        모 제약회사 근로자들의 직업성 천식 유병률과 약제 분말 노출수준

        목적: 제약회사에서 직업상 천식의 유병 규모를 파악하고 작업공정에 대한 작업환경 측정을 통해 제약회사의 약제 분말에 대한 노출수준을 평가하고자 하였다. 방법: 제약회사에서 약제 분말에 노출되는 근로자를 대상으로 천식을 선별하기 위하여 설문조사 및 진찰, 혈액검사, 폐기능 검사 그리고 피부단자시험을 실시하였고, 각 부서별 공정에 따라서 총분진과 항생제 또는 효소제 분말에 대한 작업환경 측정을 실시하였다. 직업상 천식이 의심되는 근로자에 대하여 기관지과민성에 대한 검사(메타콜린을 이용한 비특이 기관지유발시험)와 약제 분말 기관지유발시험을 실시하여 확진하였다. 결과: 설문조사 및 진찰에서 작업 관련 천식 증상이 있는 근로자는 11명(34.4%)이었고, 약제 분말을 이용한 피부단자시험에서 1+ 이상 반응을 보인 근로자 10명(31.2%)이었다. 비특이 기관지유발시험 결과 8명(25.0%)이 양성(PC20 ≤16 mg/ml)이었다. 약제 분말 기관지유발시험을 통해 직업상 천식으로 확진된 근로자는 5명(15.6%)이었다. 작업환경 측정에서 원료를 충진하거나 투입하는 작업 그리고 정립작업에서 약제 분말에 대한 노출이 많았다. 결론: 제약회사에서 직업상 천식은 주로 제조공정 중 약제 분말에 노출된 근로자에서 발생하여, 증상 및 직업성 천식의 유병률이 높았다. 따라서 원료를 충진 또는 투입하는 작업과 정립작업 등 제조과적에서 약제 분말 노출을 줄이기 위한 세심한 주의가 필요하여, 약제 분말을 취급하는 근로자에 대해서는 천식 증상 조사뿐 아니라 정기적으로 피부단자시험, 비특이 기관지유발시험 등을 실시하여야 할 것이다. Objectives: After the investigation of one worker with occupational asthma, we surveyed the preva- lence of occupational asthma and the exposure level of pharmaceutical dust of 32 workers in a pharmaceutical company. Methods: Thirty-two of the 90 employees participated in the survey which consisted of questionnaire, blood sampling, spirometry and skin prick tests with 8 common allergens as well as 9 antibiotics and 2 enzymes. Various indices of the working environment were also measured. Subjects who had a symptom suggestive of work-related asthma or positive skin prick test were further investigated by PC20 methacholine. Nine subjects who had a PC20 result of 16 mg/ml or less (n=7) or had work-related symptoms and positive skin prick test (n=2) were referred to undergo a specific bronchial provocation test to phar- maceutical dust in an academic allergic disease center. Results: Eleven of 32 workers (34.4%) had a work-related symptom suggestive of occupational asthma. Ten (31.2%) showed positive skin prick test. 8 (25.0%) had a PC20 result of 16 mg/ml or less (indicative of significant bronchial hyperresponsiveness), and 5 (15.6%) had a positive result on the specific bronchial provocation test. Exposure levels of stuffing, input of raw materials and screening process were relatively high. Conclusions: This survey showed that pharmaceutical workers have an increased risk of occupational asthma. Although pharmaceutical factories maintain a relatively good working environment, careful control of respiratory tract exposures, especially during stuffing, input or raw materials and screening process, is important to prevent occupational asthma. Pharmaceutical workers need to undergo regular skin prick and methacholine bronchial provocation tests, as well as asthmatic symptom survey, to ensure the early detection and prevention of occupational asthma.

      • 마이크로파를 이용한 터널현상 연구

        We observed the tunneling effect, one of the quantum mechanical phenomena. by using some experimental equipment and materials. Microwaves were directed through two prism-shaped mediums which consisted of wood. paraffin. plastic, and basalt. An incident microwaves with 32mm wavelength was directed into the media through an airgap. As a result, the transmission rate was determined by the decay coefficient of each medium material. The more the refractive index grew, the more the decay coefficient increased. As the airgap grew, the transmission rate exponentially decreased in all the materials. It was observed that as the decay coefficient increased the transmission rate decreased in each of wood, paraffin. plastic and basalt medium materials.

        Whirl Fire와 Pool Fire의 화염 거동에 관한 연구

        4-panel of Im height and 45cm width were fixed on the 40cmx40cm bottom plate and the opening of the panel comer was 5cm. Diameter of stainless vessel is 10em and its height is 2cm and it located at the center of the bottom plate. 78mL liquid fuel was filled in the vessel and its depth was lcm. Flame temperature was measured with K type thermocouple, and radiation heat of flame was measured with heat flux meter. Flame height and its behavior was visualized with video camera. and mass burning rate was measured by fuel combustion time. According to the development of fire, flame swirling was begin. From the experiment the mass burning rate was larger and the height of flame was higher than the usual pool fire flame. Flame temperature and heat flux also increased far more than the pool fire. Consequently the swirling air flow through the openings between the panel and thermal buoyance contribute to increase of heat release rate, flame length and mass burning rate.

        만화 과제를 이용한 정신분열병 환자의 사회인지 능력의 측정

        Objectives : This study is to investigate the hypothesis that schizophrenic patients have difficulty in inferring mental state of others. Methods : Twenty schizophrenic patients and twenty normal subjects were administered to the Cartoon Task required inferring the character's intention in a given context. Results : The schizophrenic patients showed a statistically significant impairment in the Cartoon Task. Both groups chose the card depicting a frequent everyday action significantly more rather than the similar card to the last picture in each story for wrong answers. Conclusion : These preliminary results suggest that schizophrenia is associated with a specific deficit of the cognitive ability referred to as social cognition, and this deficit can be detected not only in the acute phase as found in previous research studies, but also in remission. Also when subjects fail to appreciate what is going on in the minds of other people, they seem to rely on a socially familiar experience.

        폐부종과 심정지가 발생한 페노바비탈 중독환자

        Phenobarbital is a long-acting barbiturate causing generalized depression of neuronal activity in the brain. Its effect is primarily achieved through enhanced GABA-mediated synaptic inhibition. Its use as an antiepileptic agent was first described in 1912. Before the introduction of phenytoin, phenobarbital is used as sedative-hypnotics. It is used for the treatment of epilepsy and status epilepticus. All barbiturates, including phenobarbital, have a high potential for abuse. They were frequently used for suicide attempts in the past, but they have in large part been replaced by benzodiazepines. The onset of symptoms depends on the drug and the route of administration. Mild to moderate barbiturate intoxication resembles ethanol inebriation with slurred speech, ataxia, and lethargy. Severe acute barbiturate intoxication is life threatening. Early deaths are generally cardiovascular-related. Hypotension, shock, pulmonary edema, and cardiac arrest that occurs with large doses are caused by depression of central sympathetic tone and as well as by direct depression of cardiac contractility. The potentially fatal oral dose of phenobarbital is 6-10g. We describe an 23-year-old woman with pulmonary edema, and cardiac arrest ingestion of 18grams of phenobarbital. She was completely recovered by successful cardiopulmonary resuscitation and homoperfusion. We report a case with literature review.

      • Fusobacterium nucleatum의 유황화합물 생성에 영향을 미치는 인자

        Fusobacterium nucleatum, one of the bacteria causing halitosis, produces the volatile sulfur compounds (VSC) such as H2S in the media containing sulfur components, and forms FeS by binding with iron component. The various factors of oral cavity affect the concentration of sulfur compounds produced by Fusobacterium nucleatum. In this study, the effect of nutrients and pH on the production of sulfur compounds by Fusobacterium nucleatum was studied with the following results; 1. The optical density of broth was increased to 0.817±0.032 and 1.297±0.024 by adding 1.0% sodium thiosulfate and 0.05% L-cysteine hydrochloride in the media, respectively. 2. Though the optical density of broth was 0.799 ± 0.032 by adding volatile sulfur compounds (VSC) only in the media, it was increased to 1.775±0.003 and 1.648±0.022 by adding xylitol combined with glucose and fructose, respectively. 3. The concentration of VSC was above 20,000 ppb in the media above pH 5.5. The optical density of broth was still high in the media with L-cysteine hydrochloride of higher concentration, being low in the media of lower pH. 4. The concentration of VSC was high when there was distilled water or saline solution on the media, and their amount was small. These results suggest that the production of sulfur compounds by Fusobacterium nucleatum is inhibited by xylitol and acid.

        Chlorhexidine 처리가 상아질 접착제의 미세인장결합강도에 미치는 영향

        본 연구는 상아질 혼성층의 교원섬유를 가수분해하는 효소인 MMPs (Matrix metalloproteinses) 억제제로 알려진 chlorhexidine (CHX)을 적용 후 결합강도를 측정하였으며, 이를 각각 열순환 처리 후 결합강도를 측정하였다. 또한 주사전자현미경으로 접착계면메서의 파괴 양상을 비교 분석하였다. 우식이 없는 발거한 32 개의 제 3 대구치의 교합면 상아질을 노출시키고 GI그룹에서는 dentin conditioner를 처리 후 2% chlorhexidine을 적용시키고, 산부식 접착제 그룹에서는 인산 산부식을 시행하고 2 % chlorhexidine을 적용 후 3단계 산부식형 상아질 접착제 (Scotchbond Multipurpose, SM), 2단계 산부식형 상아질 접착제 (Single Bond, SB)를 도포하고, 자가부식 접착제 그룹에서는 2 % chlorhexidine 적용 후 자가부식 상아질 접착제 (Clearfil Tri-S, TS)를 도포한다. 이후 복합 레진 (Z-250)과 GI (Fuji-II LC)를 충전한 시편을 1 ㎟의 단면을 갖는 beam으로 제작하여 열순환 하지 않거나, 10,000 회 열순환 (5 ~ 55 ℃)하였다. Universal testing machine (EZ-test: Shimadzu, Japan)에서 cross head speed 1 mm/min로 인장력을 가하여 , 미세인장결합강도를 측정하였다. 실험 결과는 유의수준 0.05 1evel에서 two-way ANOVA를 이용하여 통계분석하였다. 그 후 파절된 시편의 파괴 양상을 현미경 (SEM)으로 관찰하여 다음과 같은 갈론을 얻었다; 1. 2 % CHX을 적용한 모든 실험군에서 상아질과의 미세인장결합강도가 증가하였고, 열순환은 상아질과의 미세인장 결합강도를 감소시켰다 (P>0.05). 2. CHX 적용 후 열순환 한 군은 CHX을 적용하지 않고 열순환한 군에 비하여, 상아질과의 미세인장결합강도가 높았으며 , 특히 GI와 TS군에서 유의한 차이를 나타내었다 (P<0.05). 3. 파괴 양상 분석 결과, 혼성층에서의 접착성 파괴를 보이며, CHX을 적용하면 혼성층 기저부에서 상부로 파괴 부위가 옮겨가는 양상을 나타내었다. 이상의 연구 결과를 토대로, MMPs 억제제인 2 % CHX은 글래스 아이오노머 시멘트와 상아질 접착제의 초기 미세인장결합강도에는 영향을 미치지 않으며 ,CHX 적용이 접착 내구성을 유지하는데 도움이 되었다. The purpose of this study was to evaluate the effect of chlorhexidine (CHX) on microtensile bond strength (μTBS) of dentin bonding systems. Dentin collagenolytic and gelatinolytic activities can be suppressed by protease inhibitors. indicating that MMPs (Matrix metalloproteinases) inhibition could be beneficial in the preservation of hybrid layers. Chlorhexidine (CHX) is known as an inhibitor of MMPs activity in vitro. The experiment was proceeded as follows : At first, flat occlusal surfaces were prepared on mid-coronal dentin of extracted third molars. GI(Glass lonomer) group was treated with dentin conditioner, and then, applied with 2 % CHX. Both SM (Scotchbond Multipurpose) and SB (Single Bond) group were applied with CHX after acid-etched with 37% phosphoric acid. TS (Clearfil Tri-S) group was applied with CHX, and then, with adhesives. Hybrid composite Z-250 and resin-modified glass ionomer Fuji-II LC was built up on experimental dentin surfaces. Half of them were subjected to 10,000 thermocycle, while the others were tested immediately. With the resulting data, statistically two-way ANOVA was performed to assess the μTBS before and after thermocycling and the effect of CHX. All statistical tests were carried out at the 95 % level of confidence. The failure mode of the testing samples was observed under a scanning electron microscopy (SEM). Within limited results, the results of this study were as follows; 1. In all experimental groups applied with 2 % chlorhexidine, the microtensile bond strength increased, and thermocycling decreased the microtensile bond strength (P > 0.05). 2. Compared to the thermocycling groups without chlorhexidine, those with both thermocycling and chlorhexidine showed higher microtensile bond strength, and there was significant difference especially in GI and TS groups.

        생검펀치를 이용한 점액낭종의 절제

        점액낭종은 소타액선 분비관의 폐쇄나 파열로 타액이 저류되어 발생하는 연조직 부종이고, 비교적 높은 빈도로 발생하는 구강내 가성 낭종으로 스스로 파열되어 치유되기도 하지만 대개는 만성적으로 진행되어 몇 주에서 몇 달간 지속되기 때문에 치료가 필요하다. 점액 낭종의 발생시, 주변에 이환된 소타액선을 포함하여 제거하는 완전절제술 외에 재발율을 낮추기 위한 스테로이드의 국소적 주입, 냉동동결요법(cryotherapy), 미세조대술(micromarsupialization) 등의 여러 가지 치료방법이 제시되고 있다. 완전절제술의 한 방법으로 생검펀치(biopsy punch)를 이용할 수 있는데, 조작이 간편하고 출혈이 적으며 술 후 처치가 거의 필요없다는 장점이 있다. 이 증례는 점액낭종이 있고 행동 조절이 힘든 소아에서 생검펀치를 사용하여 점액낭종을 완전히 제거한 경우로, 양호한 결과를 얻었다. Mucocele is a clinical term of the localized superficial mucosal swelling caused by salivary retention after the obstruction or the rupture of minor salivary ducts. Various treatment methods have been suggested to lower its recurrence rate, including complete excision of lesion Biopsy punch can be used easily and simply for complete excision of mucocele. It has several advantages, such as convenience in use, low bleeding tendency, and nearly no requirements for post-operative management. In this case, biopsy punch was used for the removal of mucocele in an uncooperative child, which enable fast, simple and safe procedure, with a good result.

      • 光多變色 초파리 迷路集團의 性的 選擇利益에 관한 遺博的 行動

        光多變色 迷路集團裝置를 사용하여 4가지 色光에 대한 초파리의 性的 選擇利益에 따른 光鮮好性 差異의 形質을 分析하고자 實驗하였다. 光多變色 迷路集團裝置 속에 野生型 초파리와 突然變異型 黃色體-小翅 초파리를 各各 50쌍씩 즉, 암·수 各各 25%씩을 混入하여 그후 異型接合體 野生型 암컷의 交叉에 의해서 出現된 突然變異型 黃色體 초파리와 小翅 초파리를 同時에 觀察하였던 바, 各 色光 房 초파리 集團마다 差異는 있었으나 모든 경우에서 野生型 초파리의 選擇利益率이 增加하고 突然變異型 초파리의 選擇利益率은 減少하는 傾向이었다. 各 色光 房 野生型 초파리의 +遺傳子를 운반한 梁色體의 性的 選擇利益率은 25世代까지 全體 平均 頻度가 黃色光 房에서는 64.20%, 赤色光 房에서는 64.36%, 靑色光 房에서는 65.42 %, 暗色光 房에서는 61.74%로 나타났다. 이러한 實驗結果는 陽性 走光性 步行行動을 나타낸 靑色光 房 초파리 集團의 性的 選擇利益率(65.42%) 의 效率이 가장 강했음을 보여 주었고 반면에 陰城 走光性 步行行動을 나타낸 暗色光 房의 초파리 集團은 그 效率(61.74%) 이 比較的 약한 것으로 觀察되었다. 한편 野生型과 突然變異型 모든 초파리가 各 色光 房의 光鮮好性에 따른 步行行動에 의한 發生率은 各各 25%로 시작했던 實驗이 25世代까지 全體平均 發生率은 黃色光 房, 30.41%; 赤色光 房, 23.24%; 靑色光 房, 41.96%; 暗色光 房, 4.38%로 算定되었다. 이런 實驗結果를 中立値 頻度 25%로 分析하여 보면 黃色光 房과 靑色光 房의 초파리 發生率은 各各 5.41%와 16.96%의 陽性 遺傳率을 보였고 赤色光 房과 暗色光 房에서는 各各 -1.76% 와 -11.62%의 陰城 遺傳率로 觀察되었다. 이런 現象은 모든 초파리가 靑色光을 가장 鮮好하고 다음으로 黃色光, 赤色光, 暗色光 순으로 鮮好한다는 것을 보여준 것으로 思料된다. A study on the difference of photo-preference subjected to sexually selective advantage of Drosophila melanogaster in four light chambers was carried out. Each 25% of the wild type and the visible mutants(the yellow bodied-miniature winged) of male and female flies put into the photo-varicolored labyrinth population apparatus. The yellow bodied and the miniature winged mutants were observed by crossing over in the wild type heterozygous female flies. In all cases the seletive advantage of the wild type increased and that of the mutants decreased. The sexually selective advantages of +gene bearing chromosome of the wild type in each light chamber were observed as 64.20% in the yellow light chamber for the whole average frequency through initial to twenty-five generations, as 64.36% in the red light chamber, as 65.42% in the blue light chamber and as 61.74% in the dark light chamber respectively. These results showed that the efficiency of the sexually selective advantage(65.42%) of the blue chamber flies group was appeared high in the walking behavior of the positive phototaxis but to the contrary the flies group in the dark chamber showing negative phototaxis was observed as comparably weak(61.74%). On the other hand, the developmental ratio of all the wild type and the mutants by their walking behavior subjected to photo-preference of each chamber was computed to be of 30.41% in entire average through initial to twenty-five generations in the yellow chamber, 23.24% in the red chamber, 41.96% in the blue chamber, and 4.30% in the dark chamber respectively. As such results were analyzed by the neutral freuency(25%), the developmental ratio of files of the yellow and the blue chambers showed 5.41% and 16.96% respectively as positive realized heritability and in the red and the dark chamber it was observed as -1.76% and -11.62% respectively as negative realized heritability. All these results indicated that all the flies prefered the blue, yellow, red and dark light in order.
