These results ensure the safety of freshwater aquaculture species and will be useful for developing consumption advisories of freshwater fishes.

아스페르길루스 심내막염은 국내에서도 7증례만 보고된 바 있는 드문 질환이며 명확한 치료 방안이 정립되지 못하여 사망률 또한 매우 높은 질환이다. 최근 심장수술의 증가, 장기이식, 광범위 항생제 사용 등으로 입원환자 중에 그 발생빈도가 증가하고 있어 해당 질환에 대한 관심이 필요하겠다. 현재까지 아스페르길루스 심내막염의 치료는 외과적 수술 및 적절한 항균제의 사용이 고려되고 있으나 병합요법의 필요성 및 추후 이차적 예방에 관해 더 많은 연구가 요구되고 있다. 저자들은 대동맥판막 치환술및 관상동맥 우회술을 시행한 환자에서 판막파열 및 가성동맥류가 발생하였고, 수술 후 조직검사에서 A. fumigatus를 확인했던 아스페르길루스 심내막염 환자를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Aspergillus endocarditis is a rare infection that occurs most commonly after heart valve replacement surgery. The outcomes for patients with prosthetic valve endocarditis due to Aspergillus species have been extremely poor. Therapeutic management conventionally consists of combined medical and surgical approaches. However, there was no clear evidence to determine whether or not combination therapy was superior to liposomal amphotericin B alone. And no clear pattern could be determined in relation to the optimal duration of initial antifungal treatment or of long-term secondary prophylaxis. We report a case of aspergillus endocarditis in a patient who underwent aortic valve replacement surgery. The results of repeated blood culture were negative. Aortic root replacement was performed and Aspergillus fumigatus was isolated from aortic valve tissue. Despite of postoperative liposomal amphotericin B therapy, he died of multiple organ failure associated with diffuse brain damage.

The endoplasmic reticulum (ER) is not only important for protein synthesis and folding but is also crucial for lipid synthesis and metabolism. In the current study, we demonstrate an important role of ceramide synthases (CerS) in ER stress and NAFLD progression. Ceramide is important in sphingolipid metabolism, and its acyl chain length is determined by a family of six CerS in mammals. CerS2 generates C22-C24 ceramides, and CerS5 or CerS6 produces C16 ceramide. To gain insight into the role of CerS in NAFLD, we used a high-fat diet (HFD)-induced NAFLD mouse model. Decreased levels of CerS2 and increased levels of CerS6 were observed in the steatotic livers of mice fed a HFD. In vitro experiments with Hep3B cells indicated the protective role of CerS2 and the detrimental role of CerS6 in the ER stress response induced by palmitate treatment. In particular, CerS6 overexpression increased sterol regulatory elementbinding protein-1 (SREBP-1) cleavage with decreased levels of INSIG-1, leading to increased lipogenesis. Blocking ER stress abrogated the detrimental effects of CerS6 on palmitate-induced SREBP-1 cleavage. In accordance with the protective role of CerS2 in the palmitate-induced ER stress response, CerS2 knockdown enhanced ER stress and SREBP1 cleavage, and CerS2 heterozygote livers exhibited a stronger ER stress response and higher triglyceride levels following HFD. Finally, treatment with a low dose of bortezomib increased hepatic CerS2 expression and protected the development of NAFLD following HFD. These results indicate that CerS and its derivatives impact hepatic ER stress and lipogenesis differently and might be therapeutic targets for NAFLD.
