        일부 조선업 근로자의 직업성 요통 발생실태 및 위험요인에 관한 연구

        목적 : 이 연구에서는 한 사업장의 동일한 코호트내에서 직업성 요통의 발생 및 발생원인에 대해 조사하고자 하였다. 방법 : 연구대상은 1995년 12월 현재 한 조선업체에 종사하는 근로자 중에서 1995년 12월 31일 까지 요통이 이미 발생한 근로자와 1995년 당시 검진 미수검자를 제외한 9,784명을 연구대상으로 하였다. 요통 발생군은 1996년부터 1998년까지 3년간 추적 관찰하여 요통으로 4일 이상 통원 또는 입원치료를 받은 근로자로 하였다. 발생률은 요통발생 당시 연도를 기준으로 하여 해당 연도의 4일 이상 신규치료를 받은 중등도 이상의 요통근로자를 분자로 하였고, 해당년도 사업장의 종사하는 근로자 총수(과거 요통자 제외)를 분모로 간주하였다. 요통 발생군은 외상성 요통과 직업관련성 요통으로 대별하였으며, 위험요인과 질병 발생간의 관련성을 보기 위한 Cox모형에서 그 위험도를 파악하고자 할 때에는 직업성 요통만을 환자군으로 선정하였다. 결과 및 결론 . 요통 발생률은 근로자 천명당 1998년 3.1명, 1997년 8.8명, 1996년 7.8명이었다. 요통점유율의 범위는 10.99∼25.66이었다. 연령별 요통 발생률은 30대, 40대에서 높은 발생률을 보였다. 진단명에 따른 발생률은 추간판 탈출증, 염좌, 척추골절, 척추분리증, 척추협착증 순이었다 직업성 요통 발생자들의 직업적 요인을 알아보기 위하여 요통 발생 당시 작업형태를 알아본 결과, 물건을 들다가 66명(35.3%)이 발생하여 가장 큰 빈도를 차지하였고, 물건을 옮기는 과정에서 19명(10.2%)이 발생하여 2위를 차지하였고, 넘어지거나(8.6%), 물건을 당기는 자세(8.0%), 값자기 일어서다(7.5%) 순으로 요통이 발생하였다. 직업성 요통에 영향을 주는 일반적 특성 및 직업적 특성에 판한 상대위험도 및 95% 신뢰구간를 구한 결과, 요통발생에 영향을 주는 유의한 변수로는 체질량 지수(비만도), 교대여부, 중량물 취급여부였다. Objectives : This study was conducted to estimate the incidence rate, and to identify the risk factors for the occupational low back pain among shipyard workers. Methods : The study subjects consisted of 9,784 workers who were employed in a ship-building industry (excluded workers who had a history of low pack pain before 1995 or did not take periodic health examination in 1995) . The cases were 220 people who experienced back pain from January 1, 1996 to December 31, 1998. To assess risk factors for occupational low back pain, Cox propotional hazard model was used. Results : During the recent three years from 1996 to 1998, the incidence rate per 1,000 persons was 7.8 in 1996, 8.8 in 1997, and 3.1 in 1998. The main causes of work-related low back pawn were sprain, strain and disc herniation. Lifting was the most common cause of back pain (35.3%) , and carrying (10.2%) and pulling (8.0%) were followed. In Cox proportional harzard model, independent risk factors for back pain were body mass index(R.R. ; 1.54, 95% C.1. : 1.06-2.25), shift work(R.R. ; 1.65, 95% C.1. : 1.19-2.28), and lifting heavy material(R. R., 3.95, 95% C.1. : 2.29-6.82). Conclusions . This study suggests that the risk factors of back pawn In shipyard work-ers were body mass index, shift work and lifting.

      • 강원지역의 자원관리를 위한 지리정보시스템 개발

        강원도는 오랫동안 금속, 비금속, 석재 및 석탄과 같은 지하자원들을 공급하는 국내 주요생산지로 알려져 왔으나, 장기적인 채광과 비 계획적인 생산으로 인하여 매장량이 고갈되고 있으며, 대외적으로 생산 경쟁력 마저 잃어가고 있다. 이러한 문제점에 대한 해결방안의 일환으로 본 연구에서는 강원도내에 산재되어 있는 지하자원의 분포, 자원 생산시설 및 생산량, 경제성, 자원개발, 환경문제 등 자원관리에 필요한 제반사항을 수행할 수 있는 강원지역 자원 종합관리 GIS 시스템을 개발하였다. 이 시스템의 개발은 방대한 양의 정보 조사 및 저장, 오랜 개발기간, 많은 소요인력 및 개발 비용이 요구되기 때문에 본 연구에서는 조기에 자원관리 GIS 시스템을 개발하여 이와 관련된 생산 및 연구 분야에 조기에 적용할 수 있도록, 일차년도에는 현재 석재복합 신소재제품 연구센터의 집중적 연구대상 지역인 태백지역에 대한 탄광지역을 중심으로 자원 종합관리 GIS 시스템을 개발하였다. The Kangwon province has been well-known to be one of major underground resource producing districts which are mainly supplying metals, non-metals, stones, and coals. However, due to the long-term mining and unplanned production, the natural resource reserves have been rapidly drained and oversea production competition has been losing. In this study, we has developed a GIS system for regional resource management in the Kangwon province, which can operate, maintain, and manage the distribution of underground resources, resource production facilities, production and economic development, and environmental issues. The development of this system requires the collection, analysis, evaluation, and storage for a huge amount of research data, and in addition, the long period of time, the big research man power, and big development cost. Accordingly, to shorten the development period of time and to early apply itself to production, this stage of study has been intensively developed a regional resource GIS system for managing the mines scattered in the Tabak province, which is one of hottest RRC's research areas.

        둥시 장아찌 제조 과정 중 이화학적 특성 변화

        떫은감 품종의 하나인 등시를 이용하여 감장아찌 제조 중 과육의 이화학적 변화를 조사한 결과, 염도 변화는 저농도 간장 및 된장 침지의 경우 다소 완만한 증가를 보였고, 고농도의 간장 및 된장 침지의 경우 침지 기간내내 증가하는 것으로 나타났다. 간장 침지액 감장아찌의 경도는 간장 및 된장 모두 20-80% 농도에서는 침지 초기에는 증가하다가 침지 기간이 경과할수록 감소하는 경향을 나타내었다. 간장 침지 감장아찌의 색도 변화는 저장기간이 경과할수록 명도가 낮아지는 경향이었고, 적색도와 황색도는 침지액 농도를 달리하였을 경우 농도가 낮을수록, 침지 기간이 길어질수록 높았으며, 침지액 농도에 따른 차이는 크지 않았다. 침지액의 간장 및 된장의 농도를 달리하였을 때 감장아찌 중의 탄닌함량의 변화는 모든 침지액 농도에서 침지 기간이 지속될수록 탄닌의 함량은 저장 초기에 서서히 감소되다가 침지 기간이 길어질수록 급격히 감소하였으며, 첨가량에 따른 탄닌함량의 변화폭은 크지 않았다. 침지액의 간장 및 된장의 농도를 달리하였을 때 감과육에 존재하는 연화효소인 polygalacturonase와 pectinesterase 활성 변화는 간장침지 및 된장침지 모두 침지 초기부터 효소활성이 증가하기 시작하였으며 침지 기간이 경화할수록 효소활성은 더욱 높아지는 것을 알 수 있었고, 침지액의 농도가 높아질수록 효소활성이 억제되었다. Changes of physicochemical properties during the preparation of persimmon pickles were investigated. The salinity in persimmon pickles increased during storage time (0~50 days) with soy sauce and soy paste. When the fermented soy sauce and soy paste was added to the soaking solution with 80~100%, the salinity increased more rapidly. The hardness of persimmon pickles with soy sauce and soy paste was slightly increased up to the 20 or 30th day of storage and then decreased. L value of persimmon pickles was gradually decreased, but a and b value were slightly increased. The concentration of soluble tannin in persimmon pickles was slightly decreased down to the 20th day of storage and then decreased rapidly. The activities of polygalacturonase and pectinesterase as softening enzyme in persimmon pickles with soy sauce and soy paste increased during storage time (0-50 days) and enzyme activity was inhibited by high concentration of soaking solution.

      • 후천성 면역 결핍증에서의 폐렴구균 감염 부검 1예 : An Autopsy Report

        28세 남자의 후천성 면역 결핍증 환자에서 S. pneumoniae 폐렴이 합병되어 사망한 1예를 경험하여 부검소견과 함께 보고하는 바이다. Pulmonary infections such as Pneumocystis carinii, cytomegalovirus, and tuberculosis are a major cause of morbidity and mortality in patients with human immunodeficiency virus(HIV) infection. Patients with HIV infection are at increased risk for bacterial pneumonia in addition to opportunistic infection. Although in a primary defect, HIV infection is associated with substantial dysfunction of humoral immunity, which predisposes patients to bacterial infections, particularly with encapsulated organisms, such as Streptococcus pneumoniae. We report a case of 28 year-old Korean male with HIV infection who died of respiratory failure due to pneumococcal pneumonia with autopsy findings.

      • 반코마이신 내성 장구균 감염에 대한 quinupristin-dalfopristin 치료 경험

        VRE 감염이 중환자에서 급속히 증가하고 있으나 이에 대한 확실한 치료제가 없어 새로운 항생제에 대한 활발한 연구가 있는데 그 중 하나가 quinupristin/dalfopristin이고, 저자들은 중증 환자들의 VRE 감염에 대해 quinupristin/dalfopristin를 투여하여 치료한 임상적 경험을 하였기에 문헌고찰과 함께 보고하는 바이다. Vancomycin-resistant enterococci (VRE) was first recognized in 1992 in Korea. VRE infection have been increasingly reported in immunosuppressed patients over the past decade and have become one of major nosocomial pathogens. Clinicians carings for patients with VRE infections face severe constraints in the selection of treatment. Quinupristin/dalfopristin is active in vitro againt vancomycin-resistant E. faecium (VREF), with a MIC_(90) of 1.0㎍/mL. We studied the clinical efficacy and safety of this agent in the treatment of VREF infection. Patients were included if they had signs and symptoms of active infection including bacteremia, intra-abdominal infection, and wound infection. A total of 13 patients with VREF infection were enrolled. A favorable clinical response (cure or improvement) occured in 10 of 11 evaluable patients. The only adverse events related to quinupristin/dalfopristin were arthralgia and myalgia, which occurred in 2 of 13 patients. These results suggest that quinupristin/dalfopristin is effective and safe as treatment for VREF infections in critically ill patients with serious underlying conditions.

      • Mirizzi 증후군의 변형된 분류와 치료

        Purpose : The Mirizzi syndrome is relatively rare and preoperative diagnosis of this disease is difficult. In 1978, Morelli suggested the subclassification of the Mirizzi syndrome into acute or chronic form. We experienced 5 cases of acute form. We analysed clinical features, preoperative radiologic findings and operative findings of 18 cases including acute forms which were diagnosed as Mirizzi syndrome and should suggest the modified classification of Mirizzi syndrome for choice of appropriate treatment. Method : From January 1995 to December 1998, 18 cases, of which 8 cases were diagnosed at Soonchunhyang University Chunan Hospital, and 10 cases were reported in the Korean Journal were retrospectively analysed with regard to clinical features, preoperative radiologic findings and operative findings. According to the clinical features, whole cases were divided into type Ⅰ(acute form) and type Ⅱ(chronic form) and then each type of cases were subclassified according to preoperative radiologic findings and operative findings. Results : Of 18 cases there were 5 cases in type Ⅰ(27.8%), 13 cases in type Ⅱ(72.2%). Type Ⅱb was most common. Type Ⅰa cases were treated only with cholecystectomy. We applied cholecystectomy, T-tube choledochostomy and patch technique in type Ⅰb and thpe Ⅰc cases. Cholectystectomies including removal of gallstones and internal drainage procedures were done in type Ⅱ chronic forms. Conclusion : The acute form(Type Ⅰ) of Mirizzi syndrome was suggested by Morelli might be subclassified into typeⅠa,Ⅰb and Ⅰc following the presence of the necrotic defect in common hepatic duct. Through the modified classification of Mirizzi syndrome based on clinical feature, preoperative radiologic findings and operative findings, we can choice appropriate treatment.

        Genetic Characteristics and Phylogenetic Analysis of Influenza Type B Viruses Isolated from Nasopharyngeal Suction Samples of Korean Patients

        To investigate the genetic characteristics of human influenza type B viruses circulating in Chungbuk province, Korea, we tested 510 clinical samples of nasopharyngeal suction from pediatric patients diagnosed with respiratory illness between June 2007 and June 2008. Twelve out of thirty-six isolates were identified as type B influenza virus by RT-PCR and sequencing analysis. Interestingly, genetic characterization of type B viruses isolated in this study revealed that all type B influenza viruses were the Yamagata lineages, a vaccine strains of southern hemisphere during $2007{\sim}2008$, rather than the Victoria lineage of northern hemisphere during $2007{\sim}2008$. Furthermore, there were a total of twelve unique mutations (HA: H40Y, D/G230S, V252M and K272R and NA: P3H, P/T/S42Q, N59S) occurred in our type B isolates. These results suggest that relative high prevalence of type B viruses in Korea during $2007{\sim}2008$ season might be due to the wrong vaccine strains selection. Taken together, the results of this study demonstrate continuous evolutions of human type B viruses by antigenic drift and also highlight the need to closely monitoring of influenza viruses to aid the early detection of potentially pandemic strains as well as underscore the need for new therapeutics.

        Efficient Interleukin-21 Production by Optimization of Codon and Signal Peptide in Chinese Hamster Ovarian Cells

        Interleukin-21 is a common γ-chain cytokine that controls the immune responses of B cells, T cells, and natural killer cells. Targeting IL-21 to strengthen the immune system is promising for the development of vaccines as well as anti-infection and anti-tumor therapies. However, the practical application of IL-21 is limited by the high production cost. In this study, we improved IL-21 production by codon optimization and selection of appropriate signal peptide in CHO-K1 cells. Codon-optimized or non-optimized human IL-21 was stably transfected into CHO-K1 cells. IL-21 expression was 10-fold higher for codon-optimized than non-optimized IL-21. We fused five different signal peptides to codon-optimized mature IL-21 and evaluated their effect on IL-21 production. The best result (a 3-fold increase) was obtained using a signal peptide derived from human azurocidin. Furthermore, codon-optimized IL-21 containing the azurocidin signal peptide promoted IFN-γ secretion and STAT3 phosphorylation in NK-92 cells similar to codon-optimized IL-21 containing original signal peptide. Collectively, these results indicate that codon optimization and azurocidin signal peptides provide an efficient approach for the high-level production of IL-21 as a biopharmaceutical.

        Cullin 3/KCTD5 Promotes the Ubiqutination of Rho Guanine Nucleotide Dissociation Inhibitor 1 and Regulates Its Stability

        Rho guanine nucleotide dissociation inhibitor 1 (RhoGDI1) plays important roles in numerous cellular processes, including cell motility, adhesion, and proliferation, by regulating the activity of Rho GTPases. Its expression is altered in various human cancers and is associated with malignant progression. Here, we show that RhoGDI1 interacts with Cullin 3 (CUL3), a scaffold protein for E3 ubiquitin ligase complexes. Ectopic expression of CUL3 increases the ubiquitination of RhoGDI1. Furthermore, potassium channel tetramerization domain containing 5 (KCTD5) also binds to RhoGDI1 and increases its interaction with CUL3. Ectopic expression of KCTD5 increases the ubiquitination of RhoGDI1, whereas its knockdown by RNA interference has the opposite effect. Depletion of KCTD5 or expression of dominant-negative CUL3 (DN-CUL3) enhances the stability of RhoGDI1. Our findings reveal a previously unknown mechanism for controlling RhoGDI1 degradation that involves a CUL3/KCTD5 ubiquitin ligase complex.
