        건강보험 본인부담상한제의 효과성에 관한 연구

        Out-of-Pocket Maximum in the National Health Insurance is a institutional device to maximize positive aspects of cost-sharing and minimize negative effects of pay system related to in cash benefit. Out-of-Pocket Maximum has a limit that do not effects on uninsured benefit. Nevertheless, it is necessary to improve the role of social security by preventing poverty transition or restricting health care utilization. In this aspect, the differentiation of Out-of-Pocket Maximum by income level in 2009 has important meaning on national health insurance system. However, there was no preceding research. This study analysis statistical findings as follows. First, in spite of differentiation by income level in Out-of-Pocket Maximum, the increasement rate of cost-sharing is ascending. In addition, the real coverage is too small range so the system has limitation to perform the institutional goals. Moreover, the differentiation by seven grades in 2014 made just a slight increase in policy target comparing to previous three grades. As a result, the current system is just a nominal system. Meanwhile, setting the upper limit should consider catastrophic health expenditure based on personal income status. Therefore, it is necessary to improve the system of Out-of-Pocket Maximum to perform a substantially policy objectives in national health system.

        NYU 응급환자 분류 알고리즘을 적용한 응급의료 이용 연구

        The purpose of this study is to analyze the current state of emergency medical services in Korea in a variety of ways by identifying and categorizing patients’ use of emergency medical services. The New York University Emergency Department visit algorithm(NYU Algorithm), a major measurement tool used in the U.S., has been applied. The data were analyzed from 2012 to 2017 using the Korean Health Panel(KHP). The summary of the analysis results is as follows. First, the probability of emergency medical service use per person is increasing, and the proportion of the top five chronic diseases in emergency medical services is high, with J00(Cold Flu) and A09(Cystitis and colonitis). Second, using the NYU algorithm, the use of emergency medical services was divided into nine categories, with 24.26% of non-ED, 24.00% of Injury group, 23.67% of PCT, and 11.45% of EDNNP. Third, considering total medical and transportation costs, the average cost per person is 81,284 won per year and the maximun cost is 2,961,510 won per year. Based on NYU algorithm, the percentage of emergency medical expenses occurred by non-ED group and injury group. Fourth, a panel probabilistic effect model analysis showed that non-ED groups are lower in age than other categories, more men than women, and more chronic diseases, in order to analyze the factors of emergency medical use in non-emergency groups. For the efficiency of the use of emergency room resources, patients with high severity should be made available in a timely manner, and the proper use of emergency medical services should be induced. Where non-emergency patients or diseases are preventable and treatable in primary care, policy should strengthen mechanisms such as appropriate patient classification and economic incentives during emergency room visits to attract appropriate medical use.

      • 병원 경쟁과 보장성 강화정책이 의료서비스 가격에 미치는 영향

        의료공급자의 행태변화에 영향을 미치는 요인은 경쟁에 기반을 둔 시장요인과 정책요인으로 구분된다. 본 연구는 의료공급자가 가격을 내생적(endogenous)으로 결정할 수 있는 비급여권에서 병원이 시장경쟁과 건강보험 정책에 따라 가격(price)과 의료 질(quality)을 어떻게 변화시키는지에 대해 분석한다. 기존의 병원경쟁효과 문헌과 정책효과 문헌은 건강보험 급여권에 국한하여 비급여에 대한 연구는 부재하다. 본 연구는 경제모형과 실증분석을 통하여 비급여권에서의 병원경쟁과 정책효과를 분석하였다. 먼저, 경제학적 이론모형인 Salop(1979)의 원형도시모형(circular city model)으로 분석한 결과, 병원경쟁과 의료가격 및 의료 질은 정(+)의 관계가 있으며, 정책효과인 건강보험 보장성강화 역시 의료가격 및 의료 질에 정(+)의 관계가 있음을 도출하였다. 이론모형의 명제가 실증적으로 지지되는지 분석하기 위하여 비급여 진료비 정보자료 중 공급 민감적(supply-sensitive) 특성이 있는 MRI와 초음파 자료를 이용하여 패널 SUR(Panel Seemingly Unrelated Regression) 모형으로 실증분석을 하였다. 분석 결과, 이론모형에서 도출된 명제가 실증적으로 지지됨을 보였다. 의료기관 경쟁과 의료정책이 의료 질과 가격을 상승시키는 의료군비경쟁으로 나타나지 않도록 경쟁을 고려한 의료자원 분배 필요, 지속적인 건강보험 보장성 강화를 통한 필수적 비급여의 급여화, 국민이 체감할 수 있는 비급여 가격 공개 등을 통하여 비대칭정보를 강하게 완화할 수 있는 심도 있는 기제 마련의 필요성을 제시한다. Factors affecting the behavior of medical providers are divided into market factors and policy factors based on competition. This study analyzes how hospitals change prices and quality of care according to market competition and healthcare policies where health providers can determine prices as endogenous. As previous studies only focused on health insurance, and there is no research on the non-coverage national insurance sector. This study analyzed hospital competition and policy effects in the non-coverage benefit sector through an economic model and empirical analysis. First, the derived propositions based on the circularity model of Salop (1979) show hospital competition and healthcare policy for Benefit Expansion in NHI has positive relationships with both medical quality and medical price. In order to analyze whether the proposition of the theoretical model is empirically supported, an empirical analysis was performed using a panel SUR (Panel Seemingly Unrelated Regression) model focused on MRI and sonography with the disclosure of non-covered benefit medical expenses data. The empirical results shown that the proposition derived from the theoretical model was empirically supported. Policy implications are discussed as follows. Firstly, effective healthcare resource distribution concerning regional competition should be considered. Secondly, the consistent benefit expansion policy of NHI that includes non-covered benefit (uninsured services) is needed. Lastly, the role of an informed agent is strengthened by the disclosure of non–covered benefit medical expenses is important for the empowerment of consumers to release asymmetric information in healthcare.

        A Review of Current Digital Mental Health Care Applications for Anxiety Symptoms and Future Prospects

        Objective Since the impact of the coronavirus disease-2019 pandemic, the need for efficiency in medical services has become more urgent than ever. The digital treatment market is rapidly growing worldwide and digital therapeutics (DTx), a major part of the digital medical services, is also emerging as a new paradigm for treatment, with its industry growing rapidly as well. Increasing research is done on the effectiveness of mobile DTx in improving mental health conditions such as insomnia, panic, and depression. Methods This review paper investigates 1) the functions and characteristics of mobile digital mental health care applications for the treatment of anxiety symptoms, 2) extracts common attributes of the applications, and 3) compares them with existing traditional treatment mechanisms. Results Among the 20,000 mental health management applications that have been developed so far, 8 applications that are relatively widely used were selected and reviewed. Check-in, self-help tips, quick relief, journal, courses for practice are common features of the digital mental health care applications for anxiety and are also widely used feature in the cognitive behavioral therapy. Conclusion Based on this review, we have proposed the essential elements and directions for the development of a Korean digital mental health care applications for anxiety disorders.

        전 세계적으로 지구 온난화로 인해 지구의 환경이 변화하고 있다. 이는 인간의 활동이 주된 원인으로 꼽히고 있다. 결국 이러한 변화는 모든 생명체에 막대한 영향을 끼치고, 특히 식생에서의 큰 변화를 낳을 것으로 예상된다. 따라서 본 연구는 앞서 발간된 여러 논문들을 바탕으로 기온과 강수량에 초점을 맞춰 세계와 한국의 기후 변화를 알아보고 식생의 분포 변화를 분석하는 형식으로 이루어졌다. 식생의 변화는 분포지가 점차 확대되고 있는 난대성 작물과 분포지가 점점 축소되고 있는 냉대성 작물의 대표적인 작물을 선택하여 연구를 진행하였다. 난대성 작물에서는 대나무와 차나무, 냉대성 작물에서는 배추나무, 구상나무, 사과나무, 그리고 풍혈을 선정하여 각각의 작물 특성과 이에 따른 재배지의 변화를 예측하였다. The environment of the earth is changing globally because of the global warming. The basic cause of this phenomenon is human activities. It is expected that this change will eventually affect all living things, especially vegetation will be affected a lot by this change. Therefore this study investigated the climate change of the world and the South Korea focusing on temperature and precipitation based on preceding research data, and analyzed the change of vegetation distribution. It was proceeded by choosing some samples of flora of warm climate expanding its realm and some samples of flora of subarctic climate decreasing its realm. There are bamboo and tea plant in samples of warm climate and napa cabbage, Korean fir, apple trees and wind-hole in samples of subarctic climate. This study predicted the natures and changes of plantation of each samples.

      • Electrically conductive graphene/polyacrylamide hydrogels produced by mild chemical reduction for enhanced myoblast growth and differentiation

        <P><B>Abstract</B></P> <P>Graphene and graphene derivatives, such as graphene oxide (GO) and reduced GO (rGO), have been extensively employed as novel components of biomaterials because of their unique electrical and mechanical properties. These materials have also been used to fabricate electrically conductive biomaterials that can effectively deliver electrical signals to biological systems. Recently, increasing attention has been paid to electrically conductive hydrogels that have both electrical activity and a tissue-like softness. In this study, we synthesized conductive graphene hydrogels by mild chemical reduction of graphene oxide/polyacrylamide (GO/PAAm) composite hydrogels to obtain conductive hydrogels. The reduced hydrogel, r(GO/PAAm), exhibited muscle tissue-like stiffness with a Young’s modulus of approximately 50kPa. The electrochemical impedance of r(GO/PAAm) could be decreased by more than ten times compared to that of PAAm and unreduced GO/PAAm. <I>In vitro</I> studies with C2C12 myoblasts revealed that r(GO/PAAm) significantly enhanced proliferation and myogenic differentiation compared with unreduced GO/PAAm and PAAm. Moreover, electrical stimulation of myoblasts growing on r(GO/PAAm) graphene hydrogels for 7days significantly enhanced the myogenic gene expression compared to unstimulated controls. As results, our graphene-based conductive and soft hydrogels will be useful as skeletal muscle tissue scaffolds and can serve as a multifunctional platform that can simultaneously deliver electrical and mechanical cues to biological systems.</P> <P><B>Statement of Significance</B></P> <P>Graphene-based conductive hydrogels presenting electrical conductance and a soft tissue-like modulus were successfully fabricated via mild reduction of graphene oxide/polyacrylamide composite hydrogels to study their potential to skeletal tissue scaffold applications. Significantly promoted myoblast proliferation and differentiation were obtained on our hydrogels. Additionally, electrical stimulation of myoblasts via the graphene hydrogels could further upregulate myogenic gene expressions. Our graphene-incorporated conductive hydrogels will impact on the development of new materials for skeletal muscle tissue engineering scaffolds and bioelectronics devices, and also serve as novel platforms to study cellular interactions with electrical and mechanical signals.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

      • 중간언어의 VC 생성을 통한 바이트코드 검증

        프로그램 신뢰성을 높이기 위한 방법 중 하나로 쓰이는 것이 명세 된 언어의 검증이다. 명세 된 언어를 검증하기 위해 소스 프로그램을 논리식으로 바꾸어 검증하는 방법을 사용한다. 소스 프로그램 뿐만 아니라 바이트코드 역시 프로그램 신뢰성을 높이기 위해서 검증이 필요하다. 본 논문에서는 바이트 코드의 검증을 위해 바이트코드의 정보를 가지고 있는 중간언어의 verification condition을 생성하는 방법을 보인다.
