A 29-year-old man developed sudden dysarthria and right-sided weakness 3 days before admission to hospital. He was diagnosed with parotitis in the emergency room of Soonchunhyang University Gumi Hospital. The plasma levels of homocysteine were elevated (30.48 mg/dL). A brain magnetic resonance imaging scan revealed a high signal intensity lesion in the left paramedian pons and computed tomography angiography showed a marked narrowing of the mid-basilar artery. We report a rare case of spontaneous basilar artery dissection caused by a recent infection and hyperhomocysteinemia.

Valacyclovir is a prodrug of acyclovir, transformed to acyclovir by first-pass metabolism in the liver. In recent years, valacyclovir has largely replaced acyclovir in the treatment of herpesvirus infections, because it is more effective by oral administration. Valacyclovir is used increasingly to treat herpes zoster, although neuropsychiatric symptoms [valacyclovir neurotoxicity (VAN) or acyclovir neurotoxicity], may accompany use of this drug. This paper reports two cases of neurotoxicity of valacyclovir in a patients with end-stage renal disease who was undergoing maintanence hemodialysis. A 68-year-old male, receiving hemodialysis three times a week, prescribed valacyclovir 1000 mg/day on every dialysis day, for herpes zoster of left thigh . After he took his second dose orally, he developed lethargy and disorientation. His mental status progressed to semicoma. Daily conventional hemodialysis was started. After three sessions of hemodialysis, the patient became alert A 64-year-old male who was being treated with hemodialysis was admitted for dysarthria and euphoria. He took 1,000 mg of valacyclovir twice for hepes zoster of fourth level of thoracic nerve. After two days of treatment with oral valacyclovir, he had several neurologic symptoms as mentioned above. We performed daily hemodialysis and after three session of hemodialysis, he fully recovered. In conclusion, valacyclovir can induce life-threatening neurotoxicity, especially in ESRD patients with appropriate dose reduction. But which can be effectively managed by hemodialysis.

Sclerosing mesenteritis is a rare inflammatory disease of the bowel mesentery. It produces tumor-like masses of the mesentery composed of varying degrees of fibrosis, chronic inflammation, and fat necrosis. It has been described variously as fibrosing mesenteritis, retractile mesenteritis, mesenteric Weber Christian disease, and systemic nodular panniculitis. The etiology and pathogenesis of the disease are as yet unknown, but autoimmune disorder, previous abdominal surgery, trauma, and ischemia could play a role. The clinical features include abdominal pain, vomiting, diarrhea, and constipation. Occasionally, patients with this condition may present with bowel obstruction. Rarely, It can be associated with other idiopathic inflammatory disorders such as retroperitoneal fibrosis, sclerosing cholangitis, and orbital pseudotumors. We report a case of idiopathic sclerosing mesenteritis with retroperitoneal fibrosis in a 58-year-old man. (Korean J Gastroenterol 2011;58:221-225)

A bulk-grounded DRAM cell, which sets the substrate bulk voltage to the ground level by eliminating the conventional charge pumping V_(BB) generator, was proposed for use in multi-gigabit DRAMS using triple-well CMOS technology. This cell reduced the leakage current to half, increased the refresh time by 40-70% and reduced the stand-by power consumption, compared to the conventional cell.

N/A The purpose of this study was to assess the ability of quantitative Tl-201 tomography to identify and localize coronary artery disease (CAD). The study population consisted of 4l. patients (31 males, 10 females; mean age 55 +- 7 yr) including 14 with prior myocardial infarction who underwent both exercise Tl-201 myocardium SPECT and coronary angiography for the evaluation of chest pain. From the short axis and vertical long axis tomograms, stress extent polar maps were generated by Cedars-Sinai Medical Center prograrn, and the 9 stress defect extent (SDE) was quantified for each coronary artery territory. For the purpose of this study, the coronary circulation was divided into 6 arterial segments, and the myocardial ischemic score (MIS) was calculated from the coronary angiogram. Sensitivity for the detection of CAD (>50% coronary stenosis by angiography) by stress eXtent polar map was 95% in single vessel disease, and 100% in double and triple vessel deseases. Overall sensitivity was 97%<. Sensitivity and specificity for the detection of individual diseased vessels werc, respectively, 87% and 90% for the left anterior descending artery (LAD), 36% and 93% for the left circumflex artery (LCX), and 71% and 70%, for the right coronary artery (RCA). Concordance for the detection of individual diseased vessels between the coronary angiography and stress polar map was fair for the LAD (kappa=0.70), and RCA (kappa=0.41) lesions, whereas it was poor for the LCK lesions (kappa =0.32) There were siginificant correlations between the MIS and SDE in LAD (rs=0. 56, p=0.0027), and RCA territory (rs=0.60, p=0.0094). No significant correlation was found in LCX territory. When total vascular territories were combined, there was a significant correlation between the MIS and SDE (rs=0.42, p=0,0116). In conclusion, the quantitative analysis of TI-201 tomograms appears to be accurate for determining the presence and location of CAD.

Intestinal ischemia is divided into three categories, namely, acute mesenteric ischemia (AMI), chronic mesenteric ischemia (CMI), and colonic ischemia. AMI can result from arterial or venous thrombi, emboli, and vasoconstriction secondary to low-flow states. It is an urgent condition which can result in high mortality rate. The predominant causative factor of CMI is stenosis or occlusion of the mesenteric arterial circulation, and it is characterized by postprandial abdominal pain and weight loss. Surgery is the treatment of choice for intestinal ischemia. However, it has been recently reported that percutaneous transluminal angioplasty with stent placement and/or thrombolysis is an effective therapy in various types of mesenteric ischemia. We report six cases of mesenteric ischemia which were successfully treated by percutaneous angioplasty, and review the literature from South Korea. (Korean J Gastroenterol 2011;57:243-248)
체내 안정형 HSV1-tk (Herpes Simplex Virus Type-1 Thymidine Kinase) 영상용 IVDU 유도체의 합성

목적: 화합물이 가지는 항바이러스 및 항암 작용으로 인하여 5-iododeoxyuridine 유도체들에 관한 연구는 매우 광범위하게 이루어져왔다. 방사 요오드가 치환된 이들 화합물에 대한 연구의 주요 목표는 체내 및 체외 평가 시 효과적 일뿐만 아니라, 보다 안정한 화합물을 개발하는 것이다. 본 연구를 통해, 다양한 방사성의약품의 중간체로도 이용되어질 수 있는 carbocyclic radioiododeoxyuridineanalogue (ddIVDU)를 합성하여 생물학적 평가를 수행하고자 하였다. 대상 및 방법: 목표 화합물의 합성은 Pd(0)-촉매 결합반응, LAH 환원반응, hetero Diels-Alder 반응을 주요 반응으로 하여 수행하였다(Sch 3). 합성된 화합물의 생물학적 평가를 위해 MCA와 MCA-tk 세포에 다양한 농도의 GCV와 carbocyclic ddIVDU를 처리하여 MTS 법을 이용해 세포독성을 보았고, [125I]carbocyclic ddIVDU를 이용해 MCA와 MCA-tk 세포에서 HSV1-tk에 의한 선택적 섭취를 비교 평가 하였다. 결과: Carbocyclic ddIVDU 및 요오드 도입을 위한 전구물질의 합성을 cyclopentadiene을 출발물질로 하여 높은 수율에 의해 수행하였다. 방사요오드를 이용한 동위원소 도입 반응은 각각, 80% 이상의 방사표지 수율 및 95% 이상의 방사 화학적 순도를 나타내었다. 세포독성 결과 MCA-tk 세포에서 carbocyclic ddIVDU는 GCV에 비해 세포독성이 작았고, MCA에서는 유사한 세포독성을 보였다. [125I]carbocyclic ddIVDU는 MCA-tk 세포에서 uptake는 8시간째까지 계속 증가하는 양상을 보였으며, MCA 세포에서 uptake는 4시간째까지 일정하게 유지되다가 8시간째 급격히 증가하였다. [125I]carbocyclic ddIVDU의 uptake는 MCA보다 MCA-tk에서 약 2-5배 정도 높은 결과를 보였다. 결론: 진행되어진 생물학적 결과로부터 이 유도체는 체내 평가 시 매우 안정할 뿐만 아니라, GCV에 비해 독성이 덜하고, HSV1-tk cell에 선택적임을 보여준다. 따라서 신규 화합물인 carbocyclic ddIVDU는 HSV1-tk를 영상화하는데 유용하게 이용되어질 수 있을 것이다. Purpose: 5-iododeoxyuridine analogues have been exclusively developed for the potential antiviral and antitumor therapeutic agents. In this study, we synthesized carbocyclic radioiododeoxyuridineanalogue (ddIVDU) and carbocyclic intermediate as efficient carbocyclic radiopharmaceuticals. Materials and Methods: The synthesis is LAH reduction, hetero Diels-Alder reaction as key reactions including Pd(0)-catalyzed coupling reaction together with organotin. MCA-RH7777 (MCA) and MCA-tk (HSV1-tk positive) cells were treated with various concentration of carbocyclic ddIVDU, and GCV. Cytotoxicity was measured by the MTS methods. For in vitro uptake study, MCA and MCA-tk cells were incubated with 1uCi of [125I]carbocyclic ddIVDU. Accumulated radioactivity was measured after various incubation times. Results: The synthesis of ddIVDU and precursor for radioiodination were achieved from cyclopentadiene in good overall yield, respectively. The radioiododemetallation for radiolabeling gave more than 80% yield with >95% radiochemical purity. GCV was more toxic than carbocyclic ddIVDU in MCA-tk cells. Accumulation of [125I]carbocyclic ddIVDU was higher in MCA-tk cells than MCA cells. Conclusion: Biological data reveal that ddIVDU is stable in vitro, less toxic than ganciclovir (GCV), and selective in HSV1-tk expressed cells. Thus, this new carbocyclic nucleoside, referred to in this paper as carbocyclic 2`,3`-didehydro-2`,3`-dideoxy-5- iodovinyluridine (carbocyclic ddIVDU), is a potential imaging probe for HSV1-tk. A significant injury of ligament or tendon that might result in hypermobility and/or instability has been termed connective tissue insufficiency (CTI). Load bearing on CTI can sometimes create a malfunction of the mechanoreceptor and this in turn can aggravate the problem thus creating a chronic condition. Prolotherapy means the injection of growth factors or growth factor stimulators into the fibroosseous junction of the disabled ligament, tendon or cartilages in order to strengthen and promote proliferation of the injured Connective tissue. In the case of frequent recurrence or unresponsiveness to conservative treatment including nerve blocks in the chronic musculoskeletal disorder. We obtained good results after the prolotherapy treatment. This study will report on 5 cases of prolotherapy treatment.
Improvement of Frontal Lobe Dysfunctions in Neuromyelitis Optica after Treatment: A Case Report

BackgroundNeuromyelitis optica (NMO) is characterized by optic neuritis and longitudinally extensive transverse myelitis. Generally, the brain had been considered healthy in NMO patients, though recent studies have demonstrated that T2-weighted abnormalities may be observed in various brain regions. Logically, NMO brain lesions are localized at sites of high aquaporin-4 expression. Case ReportA 68-year-old right-handed man with dysuria, weakness in the bilateral upper and lower limbs, and decreased sensation of the lower extremities, was diagnosed with neuromyelitis optica. The patient was gradually speaking less, was showing reduced interest in hobbies, and had undergone changes in character and behavior. An examination was performed using the Seoul Neuropsychological Screening Battery (SNSB), which revealed that the profile of frontal lobe dysfunctions was prominent as compared with other cognitive domains. The patient was treated with prednisolone and azathioprine for about 1 year without recurrence, and showed prognostic improvement according to further SNSB testing. ConclusionsFurther studies are considered necessary in order to find the most effective medication regimen for improving cognitive functions in those accurately diagnosed with NMO, and to develop systematic treatment using even more diversified immune-related agents.
