        부인암에 있어서 131I-Hippuran Renogram,131I-Hippuran 뇨배설율 및 정맥성 신우상의 비교 관찰

        부인암 61예 (자궁경암 58예, 기타 3예)에서 (131)^I-hippuran renogram, IVP 및 131I-hippuran 배설율을 비교관찰하여 다음과 같은 결론을 얻었다. 1) 정상정맥성 신우상 (IVP)을 보여준 40예의 자궁경암환자중 7예 (17.5%)에서 일측 혹은 양측 renogram상 배설지연소견을 보였고, 반대로 정상 renogram을 보여주었던 35예의 자궁경암환자중 2예 (5.7%)에서는, 정맥성 신우상 (IVP)에 경한 caliectatic 변화를 인지하였다. 2) Renogram상 일측 혹은 양측에 배설지연형소견이 있었던 군에서는 처음 20분간의 (131)^I-hippuran 배설이 감소되었으나 60분이내에 대상되었었다. 3) 일측 renogram이 무기능형이고 타측 renogram이 정상인 군에 있어서는 (131)^I-hippuran 배설율은 정상과 차이가 없었다. 4) (131)^I-hippuran renogram은 X-선학적인 정맥성 신우촬영술과 마찬가지로 폐색성 비뇨기계 질환의 검사에 있어서 중요한 검사방법일 뿐만 아니라 환자에게 부담을 주지 않고 용이하게 검사할 수 있는 장점이 있으며 자궁경암의 침범범위가 확대됨에 따라서 보다 심한 renogram상의 요류 장애상을 볼 수 있었고 일측이 무기능형의 renogram을 보일지라도 타측이 정상이면 131I-hippuran 배설율을 대상함을 보았 A comparative study of (131)^I-hippuran renogram, (131)^I-hippuran excretion test and intravenous pyelogram were performed in 61 cases of gynecological cancer. The following were the results: 1) Among 40 cases of cervix cancer showing normal excretory urography 7 cases (17.5%) were found to have unilateral or bilateral delayed excretory pattern on (131)^I-hippuran renogram and on the contrary only 2 cases (5.7%) showed a mild caliectatic change on excretory urography among 35 cases of gynecological cancer showing normal pattern of (131)^I-hippuran renogram. 2) In the group showing unilateral of bilateral delayed excretory pattern of (131)^I-hippuran renogram there was a reduction of (131)^I-hippuran excretion in the first 20 minutes, but there was no significant difference of 131I-hippuran excretion in 60 minutes compared with that of normal renogram group. 3) In the group showing unilateral non-functioning pattern of (131)^I-hippuran renogram in one side and normal pattern in the other side there was found to be no difference in (131)^I-hippuran excretion amount compared with that of normal renogram group. 4) It was evident from these experimental study that (131)^I-hippuran renogram was considered as a good examination method for the evaluation of obstructive uropathy, and if one side kidney was intact it might compensate for the other diseased kidney so far as to renal excretory function. It was also shown that the more severe the cancerous spread in the pelvic wall the more changes on (131)^I-hippuran renogram.

      • Processed Xanthii fructus increases cell viability of mast cell line, RBL-2H3

        The effect of aqueous extract of processed Xanthii fructus (PXF) on cell viability and histamine release from mast cell has been investigated. PXF showed higher cell viability than unprocessed Xanthii fructus (XF) at the concentrations of 5 and 10 mg/ml. Aqueous extract of PXF and unprocessed XF inhibited compound 48/80-induced systemic anaphylaxis in mouse. Both PXF and unprocessed XF dose-dependently inhibited histamine release from rat peritoneal mast cells by compound 48/80 to the similar extent at 0.01, 0.1 and 1 mg/ml. Our studies provide evidence that processing of XF may be beneficial to reduce cytotoxicity in high concentration (at 5 and 10 mg/ml) but does not affect on anti-allergic activity.

        간경변증에 있어서 교질형방사성금(198Au)을 사용한 간스캔과 간기능과의 관계에 관한 임상연구

        간경변증에 있어서의 간스캔상 변화가 간기능장애의 정도에 따라서 어떻게 진행되는가를 관찰하기 위하여 교질형방사성금을 사용하여 간 스캔을 실시함과 동시에 간기능검사의 수단으로서 교질형방사성금의 혈중소실반감시간을 체외계측법으로 측정하여 다음과 같은 결론을 얻었다. 1) 간경변증에 있어서 간기능장애가 심하여짐에 따라서 관찰된 교질형방사성금, 간스캔상의 변화는 간의 전체적인 종대, 간좌엽의 종대 및 우엽의 위축상, 간좌엽의 우엽에 대한 상대적 방사능의 높은 집적, 비상의 출현등의 순서로 나나났다. 2) 간경변증에서 간 스캔이 정상으로 나타난 빈도는 12.5±3.56%, 교질형방사성금의 혈중 소실반감시간이 정상범위내로 나타난 빈도는 5.0±2.43%이었으며 양자 모두 정상으로 나타난 빈도는 3.6±2.06%이었다. $quot; Correlation between the blood clearance half time and findings of liver scan using the colloidal radiogold in patients of liver cirrhosis is observed through the scoring system, in which the more changes in size, shape and density in the liver scan, the more points are given (table 1). Results: 1) Within the increase in severity of hepatocellular dysfunction in liver cirrhosis, the degree and frequency of following changes in liver scan (done with colloidal radiogold) were increased in order. a) generalized hepatomegaly b) enlargement of th left lobe & reduction of the right lobe c) relatively increased radiodensity in the left lobe and d) visualization of spleen. 2) Frequency of the normal scan in liver cirrhosis was 12±3.56%, frequency of normal value in blood clearance half time of the radiogold was 5.0±2.34% and frequency of normal scan & normal blood clearance rate in liver cirrhosis was 3.6±2.06%.

        원발성 뇌교출혈에 대한 임상적 고찰

        We have experienced 80 cases of primary pontine hemorrhage who were admitted to Kyung Hee Medical Center from October 1982 to September 1987. All cases were confirmed by brain CT scan and treated conservatively. The hematomas have classified according to their modes of extension on computerized tomography. The outcome was assessed on their basis of activity of daily living. The results were as follows : 1) Pontine hemorrhage represented 7.8% of all primary intracerebral hemorrhages. 2) Age distribution was ranged from 3rd decade to 7th decade, and 65% of them were in 4th and 5th decade. The ratio of male to female was 5 : 3. 3) 91.2% of patients had history of systemic hypertension and most of them did not received antihypertensive medication. 85% of patients showed the systolic blood pressure on admission above 160㎜Hg. 4) Outcome Ⅰ and Ⅱ according to ADL were only 7.5%. The prognosis was affected by level of consciousness, blood pressure on admission, and size and extent of hemorrhage. 5) Majority of causes of death were injury to brain itself, and the mortality rate was 33.7%.

        A Feasibility Study on the Use of Optical Fibers for the Transfer of Scintillation Light to Silicon Photomultipliers

        <P>Integrated PET/MRI units with simultaneous acquisition capability are set to play an important role in studies of human breast and prostate imaging and brain function. However, to take advantage of existing MRI units in hospitals and institutions, minimally modified combined PET MRI is highly desirable. In addition, the current MRI trend is to utilize powerful body coils to transmit radio-frequency (RF) waves and local RF coils to receive signals. The authors propose a silicon photomultiplier (SiPM) PET equipped with optical fiber bundles that transfer photons from scintillation crystal to SiPM. To investigate the feasibility of SiPM PET using optical fiber bundles, the authors studied the performances of SiPM/scintillator couplings using single optical fibers and a fiber bundle. GEANT4 Monte-Carlo simulation was used to study scintillation photon transfer from scintillation crystals to the SiPM. This simulation showed that light loss, due to the bending of an optical fiber, is not significant for a fiber with a diameter of 2.0 mm and a bending radius of greater than 25 mm. To validate the GEANT4 Monte-Carlo simulation, several simple detectors were assembled and tested. Simulation results agreed reasonably well with experimental results. Two Hamamatsu multi-pixel photon counters (MPPCs) were tested using double clad optical fibers of 1.5 mm and 2.0 mm diameter, and 25 mm and 50 mm bending radius, respectively. When two MPPCs were directly attached to 2.0 × 2.0 × 10.0 mm<SUP>3</SUP> LYSO crystals, a ~14% energy and a ~1.3 ns coincidence timing resolution were obtained at full width half maximum (FWHM). With one of the MPPCs attached to an optical fiber of 1.5 mm diameter, 50 mm bending radius, and length 300 mm, energy and coincidence timing resolutions were 27% and 2.2 ns, respectively. With an optical fiber bundle made of bare fibers with 1.5 mm diameter and length 100 mm, an ~ 26% energy resolution was obtained. Even though the Monte-Carlo simulation showed light loss was not significant for a single 90° turn of bending, the mechanical integrity of the optical fiber, especially the absence of cracks which can be caused by sharp bending, seemed to be a far more important constraint on sharp bending. These initial results are encouraging with respect to the use of combined SiPM PET using optical fibers.</P>

        113mIn 교질에 의한 간주사에 관한 연구

        간 scan용 113mIn 교질을 만들어 이의 간섭취율 및 혈중 clearance을 측정하고 간 scan을 시행하여 198Au 교질의 성적과 비교관찰하여 다음과 같은 결론을 얻었다. 1) 정상인에 있어서 113mIn 교질의 간섭취율 반감시간은 1.40±0.24분 및 혈중 clearance의 반감시간은 1.47±0.27분으로 198Au 교질의 경우보다는 유의한 단축을 보였다. 그러나 이로서 간기능을 추정하는 것은 반드시 정확한 index가 되는 것 같지 않았다. 2) Space occupying lesion의 해상능은 113mIn 교질을 사용하면 198Au 교질보다 우수함을 알 수 있었다. 3) 113mIn 교질의 간섭취율은 간기능의 변화에 따르는 198Au 교질의 간섭취율의 변동과 반드시 일치하는 것은 아님을 알 수 있었으며 따라서 이는 RES의 탐식능의 다양성에 기인한 듯 하였다. 4) 113mIn 교질에 의한 간 scan에 있어서는 정상인에 있어서도 비장의 출현이 있었다. 5) 간기능장애가 있을때는 간의 교질 섭취성이 113mIn 교질에 있어서 198Au 교질보다 더욱 장애를 받으며 비장의 교질 섭취능은 상반되는 관계에 있음을 알 수 있었다. 6) 113mIn 교질을 만든 다음 인체에 사용하기 전에 coarse filter에 걸려서 사용하는 경우 더욱 좋은 간 scan상을 볼 수 있었다. 7) 간 scan에 113mIn 교질을 사용함으로서 scan시간을 더욱 단축시킬 수 있었다. There have been reported numberous cases of liver scanning in use of 198Au colloid by many investigators, however, one in use of 113mIn colloid has not been reported as yet in this country. The dose of 113mIn for high diagnostic value in examination of each organ was determined and the diagnostic interpretability of liver scanning with the use of 113mIn was carefully evaluated in comparison with the results of the liver scanning by the conventionally applied radioisotope. The comparative study of both figures of liver scanning with the use of 113mIn colloid and 198Au colloid delivered following results:1) The liver uptake rate and clearance into peripheral blood were accentuated more in case of 113mIn colloid than in case of 198Au colloid. 2) The interpretability of space occupying lesion in liver scanning with 113mIn was also superior to one with 198Au. 3) The figure of liver scanning with 113mIn colloid corresponds not always to the figure with 198Au. This difference can be explained by difference of phagocytic ability of reticuloendotherial system within liver. 4) In the liver scanning with 113mIn colloid, the spleen is also visualized even in normal examine. 5) In the cases of disturbed liver function, uptake is more decreased in use of 113mIn colloid than in 198Au, in the spleen, however, the way is contrary. 6) With use of 113mIn colloid, the time required for scanning could be shortened in comparison with 198Au. 7) The filtration of 113mIn colloid for scanning prior to human administration gives an expectation for better scanning figure.
