      • Celecoxib 유도체인 PCX-계열 화합물의 항염증 및 혈관신생 억제 작용

        This study aimed to evaluate the pharmacological efficacies of PCX-series compounds, which were synthesized as prodrugs of celecoxib, a COX-2 selective inhibitor. Celecoxib, PCX-1, PCX-2, PCX-3, PCX-7, PCX-15 and PCX-16 provide potent anti-angiogenic activity in chorioallantoic membrane (CAM) assay. In addition, celecoxib, PCX-2, and PCX-16 showed anti-angiogenic activity in a dose-dependent manner. PCX-2 inhibited swelling in the carrageenan-induced rat paw edema test 24 hours after the administration. In the acetic acid-induced writhing test, PCX-7 and PCX-15 were verified to provide more potent analgesic effect than celecoxib 12 hours after the administration, and PCX-2 showed potent analgesic effect 24 hours after the administration. In the carrageenan-induced rat air pouch model, PCX-2 significantly decreased exudate volume, number of leukocyte and NO level. In the ulcerogenic activity test, PCX-2, PCX-7, PCX-15, and PCX-16 showed significantly less ulcer formation than indomethacin. Especially, PCX-7 showed the highest safety in the GI safety test. These findings provide novel pharmacological knowledge on pharmacological actions of celecoxib and its PCX-series compouds.

        Adjuvant role of macrophages in stem cell-induced cardiac repair in rats

        <▼1><P>Bone marrow-derived mesenchymal stem cells (BMMSCs) are used extensively for cardiac repair and interact with immune cells in the damaged heart. Macrophages are known to be modulated by stem cells, and we hypothesized that priming macrophages with BMMSCs would enhance their therapeutic efficacy. Rat bone marrow-derived macrophages (BMDMs) were stimulated by lipopolysaccharide (LPS) with or without coculture with rat BMCs. In the LPS-stimulated BMDMs, induction of the inflammatory marker iNOS was attenuated, and the anti-inflammatory marker Arg1 was markedly upregulated by coculture with BMMSCs. Myocardial infarction (MI) was induced in rats. One group was injected with BMMSCs, and a second group was injected with MIX (a mixture of BMMSCs and BMDMs after coculture). The reduction in cardiac fibrosis was greater in the MIX group than in the BMC group. Cardiac function was improved in the BMMSC group and was substantially improved in the MIX group. Angiogenesis was better in the MIX group, and anti-inflammatory macrophages were more abundant in the MIX group than in the BMMSC group. In the BMMSCs, interferon regulatory factor 5 (IRF5) was exclusively induced by coculture with macrophages. IRF5 knockdown in BMMSCs failed to suppress inflammatory marker induction in the macrophages. In this study, we demonstrated the successful application of BMDMs primed with BMMSCs as an adjuvant to cell therapy for cardiac repair.</P></▼1><▼2><P><B>Heart attacks: mixed cell therapy for heart regeneration</B></P><P>A tailored technique involving stem cells and anti-inflammatory immune cells shows promise for repairing heart tissue damage. Immune cells called anti-inflammatory macrophages are vital for healing of the heart following a heart attack. Youngkeun Ahn, Yong Sook Kim and co-workers at Chonnam National University Hospital in Gwangju, South Korea trialed a novel stem cell therapy on rats to improve cardiac repair. They took bone marrow-derived macrophages and stem cells from each rat and incubated the two cell types together to create individualized treatments. Following induced heart attacks, they injected one group of rats with both cell types, and another group with stem cells only. While heart function improved in both groups, the group treated with both cell types showed significant improvements with a greater reduction in cardiac fibrosis and increased the reparative activity of macrophages.</P></▼2>

        중성어혈(中性瘀血) 약침(藥鍼)과 당귀수산(當歸鬚散)이 흰쥐의 골절초기(骨折初期) 골유합(骨癒合)에 미치는 영향

        Objectives: This study was designed to evaluate the effects of Neutral Eohyeol(Yuxue) herbal acupuncture and Dangkisoo-san(Dangguixu-san) on fracture healing in rats. Methods : We divided the subjects into 5 groups : normal, control with no treatment, Sample Ⅰ taken with Dangkisoo-san(Dangguixu-san), Sample Ⅱ taken with Neutral Eohyeol(Yuxue) herbal acupuncture and sample Ⅲ taken with Neutral Eohyeol(Yuxue) herbal acupuncture and Dangkisoo-san(Dangguixu-san) groups. We induced penetrating fracture on femur in control, sample Ⅰ, Ⅱ and Ⅲ groups. And after serum T-ALP, serum osteocalcin, histological changes, and the degree of TGF-β1 in each group were observed at the 7th day and at the 14th day respectively. Results : All the sample groups showed significant changes in serum T-ALP, histological observation and TGF-beta 1 compared with control group. Especially sample Ⅲ was more effective than other groups in fracture healing. However, serum osteocalcin insignificantly increased at the 14th day. It seemed that 14 day was too short to evaluate osteocalcin. Conclusions : It is suggested that Neutral Eohyeol(Yuxue) herbal acupuncture and Dangkisoo-san(Dangguixu-san) may play a significant role in fracture healing and these two treatments could be more effective when used together

        외상성(外傷性) 측두골(側頭骨) 골절(骨折)로 인한 안면신경마비 치험 1례

        Objectives: The purpose of this study is to report that a patient with facial nerve paralysis caused by traumatic temporal bone fracture was improved by non-surgical treatment. Methods : We treated this patient with electric acupuncture, physical therapy and herbal medication. We evaluated the state of improvement by House-Brackmann facial grading scale, Yanagihara`s unweighted grading system, DITI(digital infrared thermographic imaging) and EMG(electromyography). DITI was measured twice before treatment and after 25 weeks ; EMG before treatment and after 12 weeks. Results : The subject was improved in estimation by House-Brackmann facial grading scale, Yanagihara`s unweighted grading system, DITI and EMG. Conclusions : Although this case was supposed to take surgical treatment, non-surgical treatment(electric acupuncture, physical therapy and herbal medication) was effective to improve many symptoms of facial nerve paralysis. We expect we can use non-surgical treatments more broadly for diseases which can be taken only by surgery with subsequent studies

        산후 비만의 효과적인 접근을 위한 문헌적 고찰

        Weight change in the postpartum period is a dominant concern of new mothers recently. Postpartum weight retention has important public health implication as well, because retention of gestational weight can be a significant contributor to long-term obesity and associated health risks. This study was designed to collect the latest papers and find more effective approach for postpartum obesity. Methods We searched 81 papers in English from PubMed and selected 41 in favor of our study. In addition to that, we searched 11 papers in Korean and 2 papers in Chinese. Results are as follows. Results and Conclusions These papers show that many different factors were related with postpartum weight retention. This study suggests that moderate weight gain during pregnancy is important to prevent postpartum obesity. Adequate caloric restriction and exercise are needed to prevent from postpartum weight retention. In addition, husbands, some relatives and friends are primary sources for postpartum female to lose and keep up weight. The effect of weight loss associated with lactation significantly differs from paper to paper. There are not many treatments for postpartum weight retention. So, more effective treatments using Korean traditional medicine must be improved even from now on. This study can offer many ideas to treat postpartum weight retention.

        좌골신경 압좌 손상 후 전침 및 녹용 약침의 통증 감소와 신경 재생에 관한 실험적 연구

        Objectives: This study was designed to evaluate effects of electroacupuncture and Cervi Pantortichum Cornu pharmacopuncture on pain and nerve regeneration after crush injury of sciatic nerve in rats. Methods: We divided the subjects into 5 groups: control group, acupuncture treated group, electroacupuncture treated group, Cervi Pantortichum Cornu pharmacopuncture treated group, electroacupuncture and Cervi Pantortichum Cornu pharmacopuncture group. Hot plate test and degree of Substance P antibody in each group were observed at the 1st, 2nd, 3rd and 5th day. Sciatic function index and change of BDNF immunoreactivity had been estimated at the 7th, 14th, 21st day respectively. Atrophy of the gastrocnemius at the 21st day was observed. Results: All the experimental groups showed significant changes in hot plate test, degree of Substance P antibody, sciatic function index and change of BDNF immunoreactivity compared with control group. Especially, when electroacupuncture and Cervi Pantortichum Cornu pharmacopuncture used together, results were more effective than other groups. Recovery of the gastrocnemius was also the same. Conclusions: It is suggested that electroacupuncture and Cervi Pantortichum Cornu pharmacopuncture may play a significant role on pain decrease and nerve regeneration after crush injury of sciatic nerve in rats and these two treatments could be more effective when used together.

        슬링 운동이 요통 환자의 통증 감소 및 근력 강화에 미치는 영향

        Objectives :Sling exercise therapy(SET) is a system for spinal muscle stabilization and strengthening using fixed string and mobile band. Nowadays, patients with spinal disorders are spreading more and more because of many working conditions. At the same time, non-operation therapies including SET come to have important meaning from a viewpoint of prevention and treatment of spinal disorders. Methods : The subjects for this study are 32 patients suffering lumbar pain in our clinic. They were divided into two groups(Control group and SET group). All the patients were taken basic treatments including acupuncture, cupping, moxibustion and physical therapy. The subjects in SET group were taught special sling program for pain. Every subjects visited 12 times. And they were estimated by VAS, flexibility index and disability index(Oswestry Score) 3 times(before, middle and final of this study). Results : In the above index, the score were decreased after treatments. Of course, patients in both groups were improved. Especially, subjects in SET group were satisfied with pain relief. Conclusions : These results suggest that SET may be used for spinal disorders for pain management and muscle strengthening as an essential treatment

        Feasibility of ultrasound-guided posterior approach for interscalene catheter placement during arthroscopic shoulder surgery

        Background: Continuous interscalene block has been known to improve postoperative analgesia after arthroscopic shoulder surgery. This was a prospective study investigating the ultrasound-guided posterior approach for placement of an interscalene catheter, clinical efficacy and complications after placement of the catheter. Methods: Forty-two patients undergoing elective arthroscopic shoulder surgery were included in this study and an interscalene catheter was inserted under the guidance of ultrasound with posterior approach. With the inplane approach, the 17 G Tuohy needle was advanced until the tip was placed between the C5 and C6 nerve roots. After a bolus injection of 20 ml of 0.2% ropivacaine, a catheter was threaded and secured. A continuous infusion of ropivacaine 0.2% 4 ml/hr with patient- controlled 5 ml boluses every hour was used over 2 days. Difficulties in placement of the catheter, clinical efficacy of analgesia and complications were recorded. All patients were monitored for 48 hours and examined by the surgeon for complications within 2 weeks of hospital discharge. Results: Easy placement of the catheter was achieved in 100% of the patients and the success rate of catheter placement during the 48 hr period was 92.9%. Postoperative analgesia was effective in 88.1% of the patients in the post anesthetic care unit. The major complications included nausea (7.1%), vomiting (4.8%), dyspnea (4.8%) and unintended vascular punctures (2.4%). Other complications such as neurologic deficits and local infection around the puncture site did not occur. Conclusions: The ultrasound-guided interscalene block with a posterior approach is associated with a success high rate in placement of the interscalene catheter and a low rate of complications. However, the small sample size limits us to draw definite conclusions. Therefore, a well-designed randomized controlled trial is required to confirm our preliminary study.
