소아, 청소년 아토피 피부염에서의 DPCP 면역요법의 임상적 치료효과

N/A Background: Atopic dermatitis is the most common chronic skin disease in childhood. Severe atopic dermatitis may require use of topical or systemic corticosteroids. These treatments are prone to several adverse side effects. This paper highlights the impressive effect of diphenylcycloprope-none(DPCP) sensitization in the some cases of childhood and adolescent atopic dermatitis. Objective: The purpose of this study was to examine the effect of DPCP in treating severe atopic dermatitis of children and adolescents. Methods: Twenty eight patients, who had moderate to severe atopic dermatitis, were recruited in the treatment of DPCP weekly for 10 weeks and followed up for another 4 weeks. Severity of atopic dermatitis was measured using SCORAD index. Results: Sixteen of 28 patients had finished the treatment courses. After 10 weeks of therapy with DPCP, the patient`s symptoms were improved and the mean SCORAD index was significantly reduced from 48.9±17.8 to 18.0±9.1 (p<0.01). During the follow-up period, patients could be managed by topical steroid and oral antihistamine and flare-up was no observed. Conclusion: According to our study, DPCP can be an effective, safe and well-tolerated therapy for moderate to severe atopic derm atitis in children and adolescents. We suggest that DPCP may be effective in the treatment of moderate to severe atopic dermatitis of children and adolescents without serious complications. (Korean J Dermatol 2004;42(1):29-36)

Where the loss or damage to a vessel has occurred in relation to hull insurance, the assured plaintiff, having pleaded a loss or damage by fire, barratry, and/or perils of the sea as the cause of loss, often are faced with the defense that there was no reason for the underwriter defendant to answer because that plaintiff had failed to discharge its persuasive burden and that, additionally or in the alternative, the loss was caused by the willful misconduct of the owners. In hull insurance cases, like any other civil action, the burden of proof of a claim lies with the plaintiff, which must prove to the satisfaction of the trial judge that the loss of damage was caused by a peril insured against. The above maxim, however, may require a modification when defendant avails himself of the positive defense contained in section 55(2)(a) of the Marine Insurance Ac 1906, whereby the burden of proving a willful misconduct will be upon defendant. Nevertheless, this modification is much debated. In this regard, this article examines he nature of the burden of proof in hull insurance under English law and also proposes a general principle of interpretation regarding the burden of proof in hull insurance cases.
초고온원자로 중간열교환기 미니챈널에서의 Molten Salt 열수력 특성 연구

초고온원자로(VHTR) 설계에 있어 중간열수송루프(IHTL) 및 중간열교환기(IHX) 설계는 고온의 운전조건(950 ℃ 이상)으로 인하여 공학적으로 어려운 과제 중 하나로 알려져있다. 본 연구에서는 LiF, NaF 및 KF(46.5:11.5:42.0 mole %)의 공융혼합물인 Flinak molten salt 를 IHTL 의 열수송매체로 고려하였다. Flinak molten salt 의 세관에서의 열수력 특성을 평가하기 위하여 직경이 1.4 ㎜ 인 원형관을 이용하여 고온의 가스와 Flinak 을 열교환할 수 있는 이중관식 열교환기를 구성하여 실험하였다. 실험 결과 층류유동에서 측정된 Flinak 의 마찰계수는 이론식인 64/Re 에 근접하였고 Nusselt 수는 일반적으로 3.66 에서 4.36 범위에 들었다. For Very High Temperature Reactors (VHTR), the designs of the Intermediate Heat Transport Loop (IHTL) and the Intermediate Heat Exchanger (IHX) are particularly difficult because of the high-temperature operation (up to 950℃). In this study, Flinak molten salt, a eutectic mixture of LiF, NaF, and KF (46.5:11.5:42.0 mole %) is considered as the heat transporting fluid in the IHTL. To evaluate the flow and heat transfer performance of the Flinak molten salt in small channels with hydraulic diameters in the millimeter range, a double-pipe heat exchanger was constructed using small-diameter tubes for the heat exchange between the Flinak and the gas flow. The experimental data showed that, for laminar Flinak flow, the measured friction factors were close to the 64/Re curve and the Nusselt numbers were generally between 3.66 and 4.36.

The immunotactoid glomerulopathy is a recently described entity characterized histologically by highly organized ultrastructural deposits that appears to be composed of immunoglobulin and complement and are negative for amyloid by Congo red stain. The idiopathic hypereosinophilic syndrome is a disease characterized by idiopathic eosinophilia in blood and eosinophilic infiltration of multiple organs. Although all organ systems can be virtually involved, renal involvement is rare. The authors have experienced a case of idiopathic hypereosinophilic syndrome in a 18-year-old male who showed asymptomatic proteinuria of nephrotic range. Renal biopsy revealed the findings of thickening of the glomerular capillary basement membranes and deposits within the capillary lumen by light microscopy. Immunofluoresence microscopy disclosed IgG and C3 in mesangial regions and capillary walls. Electron microscopy showed highly organized, fibrillary structures measuring about 20 nm in the mesangial matrix and basement membrane. Congo red staining for amyloid was negative. This case was thought to represent an instance of immunotactoid glomerulopathy.
