        파킨슨씨병 환자에 있어서 자가부신수질 이식술

        Recent experimental studies and clinical cases have suggested that grafting tissue from the adrenal medulla into the brain may alleviate the symptoms of Parkinson's disease. Autologous adrenal medullary grafts into the striatum was performed stereotactically in two patients with Parkinson's disease. Both patients were bedridden preoperatively, but 4 months after the surgery both became ambulatory and one was even capable of returning to his job. Even though the number of cases was small⑵ and the follow-up period was short, this study shows that adrenal medullary transplantation into the striatum may have some beneficial effects in patient with Parkinson's disease. Further experimental and clinical trials are necessary to see whether this procedure will be a recommendable surgical treatment for patients suffering from Parkinson's disease.

        대정맥-심방 우회술로 치료한 기질화된 혈전에 의한 하대정맥 폐쇄 : 증례 보고

        Suprarenal IVC obstruction occurs rarely but has various causes. Because this obstruction proceed chronically and usually has collateral circulation, if there is no IVC or hepatic vein obstruction symptom such as Budd-Chiari syndrome, operation is usually needless. However, although symptom is not combined, if malignancy can not be ruled out and there is no proper and radiologically visible collateral, mass resection with IVC wall and bypass graft should be done. 58 year-old female patient visit the hospital for IVC mass that is occasionally discovered by routine abdominal ultrasonography examination. After abdominal CT scanning and IVC venography, IVC obstructive mass between renal vein and hepatic vein was found. Patient didn't show any abnormality in hematological examination such as coagulation and platelet counts. There was no IVC obstruction symptom such as lower limb swelling. Inferior hepatic vein was abnormally dilated and this was regarded as collateral vessel for IVC obstruction. Radiologically, primary leiomyosarcoma was not ruled out and so operation was decided. Suprarenal IVC was dissected and mass was exposed. And with the use of femoral vein and right atrium, temporally veno-veno bypass was performed. Mass including IVC wall was excised and upper end of divided IVC was sutured. Lower end of divided IVC was anastomosed with 16 mm Dacron graft and graft was anastomosed with right atrium by end-to-end methods (Cavoatrial shunt). Postoperative pathologic examination revealed the mass to be organized thrombi. After 2 weeks later, follow-up IVC venography was performed and good patency was found from IVC to right atrium through artificial bypass graft and patient was discharged without complications.

        Replacement techniques to reduce animal experiments in drug and nanoparticle development

        Background Animals have been used for the testing of new drugs and nanoparticles to determine their safety and effectiveness before application in humans. Numerous drugs and nanoparticles have been developed through animal tests, and development in medical nanotechnology is progressing rapidly. However, a good experiment requires many animals. The primary measures to reduce animal experiments include reducing the number of animals used in each experiment and replacing laboratory animals with inanimate, cellular, or inferior animals to reduce sacrifices. Area covered Developing nanoparticles using simulations rather than experiments may be another alternative. Instead of animals, human cells or animal-derived organs, tissues, or cells could be used in experiments. These methods allow rapid toxicity and efficacy testing at low cost. However, the disadvantage of these methods is that they cannot accurately replicate the complex interrelationships between human organs, biological reactions to specific routes of administration, or toxicity of substances resulting from metabolic processes. To overcome these shortcomings of in vitro tests, new technologies such as organoids and organ-on-chips are progressing. These can be used to quickly examine the efficacy of newly developed drugs and nanoparticles and can be useful in developing patient-tailored agents. Expert opinion These technologies offer some promise but it is still difficult to entirely replace animal experiments. However, the scope of methods to replace experimental animals is widening, suggesting the potential to refine, reduce, and ultimately replace animal testing.

        The axon guidance defect of the telencephalic commissures of the JSAP1-deficient brain was partially rescued by the transgenic expression of JIP1

        <P><B>Abstract</B></P><P>The JNK interacting protein, JSAP1, has been identified as a scaffold protein for mitogen-activated protein kinase (MAPK) signaling pathways and as a linker protein for the cargo transport along the axons. To investigate the physiological function of JSAP1 in vivo, we generated mice lacking JSAP1. The JSAP1 null mutation produced various developmental deficits in the brain, including an axon guidance defect of the corpus callosum, in which phospho-FAK and phospho-JNK were distributed at reduced levels. The axon guidance defect of the corpus callosum in the <I>jsap1</I><SUP><I>−/−</I></SUP> brain was correlated with the misplacement of glial sling cells, which reverted to their normal position after the transgenic expression of JNK interacting protein 1(JIP1). The transgenic JIP1 partially rescued the axon guidance defect of the corpus callosum and the anterior commissure of the <I>jsap1</I><SUP><I>−/−</I></SUP> brain. The JSAP1 null mutation impaired the normal distribution of the Ca<SUP>+2</SUP> regulating protein, calretinin, but not the synaptic vesicle marker, SNAP-25, along the axons of the thalamocortical tract. These results suggest that JSAP1 is required for the axon guidance of the telencephalic commissures and the distribution of cellular protein(s) along axons in vivo, and that the signaling network organized commonly by JIP1 and JSAP1 regulates the axon guidance in the developing brain.</P>

        부분 간절제술로 급성간부전이 유도된 흰쥐에서 비장내 동종 간세포 이식

        Background/Aims: Various techniques of hepatocyte transplantation were actively studied as the alternatives to liver transplantation because of potential advantages including technical simplicity, low risk of operation and use of cells from a liver of single donor for multiple recipients, We investigated the effect of allogenic hepatocellular transplantation into spleen on the survival rate after 90% partial hepatectomy-induced acute hepatic failure in rats. Methods: Acute hepatic failure was induced by resection of all lobes except caudate lobe in Sprague-Dawley rats. Hepatocytes, isolated by collagenase perfusion of the liver via the portal vein, were transplanted into the spleen of rats after 90% partial hepatectomy (2.0×10^7 hepatocytes/rat). Results: There was significant difference in the survival rate between control group without treatment after 90% partial hepatectomy and transplanted group receiving intrasplenic allogenic hepatocellular transplantation after 90% partial hepatectomy. Engraftment and survival of transplanted hepatocytes were observed in spleen 2 days after transplantation. Conclusioas: We could observe significant improvernent of survival rate by intrasplenic hepatocellular transplantation in rats with 90% partial hepatectomy-induced acute liver failure. These results suggest that intrasplenic hepatocellular transplantation may be effective in the treatment of acute liver failure.

        비접촉성 안압계의 임상적 유용성

        The non-contact tonometer can measure intracocular pressure without touching the eye. A central area of the cornea(3.60 mm in diameter) is deformed by an air-pulse of linearly increasing force and the intant of applanation is determined by amonitoring system that senses light reflected from the corneal surface. The interval of time required for the air-pulse to produce applanation is proportional to IOP. The noncontact tonometer utilize the fundamental principles of applanation tonometry. It is calibrated against the Goldman tonometer. To evaluate non-contact tonometer clinically, intraocular pressure was measured with the non-contact tonometer(NCT) and the Goldman applanation tonometer(GAT) in 174 eyes of 101 subjects with noraml corneas. The results were as follows. Mean intraocular pressue was 15.26±4.07 mmHG with the NCT, and 16.36±4.03 mmHG with the GAT respectively. The correlation coefficient at 0.79 was statistically significant at the p<0.0001 level. The regression analysis equation was y=4.34748+0.78861x. Intraocular pressures were gradually decreased with aging (p<0.05), but there was no sexual difference in intraocular pressure with the NCT and GAT.

      • 극후판 적용 강교량 용접구조상세의 피로강도 평가

        The main objectives of this study are to obtain the fatigue test data and investigate the influence of the plates thickness. Two types of specimens with plate thicknesses of 90, 75 and 100 mm are used to examine the influence of plate thickness on the fatigue behavior of welded steel plates. All types of fatigue specimens are tested to evaluate the fatigue strength of structural welding details often used in a steel bridge such as a butt-welded member and lateral attachment to the flange with constant amplitude sinusoidal loading under axial-tension in air, which were fabricated by manual gas weld. Results of the test show that all of the specimens satisfied with the fatigue strength corresponding to their fatigue design category, which means there seems to be no problem using the thick-plate in a real steel bridge. However, most fractured specimens showed brittle failure(rapid crack growth) after a crack is developed. Countermeasures against this phenomena are needed. And through the inspection of cracked surfaces, we found that manual CO₂ welds applied in this experiment could be one of the reasons for brittle failure in the welding zone.

        2004년 3월 4일 대설 사례에 관한 분석과 예보를 위한 제안

        A heavy snowfall event occurred over the middle part of the Korean peninsula on March 4, 2004. The numerical models of KMA failed to forecast this heavy snowfall event because this event was due to small scale disturbance by low lever convergence and atmospheric instability. The analyses for this heavy snowfall have been performed to give forecasters useful suggestions for forecasting heavy snowfall events in the future. The analyses for the snowfall event were recounted by the Hourly Korean Peninsula Analysis Weather Chart (HKPAWC) presenting on the KMA intranet system. We confirmed that warm air flows of low level into south central Korea in conjunction with strong southwesterly winds played important role in the heavy snowfall event. We suggested several check points to improve the forecast of heavy snowfall events in the future through the results of the analyses.

        A Single Center`s 30 Years` Experience of Esophageal Adenocarcinoma

        N/A Background: Adenocarcinoma of the esophagus has been reported to be increasing in incidence in a number of regions throughout the world, while the incidence of squamous cell carcinoma (SCCA) of the esophagus is mostly stable or decreasing. To evaluate the increasing tendency of adenocarcinoma of the Methods: we studied retrospectively the records of patients with histologically proven esophageal cancer between 1970 and 1999 at the Yonsei Medical Center. Results: Total cases of esophageal cancer were 969 patients of which the cases of adenocarcinoma and SCCA were 27 patients and 918 patients, respectively. The ratio of esophageal adenocarcinoma to SCCA was 0.0375 in the 1970s, 0.0241 in the 1980s and 0.0292 in the 1990s. There was no statistical difference (p=0.811) in the ratios of adenocarcinoma of the esophagus between the three consecutive 10-yeargroups. Conclusion: In conclusion, unlike the US and other western countries, it seems that the ratio of esophageal adenocarcinoma compared to SCCA has not increased among patients with esophageal carcinoma at the Yonsei Medical Center.

      • Low Thermal Expansion of $Al_2TiO_5$ Ceramics Prepared from Electrofused Powders

        The synthesis of polycrystalline $Al_2TiO_5$ ceramics with low thermal expansion by fusion in an electric arc furnance was investigated. The thermal expansion curves of $Al_2TiO_5$ ceramics were lowered because of microcracks caused by the strong thermal expansion anisotropy of the crystal axes and were accompanied by hysteresis curves. These phenomena are explained by the opening and closing of microcracks. The difference in microcracking temperatures of dilatometric cooling curves in the range of 400~$620^{\circ}C$ is caused by the difference in sintering temperature, grain size and stabilization status.
