        노인정신장애 평가척도(Psychogeriatric Assessment Scale)의 한국판 표준화 연구

        연구목적 : 본 연구는 치매와 우울증을 다차원적으로 평가할 수 있는 도구인 Psychogeriatric Assessment Scale(PAS)의 한국판(PAS-K)을 개발하기 위한 것이다. 방 법 : 번역, 역번역, 그리고 세차례의 예비조사를 통하여 PAS-K를 제작하였다. 60세 이상의 노인 291명(임상환자군 : 58명, 지역사회노인군 : 168명, 수용시설 거주자 : 67명)과 그들의 정보제공자들을 대상으로 PAS-K, Geriatric Depression Scale(GDS), Korean version of Mini-Mental State Examination(MMSE-K), Hamilton Rating Scale for Depression(HRSD), 그리고 Hachinski Ischemic Score 를 적용하고, Diagnostic Interview chedule(DIS-Ⅲ-R)을 사용하여 치매와 우울증을 진단하였다. PAS-K의 신뢰도와 타당도를 평가하고, 예민도와 특이도, Kappa값, 그리고 ROC커브 분석법을 이용하여 PAS-K의 6개의 소척도들(피검자면접 : 인지기능 장애척도, 우울척도, 뇌졸중척도, 정보제공자면접 : 인지기능 저하척도, 행동변화척도, 뇌졸중척도)의 최적 절단점을 구하였다. 결 과 : PAS-K는 높은 내적 일관성을 보였고, 검사자간 신뢰도도 만족할 만한 수준이었다. PAS-K의 소척도들을 다른 표준화된 검사도구들과 비교하였을 때 인지기능 장애척도와 인지기능 저하척도는 MMSE-K와, 우울척도는 GDS 및 HRSD와 유의한 상관관계를 보였으며, 뇌졸중척도는 Hachinski Ischemic Score와 통계적으로 의미있는 상관관계를 보였다. 최적절단점은 인지기능 장애척도 10점, 인지기능 저하척도 3점, 우울척도 5점, 피검자면접과 정보제공자 뇌졸중척도 모두 1점, 그리고 행동변화척도 2점으로 추정하였다. 결 론 : 본 연구를 통해 PAS-K의 신뢰도와 타당도가 검증되었고, 노인정신의학 역학연구나 보건분야에서 치매와 우울증의 임상경과관찰 및 일차 선별도구로서 유용하게 사용될 수 있게 되었다. Objectives : This study was designed to develop the Korean version of the Psychogeriatric Assessment Scale(PAS-K), a multidimensional screening tool for the dementia and depression. Methods : Through three times preliminary trials, the authors translated PAS into Korean. The PAS-K, Geriatric Depression Scale, Hamilton Depression Rating Scale, Hachinski Ischemic Scale and Mini-Mental State Examination-Korean version were administered to 291 subjects over the age of 60 and the same number of their informants(67 subjects were institutionalized, 168 subjects were in community, 58 subjects were psychiatric hospital patients). And the Diagnostic Interview Schedule(DSM-Ⅲ-R) was independently administered to exactly diagnose dementia and depression. The reliability and validity test, optimal cut-off point estimation for six each scale and ROC curve analyses were done to investigate the diagnostic validity of PAS-K. Results : Internal consistency and interrater reliability of the PAS-K were high. Concurrent validity of each scales of PAS-K was good in being measured with other standardized scales. The optimal cut-off points of each scale of the PAS-K were estimated as follows : 1) Stroke Scale of subject interview : 1, 2) Cognitive Impairment Scale of subject interview: 10, 3) Depression Scale of subject interview : 5, 4) Stroke Scale of informant interview : 1, 5) Cognitive Decline Scale of informant interview : 3, and 6) Behavioral Change Scale of informant : 2. conclusions : The PAS-K was valid and reliable screening tools for detecting dementia and depression. Therefore the PAS-K could be widely and extensively used in psychogeriatric epidemiological research or clinical setting of primary screening for dementia and depression.

        한국 정신장애의 역학 조사 연구[I] : 각 정신장애의 유병률

        Objectives : This study aims to estimate the prevalence of the DSM-IV psychiatric disorders in Korean population using the Korean version of Composite International Diagnostic Interview (K-CIDI). Methods : Subjects were selected by taking multi-stage, cluster samples of 7,867 adult household residents, 18 to 64 years of age, in ten catchment areas. Total 78 trained interviewers administered the K-CIDI to the selected respondents, from June 1 to November30,2001. Results : Total 6,275 respondents completed the interview. Some 33.5% of respondents reported at least one lifetime disorder, 20.6% reported at least one-year disorder, and 16.7% reported at least one-month disorder. The most common lifetime disorders were alcohol abuse/dependence (17.24%), nicotine dependence/withdrawal (11.19%), specific phobia (5.16%), and major de-pressive disorder (4.25%). The lifetime prevalence of substance abuse/dependence (0.25%) and schizophrenia (0.16%) was very low. Nicotine and alcohol use disorder showed very high male/female ratio. Mood disorder and anxiety disorder were more prevalent among female than male. Conclusion : The prevalence of psychiatric disorders was high. In comparison with other studies, remarkable differences in distributions of psychiatric disorders across the areas and times were observed.

      • 악성 혈액질환에서 성공적인 동종골수이식 후 숙주 기질 미세환경의 구축

        인체의 골수는 간엽모세포를 함유하고 있으며 이들은 골수미세환경의 주된 세포들로 분화가 가능하여 조혈기능을 지지한다. 본 연구에서는 성별이 다른 동종조혈모세포이식 환경에서 조혈모세포의 완전 생착을 보이고, 이식 후 1년에서 8년이 지난 11예의 재생불량성 빈혈 및 백혈병 환자들을 대상으로 하여 골수에서 MSC를 분리하고 체외 확장을 통해 배양된 MSC에서 X 염색체 탐식자를 이용한 FISH 및 microsatellite polymorphism PCR 기법으로 그 기원을 확인하였다. 그 결과 조혈모세포는 완전히 공여자 기원으로 대치되었음에도 불구하고 MSC는 모두 수여자 기원임을 알 수 있어, 동종조혈모세포이식에서 미세환경의 구축은 수여자의 자가 생산에 의한 골수 간질세포에 의한 것으로 생각된다. Background: Human bone marrow (BM) contains mesenchymal stem cells (MSC) that can differentiate into various cells of mesenchymal origin. It remains a matter of controversy whether donor-derived stromal cells are capable of engraftment following hematopoietic stem cell transplantation (HSCT) or not. Methods: To determine if donor-derived stromal cells are transferred to the recipients of allogeneic HSCT, we investigated the characterization of MSC in 11 patients 1 to 8 years after sex mis-matched allogeneic HSCT in severe aplastic anemia and leukemia. Results: All patients had complete engraftment with donor- derived stem cells as shown by detection of donor type DNA in peripheral blood mononuclear cells. Following culture, MSC showed the expression of SH2 and SH4, but none of the hematopoietic markers of CD14, CD34, or CD45. MSC which can be differentiated to osteogenic lineage showed the genotype of recipient completely using FISH or PCR analysis. Conclusion: This study confirmed that MSC isolated from recipients of allogeneic HSCT in severe aplastic anemia and leukemia are not of donor genotype despite of full hematopoietic engraftment with donor type. Donor cells did not contribute to reconstitute the marrow microenvironment.

        초등학생의 음악 적성과 음악 환경에 대한 조사 연구

        The purposes of this study were:(1) to examine factors related to musical environments of elementary students in Seoul, and :(2) to determine whether these factors have effects on the studets' music aptitude. Students(n=950) enrolled in an elementary school in Seoul participated in this study. All the participating students took the music aptitude test(IMMA by Gordon). Prior to the test, students filled out questionnaires. The questionnaires were designed to elecite information on: (1) their experiences of taking private lessons in music instrument: (2) kind of instruments they learned; (3) performing abilities of their family member; (4) attitude towards music; (5) kind of most prefered music; (6) experiences of visiting concerts,; (7) and the devices used in music listening. The questionnaires and the scores of music aptitude test were analyzed. In case of grade 3-6, the correlation between the scores of eight academic subjects and the scores of music aptitude test were analyzed. Based on the statistic analysis of the data. following facts were founded: (1) majority of participating students have taken private lessons in music instruments(mostly piano), and taking private lessons have positive effects on developing music aptitude: (2) no relationship was founded between students' music aptitude and performing abilities of their family members: (3)most students have visited concerts, and this experience has positive effects on developing music aptitude: (4) most students have positive attitude towards music, and this factor closely related to music aptitude: (5) classical music is most preferred by participating students, and students who liked classical music had relatively higher music aptitude: (6) repondents' prefered devices for music appreciation may have effects on developing music aptitude: (7) there was little correlation between music aptitude and academic scores.

      • 피부 병변을 동반한 악성조직구증 1예

        Malignant histiocytosis, originally described in 1939 as histiocytic medullary reticulosis by Scott and Robb-Smith, is a rare histiocytic proliferative disorder that often shows an acute onset and used to progress to death within a few months. This disorder characterized clinically by fever, generalized weakness, lymphadenopathy and hepatosplenomegaly, and shows a variable range of pancytopenia. Extranodal involvement is common, with an incidence ranging from 50% to as high as 90%, skin involves8ment was noted in 10% to 15%. Typical skin lesions were mainly founded in extremity. i.e. erythematous papule and nodule occasionally become to necrosis and ulceration. We experienced one case of malignant histiocytosis in a 46-years-old female. The major clinical findings are fever, malaise, hepatosplenomegaly and erythematous skin lesion. In the laboratory study, pancytopenia is noted on the peripheral blood. And also aggregation of many atypical histiocytes were shown on skin and bone marrow biopsy. So we reported one related case with malignant histiocytosis as well as reviewing literature .

      • 유아의 상징놀이와 사회인지 발달: 통합적 고찰

        This study is an integrative review of research on children's symbolic play development and its outcomes. The present study examines a developmental progression in the ability to engage in pretend play that seems to reach a crescendo during the preschool years. Recognition of make-believe as a central factor in play is reflected in a series of training studies n which adults have attempted to foster pretense in childern's play. The majority of training studies have investigated the transfer effects of training in pretend play to a variety of related social-cognitive skills, and mostly reported positive findings. These results provide credible evidence to support the theories of cognitive-structural-symbolic aspects of development(e.g., Piaget, Vygotsky), and underscore the educational value of training for the development of play ability. Results of these studies are reviewed and critiqued in terms of methodological issues. Then, areas in need of further inquiry are discussed with respect to issues that require empirical clarification

      • 유아의 도덕성 발달과 도덕교육 연구

        Since theory or moral development has not yet settled down into an accepted system in many early childhood programs, this study was presented to review theories of moral development, to compare educational approaches to moral development, and to combine the most useful suggestions into an educational setting. From the theoretical ideas of Piaget and Kohlberg and some other research findings, it is indicated that the process of moral development is not imprinting rules and virtues, but rather the process of transforming mental sttuctures, a process that is contingent on cognitive growth combined with stimulation from the social environment. Influences on the Kohlberg stages include cognitive conflict, peer relation, and perspective taking. Criticisms include an overemphasis on cognition and an underemphasis on behavior, the quality of the research, an inadequate consideration of the care perspective, and an underestimation of the role of culture. As three educational approaches to moral development, character eduction, values clarification, and the cognitive-develomental approach were examind. Cognitive morl education was considered to help children understand and value the resoning behind the moral choices. With this approach, moral education means facilitating children's propgess through the stages of moral reasoning, leading to the adoption of freely chose moral values. Damon's view is more comprehensive than the traditional cognitive moral education viesws, because it recognizes the importance of emotions, parent-child relations, and culture in moral development, integrating them with the influence of schools, peers, and cognition in a meaningful way. Implications for future research and educational parctices were suggested, pursuing the development of a conscience and of a consistent set of moral behaviors.
