        Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

      • Large-Scale Synthesis of Uniform and Extremely Small-Sized Iron Oxide Nanoparticles for High-Resolution <i>T</i><sub>1</sub> Magnetic Resonance Imaging Contrast Agents

        <P>Uniform and extremely small-sized iron oxide nanoparticles (ESIONs) of < 4 nm were synthesized via the thermal decomposition of iron–oleate complex in the presence of oleyl alcohol. Oleyl alcohol lowered the reaction temperature by reducing iron–oleate complex, resulting in the production of small-sized nanoparticles. XRD pattern of 3 nm-sized nanoparticles revealed maghemite crystal structure. These nanoparticles exhibited very low magnetization derived from the spin-canting effect. The hydrophobic nanoparticles can be easily transformed to water-dispersible and biocompatible nanoparticles by capping with the poly(ethylene glycol)-derivatized phosphine oxide (PO-PEG) ligands. Toxic response was not observed with Fe concentration up to 100 μg/mL in MTT cell proliferation assay of POPEG-capped 3 nm-sized iron oxide nanoparticles. The 3 nm-sized nanoparticles exhibited a high <I>r</I><SUB>1</SUB> relaxivity of 4.78 mM<SUP>–1</SUP> s<SUP>–1</SUP> and low <I>r</I><SUB>2</SUB>/<I>r</I><SUB>1</SUB> ratio of 6.12, demonstrating that ESIONs can be efficient <I>T</I><SUB>1</SUB> contrast agents. The high <I>r</I><SUB>1</SUB> relaxivities of ESIONs can be attributed to the large number of surface Fe<SUP>3+</SUP> ions with 5 unpaired valence electrons. In the in vivo<I> T</I><SUB>1</SUB>-weighted magnetic resonance imaging (MRI), ESIONs showed longer circulation time than the clinically used gadolinium complex-based contrast agent, enabling high-resolution imaging. High-resolution blood pool MR imaging using ESIONs enabled clear observation of various blood vessels with sizes down to 0.2 mm.       • The association of disease activity, pro-inflammatory cytokines, and neurotrophic factors with depression in patients with rheumatoid arthritis

        <P><B>Abstract</B></P> <P>Inflammation and trophic factors (brain-derived neurotrophic factor [BDNF], vascular endothelial growth factor, glial cell line-derived neurotrophic factor, and insulin-like growth factor-1) are associated with depression in the general population. Rheumatoid arthritis (RA) is a chronic representative inflammatory autoimmune disease; however, the association of disease activity, pro-inflammatory cytokines, and neurotrophic factors with depression has not been sufficiently investigated. Therefore, we determined the prevalence of depression and risk factors for depression and deterioration of depressive symptoms in RA patients. In addition, we analyzed the association between disease activity, pro-inflammatory cytokines, trophic factors, and depression in RA (N = 474). Demographic and laboratory data were examined, and routine assessment of patient index data 3 (RAPID 3) and disease activity score 28-joint count C-reactive protein (DAS 28-CRP) was performed to assess disease activity of RA. Depression was measured using the Korean version of the Beck Depression Inventory-second edition (K-BDI II). A K-BDI score ≥18 was considered the cut-off for depression in accordance with a previous validation study. The serum level of pro-inflammatory cytokines and neurotrophic factors was assessed by enzyme-linked immune sorbent assay. The prevalence of depression was 32.4% in patients with RA. The severity of disease activity of RA (RAPID 3 score [OR 2.34; 95% confidence interval, CI 1.22–4.51], DAS 28-CRP [≥3.2] [OR 1.60, 95% CI 1.01–2.53]) and severity of fatigue (OR 1.26 95% CI 1.15–1.38) were associated with depression and deterioration of depressive symptoms in the multivariate analysis. Among the components of RAPID 3 and DAS 28-CRP, patient assessment for global health and abilities for daily performance were more related to depression. The level of pro-inflammatory cytokines (IL-1β, IL-6, TNF-alpha) was not related to depression. The level of BDNF was significantly lower in RA patients with depression and was negatively correlated with K-BDI II score. Depression was related with the level of fatigue, low expression of BDNF, and high RA disease activity, which was associated with impaired ability to perform activities of daily life. Strict control of fatigue and disease activity to improve one’s capacity to perform daily life activities would be important to regulate depression.         Effect of abatacept versus conventional synthetic disease modifying anti-rheumatic drugs on rheumatoid arthritis-associated interstitial lung disease

        Background/Aims: To compare the effects of abatacept and conventional synthetic disease modifying anti-rheumatic drugs (csDMARDs) on the progression and development of rheumatoid arthritis-associated interstitial lung disease (RA-ILD). Methods: This multi-center retrospective study included RA patients receiving abatacept or csDMARDs who underwent at least two pulmonary function tests and/or chest high-resolution computed tomography (HRCT). We compared the following outcomes between the groups: progression of RA-ILD, development of new ILD in RA patients without ILD at baseline, 28-joint Disease Activity Score with the erythrocyte sedimentation rate (DAS28-ESR), and safety. Longitudinal changes were compared between the groups by using a generalized estimating equation. Results: The study included 123 patients who were treated with abatacept (n = 59) or csDMARDs (n = 64). Nineteen (32.2%) and 38 (59.4%) patients treated with abatacept and csDMARDs, respectively, presented with RA-ILD at baseline. Newly developed ILD occurred in one patient receiving triple csDMARDs for 32 months. Among patients with RA-ILD at baseline, ILD progressed in 21.1% of cases treated with abatacept and 34.2% of cases treated with csDMARDs during a median 21-month follow-up. Longitudinal changes in forced vital capacity and diffusing capacity for carbon monoxide were comparable between the two groups. However, the abatacept group showed a more significant decrease in DAS28-ESR and glucocorticoid doses than csDMARDs group during the follow-up. The safety of both regimens was comparable. Conclusions: Abatacept and csDMARDs showed comparable effects on the development and stabilization of RA-ILD. Nevertheless, compared to csDMARDs, abatacept demonstrated a significant improvement in disease activity and led to reduced glucocorticoid use.

        Genetic Variation in Wild and Cultured Populations of the Sea Squirt Halocynthia roretzi Inferred from Microsatellite DNA Analysis

        We used nine microsatellite DNA markers to estimate genetic variation among wild and cultured populations of the sea squirt Halocynthia roretzi. The loci were polymorphic, with 6-32 alleles, and allelic richness ranged from 6.0 to 26.1 in each population. The wild and the cultured populations had similar mean heterozygosities ($H_O$ and $H_E$), allele numbers, and allelic richness. One cultured population with softness syndrome had a lower mean in the observed heterozygosity ($H_O$ = 0.57) and higher mean inbreeding coefficient ($F_{IS}$ = 0.261) than any other populations. This suggests that the loss of genetic variation in the diseased population might be due to increased inbreeding. A neighbor-joining tree and pairwise population estimates of $F_{ST}$ showed moderate genetic differentiation between the wild and the cultured populations. Additionally, the softness syndrome population was genetically divergent from wild populations, but it was genetically close to the cultured populations.

        근대전환기 한국 유교의 성찰과 전망 - 우리철학 총서 집필내용을 중심으로 -

        본 논고는 조선대학교 우리철학연구소의 총서사업인 “근대전환기 한국철학의 도전과 응전”의 유교분야 중간 연구 성과를 정리한 것이다. 유교분야 연구는 ‘리(理)의 철학’, ‘심(心)의 철학’, ‘기(氣)의 철학’, ‘실(實)의 철학’으로 세분화하여 각각의 이론적 계승과 변용, 실천적 대응을 고찰하였다. 리(理)의 철학은 전통 성리학을 중심으로 선행연구에서 미처 다루지 못했던 후기 호락논변(湖洛論辨)의 전개와 변용을 심도있게 검토하였다. 호락논변은 인간과 동물의 성(性)에 대한 이견에서 출발하였으나 후기로 갈수록 양자의 종합을 지향하며 시대적 변화를 반영한 새로운 학설로 이어졌다. 심(心)의 철학은 조선의 대표적인 양명학자인 하곡학파의 후기 사상과 실천을 중점적으로 살펴보았다. 후기 하곡학파의 사상은 개인수양을 넘어 사회비판론을 포함한 일진무가(一眞無假)의 실학(實學)으로 확장되어 1930년대 조선학(朝鮮學)운동을 주도하였다. 기(氣)의 철학에서는 양명학과 노장학을 매개로 서학을 수용하여 현실에 접목시킨 홍대용, 이규경, 최한기 등의 기 철학을 고찰하였다. 이들은 서양의 신(神)관념을 비판하고 전통적인 음양오행론의 한계를 지적하면서 주체적이고 독창적인 기 철학을 수립했다. 실(實)의 철학에서는 현재 통용되는 ‘실학(實學)’ 개념의 형성과정을 고찰하고, 그 보다 상위 개념으로써 실의 철학을 분석하였다. ‘실학’이란 말은 『중용장구(中庸章句)ㆍ서(序)』에서 처음 언급되었고, 현재까지 4가지의 범주로 발전해 왔다. 본 논고에서는 그 중 근대시기 형성된 ‘실학’ 개념을 분석하였다. 이를 통해 근대이후 형성된 ‘실학’ 개념 속에 있는 맹목적 서구 수용관을 비판하고, 동시에 국가와 민족을 중시하는 새로운 실의 철학의 등장도 함께 고찰하였다. 본 논고는 근대전환기 전통 유교철학의 논리적 심화와 현실대응을 통섭적으로 고찰하고자 하였으며, 후속 연구를 진행 중이다. 당대 유교지식인들의 철학적 고민과 대응은 현재 우리들의 삶과도 밀접한 관계가 있다. 과거 전통의 몰락과 극복, 그리고 재구성의 문제는 현재까지도 영향을 미치며, 새로운 우리 철학을 세워나가는데 중요한 선례를 제공하기 때문이다. This thesis is a summary of the mid-results of the interim study, "Woori Philosophy, How do we study it?" of the Confucianism field of Woori Philosophy Institute, Chosun University. In the field of Confucianism, we studied the theoretical assumption, the change, and the practical question of each, by subdividing into ① the philosophy of Li(理), ② the philosophy of Sin(心), ③ the philosophy of Gi(氣), and ④ the philosophy of Sil(實). The philosophy of Li studied deeply about the development and change of the late Ho-Rak theory(湖洛論辨) which could not be explained in previous studies. The theory was originally born out of difference in perspectives on human and animal, but in the late period, it became a new theory that reflects the changes of the two, focusing on the synthesis of the two schools. We will continue to explore the modern values contained in the process of the development of the Ho-Rak theory. The philosophy of Sim examined the later thought and practice of the Hagok school(霞谷學派) of the Yangming school(陽明學) of Joseon. The late Hogok school expanded the realist school(實學) included social criticism, and led the movement of Chosun Studies(朝鮮學) in the 1930s. In addition to the Hagok school, we also search for and sort out the modern Yangming scholars who have not been known. The philosophy of Gi studied philosophy such as Hong Dae-yong(洪大容), Lee Gyu-Gyeong(李圭景), and Choi Han-gi(崔漢綺), which received and accepted the West School(西學) which was valued as a heretic in the traditional philosophy. They denied the idea of worship in the West and the theory of traditional pentagram, and instead established a fundamental and philosophy of Gi. In this study, we will proceed with a deeper examination of the philosophy of Gi and its contemporary application. In the philosophy of Sil, we first clarified that there is a difference between the present Silhak(實學) and the original Silhak(實學). Silhak have come out in Moderation(中庸) and do not simply mean science technology. In this study, we criticized the current "practice" or “reality” concept known only in science and technology and explained the development of philosophy of Sil, which criticizing blind Western acceptance in modern times and giving priority to the nation and the people. we will examine the changes in the concept of Silhak and the view of the nation after the 1920s. The concept and development of each field are different, but it is the same as that which emphasized Korea's subjective awakening and fulfillment in the modern period through the inheritance and change of the traditional philosophy. And the philosophical works to deal with the actual change is very effective in modern times.

        허혈성뇌졸중과 출혈성뇌졸중에서 Apolipoprotein E 유전자의 다형성

        Apolipoprotein E (APOE) 대립유전자의 분포는 동맥경화증, 심장동맥질환 및 목동맥협착증 환자에서 일반 인구군에 비해 유의한 차이를 나타내는 것으로 알려져 있지만, 뇌졸중에서의 역할에 대해서는 명확하게 알려져 있지 않다. 이 연구는 뇌경색과 뇌출혈에서 APOE 유전자의 다형성을 조사하고 APOE 대립유전자와 뇌졸중의 발생 및 유형과의 상관관계를 알아 보기 위해 시행하였다. 113명의 뇌졸중 환자군과 111명의 대조군을 대상으로 APOE 대립유전자의 분포 및 유전자형을 조사하고 뇌혈관질환과 이들 사이의 상관관계를 분석하였다. 대상군과 대조군, 그리고 뇌출혈군과 뇌경색군 사이에 APOE 대립유전자와 APOE 유전자형의 분포는 유의한 차이가 없 었다. 따라서 뇌졸중 환자군에서 APOE의 다형성이 뇌졸중의 발생과 유형에 영향을 끼치지 않는 것으로 추정된다. 이는 지역적 차이와 표본 추출 상태 및 표본 수의 영향을 고려해야 함이므로 뇌졸중의 유전적 병태 생리를 이해하기 위해서 더 많은 표본을 대상으로 한 연구가 필요할 것이다. There is an evidence for a role of apolipoprotein E (APOE) in atherosclerosis, coronary heart disease and carotid artery stenosis. Morbidity of carotid artery atherosclerosis is higher in persons carrying an ε4 allele, but the association of cerebrovascular disease and apoE genotype is controversial. We studied the association between APOE genotype and allele (ε2, ε3, ε4) frequency and stroke. We evaluated APOE genotype in 133 first-ever stroke patients and 111 healthy controls. We also estimated the risk factors of stroke such as hypertension, diabetes and lipid profiles in both groups. APOE genotypes were analyzed by polymerase chain reaction. APOE genotypes and allele distributions were not different in patients and controls. There is also no difference of APOE allele frequencies between ischemic small artery occlusive disease and hypertensive subcortical intracerebral hemorrhage. We concluded that the APOE-ε4 allele is not associated with stoke including ischemic and hemorrhagic stroke.
