        Factors associated with the risk of colorectal neoplasia in young adults under age 40

        Background/Aims: Recent epidemiologic studies have shown a continued increase in colorectal cancer incidence among younger adults. Little is known about the factors that contribute to the development of young-onset colorectal neoplasia (CRN). Methods: A cross-sectional analysis was performed for individuals younger than 40 years who underwent colonoscopy in Seoul St. Mary’s Hospital and its affiliated health screening center. High-risk CRN was defined as adenoma or sessile serrated lesion ≥ 10 mm, with three or more adenomas, villous histology, high grade dysplasia, or carcinoma. Results: Of these 13,621 included participants, 2,023 (14.9%) had one and more CRN. Young patients with CRN tended to be elderly, male, obese, smoker, having a habit of drinking, and having comorbidities such as hypertension, dyslipidemia, diabetes, and chronic kidney disease. In a multivariate analysis adjusted for age, sex, obesity, smoking status, and alcohol intake, old age (odds ratio [OR], 1.086; 95% confidence interval [CI], 1.054 to 1.119), male sex (OR, 1.748; 95% CI, 1.247 to 2.451), obesity (OR, 1.439; 95% CI, 1.133 to 1.828), and smoking (OR, 1.654; 95% CI, 1.287 to 2.127) were independent risk factors for overall CRN. Obesity and smoking as two modifiable factors increased the risk for high-risk CRN even more than for overall CRN (OR, 1.734; 95% CI, 1.168 to 2.575 and OR, 1.797; 95% CI, 1.172 to 2.753, respectively). Conclusions: Obesity and smoking were modifiable risk factors for CRN in young adults. They increased the risk for highrisk CRN even more than for overall CRN. A colonoscopy might be beneficial for young individuals with these factors.

        오스테나이트계 스테인리스강과 SM45C의 연속파형 Nd:YAG 레이저 용접특성비교

        Welding characteristics of austienite 304 stainless steel and SM45C using a continuous wave Nd:YAG laser are experimentally investigated. Laser beam welding is increasingly being used in welding of structural steels. The laser welding process is one of the most advanced manufacturing technologies owing to its high speed and deep penetration. The thermal cycles associated with laser welding are generally much faster than those involved in conventional welding processes, leading to a rather small weld zone. Experiments are performed for 304 stainless steel plates changing several process parameter such as laser power, welding speed, shielding gas flow rate, presence of surface pollution, with fixed or variable gap and misalignment between the similar and dissimilar and plates, etc. The Nd:YAG laser welding process is one of the most advanced manufacturing technologies owing to its high speed and penetration. This paper describes the weld ability of SM45C carbon steel for machine structural use by Nd:YAG laser. The follow conclusions can be drawn that laser power and welding speed have a pronounced effect on size and shape of the fusion zone. Increase in welding speed resulted in an increase in weld depth/aspect ratio and hence a decrease in the fusion zone size. The penetration depth increased with the increase in laser power.

        This study focuses on the inspect of effective net area design code, according to limited state design criteria of steel structures, recently established in Korea, by an experiment on the joint of angle tension members on the ground. reduction factor. The methods of this study were to compare other study results on effective net area rupture mode and ultimate capacity, and to evaluate the propriety of the criteria design code. The result is that code in force limited state design criteria of steel structures and AISC-LRFD joint of angle tension members predicted load and experiment depend on ultimate capacity difference. Therefore, it is thought that there is need for supplemental test and study to effective net area of tension joint focused on bolted connection of angle.

      • 영아 급성 출혈설 부종 1례

        영아 급성 출혈성 부종 (acute hemorrhagic edema of infancy, AHEI)은 4개월에서 2세 사시의 소아에서 주로 얼굴과 사지에 국한된 염증성 부종과 특징적인 자반성 피부 병변을 보이는 백혈구 파쇄성 혈관염으로, 내부 장기의 침범 없이 급성의 피부 병변을 보이며 대게 1-3주 내에 자연 치유되는 양성질환이다. 이 질환은 비슷한 임상양상이나 조직소견을 보이는 다양한 질환들과의 감별이 필요한데, 특히 소아에서는 Henoch-Schonlein 자반증과의 감별이 특히 중요하다. 이에 저자들은 자반성 피부병변과 얼굴과 사지의 부종 등의 특징적 피부증상과 피부 병리소견을 통하여 AHEI로 진단한 17개월 남아 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Acute hemorrhagic edema of infancy (AHEI) is a variant of leukocytoclastic vasculitis which is characterized by edema and pupuric skin lesions involving mainly the face and extremities. However, there is usually no visceral involvement. The onset of the disease is acute but self-limited, bearing a benign clinical course within 1 to 3 weeks. The main differential diagnosis of AHEI is Henoch-Schonlein purpura. We report a 17-month old boy with AHEI presenting purpuric skin lesions, edema on face and extremities, and leukocytoclastic vasculitis on skin pathology.

      • 上東地區 重石鑛의 破壞特性에 關한 硏究

        A Study on failure strength of rock under uniaxial and triaxial compression tests was carried out for underground structural design in domestic and foreign countries recently. An experimental study was made for the core specimens of SANG-DONG tungsten mine. The results are as follows; 1. Failure strength was increasing gradually with increase of confining pressure. 2. Longitudinal and lateral strain increased in proportion to confining pressure under triaxial compression tests. 3. Stress-strain curves showed straight line to 62% of failure strength. 4. Effect of failure strength about porosity was 3.0% on the average and it represented lower value comparatively. 5. The values of triaxial compressive strength were distributed between 345 and 3082㎏/㎠, the mean value of which was 1063㎏/㎠. 6. The mean value of modulus of longitudinal elasticity(E) was 3.6X 10^5㎏/㎠ and poisson's ratio was 0.35.

      • 太谿穴의 臨床 活用에 대한 文獻考察

        Literally, the mean of Tae-gye is highest brook. And on the basis of the acupuncture theory, Tae-gye point is the Soo earth point and source point of the Kidney Channel as well. In addition, Tae-gye point is one of the Yang-Returning Nine points. We were trying to study bibliographically on the Tae-gye point because Tae-gye point can be used very broadly across the symptoms in the theoretical aspect described above. As a results, we found out that according to the classical books of acupuncture, Tae-gye point is entering point of the Meridian Water as a source point and can be used at both Kidney-Sufficient Syndrom and Kidney-Deficient Syndrom. And Tae-gye can be applied to the disease of kidney or bladder that is urogenital symtoms, Also, Tae-gye is an useful option of tooth-ache, asthma, indigestion, constipation, edema etc whose cause is related with decrease of Kidney ki.

      • 적외선 열화상 카메라를 이용한 Plume 형상에 따른 Nd:YAG 레이저 용접 결함 평가

        In Nd:YAG laser welding, evaluation methods of welding flaw are various. But, the method due to plume shape is difficult to classification of welding flaw. The Nd:YAG laser process it known to have high speed and deep penetration capability to become one of the most advanced welding technologies. At the present time, some methods are studied for measurement of plume shape by using high-speed camera and photo diode. This paper describes the machining characteristics of SM45C carbon steel welding by use of an Nd:YAG laser. In spite of its good mechanical characteristics, SM45C carbon steel has a high carbon contents and suffers a limitation in the industrial application due to the poor welding properties. In this study, plume shape was measured by infrared thermal camera that is non-contact /non-destructive thermal measurement equipment through change of laser generating power, speed, focus. Weld was performed on bead-on method. Measurement results are compared as two equipments. Here, two results are composed of measurement results of plume quantities due to plume shape by infrared thermal camera and inspection results of weld bead include weld flaws by ultrasonic inspector.

        Provisional implant의 임상적 적용례

        It has been reported that for implatns to become osseointegrated, they must heal in the absence of functional loads for 4 to 6 months. The provisional implants were designed to support provisional restorations and to allow for load-free osseointegration of definitive implants, while the was provided with immediate esthetic and function. In addition, the provisional implant protects graft site. Also the provisional implant provides anchorage for orthodontic treatment and reconstructs vertical dimension in edentulous condyle fracture to practice functional therapy. The provisional implant system is a sound and economical method to achieve patient's immediate masticatory and esthetic function. Three cases are presented to document and illustrate the clinical application.

      • 관상동맥 스텐트 시술 후의 재협착에 관한 연구

        Coronary stent implacement is known as an effective treatment in the intimal dissection after percutaneous transluminal coronary angioplasty and the prevention of restenosis. However, In-stent restenosis still remains a major concern in clinical stenting. The stents were placed in 103 patients from July 1996 to March 1999 and performed follow-up coronary angiograms in 59(57.3%) patients. To identify the clinical, angiographic and procedurerelated variables 'which predict late restenosis within the stented artery, 59 patients(58.3±9.9, M:F= 41:18) were studied. The clinical characteristics of the patients were stable angina in 23(39.0%), unstable angina in 14(23.7%), acute myocardial infarction in 21(35.6%) and old myocardial infarction in 1(1.7%). Coronary stenting was performed in 1 patient(1.7%) for primary lesion, 50 patients(84.7%) for suboptimal results after PTCA, 6 patients(10.2%) for bail-out procedure, and 2 patients(3.4%) for restenotic lesions. All patients were treated with aspirin and ticlopidinc. The follow-up angiograms were obtained at 7±4 months. The overall in-stent restenosis rate was 27.1%. The coronary angiographic findings were 32 single vessel(54.2%), 19 two vessel(32.2%) and 8 three vessel disease(13.6%). The angiographic morphological characteristics were type A in 33(55.9%), type B in 14(23.7%), type C in 12(20. 3%) cases. Variables of 16 patients with restenosis were compared with those of 43 patients without restenosis. Previously known predictors for in-stent restenosis were multiple stenting, stenting for restenotic lesions, residual stenosis after stenting, stenting for total occlusion lesions, reference diameter, balloon to vessel ratio, acute gain and minimal luminal diameter after procedure, design and characteristics of stents, ostial lesion of aorta, high pressure method for stenting, lesion length, diabetes mellitus, size of artheroma, saphenous vein grafts, ulcerlating lesions and calcified lesions. In this study, Reference diameter before stenting(2.43±0.54mm vs. 2.88±0.65mm, p=0.016) and balloon-to-artery ratio(1.28±0.26 vs. 1.11±0.18, p=0.006) were predictors for in-stent restenosis. 1) The overall in-stent restenosis rate was 27.1%. 2) In the analysis of predictors for in-stent restenosis, there was no significant differences in clinical, angiographic factors between group with restenosis and without restenosis. But, Only reference diameter before stenting and balloon-toartery ratio were predictors of late in-stent restenosis. In conclusion, stenting is effective revascularisation method for selected patients with ischemic heart disease, and to minimize in-stent restenosis rate, stent implanting is achieved in a large vessel on the basis of an artery-to-stnet ration of 1:1, if possible.
