        Background : The reported incidence of early gastric cancer located in the upper portion of the stomach has been increasing with the recent advances in its diagnosis and screening. Recently, we have successfully performed laparoscopic assisted proximal gastrectomy and gastric tube reconstruction without pyloroplasty on three patients with early gastric carcinoma localized to the upper third of the stomach. We describe our modification of this procedure in this report. Method : After creating an surgical pneumoperitoneum, the stomach was mobilized using laparoscopic coagulating shears. Upper half of the greater curvature and three-quarters of the lesser curvature were then dissected along with regoinal D2 lymphadenectomy. This was followed by a 5 cm, longitudinal mini-laparotomy in the upper abdomen, and the construction of the exteriorized stomach with a gastric tube measuring 20 cm long and 4 cm wide. Reconstruction with an esophagogastrostomy was performed using a circular stapler. Result : No post-operative morbidity or mortality was observed in this small series of patients. The average operative time was 250 minutes (range 220-300 minutes), and the average blood loss was 150 ml (range 90-180 ml). The mean number of lymph nodes harvested during these laparoscopic proximal gastrectomies was 24 nodes (range 22-25 lymph nodes). The average postoperative hospital stays was 7.5 days (range 7-8 days). Conclusion : Our technique of laparoscopic assisted proximal gastrectomy and gastric tube reconstruction without phloroplasty offers a minimally invasive technique with the potential of impoving the post-operative quality of life patients with an early-stage proximal gastric cancer.

      • 柔道 밧다리 후리기의 kinematic 分析

        The purpose of the study was to analyze the Kinematics component of O-soto-gari in Judo. The subjects were 4 male college judo caliber players. They were filmed at 100f/sec. The analyzed Variables were angular velocity and angular accee ation of leg and angular change during O-soto-gari. The results were summarized as follow; 1. During Kuzushi-Tsukuri phase the stride and volocities were 1.093m and 1.497m/sec in each. 2. And during in the Tsukuri-Kake phase the maximum angular velocities in 0.2 seconds were 400 degree/sec in low leg and 350 degree/sec in upper leg. The maximum angular acceleration was -4000 degree/sec²in low leg and 4000 degree/sec²in upper leg. 3. During Kake phase maximum angular velocities were 680 degree/sec in low leg and 400 degree/sec in upper leg. And maximum angular acceleration was 9500 degree/sec² in low leg and 3000 degree/sec²in upper leg. 4. In order to analyze the Kinematic and Kinetic components of O-soto-gari accuretely, the study should be performed with 3-D and force plat-form system.

        In this study, chemical compositions of Godulbaegi(leaves and root) were investigated. As for free sugar of the leaves and roots of Godulbaegi, fructose, glucose and sucrose were detected. The total phenolic content was shown higher in the leaves than in the roots. As for amino acid of the leaves of Godulbaegi, 20 kinds amino acids were detected and among them, L-proline and L-glutamic acids were prominent, root of Godulbaegi, 19 kinds amino acids were detected and among them, L-glutamin, L-proline were prominent. As for fatty acid, linolenic acid was prominent in the leaves of Godulbaegi and linoleic acid was prominent in the roots of Godulbaegi. As for vitamin, only the ascorbic acid was detected in the leaves and roots of Godulbaegi and the rest was trace. As for minerals K, Ca, Mg and Na were prominent in the leaves and roots of Godulbaegi.

        To find anti-tumor components from korean Basidiomycetes, the mycelium of Coriolus versicolor Fr. was grown in jar-fermentor culture and a proteinbound polysaccharide was isolated. The experimental results obtained are summarized as follows: The korean yeast extract for the production of crude polysaccharide and cell by the cultivation of jar-fermentor was more efficient than of foreign production. The addition of 1.5% yeast extract to culture medium promoted crude polysaccharide production but pH-control had no effect on cell growth and crude polysaccharide production.

        Expression of the survivin-2B splice variant related to the progression of colorectal carcinoma

        Purpose: Recently, two alternatively spliced survivin variants, survivin-ΔEx3 and survivin-2B, were identified in a single copy of the survivin gene. It has been reported that the expressions of survivin splice variants significantly correlates with the clinical results in many types of human carcinoma. We investigated the transcription levels of survivin and its splice variants in human colorectal carcinomas, and analyzed correlations between survivin expression levels and clinicopathologic features. Methods: We used Western blot and real-time quantitative reverse transcription polymerase chain reaction (RT-PCR) to analyze the protein and mRNA expression levels of survivin variants in 51 colorectal carcinomas. The quantitative RT-PCR was performed using primer pairs specific for survivin and each of its splice variants, then normalized for the gene that encodes glyceraldehydes-3-phosphate dehydrogenase. Results: In Western blotting, the protein levels of survivin were higher in the tumor tissue than in normal tissue. The expression of survivin, survivin-2B and survivin-ΔEx3 mRNA was present in 96%, 64.7%, and 82.4% of the samples, respectively. When the pathologic parameters were compared, colorectal cancers of advanced pT stages showed significant decrease in survivin-2B mRNA expression by the quantitative RT-PCR (P < 0.001). Conclusion: The decreased expression of survivin-2B might be related to tumor progression in colorectal cancers. This finding indicates that alternatively spliced variants of survivin may be involved in refining the functions of survivin during tumor progression.

      • Immunoregulatory effects of allogeneic mixed chimerism induced by nonmyeloablative bone marrow transplantation on chronic inflammatory arthritis and autoimmunity in interleukin-1 receptor antagonist–deficient mice

        <B>Objective</B><P>To investigate the immunoregulatory effects of allogeneic mixed chimerism induced by T cell–depleted, nonmyeloablative bone marrow transplantation (BMT) on chronic inflammatory arthritis and autoimmunity in mice deficient in interleukin-1 receptor antagonist (IL-1Ra).</P><B>Methods</B><P>IL-1Ra<SUP>−/−</SUP> mice (H-2K<SUP>d</SUP>) were treated with antibody to asialoganglioside G<SUB>M1</SUB> (anti–natural killer cell), total body irradiation (500 cGy), and T cell–depleted, nonmyeloablative BMT derived from C57BL/6 mice (H-2K<SUP>b</SUP>). Engraftment and chimerism were evaluated in peripheral blood, lymph nodes, and spleen by multicolor flow cytometry. The severity of arthritis was evaluated by clinical scoring and histopathologic assessment. Levels of IgG1 and IgG2a subtypes of anti–type II collagen (anti-CII) antibodies were measured in serum samples. After T cells were stimulated with CII, ovalbumin, and phytohemagglutinin, T cell proliferative responses and levels of cytokine production (interferon-γ [IFNγ], tumor necrosis factor α [TNFα], interleukin-10 [IL-10], and IL-17) were assayed in culture supernatants.</P><B>Results</B><P>All IL-1Ra<SUP>−/−</SUP> mice receiving BMT showed marked improvement in arthritis within 3 weeks, as well as successful induction of mixed chimerism. These mice showed higher levels of IgG1, and lower levels of IgG2a anti-CII antibodies and weaker T cell proliferative responses than did mice in the control groups (either no treatment or conditioning alone without bone marrow rescue). In mixed chimeras, the levels of IFNγ, TNFα, and IL-17 produced from CII-stimulated T cells were significantly suppressed and IL-10 production was significantly higher as compared with controls.</P><B>Conclusion</B><P>The introduction of allogeneic mixed chimerism showed a strong immunoregulatory potential to correct established chronic inflammatory arthritis and autoimmunity originating from a dysregulated proinflammatory cytokine network.</P>

        Development of a Gd2O2S:Tb Phosphor Screen with an Anti-reflective Layer Fabricated Using Poly Chloro-para-xylylene to Improve Optical Properties in Radiography

        In this study, we developed a Gd2O2S:Tb screen with a parylene-C-based antireflective (AR) layer and evaluated its optical and X-ray response properties; in particular, we focused on developing an AR coating layer to improve transmission efficiency. The phosphor screen with the parylene-C-based AR layer can be fabricated via screen printing and chemical vapor deposition. In particular, experimental results obtained using the AR layer showed improvements of 1.47% in luminescence intensity, as well as a lower full width at half maximum of 9.40, compared to the case without using the AR layer. We observed that the roughness of the AR layer was higher than that of the phosphor screen; the image resolution improved owing to the narrow distribution of the secondary photons resulting from forward scattering attributable mainly to Mie scattering. These results indicate that parylene-C is suitable for developing AR coating materials to improve exposure efficiency while maintaining resolution in radiography systems. Therefore, we believe that the number of photons would be further increased if the AR layer were utilized in commercial products.

        염료감응형태양열 물질이 요오드화수은의 전기적 특성에 미치는 영향에 관한 연구

        요오드화수은은 우수한 엑스선 민감도 특성을 가진 광도전체로 비정질 셀레늄을 대체할 수 있는 후보물 질로 많은 연구가 진행되고 있지만 높은 누설전류로 인해 상용화에 많은 한계점을 나타내고 있다. 본 연구 에서는 요오드화수은의 높은 누설전류를 저감하기 위해 요오드화수은에 비해 입자가 작은 이산화규소 및이산화티타늄을 물리적으로 혼합하여 단위시편을 제작하였으며 제작된 단위시편의 전기적 특성을 비교·분 석하였다. 그 결과 혼합한 두 물질 모두 요오드화수은의 높은 누설전류를 저감하는데 효과가 있었으며 요오드화수은-이산화티타늄 혼합물에서는 방사선 민감도 특성이 상당히 높아짐을 확인하였다. As a photoconductive material with a high X-ray sensitivity, many researches about mercury iodide has been carried out to substitute for amorphous selenium. However, it has many limitations in commercialization because of the high leakage current. In this study, we fabricated HgI 2 unit-cells with mixed silicon oxide(SiO 2 ) and titanium oxide(TiO 2 ) to reduce a high leakage current and we evaluated an electrical properties of the fabricated unit-cells. As a result, we confirmed that both mixtures were effective in reduing the leakage current of the HgI 2 and x-ray sensitivity were significantly increased in fabricated HgI 2 -TiO 2 unit-cell.
