        흰민들레 (Taraxacum coreanum) 추출물이 급성 수은 중독된 생쥐의 간에 미치는 효과

        본 연구는 급성 수은 독성에 대한 민들레의 효과를 알아보기 위하여 시도되었다. 30 g 내외의 생쥐를 대상으로 대조군, 수은(5mg/kg) 투여군, 수은 투여 후 민들레(3 g/kg)를 구강투여 한 후 다시 24, 48, 72, 96시간, 1주일군으로 세분하여 간장 손상 억제 효과를 알아보기 위하여 생화학적 및 조직학적 실험을 실시하였다. 혈액중 aspartate amiotransferase (AST)와 alanine aminotransferase (ALT)의 수치는 민들레 투여군이 수은 투여군보다 감소되었다. 또한 간조직의 SOD와 catalase 활성도 역시 민들레 투여군이 수은 투여군에 비하여 감소하였으나 통계학적 유의성은 없었다. 간 조직의 광학현미경적 관찰에서 심한 조직괴사가 관찰되지만 민들레 투여군에서는 문맥주위의 약간의 괴사와 심한 호중구 침윤현상이 관찰되었다. 전자현미경적 관찰 결과, 간장에서 수은 투여군은 간세포의 핵이 함입되어 불규칙했으며 미토콘드리아와 조면소포체의 수조가 팽대되고, 리보솜의 탈락이 관찰되었다. 민들레 투여군은 핵이 정상적인 상태로 관찰되었고, 전자밀도가 높은 미토콘드리아가 분포되어 있었으며, 리보솜이 부착된 상태로 층판구조를 형성하는 조면소포체가 관찰되었다. 이상의 연구 결과로 보아 민들레가 수은으로 유발된 간손상을 보호하는 효과가 있는 것으로 사료되며 민들레에 대한 다양한 연구가 필요할 것으로 사료된다. Dandelion has been frequently used as a remedy for women’s disease, inflammatory diseases and disorders of the liver and gallbladder. Dandelion extracts water extract, an herbal medication, may have an effect on the activity of hepatic antioxidant enzymes in diabetic rat. This study aims demonstrate the effect of dandelion extracts, one of the natural chelator, on the biochemical and enzyme activity changes in the mouse liver caused by HgCl₂. Mice approximately 30 gm in weight were grouped into the control, mercury chloride-treated, and the dandelion extractstreated after mercury chloride groups. HgCl₂ (5 mg/kg) and dandelion extracts (3 g/kg) were delivered orally. Serum AST and ALT were measured, enzyme activity of liver were examined by spectrophotometer and ultrastructural alteration of liver were examined by light and electron microscopy. Dandelion extracts were decreased the increase of serum AST and ALT level induced by mercury. The catalase activity was decreased in the dandelion extracts group. The activity of SOD was dereased, but did not show significant differences. Mercury chloride-treated hepatic cell were irregular nucleus, enlarged and reduced number of mitochodria, enlarged rough endoplasmic reticulum, loss of ribosomes. Cells treated with dandelion extracts were similar to those of the control group. In conclusion, dandelion extracts may protect the mercury-induced toxicity on Liver.

        Structural Characteristics on InAs Quantum Dots multi-stacked on GaAs(100) Substrates

        The InAs self-assembled quantun dots (SAQDS) were grown on a GaAs(100) substrate using a molecular beam epitaxy (MBE) technique. The InAs QDs were multi-stacked to have various layer structures of 1, 3, 6, 10, 15 and 20 layers, where the thickness of the GaAs spacer and InAs QD layer were 20 monolayers (MLs) and 2 MLs, respectively. The nanostructured feature was characterized by photoluminescence (PL) and scanning transmission electron microscopy (STEM). It was found that the highest PL intensity was obtained from the specimen with 6 stacking layers and the energy of the PL peak was split with increasing the number of stacking layers. The STEM investigation exhibited that the quantum dots in the 6 stacking layer structure were well aligned in vertical columns without any deflect generation, whereas the volcano-like deflects were formed vertically along the growth direction over 10 periods of InAs stacking layers. 분자선에피택시법에 의하여 GaAs(100)기판 위에 InAs 자발형성양자점을 성장하였다. InAs 양자점은 1, 3, 6, 10, 15 및 20층 등으로 다양하게 적층되어졌고, GaAs 층과 InAs 양자점 층은 각각 20 MLs와 2 MLs의 두께를 갖도록 하였다. 이 양자점의 나노구조적 특성은 PL과 STEM을 사용하여 분석하였다. 가장 높은 PL 강도는 6층의 적층구조를 갖는 시편에서 나타났고 PL 피크의 에너지가 적층회수가 증가함에 따라 분리됨을 알 수 있었다. STEM분석결과, 6충의 적층구조에서는 결함이 거의 없이 수직으로 형성된 구조를 보여준 반면에 10층 이상의 적층구조를 가질 때 그 성장 방향에 따라 volcano형상을 갖는 결함이 수직하게 성장되어졌다.

        Baseline Clinical and Biomarker Characteristics of Biobank Innovations for Chronic Cerebrovascular Disease With Alzheimer’s Disease Study: BICWALZS

        Objective We aimed to present the study design and baseline cross-sectional participant characteristics of biobank innovations for chronic cerebrovascular disease with Alzheimer’s disease study (BICWALZS) participants.Methods A total of 1,013 participants were enrolled in BICWALZS from October 2016 to December 2020. All participants underwent clinical assessments, basic blood tests, and standardized neuropsychological tests (n=1,013). We performed brain magnetic resonance imaging (MRI, n=817), brain amyloid positron emission tomography (PET, n=713), single nucleotide polymorphism microarray chip (K-Chip, n=949), locomotor activity assessment (actigraphy, n=200), and patient-derived dermal fibroblast sampling (n=175) on a subset of participants.Results The mean age was 72.8 years, and 658 (65.0%) were females. Based on clinical assessments, total of 168, 534, 211, 80, and 20 had subjective cognitive decline, mild cognitive impairment (MCI), Alzheimer’s dementia, vascular dementia, and other types of dementia or not otherwise specified, respectively. Based on neuroimaging biomarkers and cognition, 199, 159, 78, and 204 were cognitively normal (CN), Alzheimer’s disease (AD)-related cognitive impairment, vascular cognitive impairment, and not otherwise specified due to mixed pathology (NOS). Each group exhibited many differences in various clinical, neuropsychological, and neuroimaging results at baseline. Baseline characteristics of BICWALZS participants in the MCI, AD, and vascular dementia groups were generally acceptable and consistent with 26 worldwide dementia cohorts and another independent AD cohort in Korea.Conclusion The BICWALZS is a prospective and longitudinal study assessing various clinical and biomarker characteristics in older adults with cognitive complaints. Details of the recruitment process, methodology, and baseline assessment results are described in this paper.

        How high is too high in cutoff levels from 50-g glucose challenge test

        Objective To determine the highest 50-g glucose challenge test (GCT) value that indicates no further diagnostic test is needed to confirm a diagnosis of gestational diabetes mellitus (GDM) under the criteria of National Diabetes Data Group (NDDG) or the Carpenter and Coustan (C&C) and fasting glucose thresholds from the International Association of Diabetes and Pregnancy Study Group (IADPSG). Methods We collected the 50-g GCT results from 16,560 pregnancies and identified 2,457 gravidas with positive 50-g GCT (≥130 mg/ dL) values who underwent the 100-g glucose tolerance test. We investigated GDM prevalence in pregnancies with positive 50-g GCT according to the respective diagnostic thresholds and determined the 50-g GCT cutoff values with 100% positive predictive value for GDM under each diagnostic threshold. Results Twelve point five percent (306/2,457), 20.0% (492/2,457), and 9.6% (235/2,457) met the diagnostic criteria of GDM with the application of NDDG, C&C criteria, and fasting glucose thresholds from IADPSG (≥92 mg/dL), respectively. We also found that the prevalence of GDM increased with increasing 50-g GCT values using each diagnostic criterion. Importantly, we identified that all subjects with a 50-g GCT value ≥223, ≥217, or ≥228 mg/dL can be exclusively diagnosed as having gestational diabetes according to the criteria of NDDG, C&C, and fasting glucose thresholds from IADPSG, respectively. Conclusion We propose that women with a 50-g GCT screening value ≥228 mg/dL can be reliably omitted from further confirmative tests for GDM, such as 100- or 75-g glucose tolerance test.

        Comparison of neonatal outcomes and intrapartum events in full term vaginal deliveries conducted by staff versus resident physicians

        Objective The objective of this study was to compare the neonatal outcomes and intrapartum events conducted by staff versus resident physicians in full term vaginal deliveries. Methods We divided study population (n = 5,007) into two groups: staff versus resident physicians. These two groups were sub-divided; faculty versus fellow and senior versus junior resident, respectively. The maternal characteristics, neonatal outcomes including Apgar score, admission to the neonatal intensive care unit and umbilical arterial pH and intrapartum event which was defined as the occurrence of shoulder dystocia and vacuum delivery were also investigated. Results There was no difference in neonatal outcomes between two groups. The group delivered by staff had a higher rate of nulliparity, large for gestational age and intrapartum events than the resident physician group. The subgroup analysis revealed a higher rate of vacuum delivery in the group delivered by faculty and senior members than the group delivered by fellows and junior members. Conclusion There was no significant difference in neonatal outcomes between the two groups; staff versus resident physicians in full term vaginal deliveries in low-risk pregnant women. Also, experienced obstetricians might tend to participate in difficult labors and would prefer applying vacuum compared to the obstetricians with fewer experiences.

        목적: We aimed to determine the highest 50-g glucose challenge test (GCT) value that could omit the need to perform a 100-g glucose tolerance test (GTT) for GDM according to the National Diabetes Plasma Data Group (NDDG) and the Carpenter and Coustan (C&C) criteria, respectively. 방법: We collected 16,560 pregnancies with 50-g GCT result from January 2001 to December 2013 and identified 2,457 gravidas with positive 50-g GCT (>130mg/dl) values who underwent the 100-g GTT. Then, we investigated GDM prevalence in pregnancies with positive 50-g GCT according to the respective diagnostic thresholds and determined the 50-g GCT cutoff values with 100% specificity for GDM under each diagnostic threshold. 결과: Of the 2,457 pregnancies with positive 50-g GCT results, 12.4% (305/2,457) received GDM diagnoses under NDDG thresholds and 20.0% (491/2,457) received the diagnosis with the application of C&C thresholds. Additionally, we found that the prevalence of GDM was increased with the rise of 50-g GCT values in both criteria. Furthermore, we could find that all subjects with a 50-g GCT screening value ≥223mg/dL had GDM under NDDG criteria and those with a 50-g value ≥217 mg/dL all received GDM diagnoses under C&C criteria. 결론: The prevalence of GDM in pregnancies with positive 50-gGCT increased by using of the C&C criteria. Also, the 100-g GTT could possibly be omitted in subjects with 50-g GCT values over 223 mg/dL in NDDG and 217 mg/dL in C&C criteria, respectively. We expect that our results can provide practical information to obstetricians in GDM diagnosis.

      • Case Report : The ex utero intrapartum treatment (EXIT) procedure for a fetal neck mass

        Large fetal neck masses may be associated with fetal airway obstruction, which can lead to neonatal hypoxia, brain injury, and death in the neonatal period. The relationship between a fetal neck mass and airway structures can be assessed prenatally with ultrasonography and magnetic resonance imaging (MRI). The ex utero intrapartum treatment (EXIT) procedure is a procedure to secure the fetal airway, while preserving the feto-maternal circulation at the time of cesarean delivery. In the case reported herein, an anterior fetal neck mass measuring approximately 7 cm in size, was diagnosed with ultrasound and MRI which suggested possible airway involvement immediate after birth. Therefore, the EXIT procedure was successfully performed with a multidisciplinary approach at the time of delivery. After birth, the neonate underwent surgery and the histopathologic result indicated that the lesion was a cavernous lymphangioma. Herein we report a case involving a fetal neck mass diagnosed with ultrasound and MRI and treated with the EXIT procedure, with a brief review of the literature.
